Orientador: João Guilherme Padilha Filho / Banca: Júlio Carlos Canola / Banca: Antonio Carlos Alessi / Banca: Andréa Pacheco Batista Borges / Banca: Duvaldo Eurides / Resumo: Realizou-se acetabuloplastia extracapsular com xenoenxerto cartilaginoso auricular conservado em glicerina, fixado com dois parafusos da série 304L, para tratar 13 articulações de oito cães que apresentavam sinais clínicos e radiográficos de displasia coxofemoral de grau moderado a grave. Os animais passaram por avaliação clínica, ortopédica e radiográfica. Antes e 60 dias após a cirurgia, os proprietários responderam questionário sobre a habilidade de seus animais realizarem atividades rotineiras e como o procedimento influenciou a qualidade de vida deles. Paralelamente, realizou-se a mesma técnica em ambas as articulações de dois cães sadios a fim de verificar o comportamento macro e microscópico do enxerto e dos parafusos. No dia seguinte à cirurgia, os cães displásicos já demonstravam melhora na marcha, que continuou a evoluir durante o período de avaliação. O sinal de Ortolani estava presente nas 13 articulações antes do procedimento, em 50% delas aos 60 dias e ausente em todas aos 120 dias. Aos 60 dias, todos os cães realizavam as atividades cotidianas de maneira normal ou próxima do normal; observou-se, radiograficamente, a presença de prolongamento ósseo na borda craniodorsal do acetábulo em duas articulações. Nos exames macro e microscópico dos cães sadios, pode-se perceber que os implantes metálicos despertaram reação inflamatória de baixa intensidade e de curta duração. Já o enxerto não suscitou reação inflamatória, incorporou-se ao coxal e foi envolvido por tecido conectivo. Os achados permitiram concluir que os implantes metálicos são biocompatíveis e o enxerto se incorpora aos tecidos do receptor. Além disso, a acetabuloplastia, realizada com tal material, elimina a sintomatologia dolorosa da displasia, proporciona estabilidade articular e, em alguns casos, prolonga a borda craniodorsal do acetábulo. / Abstract: Extracapsular acetabuloplasty using auricular cartilage xenograft stored in glycerin, fixed with two 304L-series screws, was carried out in order to treat 13 articulations of eight dogs that presented clinical and radiographical signs from moderate to severe hip dysplasia. The animals were submitted to clinical, orthopedic and radiographic evaluation. Prior to and 60 days after acetabuloplasty, the animals' owners answered a questionnaire about their dogs' ability to perform routine activities, as well as how the surgery affected their life quality. At the same time, a similar technique was used in both articulations of two healthy dogs so as to verify, both macroscopically and microscopically, the behavior of the graft and the screws. The next day this procedure was applied, the dysplasic animals showed gait improvement, which continued to evolve during the assessment period. Before the surgical procedure was performed, the Ortolani sign was present in the 13 articulations; at 60 days, in 50% of the articulations, and at 120 days it was absent in all of them. At 60 days, all dogs were able to perform daily activities normally or nearly normally, and, an extension at the craniodorsal acetabular rim was observed in two articulations. The macroscopic and microscopic examinations of the healthy dogs showed that the metallic implants had caused inflammatory reaction of low intensity and for a short period of time. Such reaction did not occur in relation to the graft, which was integrated into the coxal articulation, and was involved by connective tissue. These findings lead to the conclusion that metallic implants are biocompatible, and the graft is incorporated into the receptor's tissues. Moreover, acetabuloplasty using this particular material relieves painful dysplasia symptomatology, stabilizes articulation and, in some cases, extends the craniodorsal rim of the acetabulum. / Doutor

Avaliação clínica e radiográfica da cobertura acetabular à cabeça femoral, após aplicação de cunha sacroilíaca em cães /

Para tal, foram utilizados oito cães, adultos, com peso entre 16,5 a 32 kg, quatro normais e quatro displásicos, porém sem sintomatologia dolorosa. Na junção sacroilíaca esquerda foi aplicada cunha de 30°. Foram realizadas avaliações clínicas pós-operatórias, perimetria da coxa, avaliação do apoio do membro do lado operado, aferição dos ângulos de flexão e extensão. Para avaliação da ventroversão acetabular foram empregados os métodos radiográficos do ângulo de Norberg e a porcentagem de cobertura acetabular. Os animais foram avaliados no pré-operatório, pós-operatório imediato, 30, 60 e 90 dias. Para a avaliação clínica, perimetria da coxa, avaliação da claudicação e apoio do membro, flexão e extensão, também se incluiu os períodos de sete, 14 e 21 dias. Observou-se na avaliação radiográfica aumento da cobertura acetabular logo no pós-operatório imediato, tanto no método do ângulo de Norberg quanto na porcentagem de cobertura acetabular. Não houve interferência na deambulação dos animais, uma vez que todos retornam a deambulação normal ao término do período de avaliação. Do ponto de vista clínico, a cunha comportou-se como biopolímero, suportando a aplicação de parafuso em efeito compressivo. A técnica promove aumento da cobertura acetabular, comprovado radiograficamente, podendo ser empregada em cães displásicos / Abstract: The goal of this study was to apply castor oil porous polymer wedges with addition of calcium in the sacroiliac joint; to evaluate clinically the interference of the technique in the walking of dogs and, radiographically, to check the increasing of the acetabular covering to the femoral head. In order to accomplish it, eight adult dogs weighting 16.5 - 32 kg were used, four of them normal and four of them dysplastic, although without painful symptomatology. In the left sacroiliac joint was applied 30 wedge. Postoperative clinical evaluations, thigh perimetry, evaluation of the support of the operated side member, gauging of the flection and extension angles were carried out. To evaluate the acetabular ventroversion the Norberg angle radiographic methods and the percentage of acetabular covering were used. The animals were evaluated in the preoperative, immediate postoperative, 30, 60 and 90 days. To the clinical evaluation, thigh perimetry, claudication evaluation and member support, flection and extension, they were also included the periods of seven, fourteen and twenty-one days. It was observed in the radiographic evaluation an increasing of the acetabular covering soon in the immediate postoperative, in both the Norberg angle method and the percentage of acetabular covering. There wasn't interference in the walking of the animals, considering that all of them recover the regular walking at the end of the evaluation period. From the clinical point of view, the wedge behaved as biopolymer, bearing the screw application in compression effect. Dessa forma, este estudo determinou, em cães, os efeitos da anestesia com infusão contínua de propofol em diferentes doses, associada ou não ao N20, sobre variáveis intracranianas, cardiovasculares e índice biespectral (BIS). Foram utilizados 32 cães adultos distribuídos em quatro grupos de oito animais, denominados grupo propofol decrescente + N20 (GPDO, n=8), grupo propofol decrescente (GPD, n=8), grupo propofol crescente + N20 (GPCO, n=8) e grupo propofol crescente (GPC, n=8). Os animais foram induzidos à anestesia pela administração intravenosa de propofol (10mg/kg). Após intubação orotraqueal, o GPDO recebeu N20 a 70% e 02 a 30%. Em seguida, iniciou-se a infusão contínua de propofol na dose de 0,8mg/kg/min. A dose do anestésico foi reduzida para 0,6mg/kg/min e então para O,4mg/kg/min, respeitando um intervalo de 50 minutos entre cada uma. O mesmo protocolo foi adotado para o GPD, substituindo-se o fluxo diluente por 02 a 100%. Para GPCO e GPC, iniciou-se a infusão de propofol na dose de O,4mg/kg/min. A dose do anestésico foi acrescida para O,6mg/kg/min e depois para 0,8mg/kg/min, respeitando um intervalo de 50 minutos entre cada uma, sendo que o fluxo diluente no GPCO foi de N20 a 70% e 02 a 30%, e o GPC recebeu 02 a 100%. Os dados foram submetidos à análise de variância de uma via seguida pelo teste de Tukey (p<0,05) para detectar diferenças entre os grupos. Para comparação dos momentos ao longo do tempo utilizou-se a análise de variância de uma via para medições repetidas seguida pelo teste de Tukey (p<O,05)... The aim of this study was to evaluate intracranial and cardiovascular variables, and also bispectral index (BIS) in dogs submitted to continuous infusion rates of propofol combined with nitrous oxide (N20). Thirty two adult dogs were assigned into four groups composed by eight animais: group propofol decreasing + N20 (GPDO, n=8); group propofol decreasing (GPD, n=8); group propofol increasing + N20 (GPCO, n=8); and group propofol increasing (GPC, n=8). Anesthesia was induced with propofol administered intravenously (10mg/kg). After orotracheal intubation, GPDO received 70% N20 and 30% 02. Subsequently, continuous infusion of propofol was begun using a primary dose of 0.8mg/kg/min. The anesthetic dose was decreased to 0.6mg/kg/min, and then to O.4mg/kg/min, respecting an interval of 50 minutes between them. The GPD was submitted to the same protocol; however, the diluent flow was substituted by 100% 02. For GPCO and GPC, continuous infusion of propofol was begun using a primary dose of 0.4mg/kg/min. The anesthetic dose was increased to 0.6mg/kg/min, and then to 0.8mg/kg/min, respecting an interval of 50 minutes between them. The GPCO received 70% N20 and 30% O2 and GPC 100% O2. The data were submitted to One way Anova following Tukey test (p<0.05) to detect differences between groups. For comparisons between moments, one way RM Anova followed by Tukey test (p<0.05) was used. Cerebral perfusion pressure (CPP), BIS, heart rate (HR), systolic (SAP) diastolic (DAP), and mean arterial (MAP) pressures, systemic vascular resistance (SVR) and index (ISVR) diminished with the highest infusion rate in ali groups.
