The Experience of Asian Americans Caring for Elderly Parents

This qualitative study sought to examine the experience of Asian Americans who provide housing and financial support for first-generation biological Asian parents aged 65 and older. Semi-structured interviews regarding how participants came to take care of their parents, the impact it had on close relationships and participants' plans, the impact of cultural background on taking care of their parents, and the positive aspects of caring for their parents were conducted with eight second-generation adult Asians in the U.S. The data were analyzed using thematic analysis and themes were organized around the areas of inquiry. Participants spoke about fulfilling caregiving responsibilities out of love and obligation; the positive and negative impact of caregiving on relationships with parents, siblings, and significant others; the challenges associated with their own decreased independence and the difficulty of seeing parents age; the benefits of the instrumental support that they received from parents and closer grandparent-grandchild relationships; impact on financial and housing plans; and the expectation of non-financial care from their children. Despite living in an individualistic society, participants appeared to endorse values of filial piety by taking care of their parents. Further, the participants' hopes that their children would continue taking care of elderly family members in non-financial ways in the future suggests that while they maintain the cultural value of filial piety, it is being adapted to the reality of living in an individualistic society. Limitations, clinical implications, and directions for future research are discussed. / Master of Science

Cuidado da família ao idoso portador de doença crônica: análise do conceito na perspectiva da família / Family care to the elderly with chronic disease: concept analysis from the family\'s perspective.

O envelhecimento populacional vem sendo alvo de atenção no Brasil, uma vez que está ocorrendo um aumento significativo do número de idosos na população. Observa-se que juntamente com essa transição demográfica, ocorre a transição epidemiológica. A preocupação com doenças infecto-contagiosas perde lugar para a alta prevalência das doenças crônicas não-transmissíveis, e a população idosa é a mais acometida por essas doenças. Nesta situação, a família desempenha um papel fundamental na manutenção e controle da doença de seus membros, pois cabe a ela arcar com a continuidade do cuidado ao indivíduo no seu processo de viver. Assim, o estudo tem como objetivo construir um modelo conceitual de cuidado familiar ao idoso com condição crônica, com base nos significados construídos nas interações sociais, identificando, deste modo, como a família vivencia o processo de cuidar do idoso com condição crônica e interpretando os significados do cuidado familiar à este idoso. Utilizou-se como referencial metodológico a Teoria Fundamentada nos Dados e o referencial teórico do Interacionismo Simbólico. Participaram da pesquisa oito famílias de idosos com doença crônica não-transmissível. As famílias foram selecionadas a partir da Equipe de Saúde da Família 15 do município de Maringá-PR, sendo as mesmas acompanhadas no período de agosto de 2005 a maio de 2006. Para o desenvolvimento da pesquisa, utilizou-se como técnica de coleta de dados a observação participante, entrevistas e análise de documentos, realizadas nos domicílios das famílias. A participação em diferentes momentos da vida das famílias e seus membros idosos, bem como os depoimentos permitiram compreender a vivência do cuidado ao idoso com doença crônica, expressa através de conceitos desenvolvidos em três processos: A identificação da doença crônica do idoso, A convivência com a cronicidade da doença do idoso e Os serviços de saúde na vida das famílias. O primeiro processo é constituído por quatro categorias analíticas, sendo estas: A vivência das dificuldades na fase aguda da doença; O processo de adaptação; O reconhecimento da cronicidade da doença; e A perspectiva do futuro incerto. O segundo processo também é composto por quatro categorias: A presença do idoso independente na família; A vivência do cuidado ao idoso dependente; Estratégias utilizadas no cuidado do idoso; e Perceber-se vulnerável. O terceiro processo é formado por uma categoria analítica: O serviço público de saúde no cuidado ao idoso. A organização desse modelo teórico revelou que, na construção de significados sobre o viver e envelhecer com a presença da doença crônica, a capacidade funcional do idoso é um fator significativo e determinante no processo de cuidar e ser cuidado. / The population aging has turned into the target of attention in Brazil, since a significant increase of the number of the senior citizens is going on. It is observed that while the demographic transition happens, there is also the epidemiological transition. The concern with contagious diseases loses its position to the high prevalence of non-transmissible chronic diseases, and the elderly population is the most affected by those diseases. In this situation, the family has a fundamental role in the maintenance and control of the disease of its members, for it is the family duty to continue caring for the individual in the living process. Therefore, the present study aims to build a conceptual model of the care of the family towards the aged person in chronic condition, based on the meanings built in the social interactions, identifying, this way, how the family lives the process of caring for the elderly with chronic condition and interpreting the meanings of care of the family towards the elder. As a methodological reference the Grounded Theory and the theoretical reference of the Symbolic Interactionism were used. Eight families of senior people with non-transmissible chronic disease participated of the research. The families were selected through the Family Health Team 15 in Maringá, and they were followed from August 2005 through May 2006. For the evolution of the investigation, participant observation, interviews and documents analysis in the families\' residences were used as data gathering. The participation in different moments of the families and their seniors\' life, and also their testimonies permitted to understand the experience of caring for the elder with chronic disease, expressed through developed concepts in three processes: Identifying the chronic disease, Living with the chronicity of the disease and the Health services in the families\' life. The first process is constituted by four analytical categories. They are: Living the difficulties in the acute phase of the disease; The adjustment process; The recognition of the chronicity of the disease; and The perspective of an uncertain future. The second process is also formed by four categories: The presence of the independent aged in the family; The living of the care towards the dependent aged; Strategies used on the caring of the elder; and Perceiving oneself as vulnerable. The third process is formed by an analytical category: The health public service towards the caring for the elder. The organization of this theoretical model revealed that, in the construction of meanings about living and aging with the presence of a chronic disease, the capacity of the elder is a significant factor and it is determinant in the caring and being cared process.

Assim, esta pesquisa divide-se em Estudo Transversal e Estudo Longitudinal. O objetivo do Estudo Transversal consistiu em traçar o perfil do nível de atividade física, sintomas depressivos e queixas do sono de idosos que frequentam Centros de Convivência, na cidade de Campina Grande - PB. E o objetivo do Estudo Longitudinal analisou os efeitos de um Programa de Atividade Física Generalizada na redução de sintomas depressivos e queixas de sono, bem como, a melhora da capacidade funcional dos idosos participantes. Para o Estudo Transversal, a amostra foi composta de 83 idosos de ambos os sexos nos quais foram aplicados o Questionário de Baecke Modificado para Idoso (QBMI), o Mini - Questionário de Sono (MQS), e a Escala Geriátrica de Depressão (GDS). No Estudo Longitudinal, a amostra foi composta de 29 idosos - oriundos do Estudo Transversal e que compuseram o grupo de Intervenção Motora. Esta teve duração de quatro meses, com três sessões semanais. Nestes idosos foram aplicados os mesmos testes do Estudo Transversal, acrescidos da bateria de testes da American Alliance for Health Physical Education Recreation and Dance (AAHPERD). The aim of this study is to examine how supervisors at a Jobbcentrum works with the elder unemployed and what difficulties and challenges there may be in being able to guide elder unemployed into employment. This study is based on a qualitative method with semi-structured interviews with six different supervisors at Jobbcentrum. The theories used in the study are Empowerment and Self-Efficacy. At the same time, concepts such as age discrimination and Motivational conversations were used, to have a greater understanding of the work of supervisors. The results of the study show that Motivational interviewing, Empowerment and Self-Efficacy are an important starting point in the work with the unemployed. Another important result highlighted by the participants in the study is the difficulties that the elder unemployed encounters in the form of discrimination based on age, as well as obstacles in the form of not being considered to have the language skills or the educational background that employers seek.

Nurses' recognition and identification of elder abuse by caregivers.

The purposes of this secondary study were to explore the case detection phenomena of elder abuse by determining the congruence between nurses' assessments of abuse and elders' self-reports of abuse; to identify factors that may account for differences between abusive situations and nonabusive situations; then to describe differences between abused elders correctly identified and abused elders incorrectly identified by nurses. Both quantitative and qualitative data were used. The theory of attribution directed this research. The conceptual framework consisted of four concepts: structural factors, relationship factors, elder factors, and caregiver factors. A descriptive-comparison design was used to address the research questions. The sample included 48 elder-caregiver dyads, of whom 24 were self-reported abused elders and 24 self-reported nonabused elders. Descriptive analysis was used, including chi-square and t-tests. Results indicated that the nurses' assessments of elder abuse and elders' self reports of abuse were congruent in only one-fifth (N = 5) of the abused cases (N = 24). The findings confirmed allegations that nurses have difficulty identifying elder abuse unless outright battering is observed. Five variables were significant between abused and nonabused elders, and 10 variables were significant between abused elders correctly identified by nurses and abused elders incorrectly identified by nurses.

In this series of essays about professional burnout, a veteran teacher seeks a way to continue her work and enthusiasm in it, for the sake of both her and her students. To that end, she explores her relationships with her father and mother, and how the practices of teaching and learning she brought from home have affected her present classroom experiences. A complicating factor is the presence of chronic illness and its demands both primary and secondary: her father's Alzheimer's, her mother's bi-polar disorder, and the demands of eldercare for her mother. She also explores her own habitual practice of being a student, in a reflective inquiry into the mind and situation of students from inside her own experiences. Interleaved vignettes of student interaction illustrate the kinds of difficulty that the speaker has with her teaching. They appear chronologically to suggest a developmental movement.

Introdução - Atualmente não se sabe quantos e de que forma idosos residem em instituições de longa permanência em nosso país. O poder público considera estas instituições como instrumentos de atenção social e não de saúde e, assim, não há obrigatoriedade da manutenção de profissionais da área da saúde nestes locais. Entretanto, através da atuação profissional, em visitas a, aproximadamente, quatrocentas instituições de longa permanência para idosos, sempre foi encontrado um profissional auxiliar de enfermagem prestando cuidados aos idosos residentes. Objetivo Conhecer o significado atribuído por auxiliares de enfermagem que trabalham em uma instituição de longa permanência ao termo cuidados à saúde e sua relação com as atividades por eles desenvolvidas. Método Pesquisa qualitativa, de caráter exploratório. A coleta de dados se deu por meio de grupo focal, cujos temas para discussão se basearam em três questões sobre as atividades desenvolvidas e a definição de cuidados à saúde. Também foi utilizado como instrumento de coleta de dados, um questionário estruturado, contendo oito questões relativas à caracterização da população. A população foi constituída por treze auxiliares de enfermagem que prestam serviço em uma instituição de longa permanência para idosos. Os dados foram analisados qualitativamente utilizando-se a Análise de Conteúdo. Resultados Os sujeitos conceituam cuidados à saúde, bem como descrevem os cuidados à saúde prestados como a realização de procedimentos técnicos direcionados à satisfação das atividades básicas de via diária. Um número pequeno de profissionais citou atividades de apoio emocional e de lazer como sendo cuidados à saúde. Conclusão Os auxiliares de enfermagem conceituam cuidados à saúde a partir de sua experiência de formação e de trabalho, centrada na realização de procedimentos técnicos. Cuidados psicossociais são referidos por poucos profissionais, mormente os mais velhos e com mais tempo de trabalho na instituição. / At the present time there is no information about how many people live in long term care permanence institutions are in Brazil, nor how they live in these institutions. Brazilian government considers theses institutions as intruments of social attention, not of health responsibility and, thus, there is no obligation of maintaining health professionals in these facilities. However, through professional service, during visits to, aproximately, four hundred long permanence institutions for senior citizens, there was always a licensed professional nurse assisting the elderly residents. Objectives- - Identify the meaning of the term health care for the licensed nurses who work at an elderly long term care institution and what health cares are given by them at the institution Method Qualitative research, with exploratory mather. The collection of data was presented by focal groups, in which the themes for discussion were based on three questions about the activities developed and the definition of health care. A structured questionaire was also used as a data collection instrument, containing eight questions related to population identification. The population was constituted by three Lincese Professional Nurses who work at a long permanence institution for senior citizens. The data were analyzed qualitatively using the content analysis Results The subjects have the concept that health care, such as they describe the health care offered, as the completion of technical procedures directed to the satisfaction of the basic daily life activities. A small number of professional cited activities as emotional support and leisure as health care. Conclusion The nurse aid appraises cares of health from its experience of formation and work, centered in the accomplishment of procedures technician. Psicossociais cares are related by the few professionals, mainly oldest and with more time of work in the institution.
