Comportamiento de los traumatismos dentoalveolares en pacientes atendidos en el Instituto Nacional de Salud del Niño de julio a noviembre en el año 2015

Objetivos: Determinar el comportamiento de los traumatismos dentoalveolares (TDA) en los pacientes que acuden al servicio de emergencia del Instituto Nacional de Salud del Niño. Materiales y método: En este descriptivo, observacional y transversal se tomó en muestra a toda la población de pacientes que acudían al servicio de emergencia en el Instituto Nacional de Salud del Niño con diagnóstico de TDA desde julio a noviembre en el año 2015, el registro se realizó mediante un ficha de recolección de datos y el examen clínico al paciente utilizando la clasificación de Andreasen. Los datos fueron introducidos tabulados en el programa estadístico SPSS para Windows (versión 20.0). Resultados: Acudieron un total de 101 pacientes con TDA, presentándose 146 piezas traumatizadas, entre ellas fueron 125 temporales y 21 permanentes. El traumatismo más frecuente fue la contusión labial, seguido de las avulsiones. La edad que presentó mayor frecuencia fue de 0 a 2 años, se presentó con mayor frecuencia en el sexo masculino (62%) que en el sexo femenino (41%). La causa más frecuente fueron las caídas (87%) y el tiempo transcurrido desde el traumatismo hasta la atención fue más frecuente de 0 a 4 horas (49.5%). La dentición más frecuentemente afectada fue la dentición primaria (76%) y las piezas con mayor frecuencia de traumatismos fueron los incisivos centrales superiores para ambas de denticiones (60,3%). Conclusiones: Los TDA tienen un comportamiento bastante variable, afectando con mayor frecuencia a los tejidos blandos, seguido de los tejidos periodontales, se presentan en temprana edad y con mayor frecuencia en los varones, y afectan principalmente a los incisivos centrales superiores. PALABRAS CLAVE: Traumatismo dentoalveolar, Andreasen, frecuencias, niños. / --- Objectives: To determine the behavior of dentoalveolar trauma (TDA) in patients presenting to the emergency of the National Institute of Child Health. Materials and Methods: This descriptive, cross-sectional observational sample was taken on the entire population of patients presenting to the emergency department at the National Institute of Child Health diagnosed with TDA from July to November 2015, the record it was made using a data collection sheet and clinical examination the patient using the classification of Andreasen. The tabulated data were entered in SPSS statistical program for Windows (version 20.0). Results: One hundred and a total of 101 patients with TDA, appearing traumatized 146 pieces, including 125 were temporary and 21 permanent. The most common lip contusion injury was followed by avulsions. The age had a higher frequency was 0-2 years, occurred more frequently in males (62%) than females (41%). The most common causes were falls (87%) and the time from trauma to the eye was more frequent than 0-4 hours (49.5%). The teeth most frequently affected was the primary dentition (76%) and parts more often injuries were the upper central incisors for both of dentition (60.3%). Conclusions: The TDA have a quite variable behavior, most frequently affecting the soft tissues, followed by periodontal tissues, and are presented at an early age and more frequently in males, affecting mainly the upper central incisors. Em seguida os animais foram mortos e a maxila direita coletada para análise microscópica do tecido pulpar e o ligamento periodontal da raiz mesial do primeiro molar superior. A análise microscópica demonstrou que a força de 600cN não se mostrou efetiva para promover alterações no ligamento periodontal, polpa e raiz. Por outro lado, demonstrou que um trauma de intensidade menor como a concussão, não promove alterações no ligamento periodontal, e por sua vez não representa um problema para o ortodontista. / This study has as purpose to present a method for study of the dentoalveolar traumatism, using a device capable to promote a traumatism maintaining the same intensity and sense of application of the force on molars of rats, and in that way, to be used as a method of experimental study in the induced movement of teeth previously traumatized. For the accomplishment of that traumatism a gauge was used, with one of their extremities adapted to exercise an impact on the first superior molar. The gauge was fastened in a metal support with the purpose of to standardize the direction of the application of the force and to position his extremity correctly adapted on the first superior molar. A injury was applied in the ocluso-gingival sense on the vestibular crest of the first right superior molar of 10 rats with force of 600 grams, corresponding to the approximately 600cN. Soon afterwards the animals were died and the right maxillary bone collected for histological analysis of the pulpal and the periodontal ligament of the root mesial of the first superior molar. The histological analysis demonstrated that the force of 600cN was not shown executes to promote alterations in the periodontal ligament, pulp and root, on the other hand it demonstrated that a trauma of smaller intensity as the concussion, doesn't promote alterations in the periodontal ligament, and for his time it doesn't represent a problem for the orthodontist.

PROPOSIÇÃO: O objetivo deste trabalho foi descrever as alterações das dimensões dos arcos dentários e das inclinações dos incisivos produzidas pelo Sistema Damon, durante a fase de alinhamento e nivelamento. Outro objetivo foi verificar a correlação destas alterações com o padrão de crescimento dos indivíduos. MATERIAL E MÉTODOS: modelos de estudo, telerradiografias convencionais e telerradiografias reconstruídas a partir de tomografias computadorizadas cone-beam de 22 casos tratados com o bráquetes Damon 3MXTM foram avaliados durante a fase de alinhamento e nivelamento. Os modelos de estudo foram obtidos ao início do tratamento (T0), após a primeira troca de fio (T1) e na inserção do fio de aço 0,019 x 0,025 (T2). Foram verificados os comprimentos de arco e as dimensões transversais nas regiões de caninos, primeiros e segundos pré-molares e primeiros molares mensuradas a partir das pontas de cúspides e margens gengivais linguais dos dentes, a cada etapa. Nas telerradiografias foram verificadas as alterações das inclinações dos incisivos inferiores e superiores, bem como o padrão de crescimento dos indivíduos. RESULTADOS: os comprimentos dos arcos e todas as distâncias transversais apresentaram aumentos significantes, com exceção da distância intercervical dos caninos superiores; houve aumento significante das inclinações vestibulares dos incisivos superiores e inferiores; o padrão de crescimento dos pacientes apresentou correlação significante com os aumentos transversais na região de pré-molares inferiores. CONCLUSÕES: o alinhamento e nivelamento de casos tratados sem extrações com o uso do Sistema Damon resultou no aumento significante das dimensões dos arcos dentários e das inclinações dos incisivos para vestibular. / OBJECTIVE: This study aimed to evaluate the dental arch widths and incisor torque changes produced by the Damon System during leveling and alignment. Other objective was to verify the correlation between these changes and the growth pattern. MATHERIAL AND METHODS: study casts and lateral radiographs derived from cone-beam computed tomographies of 22 subjects treated with Damon 3MXTM brackets were evaluated. The study casts were obtained at the start of treatment (T0), the first archwire change (T1), and the placement of the final 0,019 x 0,025-in archwire (T2). Arch depth and widths (intercanine, interpremolar and intermolar) were assessed at each stage. The incisor torques changes and the growth pattern were measured on the cephalometric records at T0 and T2. RESULTS: There was significant increase of the arch depths and lateral expansion of the arches for all measures, except for the intercervical of maxillary canines. The maxillary and mandibular incisors were significantly tipped labially; the growth pattern showed significant correlation with the lateral increases in lower premolars region. CONCLUSIONS: nonextraction cases treated with Damon System show significant increases in arch dimensions and incisors proclination.

Factores asociados al conocimiento de profesores de educación escolar sobre el manejo inmediato de un evento de traumatismo dentoalveolar, Lima 2018 / Factors associated with knowledge about the management of dentoalveolar trauma in teachers

Objetivo: Determinar los factores asociados al conocimiento sobre el manejo de trauma dentoalveolar en profesores. Materiales y métodos: Se analizaron 347 cuestionarios resueltos por profesores de 6 instituciones educativas de Lima. Se utilizó el instrumento elaborado por Young que estuvo estructurado en 2 partes: características demográficas y conocimiento sobre el manejo de trauma dentoalveolar. Los datos fueron analizados mediante estadística descriptiva; y se utilizó la prueba de Chi cuadrado, prueba exacta de Fisher y Regresión de Poisson con varianza robusta para determinar la asociación entre el conocimiento sobre manejo de trauma dentoalveolar y las variables de estudio. También se calculó las PR crudos y ajustados con IC95%. Resultados: Se determinó que el género, edad, entrenamiento de primeros auxilios generales, información adicional de lesiones dentales, entrenamiento de primeros auxilios sobre lesiones dentales y tipo de colegio fueron catalogados como factores asociados al conocimiento sobre el “manejo de los dientes desplazados”. También, se comprobó que el entrenamiento de primeros auxilios tanto generales como de lesiones dentales fueron factores asociados al conocimiento sobre la “decisión de reimplantar los dientes de leche avulsionados”. Conclusiones: El entrenamiento de primeros auxilios generales y el entrenamiento de primeros auxilios sobre lesiones dentales han demostrado ser factores asociados en el conocimiento sobre el manejo de las diferentes situaciones y/o emergencias de traumatismo dentoalveolar. / Objective: The objective of this study is to determine the factors associated with knowledge about the management of dentoalveolar trauma in teachers Materials and methods: 347 questionnaires were analyzed, resolved by teachers from 6 educational institutions in Lima. The instrument developed for young people was used, which was structured in 2 parts: demographic characteristics and knowledge about the management of dentoalveolar trauma. The data was analyzed by descriptive statistics; and the Chi squared test, the Fisher exact test and the Poisson regression with robust variance to determine the association between knowledge about dentoalveolar trauma management and the study variables. Crude and adjusted prevalence ratios were also calculated with 95% CI. Results: It was determined that gender, age, general first aid training, additional information on dental injuries, first aid training on dental injuries and type of school were cataloged as factors associated with knowledge about the "management of displaced teeth". Also, it was found that both general and dental injury first aid training were factors associated with knowledge about the "decision to reimplant the avulsed milk teeth". Conclusions: General first aid training and first aid training on dental injuries have been shown to be associated factors in the knowledge of the management of different situations and / or emergencies of dentoalveolar trauma. / Tesis

Relationship of epithelial cells and nerve fibres to experimentally induced dentoalveolar ankylosis in the rat.

The current study investigated the distribution of periodontal epithelial cells and nerve fibres within the furcations of rat maxillary molar teeth subjected to hypothermic injury. The upper right first molars of 30 Sprague-Dawley rats were subjected to a single 20 minute application of dry ice in order to produce aseptic necrosis within the periodontal ligament, while the contralateral first molar served as an untreated control. Five animals were each sacrificed via cardiac perfusion after 7, 10, 14, 18, 21 and 28 days respectively and the maxillae were dissected out. After fixation in paraformaldehyde and processing, the tissues were embedded in paraffin wax and cut into 7µm serial coronal sections through the furcation region. Consecutive sections were then stained with H&E, cytokeratin AE1/AE3 and PGP 9.5 immunostains. Light microscopic examination of the H&E stained sections revealed that ankylosis had not developed in all of the experimental teeth, and in some of the observation groups fewer teeth were ankylosed than unaffected. The morphology of the ankylotic areas appeared to change with time, initially consisting of fine bony trabeculae, then progressing to solid bone occupying the entire furcation before becoming less solid again by the latest observation periods. Root resorption was often seen adjacent to areas of ankylosis, but the cementum of the tooth root at the point of ankylotic union was usually intact and free of resorption. Changes within the pulp chambers of the experimental teeth were also noted, with reduction in cellularity and tissue disorganisation initially, then increasing cellularity and formation of a cementum-like material on the chamber walls later. Cytokeratin AE1/AE3 immunostaining successfully identified epithelial cells within the periodontal ligament and their distribution around control teeth was similar to previous reports. Counting of these cells revealed lower numbers around experimental teeth, with the lowest counts around experimental teeth which had developed ankylosis. No change in the epithelial cell counts was detected over time, and these cells did not appear to regenerate after necrosis regardless of whether or not ankylosis developed. Statistical analysis indicated that the probability of ankylosis decreased as the number of epithelial cells increased. The PGP 9.5 immunostain identified periodontal nerve fibres, but the use of this stain was quite technique sensitive. The furcations of the molar teeth were noted to have relatively sparse innervation, with most of the visible nerve fibres being closely associated with blood vessels and located in the outer two-thirds of the ligament. Counting of the nerve fibres revealed fewer fibres around experimental teeth compared to control teeth, especially in the part of the ligament closest to the tooth root. There was no relationship detected between nerve count and time or between nerve and epithelial cell counts. Resorption was found to be more prevalent in experimental teeth, and the probability of resorption in a given tooth decreased as the epithelial cell count increased. The findings of this study suggest that the epithelial cells within the periodontal ligament have a protective function in the prevention of dentoalveolar ankylosis and resorption. Evidence of an intimate interrelationship between periodontal nerve fibre and epithelial cell numbers could not be confirmed. Cone-beam computed tomography (CBCT) has been prescribed for the evaluation of unerupted teeth, and ankylosis is an important characteristic to be observed in the analysis of these teeth. The objective of this research was to establish diagnostic criteria, using pixel intensity in CBCT and associate with clinical and demographic factors. The pixel intensity value was performed by two analyzes. The first (analyze 1) evaluated the mean pixel intensity in different regions of the dento-alveolar complex in normal and ankylosis areas. The second (analyze 2) was observed the variation of pixel intensity values in two normal areas and one of ankylosis. Data were analyzed statistically by means of absolute and relative frequency measurements; odds ratio, Chi-square. The t-test was used to compare the means of pixel intensity. One hundred and fifty-seven CBCT exams of individuals with unerupeted or partially erupted teeth with suspected ankylosis were evaluated, totaling 206 teeth. Fifty-seven teeth (27.6%) had a presumptive diagnosis of ankylosis and were evaluated using pixel intensity value. In analysis 1, it was observed that the values of alveolar bone in the normal and ankylosis areas, practically have the same variability of pixel intensity value (t-test = 0.07). When comparing the density of normal PDL with the ankylosis area, a higher density is observed (t-test p <0.001). When compared to the alveolar bone interface with PDL and alveolar bone with ankylosis area, it is observed that the intensity of the pixels in the ankylosis region is greater than in the normal area (t-test p <0.001). In analysis 2, it was observed that there was a marked reduction of pixel intensity value in the PDL area, which did not occur in the ankylosis areas. Considering the clinical and demographic factors, the occurrence in individuals over 20 years old was 72.5%, and less or equal to 20 years was 27.5% (odds ratio 1.8 95% CI 0.87-3.73), and 77.2% (n = 44) were in the upper arch. The mesioangular inclination was observed in 49.1% (n = 28), and the association between impaction and dental ankylosis was not observed (p = 0.44 Chi-square). The cervical and middle thirds of root were the most affected, 38.6% and 35.1%, respectively. It was possible to observe ankylosis in 22 teeth (38.6%) in three multiplanar reconstructions. In summary, the results suggest that the CBCT examination allows the diagnosis of ankylosis and the analysis of the pixel intensity values and their variability are an important digital tool for the interpretation and diagnosis process. / Anquilose dentária constitui uma desordem, em que ocorre a fusão total ou parcial do cemento ou dentina ao osso alveolar, pela ausência do ligamento periodontal (LPD), podendo evoluir para a reabsorção por substituição. O exame de tomografia computadorizada por feixe cônico (TCFC) tem sido prescrito para a avaliação de dentes não irrompidos, sendo a anquilose uma característica importante a ser observada na análise desses dentes. O objetivo desta pesquisa foi estabelecer critérios de diagnóstico, utilizando intensidade de pixel em TCFC e associar com fatores clínicos e demográficos. A análise de intensidade de pixel foi realizada por meio de duas análises. A primeira (análise 1) avaliou a média de intensidade de pixel em diferentes regiões do complexo dento-alveolar em áreas normais e de anquilose. A segunda (análise 2) observou a variação dos valores de intensidade de pixel em duas áreas normais e uma de anquilose. Os dados foram analisados estatisticamente por meio de medidas de análise de frequência absoluta e relativa; odds ratio, Qui-quadrado. O teste-t foi utilizado para comparação das médias de intensidade de pixel. Cento e cinquenta e sete exames TCFC de indivíduos com dentes não irrompidos ou parcialmente irrompidos e com suspeita de anquilose, foram avaliados, totalizando 206 dentes. Cinquenta e sete dentes (27,6%) possuíam diagnóstico presuntivo de anquilose, e foram avaliados por meio da intensidade de pixel. Na análise 1, observou-se que os valores de osso alveolar na área normal e de anquilose, possuem praticamente a mesma variabilidade de intensidade de cinza (Teste-t p=0,07). Quando comparado a densidade do LPD normal com a área de anquilose, observa-se uma maior densidade (Teste-t p<0,001). Quando comparado a interface osso alveolar com LPD e osso alveolar com área de anquilose, observase que a intensidade dos pixels na região da anquilose é maior do que da área normal (Teste-t p<0,001). Na análise 2, observou-se que havia uma redução acentuada da intensidade de pixel na área do LPD, o que não ocorreu nas áreas de anquilose. Considerando os fatores clínicos e demográficos, a ocorrência em pacientes acima de 20 anos foi de 72,5%, e menor ou igual a 20 anos foi de 27,5% (Odds ratio 1,8 IC95% 0,87-3,73), sendo que 77,2% (n=44) foi localizada na arcada superior. A inclinação mesioangular foi observada em 49,1% (n=28), e não foi constatada a associação entre impacção e anquilose dentária (p=0,44 Quiquadrado). Os terços cervical e médio radiculares foram os mais acometidos, 38,6% e 35,1%, respectivamente. Foi possível observar anquilose em 22 dentes (38,6%), em três reconstruções multiplanares. Em síntese, os resultados sugerem que o exame de TCFC permite o diagnóstico de anquilose e a análise dos valores de pixel e sua variabilidade, constituem uma ferramenta auxiliar importante para o processo de interpretação e diagnóstico.
