A Proposal for the Development of an Exercise Program to Treat Post Thrombotic Syndrome

Healthcare costs continue to escalate and hospitals need to use programs that encourage cost effectiveness, reduce resources and staffing, and increase patient accountability for their own healthcare risk factors, comorbidities, and participation in their healthcare decisions. Post Thrombotic Syndrome (PTS) is a debilitating disease with few treatment options. Allowing patients to have shared decision making in a care plan for this disease process that involves an exercise program may be a significant factor in the success or failure of their own healthcare goals and outcomes. The development of a proposal for an exercise program for PTS should provide patients with input regarding their healthcare plan and participation needs, reduce healthcare risk factors, reduce comorbidities, and increase participation in their healthcare decisions. Seven stakeholders were presented with the proposal for the exercise program for PTS via PowerPoint presentation and a paper handout. Data collection was completed via a 7-question assessment tool designed to provide formative feedback on the refinement of the project and to establish whether the proposed study was a feasible option for the intended vascular population. The analysis consisted of a review and description of all stakeholder responses. Eighty-two percent of the stakeholders indicated that there is a sufficient population, a perception of potential benefit for patients, and that adequate resources are available for the proposed study. Implications for social change include the potential that, through the future implementation of this project, providers could reduce healthcare expenses by decreasing the amount of follow-up and re-hospitalization, thus leading to improved healthcare outcomes.

Comparison of Two Diet and Exercise Approaches on Weight Loss and Health Outcomes in Women

The purpose of this study was to determine the effects of following either the Curves® Fitness and Weight Management Plan or the Weight Watchers® Momentum™ Plan on body composition and markers of health and fitness in previously sedentary obese women. Fifty-one women (age 35±8 yrs; height 163±7 cm; weight 90±1 kg; BMI 34±5 kg/m2; 47±7% body fat) were randomized to participate in the Curves® (C) or Weight Watchers® (W) weight loss programs for 16-wks. Participants in the C group (n=24) followed a 1,200 kcal/d diet for 1-wk; 1,500 kcal/d diet for 3 wks (~30%:45% CHO:PRO); and 2,000 kcals/d for 2-wks (45:30) and repeated this diet while participating in a supervised Curves® with Zumba program 3-d-wk. Remaining subjects (n=27) followed the W point-based diet program, received weekly group counseling, and were encouraged to exercise. Body composition, anthropometrics, resting energy expenditure (REE), lipid biomarkers, and hormone concentrations were assessed at 0, 4, 10, and 16 weeks. Maximal cardiopulmonary exercise capacity and upper and lower body isotonic strength and endurance were assessed at 0 and 16 weeks. Data were analyzed using multivariate analysis of variance for repeated measures. MANOVA analysis of body composition data revealed overall time (Wilks’ Lamda p=0.001) and time by diet effects (Wilks’ Lamda p=0.003). Subjects in both groups lost a similar amount of total mass (C -2.4±2.0, -4.1±3.4, -5.1±3.9; W -2.3±2.3, -4.5±3.0, -5.5±4.6 kg, p=0.78). However, subjects in the C group tended to have a greater reduction in percent body fat (C -3.3±5.2, -3.2±4.6, -4.7±5.4; W 0.6±6.7, -0.6±8.3, -1.4±8.1%, p=0.10) and body fat mass (C -3.9±5.5, -4.6±5.3, -6.4±5.9; W -0.4±5.7, -2.1±6.7, -2.9±7.8 kg, p=0.09), while maintaining FFM (C 1.5±4.3, 0.52±3.7, 1.3±4.0; W -1.8±5.4, -2.4±5.8, -2.5±5.1, p=0.01). While both groups had increases cardiovascular fitness, the C group experienced improvements in upper body muscular endurance (C 1.4±3.9; W -1.2±2.4 repetitions, p=0.006). Both groups experienced improvements in lipid biomarkers; however, only the C group experienced a moderate increase in HDL-c. Results indicate that participants following the C program experienced more favorable changes in body composition and markers of fitness and health than participants in the W program.

Experiences of physical activity engagement among older adults following discharge from a medically supervised exercise program: facilitators, barriers, and suggestions

The purpose of this study was to investigate physical activity (PA) engagement among older adults (OA) following discharge from a medically supervised group exercise program and to explore the facilitators and barriers that influenced maintained PA engagement. While facilitators and barriers to PA among OA in general have been well documented, facilitators and barriers particular to maintaining PA after discharge from a supervised exercise program have not been widely explored with qualitative methods or a mixed method design. Data for this mixed method, case study approach were collected in two phases. In Phase I, questionnaires were used to investigate PA engagement as well as semi-structured qualitative interviews were completed (n = 12; Mage = 80.0 years) to explore facilitators and barriers that influenced PA engagement. In Phase II, reviews of medical charts were conducted retrospectively to gather further information on PA engagement and barriers (n = 12). All 12 individuals in Phase I remained engaged in PA activity 2 – 48 months post completion of the medically supervised exercise program. This particular group of OA identified facilitators for and barriers to maintaining PA that were personally-, socially-, and program-based, and also provided suggestions to alleviate cited barriers. Four themes identified in regards to PA engagement were: (1) Personal drive: highly aware of the need to keep moving; (2) Social connections and support: we all need people; (3) Program components matter; and, (4) Convenient, Affordable, Relevant: suggestions to improve program access. The power of multi-level, multi-sector approaches that consider the broader determinants of health was highlighted in this study. Participants identified the need for health care providers (HCP) and PA instructors to continue to communicate the benefits of PA, the importance of ongoing HCP support, and the necessity of working across sectors to reduce program related barriers to promote PA engagement among OA discharged from a medically supervised exercise program. / Graduate / melodyburgoyne@hotmail.com

The title of the thesis: The effect of exercise in the fitness center, depending on client education The aim of the thesis: The thesis aims to identify the reasons for clients to access the fitness center, their training goals and whether collaboration client with a personal trainer to improve the effectiveness of training and the effective achievement of client objectives. The methods of the thesis: We conducted a deliberate choice of people without regular physical activity history. Clients never attended a regular fitness center. The experimental group was composed of 10 clients. These were 5 women and 5 men. The age range was from clients 23-47 years, weight rating ranging from 61 kg to 105 kg, and the height was from 157 cm to 192 cm. Sedentary work has 6 surveyed respondents, 4 respondents have a job that requires some physical activity. Upplevelser av att underlätta följsamheten till hemträningsprogram för patienter i primärvården - En kvalitativ undersökning av nyexaminerade fysioterapeuter från Uppsala Universitet

Bakgrund: Inom primärvården tillämpas vanligtvis hemträningsprogram som behandling. Flera studier har undersökt underlättande faktorer för följsamhet till hemträning. Sedan fysioterapeutprogrammet på Uppsala Universitet hösten 2014 utvecklade utbildningsplanen till ett mer biopsykosocialt inriktat lärande återfinns ingen forskning på hur fysioterapeuterna med den nya utbildningsplanen upplever att de kan underlätta patientens följsamhet. Syfte: Undersöka hur nyexaminerade fysioterapeuter, som påbörjat utbildningen på Uppsala Universitet höstterminen 2014 eller senare, upplever att de kan underlätta följsamheten till hemträningsprogram för patienter i primärvården. Metod: Kvalitativ deskriptiv design. Datan insamlades genom fem semistrukturerade intervjuer med nyexaminerade fysioterapeuter och bearbetades sedan med hjälp av en kvalitativ innehållsanalys. Resultat: Analysen resulterade i sju kategorier samt 19 underkategorier. De sju kategorierna bestod av “Individanpassa träningsupplägg och uppföljning”, “Öka patientens förståelse av hemövningar”, “Tillämpa socialt stöd vid hemträning”, “Kartlägga och framkalla motivation till hemträning”, “Utforska patientens beteende och mående”, “Etablera god patientkontakt” och “Tillgångar på arbetsplatsen”. Konklusion: Resultatet framställde många strategier som redan beforskats. Samtliga kategorier förutom ”Tillgångar på arbetsplatsen” speglar ett beteendemedicinskt perspektiv. Detta kan indikera på att framtidens fysioterapeuter är behjälpliga av denna typ av perspektiv vid bemötande av patienter för att underlätta följsamheten till hemträningsprogram inom primärvården. / Background: In primary care, it is common to apply home-based exercise programs as treatment. Several studies have investigated facilitating factors that affect adherence to home-based exercise. Since the physiotherapist program at Uppsala University in autumn 2014 developed the syllabus into a more biopsychosocial-oriented learning, no research can be found on how the physiotherapists with the new syllabus experience can facilitate the patient's adherence. Purpose: Examine how newly graduated physiotherapists, who began their education at Uppsala University in the autumn 2014 or later, experience that they can facilitate adherence with home-based exercise programs for patients in primary care. Method: Qualitative descriptive design. Data was collected through five semi-structured interviews with five newly graduated physiotherapists and was conducted with a qualitative content analysis. Results: The data resulted in seven categories and 19 subcategories. The seven categories consisted of "Personalizing training arrangements and follow-up", "Increasing the patient’s understanding of home-based exercise", "Applying social support in home-based exercise", "Chart and incite motivation for home-based exercise", "Explore patient behavior and well-being", "Establish good patient contact" and "Workplace assets". Conclusion: The result presented many strategies that already have been explored. All categories except "Workplace assets" reflected a behavioral medicine perspective. This perspective might be of value for future physiotherapists to facilitate adherence with home-based exercise programs in primary care.

Performance Training Guidelines for the 1.5 and 2-Mile Runs

Very little peer-reviewed information is available to aid military personnel in selecting training programs to enhance performance on fitness tests and direct fitness-related military policy. Objective: This review provides recommendations on training programs for enhancing performance on 1.5-mile and 2-mile runs based on the available relevant literature. Design: Short review article. Methods: Collected relevant research articles by using search terms such as aerobic power, military physical fitness test, strength training, resistance training, endurance training, high intensity interval training, running economy, 3 km run, 5 km run, and 1.5/2-mile run. Results: Evidence has shown running performance can improve with a combination of traditional strength training, high intensity interval training, and distance training. Conclusion: A combination of traditional strength training, high intensity interval training, and distance training should be used to enhance running performance on the 1.5 and 2-mile run tests used by the military.

”Det hänger ju mycket på en själv” – en undersökning om hemträningsprogram och dess följsamhet

<p>Syftet med studien var att undersöka hur patienter upplevde en sjukgymnasts metodik i samband med utlärningen av hemträningsprogram de fått ordinerade i rehabiliterande syfte för sina skador. Dessutom undersöktes hur väl sjukgymnastens metodik gällande hemträningsprogram stämde överens med tidigare forsknings rekommendationer, och ifall samband kunde ses mellan patienternas följsamhet och de faktorer som påverkar den. Undersökningen var en modifierad fallstudie och genomfördes med relativt högt strukturerade telefonintervjuer. Undersökningsgruppen bestod av sju patienter som alla hade ordinerats hemträningsprogram ifrån samma sjukgymnast, för olika typer av skador. Resultatet av studien visade på att patienterna upplevde mottagningens metodik som väl fungerande. Sjukgymnastens metodik stämde även väl överens med tidigare forskningsrekommendationer för en hög följsamhet utav träningsprogrammen bland patienterna. Samband mellan patienternas följsamhet och tidigare forsknings rekommendationer visade sig vara starka.</p> / <p>The purpose of this study was to examine how patients experienced the methodology concerning the home exercise programmes they have been prescribed by a physiotherapist in rehabilitation purpose due to their injuries. Furthermore it was examined how well the methodology regarding the home exercise programmes made by the physiotherapist agreed with recommendations according to previous research studies, and if a connection could be drawn to the patients’ determination to comply with the exercise programmes. The study was a modification of a case study and it was conducted by relatively high structured telephone interviews. The target group for the study consisted of seven patients, all which had been prescribed with home exercise programmes by the same physiotherapist due their different injuries. The result of the study implies that the patients experienced the methodology of the physiotherapist reception as well-functioning. The methodology of the physiotherapist was also fall in line with what previous recommendations from scientific studies regards important in order to achieve a high compliance of the exercise programmes among the patients. The connection between the compliance of the patients and recommendations made based on previous studies appears to be highly related.</p>
