The unrecognized sexuality : A qualitative interview study on staff's views on the sexuality of the elderly in residential care

Elderly sexuality in residential care is a subject ignored in both society and in research. This qualitative study investigates staff’s beliefs and experiences of sexuality in residential care. The research was conducted in Sweden, using semi-structured in-depth interviews with staff in residential care. Thirteen caregivers participated in this study. The study embraces the concept of constructionism as its analytical framework. The following are the key findings: (1) The participants noted the presence of sexuality among elderly people; (2) Staff’s interpretation of sexual expressions varied; depending on where, to whom and how the sexual expressions were articulated; (3) The institutional barriers were: lack of privacy, overall institutional limitationsin knowledge and guidelines, and caregivers understanding about sexuality among elderly; (4) The participants also identified sexual harassment at their workplace by the elderly; (5) Caregivers emphasized Dementia as a key factor disrupting sexual boundaries. In conclusion this study shows that residential care institutions do not recognize the need of sexuality among elderly people. As a consequence of elderly´s sexual needs being unrecognized their sexual needs is not taken account for. This also legitimize lack of guidelines and the staff being put in situations that they are not properly equipped to handle.

The relationship between schedule influence, schedule satisfaction and work-related outcomes within Swedish elder care

The thesis aimed to investigate the relationship between influence over scheduling and schedule satisfaction with three central factors within the psychosocial work environment within the elder care sector: quality of work, work-life conflict and intention to quit. The JD-R model was used as theoretical approach as it provides a framework of the relationship between different work factors, characteristics, health and wellbeing, as well as a context to the different factors in the scope of the thesis. A survey was distributed among workers within the elder care sector in collaboration with a union (n=1202) and statistical analyses were made, establishing correlations between the different variables with the use of Pearson Correlation Coefficient as well as multiple linear regression analyses. The results revealed that higher degrees of influence over scheduling were associated with higher degrees of schedule satisfaction, and both variables were positively related with quality of work and negatively related with work-life conflict and intention to quit. To conclude, relatively strong relationships were found between the factors indicating that influence on scheduling and specifically schedule satisfaction are important factors associated with quality of work, work-life conflict and intention to quit within Swedish elder care.

Background: There is a burgeoning population of older adults globally and there is an increasing urgency in the policy literature to understand the health issues facing this population. A social exclusion lens provides an opportunity to understand health inequity and disadvantage among vulnerable populations. There is limited research examining how social exclusion plays out for older women, particularly in the rural context. This study examines the social exclusion experiences of Canadian, rural, older women and highlights policy implications for global health practitioners. Methods: Guided by the principles of interpretive description, this qualitative study included eight participants who identified as women, 65 years or older, spoke English, and who lived alone in private households in rural communities of Durham Region, Ontario. Concurrent data collection and analysis was conducted using an inductive approach. One-to-one, semi-structured interviews were completed. Findings: Six themes illuminate the exclusion experiences of rural, older women and they include: “Expectations of ageing in rural communities”; “Navigating the tensions of belonging within the social fabric”; “Singlehood isolation”; “Driving independence”; “Health infrastructure and changing personal health”; “Affording ageing”. Implications & Contributions: The study adds a Canadian, rural perspective to the global conceptual literature on social exclusion. It illuminates the unique ways in which female gender and rural context influence social exclusion experiences. Implications for global health policy include: recognizing health as a key category of exclusion and the need to address health equity, adopt a gender-specific and life-course approach to address social exclusion, foster a sense of belonging and peer connection, utilize unconventional channels for engagement, implement social support schemes and health programming which considers non-traditional families or single status as norm for family composition, and support affordable access to health-promoting programs and services. / Thesis / Master of Science (MSc) / This study examines how older (senior) women, living in rural communities in Durham Region, Ontario experience social exclusion, and aims to help inform global health policies for older adults. Este estudo objetivou fazer uma revisão bibliográfica sobre o tema da violência contra o idoso no período de 2005 a 2010. As bases de dados consultadas foram: LILACS, SciELO, MEDLINE, ISI Web of Knowledge e Academic Search Premier. Um total de 1098 artigos foi encontrado. Após a exclusão dos documentos que não eram artigos, que não estavam disponíveis na íntegra, materiais repetidos e que não abordavam diretamente à temática, restaram 211 artigos que foram analisados de acordo com o tipo de estudo, o país de origem e as subtemáticas mais estudadas dentro do grande tema da violência contra o idoso. Houve um predomínio de estudos quantitativos e, entre os países, Estados Unidos foi o país que mais publicou. As subtemáticas mais estudadas foram agrupadas nas seguintes categorias: 1) estudos de prevalência; 2) fatores de risco e proteção para o abuso; 3) diagnóstico e validação de instrumentos; 4) percepções sobre a violência contra o idoso; 5) conseqüências do abuso; e 6) prevenção e intervenção em situações de abuso. / Violence against the elderly is a topic of relevance to public health, largely due to the 60 years and older population growth. This study aimed to make a bibliographic review on the subject of elder abuse in the period 2005 to 2010. The databases consulted were: LILACS, SciELO, MEDLINE, ISI Web of Knowledge and Academic Search Premier. A total of 1098 articles were found. After excluding documents that were not articles, were not available in full text, repeated material and material that did not addressed directly the issue, remaining 211 articles were analyzed according to the type of study, country of origin and the most studied sub themes within the major topic of violence against the elderly. There was a predominance of quantitative studies and, between countries, the United States was the country that most published. The most studied sub themes were grouped into the following categories: 1) prevalence studies; 2) risk and protection factors for abuse; 3) instruments’ diagnosis and validation; 4) perceptions about elder abuse; 5) consequences of abuse; and 6) prevention and Intervention in situations of abuse.

On the elder long-term care system

The purpose of this study is to find existing circumstances in our country and a elder long-term care system of preventing transitions. Through the existing social insurance, for example: health insurance, national pension, labor insurance and the elderly welfare legal in our country, to compare with the other social countries, I hope to use the research analysis to find the problems of policies or legislated process that we need to prevent before the elder long-term care insurance started. Through the generalize analysis and history development of elder long-term care in many countries, use the Constitution and the Administrative Law to examine what Council of Grand Justices about Social Insurance interpretation and compare with the official policy offered by our government. I expect to avoid making mistakes and dispute like before and establishing the elder long-term care system which relieving burdens. After the analysis, I found that all of the advanced countries are almost confronting by problems like aging of population and the birthrate has been decreasing, and also confronting lack of care members and long-term care needed huge monetary payment issues. The key core of all the problems is whether it has enough money to the whole social welfare countries to be successful. Social welfare in democratic countries also face election activities carrying on social welfare politics. Ignoring national finance situations and majority political men were merely thinking off-the-shelf votes. It is priority for elder¡¦s policy but ignores the generation justice issues. Let me worry about whether descendant whom need care, not these elders, are there generations conflict being happened? In my opinion, to solve these problems is strengthening family function. If the whole social and nation wants to be stable, it is important to strengthen the family function. Therefore, the elder long-term care should be considered main family basis, in addition to ought to maintain the elder long-term care system and dualism and co-operate with National Health Insurance to work in coordination. The other elder social insurances have to adjust to unity, includes all kinds of old-age pensions similar nouns. Finally, it should be a definite principle and laws, and decrease indefinite concepts of law and reduce administrative discretion rights regarding pay items, thus it will protect people¡¦s rights instead of incurring damage beyond that could bring supervisory mechanism functions into full play after that.
