Medidas da rotação interna glenoumeral em tenistas e em nadadores assintomáticos comparados com um grupo-controle

Introdução: O déficit de rotação interna glenoumeral, freqüentemente observado em praticantes de esportes que envolvem movimentos repetidos do membro superior sobre a cabeça, tem sido associado ao surgimento de lesões secundárias no ombro. Objetivos: Medir e comparar a amplitude de rotação interna glenoumeral em praticantes assintomáticos de tênis e natação, esportes com características diferentes, mas que envolvem esse tipo de movimento. Métodos: 54 voluntários assintomáticos do sexo masculino (108 ombros) divididos em 3 grupos (tenistas, nadadores, grupo-controle) foram submetidos à medida da amplitude de rotação interna glenoumeral através do método de exame clínico com estabilização da escápula (delineamento de pesquisa: estudo transversal). Foram comparadas as medidas dos ombros dominante e não dominante dentro de cada grupo e entre os grupos. Resultados: Em todos os grupos, o ombro dominante apresentou déficit de rotação interna se comparado com o não dominante. No grupo de tenistas o déficit médio foi de 23,9° ± 8,4° (P< 0,001), no de nadadores foi de 12° ± 6,8° (P< 0,001), e no grupocontrole de 4,9° ± 7,4° (P= 0,035). Comparados os membros dominantes entre os grupos, houve diferença entre todos, sendo o déficit apresentado pelos tenistas em relação ao grupo-controle (27,6°;P< 0,001) maior do que o dos nadadores (17,9°; P<0,001); entre tenistas e nadadores, foi de 9,7°;P=0,002). Conclusões: O membro dominante apresentou menor amplitude de rotação interna glenoumeral do que o não dominante em todos os grupos, sendo o déficit dos tenistas cerca de duas vezes maior do que o dos nadadores. A diferença média entre os membros no grupo controle foi menor do que 5°, o que está dentro do parâmetro de normalidade de acordo com a maioria dos estudos. Todos os grupos apresentaram diferenças se comparados os membros dominantes entre si. Os tenistas apresentaram a menor amplitude de rotação interna seguidos pelos nadadores. / Background: Glenohumeral internal rotation deficit, often diagnosed in players of overhead sports, has been associated with the development of secondary shoulder pathologies. Aim: To measure and compare the range of glenohumeral internal rotation motion in asymptomatic tennis players and in swimmers, different sports that share this overhead movements. Methods: Fifty-four asymptomatic male volunteers (108 shoulders) divided in 3 groups (tennis players, swimmers, control group) underwent measurements of glenohumeral internal rotation using clinical examination with scapular stabilization (study design: cross-sectional study). Measurements of dominant and nondominant shoulders were compared within and between groups. Results: In tennis players, mean déficit was 23.9° ± 8.4° (P< 0.001); in swimmers, 12° ± 6.8° (P< 0.001); and in the control group, 4.9° ± 7.4° (P= 0.035). Dominant shoulders showed significant difference between all groups, and the déficit of the group of tennis players in comparison with the control group (27.6°;P< 0.001) was greater than the deficit found in the group of swimmers (17.9°; P<0.001); between tennis players and swimmers, the deficit was 9.7°;P=0.002). Conclusions: Dominant limbs showed less glenohumeral internal rotation than the nondominant limbs in all groups, being the deficit in the group of tennis players about twice the deficit found for swimmers. Mean difference between limbs in the control group was less than 5°, which is within normal parameters according to most studies. There were statistically significant differences between all groups when dominant shoulders were compared to each other. Tennis players had the least range of motion, followed by swimmers.

Total Shoulder Arthroplasty : clinical and radiological studies on the implant positioning and fixation

Shoulder arthroplasty surgery has shown remarkable progress during the last few decades. A number of factors affect postoperative range of motion, pain and prosthetic durability. Among these factors, the length of the lever arm and joint stability is the ones that can be altered by the selected prosthetic component. It is uncertain how much of the normal anatomy needs to be re-established. Stemless prostheses with total reliance on metaphyseal fixation were introduced in France in 2004 (TESS, Zimmer Biomet). The goals were to avoid stem-related complications. Stemless implants have other potential benefits, including the ability to restore shoulder anatomy. Study I: This is a prospective cohort study of 49 patients with one of two versions of the TESS prosthesis (anatomical or reverse) with clinical and radiological follow-up ranging from 9–24 months. The TESS prosthesis showed short-term results that were comparable with other shoulder prosthetic systems. Study II: This is a prospective comparative non-randomised study of 37 patients (40 shoulders) who underwent TESS reverse shoulder arthroplasty (RSA) with a follow-up ranging from 15–66 months. We found a significant improvement in functional outcome and reduction of pain in both stemmed and stemless groups. Glenoid overhang influenced the occurrence of scapular notching (SN). Study III: This is a radiological study showing that CT had a good reliability and reproducibility in estimating LHO. Study IV: This is a prospective radiological study of 69 patients (70 shoulders) with primary osteoarthritis (OA) who had undergone stemless total anatomical shoulder arthroplasty (TSA). This study showed that stemless anatomical TSA could be useful in restoring shoulder anatomy. Study V: This is a prospective study of 44 patients with OA who had undergone stemless anatomical TSA with a clinical and radiological follow up ranging from 12 – 50 months. Our study showed that LHO reconstruction close to the anatomy of a healthy contralateral shoulder improved shoulder function. Stemless anatomical TSA help to restore LHO. Increasing LHO may have a negative effect on shoulder function at three months but had no effect at 12 months. The main conclusions of this thesis are: 1. TSA (anatomic and reverse) using stemless humeral components is reliable if bone quality is adequate. The complication rate is comparable with other shoulder prosthetic systems. 2. Glenoid overhang decreased complications in RSA. 3. LHO measurement on AP radiographs is less reliable and underestimates the distance when compared with CT. 4. Stemless TSA could be of help in reconstructing shoulder anatomy. 5. Shoulder reconstruction close to the anatomy of a healthy contralateral shoulder improves shoulder function. / Axelprotes kirurgi har visat avsevärd utveckling under de senaste decennierna. Ett antal faktorer påverkar postoperativt rörelseomfång, smärta och proteshållbarhet. Bland dessa faktorer utgör längden av hävarmen och ledstabilitet de faktorer som kan ändras genom val av proteskomponent. Det är osäkert om den normala anatomin måste återupprättas. Oskaftad protes med eliminering av humerusstamm och tillit till metafysär fixering introducerades i Frankrike år 2004 (TESS, Zimmer Biomet). Målen var att undvika stam relaterade komplikationer. Ökad LHO kan ha en negativ effekt på axelnsfunktion vid tre månader, men denna effekt påvisade ej vid 12 månader. De viktigaste slutsatserna i denna avhandling är: 1. Oskaftad total axel artroplastik (anatomisk och omvänd) är tillförlitlig om benkvalitén är god med komplikationer som var jämförbar med andra axelprotessystem. 2. Glenoid overhang minskar komplikationer vid omvänd axelartroplastik. 3. LHO mätningen på röntgen är mindre tillförlitlig och underskattar avståndet jämfört med CT. 4. Oskaftad axelprotes skulle kunna vara till hjälp för att rekonstruera axelnsanatomi. 5. Axel rekonstruktion inom anatomi till att efterlikna anatomi på den friska kontralaterala axeln förbättrar axelfunktion.

Title: Effect of the range of the shoulder joint rotation in volleyball players on the development of functional changes. Objectives: The aim of this thesis is to confirm the presence of the glenohumeral internal rotation deficit in beach volleyball players and to determine whether it affects the development of functional changes of the shoulder joint located in the dominant upper limb. Methods: The thesis was a quantitative study with the character of an experiment. There were 60 active volleyball players aged 18-30 years who were deliberately selected for the purposes of the study. The essence was to ascertain whether GIRD affects the pain in the shoulder joint of volleyball players and whether it was deemed positive for all participants of the study. In order to examine the range of motion, the goniometer in the external and internal rotation according to Janda and Pavla, Janda and Sachse was used, as well as hypermobility tests and a shoulder joint pain questionnaire. The statistical data processing was performed by Microsoft Excel 2010. The sum and average functions were used. The most frequent sequelae following an obstetric brachial plexus lesion without complete functional recovery concern the impaired shoulder rotation movements and the associated structural changes of the growing glenohumeral joint. <p>This pathology is often unrecognized and may lead to a limitation in active movements, a pathologic and less efficient motion pattern in the affected limb, and the development of a severely incongruent and dysplastic glenohumeral joint prone to further arthrosis.<p><p>Hypothesis<p><p>Glenohumeral dysplasia after obstetric brachial plexus lesion has multiple etiologies: A hypothetic obstetric trauma may precede the motor imbalance, due to the initial palsy and prevalent recovery of the medial rotators of the shoulder.<p>The correction of the muscular imbalance, by neurotization of the lateral rotators (supra- and infraspinatus muscle) using a local nerve transfer or by a later muscle transfer surgery, improves function, seems to prevent the development of joint dysplasia and limits the articular deformities once they are present. <p>The early (peripartal) glenohumeral subluxation must be recognized and treated immediately to prevent the development of a severe joint contracture and dysplasia.<p> <p>Material and methods<p><p>Two retrospective and one prospective study evaluate how surgery may correct the muscular imbalance.<p>In a first series of 65 children, we analyse the recovery of the supra- and infraspinatus muscle after a nerve transfer onto the suprascapular nerve.<p>In a second retrospective analysis on 114 children, we study the outcome after secondary surgery (anterior joint release, modified Hoffer muscle transfer) dedicated to improve active and passive lateral rotation of the shoulder.<p>A prospective study of 50 magnetic resonance (MRI) scans of the glenohumeral joint describes the articular deformities.<p>Finally, 10 children presenting a very early glenohumeral subluxation have undergone a closed orthopaedic reposition and plaster immobilisation and were followed for a minimum of 2 years.<p><p>Results<p><p>In the first group, neurotization of the suprascapular nerve has been performed either by a dorsal or a ventral approach at a mean age of 14 months. The mean follow up is of 3 years and the improvement in aLR(ABD) is 68°and only 25°in aLR(ADD). None of these children with improved active lateral rotation of the shoulder developed clinical signs of a glenohumeral dysplasia within the follow up period.<p><p>Among the 114 children operated between 6 months and 44 years with a shoulder release, 74 had an isolated release procedure, 40 an associated tendon transfer or a suprascapular neurotization. The mean improvement in passive lateral rotation with the arm adducted (pLR (ADD)) was 60°. Active lateral rotation was possible in 63 % of children who underwent an isolated joint release.<p>The Hoffer muscle transfer was performed in 29 children and improved the aLR (ABD) by 60° (mean postoperative follow-up of 30 months). No signs of severe glenohumeral dysplasia developed in these children later on.<p><p>The prospective study of 50 consecutive MRI scans in children presenting at the consultation with a rotatory imbalance of their shoulder, as a sequel from obstetric brachial plexus palsy (Bahm et al. 2007) shows 37 congruent joints, 10 dorsal subluxations, 2 dorsal luxations and one complete dislocation associated with the formation of an independent neoglenoid. The humeral head was deformed in 12 cases; the glenoid in 34 children (flat in 23, biconcave 7 times, convex 3 times).<p><p>The follow up of 2 years in 10 children who underwent an immediate closed reposition shows evidence of joint congruence with a limited (30°) pLR (ADD), definitely lower than after a surgical release.<p><p>Conclusion<p><p>Some osteo-articular deformities secondary to neuromuscular diseases are well described ;those following an obstetric brachial plexus lesion are insufficiently recognized. Their etiology is unclear.<p>At the level of the shoulder joint, these sequels might be very important.<p>Our neuroorthopaedic hypothesis concerning a multifactorial etiology and treatment strategy raises the need of an early and precise screening of the deforming forces to render normal biomechanics and function.<p>The surgical strategy includes the reconstruction of the responsible motor nerve and the improvement of the passive and active range of motion of the shoulder in lateral rotation<p>It seems to be efficient to limit the progression to severe glenohumeral dysplasia and further arthrosis.<p> / Doctorat en Sciences médicales / info:eu-repo/semantics/nonPublished
