Health-care workers caring for children : The Early Childhood Development Program in La Paz, Bolivia

ABSTRACT Background: Bolivia has a high child mortality rate and many children´s living conditions are inadequate. In order to reach the Millennium Development Goals non-governmental organizations play an important role when the governmental efforts are insufficient. ChildFund Bolivia works with the Early Childhood Development [ECD] program in order to reinforce the development, growth and health for children under the age of eight. Aim: The aim of this study was to illuminate factors that affect the care given by health workers working with the ECD-program in the urban area of La Paz, Bolivia. Method: An empirical study with qualitative method was performed, using participant observations with an ethnographic approach. The data was analyzed through inductive content analysis where different themes were developed. Results: The results are presented in five themes, Creativity, Different needs, Environment, Continuity and Love and passion. All five themes showed sign of significant health promotion strategies that conform to Nola Pender´s Health Promotion Model. Conclusion: This study illuminates how ChildFund’s health workers in La Paz care for children through the globally used ECD-Program. This contributes to the shared global objective to reach the Millennium Development goals by enhancing children´s opportunity to grow and develop.  Health workers cared for the children with multiple strategies within the common method of the ECD-program. Implications for clinical practice: The outcome of this study might give people better knowledge about similarities and areas of diversity in pediatric nursing care worldwide, hence enjoy better knowledge in global nursing. / SAMMANFATTNING Bakgrund: Barnadödligheten i Bolivia är hög och många barns levnadsförhållanden är otillfredsställande.  För att nå Millenium målen spelar icke-statliga organisationer en viktig roll då de statliga insatserna är otillräckliga. ChildFund Bolivia arbetar med Early Childhood Development-programmet för att införliva utveckling, tillväxt och hälsa för barn under åtta års ålders. Syfte: Syftet med studien var att belysa faktorer som påverkar hälsoarbetares omvårdnad då de arbetar med ECD-programmet i det urbana La Paz, Bolivia. Metod: En empirisk studie med kvalitativ metod har gjorts, baserad på deltagande observationer med en etnografisk ansats. Informationen analyserades med en induktiv innehållsanalys där olika teman arbetades fram. Resultat: Resultatet presenteras i fem teman: Kreativitet, Olika Behov, Miljö, Kontinuitet och Passion och Kärlek. Samtliga fem teman visade prov på betydande hälsofrämjande strategier som överensstämmer med Nola Penders hälsofrämjande model. Slutsats: Den här studien belyser hur hälsoarbetare hos ChildFund i La Paz ger omvårdnad till barn genom det globalt använda ECD-programmet. Detta bidrar till det gemensamma globala målet att nå Millenniemålen genom att förbättra barns möjlighet att växa och utvecklas. Hälsoarbetare använde ett flertal olika strategier för att ge barnen omvårdnad, samtliga giltiga för ECD-programmets metod. Klinisk betydelse: Studiens resultat skulle kunna ge sjuksköterskor bättre kunskap om likheter och olikheter inom pediatrisk omvårdnad världen över, och därmed även ökad kunskap om global omvårdnad.

Lived Histories and the Changing Rhetoric of White Identity

Through open-ended interviews and oral history, this ethnographic project captures unique histories of cultivating critical race consciousness as a White subject in social contexts of continuing overt and covert racisms. The project studies the legacy of racist and prejudiced discourses in how White research participants embody, theorize, and perform White consciousness. I explore a spectrum of White consciousness that corresponds to shifting conceptualizations of racism (Jim Crow, Colorblind, and Critical Race Consciousness), unstable ideologies of activism and antiracism (reflecting whether or not and how subjects act against prejudice), and the changing politics of rhetorical practice in backstage settings (that is, how subjects represent and construct racialized realities in these discourse situations). The project concludes that storytelling can be strategically and effectively used in activist research and everyday conversation as a vehicle for positive social change to cultivate critical dialogue about and rearticulate lived histories of race, racialized identities, racial privileges, and racisms.

Negotiating the Margins: Aging, Women and Homelessness in Ottawa

As the population ages and income disparities increase, issues affecting older adults and marginalized individuals are examined more frequently. Despite this, little attention is paid to the community experiences of women over the age of fifty who face marginalization, criminalization and homelessness. This study is an institutional ethnography of older marginalized women in Ottawa, focusing on their identities, lives and their experiences of community life. Its findings are based on ethnographic fieldwork as well as interviews with 27 older marginalized women and 16 professionals working with this group. The women described their identities, social networks, daily activities and navigations of their communities as well as the policy and discursive framework in which their lives are situated. Regardless of whether the women had housing or were staying in shelters, upheaval, uncertainty and change characterized their experiences in the community, reflecting their current circumstances, but also their life courses. Their accounts also revealed how, through social support, community services, and personal resilience, older marginalized women negotiate daily life and find places and spaces for themselves in their communities. As an institutional ethnography, this research foregrounds participants’ responses, framing these with theoretical lenses examining mobilities, identity, social capital, governmentality, and stigma. Specifically, it uses the lenses of mobilities and identities to understand the nature of their community experiences, before moving outward to examine their social networks and the world around them. Governmentality theory is also used to describe the neoliberal context framing their community experiences. The study concludes with a reflection on the research and a set of policy recommendations arising from the study.

The Social Impacts of Street-involved Youths’ Participation in Structured and Unstructured Leisure

Little research has focused on street-involved youths’ social relationships. As some scholars have suggested that leisure is inherently social, my research sought to understand whether participation in structured and/or unstructured leisure activities influence street-involved youths’ social relationships with other street-involved youths as well with members of the mainstream community. Written in the publishable paper format, this thesis is comprised of two papers, both of which utilize Foucauldian theory. In the first paper, I examine the impacts of street-involved youths’ participation in Health Matters, a leisure program for street-involved youths in Ottawa, Canada. In the second paper, I examine street involved youths’ unstructured leisure activities (e.g., leisure in non-programmed settings) and their subsequent social impacts. Based on my findings, I argue that street-involved youths use both structured and unstructured leisure to form crucial social connections to make their lives more bearable.

Young people negotiating embodied subjectivities through (dis)engagement in physical education

The purpose of this study was to engage with a group of students from a diverse school environment about how they construct value or status in their own and others bodies in physical education (PE) and sport. This study was premised on the notion that young people s constructions of bodies that have value affect both their sense of self and their (dis)engagement with physical activity in and out of school. Sport, physical activity and education are not value-free in their purpose or practices, and constitute arenas in which young people learn about what those values are and how they apply to their own bodies. Learning more about how young people make embodied decisions to engage in physical activity can aid in understanding how best to create inclusive, positive experiences within PE and youth sport. The feminist / poststructuralist theoretical framework that this research draws upon focuses attention on the constructions of embodied subjectivities through an individual s subject positions amongst multiple discourses. These discourses are (re)produced but shift as individuals take up and negotiate positions through the multiple narratives available to them. By linking these notions to that of physical capital, this study explores how individuals practices affect how they might be seen as valued. This study pays particular attention to gendered and racialised constructions of bodies in PE and sport, as literature identifies concerns about equity in participation and representation. Data were generated over one school year with a cohort of students in Year 9 of an ethnically diverse secondary school in the East Midlands, UK. Fourteen boys and eleven girls volunteered to take part in a collaborative visual ethnographic project consisting of a fortnight s photo diary and the sharing of participant-produced images in group interviews. Taped group interviews, participants photographs, field notes from observations of the participants PE lessons and researcher s photographs of the school notice boards were collated and analysed using a combination of thematic, discourse and content analyses. Findings indicated that the participants constructed as valued bodies those that are good at PE : meaning competency, strength and a desire and ability to win. Alongside this, students also valued fit, not fat bodies, and the display of effort or trying one s best. These constructions were often tied to their potential to perform convincingly. The students took up positions in relation to these notions of status, sometimes investing in practices that would develop their bodies in these ways. Participants fluid subjectivities as they negotiated different activities, physical cultures, and assumptions about gendered and racialised bodies affected their choices not just whether to engage but in what ways they would engage in physical activity.

What happens to the radical potential of gender mainstreaming? : problems of implementation and institutionalisation in gendered organisations

Rapidly spreading in popularity over the past fifteen years, gender mainstreaming has been adopted worldwide by state, supra state and international organizations as the "most modern‟ policy for ensuring gender equality. Yet, there is general agreement that it has not succeeded in achieving its radical potential. In this thesis, I bring together policy literature on bureaucracies, the civil service, and gender mainstreaming with work done on gender, work and organizations as a way to better understand the partial success and uneven implementation and institutionalization of what is supposedly a transformational agenda. To date, gender scholars have underplayed the "stickiness‟ of gender and its effects upon actors and everyday practices in political and state organizations. I argue that the "stickiness‟ of gender in organisations presents a formidable obstacle to the implementation and institutionalization of gender mainstreaming. I also argue that insufficient attention has been paid to the "embodied costs‟ of actors who act as internal gender mainstreaming advocates both in terms of the costs to the individuals and the impact of these costs on the prospects for the successful implementation and institutionalisation of a radical change agenda. Through an ethnographic examination of the Scottish Executive from 2006-2007, I probe the analytic question framing this thesis: what happens to the radical potential of gender mainstreaming during its implementation and institutionalisation in governmental bureaucracies? Using the sociological method of institutional ethnography, I provide evidence of ways that continued reliance on highly committed individuals and everyday bureaucratic practices continue to limit the radical success of gender mainstreaming at the Scottish Executive. I contend that the radical promise of gender mainstreaming is hindered because it is a strategy which must work within the confines of fossilised norms manifested in masculinist bureaucracies and because it is paradoxically predicated on changing bureaucratic norms through the use of the same bureaucratic practices it attempts to transform. By examining the everyday experience of "doing‟ gender mainstreaming in the case of a sub-state government in the UK, the Scottish Executive, I trace the ways that the radical promises of the gender mainstreaming agenda become diluted. The thesis examines challenges to the gender mainstreaming agenda and adds to wider discussions about the plausibility of gender mainstreaming‟s radical potential. In addition, my thesis moves forward methodological discussions in feminist politics by demonstrating the possibility of using institutional ethnography in political science as an effective way to operationalise, analyze and link multiple levels of politics from a gendered perspective. My analysis of local experiences of gender mainstreaming provides insight into the international trend of gender mainstreaming because it takes seriously the experiences of individuals who work within organizations, the role of organizations in limiting change agendas, and the international context within which the mainstreaming strategy unfolds.

Constructing loss : exploring the traumatic effects of bereavement due to HIV/AIDS and tuberculosis on aid workers in South Africa

This thesis aimed to investigate, first, the potentially traumatic effects of AIDS-related bereavement on HIV/AIDS aid workers in South Africa; second, the resources that aid workers utilise in order to cope with their work; and third, differences in the experiences of local versus international aid workers. HIV/AIDS work is associated with various stresses and burnout is commonly observed among HIV/AIDS caregivers. Care of HIV/AIDS aid workers, however, has been largely overlooked; research has typically focused on the experiences of professional health workers, and often outside of an African setting. This present study, therefore, addressed these limitations with the use of participant observation ethnography and ethnographic interviewing. A period of one year was spent with an organisation in South Africa that provides care for vulnerable children in need and affected by HIV/AIDS. Openended semi-structured interviews were conducted with 63 male and female local and international staff and volunteers. The interviews were analysed using discourse analysis (DA), a methodology novel within HIV/AIDS and trauma research and particularly suitable for investigating language, social context and interaction, and identities, which are factors found to be important in HIV/AIDS work. Participants’ discourses were analysed to identify how they construct their identities, concepts such as HIV/AIDS and tuberculosis, events they experienced, and how they made sense of these phenomena. The main finding of this study was that contemporary HIV/AIDS aid work involves new challenges that have surpassed AIDS-related bereavement as the most prominent concern. The main challenges reported by participants involved the inability to control HIV/AIDS treatment and consequently inability to prevent, or control, AIDS-related death as a result of patient non-compliance. Participants further constructed HIV contraction as controllable and, therefore, avoidable, and used this micro discourse on control to counter HIV-related stigma, particularly stigma they experienced as HIV/AIDS aid workers. This rhetorical technique, however, rather maintains the macro discourse on HIV-related stigma by maintaining the blame component of the disease. Two identity constructions emerged in participants’ discourses. First, the characteristics inherent in the child identity suggested that loss is not merely a matter of death but also sadness for and on behalf of children for their various losses. Second, the caregiver identity prescribed how ‘proper’ and ‘genuine’ HIV/AIDS caregivers are expected to behave. The prescriptive nature of this identity can explain burnout among HIV/AIDS caregivers. The rewards of caregiving, however, can act as a buffer against difficult or traumatic experiences inherent in HIV/AIDS work. Managerial support and global belief systems that allow finding meaning were further identified as important coping resources for HIV/AIDS aid workers. Finally, differences between local and international participants, in terms of how they conceptualise phenomena and consequently have different needs, emphasise the role of culture in the experiences of HIV/AIDS aid workers. In the thesis I further discuss these findings in light of theories of social psychology, such as the Just World hypothesis, Cognitive Dissonance, and Identity Control Theory and Self-Categorization Theory. I conclude that although AIDS-related death no longer is a prominent issue, care of HIV/AIDS aid workers should not be overlooked. Contemporary HIV/AIDS work simply involves new challenges and traumas, and it is important that such work is continuously researched to identify evolving needs.

The development of student teacher identities through undergraduate action research projects : an Emirati case study

In recent decades, reflective practice has taken a more central role in the construction of teachers’ knowledge and practice (Elliot, 1991; Roberts, 1998). Within reflective practice, action research has developed as an approach within which teachers can systematically question, challenge and improve their teaching and recently been introduced into teacher education programmes with the rationale of encouraging student teachers to critically engage with curriculum and practice (Mills, 2003). Recent years have additionally seen interest in how teachers’ knowledge is sociodiscursively constructed with a concomitant focus on the link between teacher identity and practice (e.g. Danielewicz, 2001; Miller Marsh, 2003; Norton 2000). However, few studies have attempted to explore the influence action research may have on the construction of student teacher practice and identity (Trent, 2010). This study explores the role of an undergraduate action research project in terms of the extent of its influence on the development of student practice in English Language classes and the trajectory of their emergent teacher identities. Informed by new theoretical directions in ethnography (Denzin, 1997), data was collected using naturally occurring texts integral to the student teachers’ studies, including weekly lesson observations, post-observation feedback discussions and three focus group discussions over the course of the research project. Analysis indicates that the undergraduate action research project differentially affects students’ practice and emergent identities, but that this relationship may be tangential and students’ agency may be overshadowed by methodological preoccupations and constraints of institutions. Both global and local discursive formations combine and interact to influence this process which occurs in a theoretical ‘interzone’ a third space, sociodiscursively constructed between institutions.

Texts in performance : identity, interaction and influence in U.K. and U.S. poetry slam discourses

This thesis aims to provide a close analysis of poetry slam in the United Kingdom and United States, using the tools of ethnography and discourse analysis to produce an in-depth account, which is sensitive to the discursively constructed, situated meanings of slam participants. The aim is to explore how slam is understood by its participants, producing a partial ethnography, rather than a definitive history, defence or critique of slam. The thesis is based predominantly on research conducted in four key sites (Bristol and London in the U.K. and Chicago and New York in the U.S.), and considers how slam has been reconstructed in different geographical and social contexts. In addition, this study seeks to highlight issues around: the ways in which artists understand art worlds and their positions within them; the multiple and complex power relations with which art world participants engage; the transient, enduring and virtual communities which art world participants form; the local, translocal and transnational networks which connect these communities and individuals; and the interactions between new/avant-garde and established/dominant art worlds. It is hoped that this analysis will enrich substantially the existing meagre body of research into poetry slam, providing valuable theoretical contributions to the study of art worlds and the social construction of self and relationships. Beyond this, the thesis aims to elucidate a social scientific paradigm which links micro level analyses with macro level social structures and processes, by allying work from multiple theoretical perspectives including those of interactionism, Antonio Gramsci, Pierre Bourdieu and discourse analysis. This paradigm is mobilised to illuminate how slam participants actively construct their identities and negotiate the complex power relations which structure their everyday interactions. In line with the poetic focus of this research, each analytic chapter of this thesis concludes with a haiku. I begin with this thought: Power relations/Are complex navigations/Through interaction.

Det andra Frankrike : Reselitteraturen, revolutionen och den republikanska etnografin i Bretagne och Auvergne, 1792-1804

The last two decades of the eighteenth century saw the emergence of numerous French travel compilations and descriptive texts studying the customs and ways of life of the peasants, an ethnographical interest that developed even further during the First French Republic. In this study, I identify and study a specific genre that I call republican ethnography. The republican ethnography was part of the Enlightenment’s search for a science of society and to the quest of finding the roots of the Republic. Studying travel literature and administrative reports from two French regions, Bretagne and Auvergne, this study examines the character and the meaning of the republican ethnographical projects. The republican travellers and administrators in Bretagne and Auvergne drew on a complex array of knowledge traditions, using categories from climate theory and medical thinking as well as the vocabulary of the travel literature. The study shows that the republican ethnography worked as way of establishing difference, where the French peasant was described as rude and “savage”, as well as similarity, since the travellers and administrators set out to find the unifying principles on which the Republic could be based.
