ConcepÃÃes e vivÃncias sobre o Programa de SaÃde da FamÃlia em Fortaleza / Conceptions and experiences about the Family Health Program in Fortaleza

This research turns on the Program of Health of Family (PSF) in Fortaleza, looks for to understand, through the study of the clarifying models constructed by the medical professionals and nurses who in it act, as processed its implantation, organization and execution. It is a study of qualitative nature, that used the half- structure interview to identify perceptions, sadness and feelings of constructed satisfaction and not satisfaction through practical its of the PSF in the city of Fortaleza. Fourteen professionals had participated of the research, being seven nurses and seven doctors who act or had acted in the city during its implantation. The study he disclosed that the implantation of the Program of Health of the Family in the city of Fortaleza was marked by a series of events, between them if they detach: the exit of the professionals who acted in the old model and the arrival of the teams of health of the family; scarcity of human resources and substances; reduction of the number of qualifications and the interference partisan politics in the work process. All these had difficulty lements for the implantation process, had contributed so that it had to the eyes of the professionals interviewed, a distortion of the objectives of the PSF during its implantation and implementation. When questioned on its professional satisfaction the professionals they disclose negative and positive dimensions of practical its. Thus, the interviewed professionals live a paradoxical relation, enter the pleasure to exert practical its and the frustration generated for the informality and not the recognition of the Health of the Family, while specialty. Program Evaluation / Esta pesquisa versa sobre o Programa de SaÃde da FamÃlia (PSF) em Fortaleza, procura compreender, atravÃs do estudo dos modelos explicativos construÃdos pelos profissionais mÃdicos e enfermeiros que nele atuam, como se processou sua implantaÃÃo, organizaÃÃo e operacionalizaÃÃo. Ã um estudo de natureza qualitativa, que utilizou a entrevista semi-estrutrada para identificar percepÃÃes, angÃstias e sentimentos de satisfaÃÃo e insatisfaÃÃo construÃdos atravÃs da prÃtica do PSF em Fortaleza. Participaram da pesquisa quatorze profissionais, sendo sete enfermeiros e sete mÃdicos que atuam ou atuaram no municÃpio durante a sua implantaÃÃo. O estudo revelou que a implantaÃÃo do Programa de SaÃde da FamÃlia no municÃpio de Fortaleza foi marcada por uma sÃrie de acontecimentos, entre eles se destacam: a saÃda dos profissionais que atuavam no antigo modelo e a chegada das equipes de saÃde da famÃlia; escassez de recursos humanos e materiais; diminuiÃÃo do nÃmero de capacitaÃÃes e a interferÃncia polÃtica partidÃria no processo de trabalho. Todos esses elementos tidos como dificultadores para o processo de implantaÃÃo, contribuÃram para que houvesse aos olhos dos profissionais entrevistados, uma distorÃÃo dos objetivos do PSF durante a sua implantaÃÃo e implementaÃÃo. Quando questionados sobre sua satisfaÃÃo profissional os profissionais revelam dimensÃes negativas e positivas da sua prÃtica. Assim, os profissionais entrevistados vivem uma relaÃÃo paradoxal, entre o prazer de exercer sua prÃtica e a frustraÃÃo gerada pela informalidade e pelo nÃo reconhecimento da SaÃde da FamÃlia, enquanto especialidade.

Metodologia: Na primeira parte desse trabalho realizou-se um estudo descritivo sobre aspectos relacionados a aids e sexualidade com a populaÃÃo dos profissionais mÃdicos, enfermeiros e agentes de saÃde das equipes do PSF (programa saÃde da famÃlia) do municÃpio de Fortaleza.O instrumento utilizado para a coleta de dados foi uma adaptaÃÃo do questionÃrio da pesquisa nacional acerca do Comportamento Sexual da PopulaÃÃo Brasileira, elaborado pelo MinistÃrio da SaÃde, no ano de 2000. Na segunda fase da pesquisa foi feito um estudo qualitativo com a formaÃÃo de grupos focais, com esta mesma populaÃÃo, abordando temas relacionados à aids e a sexualidade humana. A partir de um conjunto de referÃncias para os estudos sobre AIDS (os estudos da Antropologia) e levantamentos feitos por estudiosos na Ãrea, chegou-se aos seguintes eixos norteadores: I) as prÃticas sexuais como prÃticas sociais; II) A construÃÃo social das prÃticas sexuais acompanha o dinamismo das culturas e III) as prÃticas sexuais que sÃo prÃticas sociais, sÃo vividas por sujeitos multidimensionais. Resultados da primeira etapa: a pesquisa apontou que a prÃtica sexual entre pessoas do mesmo sexo e a existÃncia de relaÃÃes extraconjugais foi amplamente rejeitada pelos participantes da pesquisa. Apesar de quase todos haverem realizado treinamentos em aids, alguns revelaram que a doenÃa se pode pegar usando banheiros pÃblicos, fazendo-se uso da camisinha masculina ou feminina ou usando o mesmo talher de pessoas contaminadas. A maioria apontou que os preservativos sÃo desagradÃveis mas mesmo assim deve-se usÃ-los para se evitar a aids. Resultados da segunda etapa: O estudo nos revelou uma concepÃÃo mÃtica que envolve aspectos relacionados à doenÃa colocando-a ,quase sempre, como uma coisa do outro; o preservativo por sua vez, adquiriu um status de instrumento de intermediaÃÃo na relaÃÃo de poder; vislumbrando-se, tambÃm, uma concepÃÃo de naturalizaÃÃo nas relaÃÃes interpessoais mediadas por essas relaÃÃes de poder. ConclusÃes: Vimos que as opiniÃes desses profissionais, apesar de todo o arsenal de conhecimentos especÃficos e treinamentos impetrados, sÃo atravessadas por uma forte influÃncia biomÃdica de assepsia dos corpos e por um conjunto de vivÃncias e valores que nÃo se diferenciam muito da populaÃÃo com a qual atuam, levando-se a refletir de que modo se està dando suporte a essa dialogicidade entre o profissional e a comunidade. / Methodology: In the first part of this work a research was carried through became fullfilled a descriptive study on the population of the medical professionals, nurses and health agents who compose the teams of the PSF of the city of Fortaleza. The instrument of collection of data were an adaptation of the questionnaire used in the national research concerning the Sexual Behavior of the Brazilian Population, carried through for the Health department, in the year of 2000. At as a moment a qualitative study with the accomplishment of focal groups was made, with this same population, that approached subjects related to the AIDS and sexuality. After bibliographical survey of the authors who present works of relevance related to the subject we arrive at the following axles that had guided our look had been: I) practical sexual as practical the social ones; Practical II) the social construction of the sexual ones follow the dynamism of the cultures and practical III) the sexual ones are lived by multidimensionais.Resultados citizens of the first stage: the research pointed that practical sexual between people of the same sex and existence of extramarital relations widely it was rejected by the participants of the research. Although almost all to have carried through training in AIDS, some had disclosed that the illness if can catch using public bathrooms, becoming use of the masculine or feminine condom in the sexual relation or using the same place setting of contaminated people. The majority pointed that the condoms are ackward but exactly thus it must be used to prevent aid. Results of the second stage: The study in it disclosed a conception to them mythical that involves aspects related to the illness placing it, almost always, as a thing of the other; the condom in turn, acquired a status of instrument of intermediation in the relation of being able; glimpsing also a naturalization in the interpersonal relations mediated by these relations of being able. Conclusions: It was observed that it has a economic difference partner enters the professionals of the team of the PSF thus characterizing a difference of classrooms and sort permeado for intrinsic relations of being able. We saw that the opinions of these professionals, although all the armory of specific knowledge and petitioned training, are crossed by one strong biomedical influence of asepsis of the bodies, characterized for a set of experiences and values that are not differentiated very of the population in general.

Program evaluation is in its infancy at the Washington County Mental Health Program; it is only within the last two years that there has been any emphasis placed on evaluation activities. The first chapter in this practicum will identify and provide a context for the recent introduction of the management by objectives approach to process evaluation within the Washington County Mental Health Program as well as present a rationale for such an approach; the second chapter in this practicum is a presentation of results obtained from the first follow-up study to employ management objectives as an evaluation tool; the third chapter explores problems inherent in the nature and design of this type of evaluation procedure as well as program implications concerning the utility of results obtained from the study and recommendations for change in both approach and procedure.

RETROSPECTIVE EVALUATION OF FACTORS THAT INFLUENCE THE IMPLEMENTATION OF C.A.T.C.H. IN SOUTHERN ILLINOIS SCHOOLS

This study is a retrospective evaluation of the Coordinated Approach To Child Health (CATCH) coordinated school health program. An abundant amount of research has been conducted concerning CATCH, but no data exist that represents the characteristics and attitudes of individuals implementing the program. This study looked to examine organizational readiness, commitment to change, leadership, implementation barriers, innovation perceptions and their influence on the diffusion of CATCH. The primary purpose of this study is to describe and explain why schools in the same area that receive the same CATCH training result in different implementation practices. This study included a retrospective evaluation that evaluated school employees' motivation of CATCH implementation over the 2011-2012 school year. A survey of 284 school employees and health department partners consisting of 33 school administers, 197 classroom teachers, 27 physical education teachers, 21 cafeteria supervisors, and 6 health department partners at elementary school located in the southernmost counties of southern Illinois was conducted. Particular attention was focused upon the differences between classroom teachers, physical education teachers, cafeteria supervisors, and health department partners. Degree of CATCH implementation was the best among cafeteria supervisors and physical education teachers while classroom teachers implemented roughly 50% of the CATCH classroom curriculum. Organizational readiness was a significant predictor of classroom teacher degree of implementation while school leadership served as a significant predictor of degree of implementation by physical education teachers. The study utilizes CATCH; however, this study could be helpful concerning other school health programs to enhance program implementation practices and delivery. The significance of these data provide health educators with evidence of why schools have different implementation practices, what constructs influence degree of implementation, and how addressed constructs that influence implementation can be rectified through school preparation and training protocols to enhance degree of implementation. Additional variables are also discussed that could account for further variation in school employee degree of implementation.

Design, Implementation, and Evaluation of a Faith-Based Nutrition Education Program for African American Women

African American women are suffering from high rates of debilitating illnesses and diseases. Over 50% of African American women are obese; close to 44% of African American women have hypertension or are taking medication for high blood pressure; 4.1% of African American women have had a stroke; and 12.4% of African American women have diabetes. Dietary behaviors play pivotal roles in improving the health of African American women. African American women have shown consistent dietary habits that include eating foods higher in fat, and foods lower in desirable nutrient content. These dietary habits have been linked to higher prevalence of obesity, hypertension, stroke, and diabetes. The culture of African American women often includes the African American church, which is a venue that may be utilized to promote dietary interventions. Eve’s Apple Nutrition Education program was an 8-week program designed using a faith-based approach to promote healthy dietary behaviors among African American women. Program objectives were to increase lowat eating behaviors and decrease negative dietary behaviors. The program was implemented with African American women (n=38) in Little Rock, Arkansas. Data were collected from pre and post surveys, as well as a focus group survey. Paired-samples t tests determined that at eight weeks, participants significantly decreased all negative dietary behaviors (emotional eating, snacking on sweets, haphazard planning, meal skipping, cultural factors) and increased lowat eating behaviors and lowat eating styles (p<.01). These results indicate Eve’s Apple Nutrition Education Program was successful in improving dietary behaviors during the program. A focus group with eight of the participants was completed post-intervention and also indicated that the faith-base was an important component in improving dietary behaviors. For African American women, faith-based nutrition education programs provide cultural relevance, support, and are unique in specifically targeting their health and nutrition needs.

Avaliação do cuidado em saúde mental na estratégia saúde da família / Assessment of the assistance in mental health in the Family Health Strategy

Esta pesquisa teve por objetivo a integralidade do cuidado ao portador de transtorno mental na Estratégia Saúde da Família (ESF). Foi realizado um estudo de caso, sendo este desenvolvido a partir de uma adaptação da avaliação proposta de Estrutura, Processo e Resultado de Donabedian (1984), com abordagem qualitativa dos dados. Para coleta de dados foi utilizado: observação e entrevista semi-estruturada. O estudo foi realizado no município de Chapecó- SC, região oeste do Estado de Santa Catarina. Foi selecionada uma Unidade de Saúde da Família da área urbana, com maior tempo de implantação dentre as pesquisadas, com equipe completa de acordo com as propostas e normas do Ministério da Saúde e com maior vulnerabilidade social da área adscrita. Os critérios utilizados para realizar a avaliação estão fundamentados nas respectivas publicações ministeriais: Plano Nacional de Inclusão das Ações de Saúde Mental na Atenção Básica, Manual de Estrutura Física das Unidades Básicas de Saúde: saúde da família, Cadernos de Atenção Básica-Programa de Saúde da Família: a implantação da unidade saúde da família (Caderno I), Guia Prático do Programa de Saúde de Família. A análise dos dados foi dividida em três temas: avaliação da Estrutura, avaliação do Processo e avaliação do Resultado. Na Estrutura, foram observados algumas limitações de área física (salas adaptadas) e de acesso (filas e tempo de espera para consultas médicas e odontológicas) importantes para oferecimento e manutenção da qualidade da assistência aos usuários. No Processo foi possível observar ruídos (na comunicação, na restrição do acesso aos prontuários pelos agentes de saúde, na identificação de situações de risco e na intersetorialidade) e potencialidades no processo de trabalho dos profissionais de saúde (tratamento odontológico na forma completada, a realização de visitas domiciliares, o vínculo e a disponibilidade para a escuta). No Resultado foi observado que o vínculo, a relação entre a equipe e comunidade, o cuidado à família, as visitas domiciliares, o olhar integral do profissional de saúde no exame físico foram relatados como motivos de satisfação dos usuários. E os motivos de insatisfação estiveram ligados ao acesso daqueles usuários que não fazem parte dos programas e sobre a área física. Ao final da pesquisa notou-se que a equipe apresentou um movimento organizacional favorável à integralidade do cuidado a saúde mental. Pois se percebeu, no caso estudado, o compromisso com a organização de uma rede de cuidados a saúde mental, tanto por parte da própria equipe, quanto pela gestão, apesar das limitações encontradas nas dimensões de Estrutura-Processo-Resultado. / This study analyzes the integraity of the system for the care of the patient with mental disorders in the Family Health Strategy (FHS). A case study was undertaken by adaptating the assessment of Structure, Process and Outcome proposed by Donabedian (1984), with a qualitative approach to the data. Observation and semi-structured interviews were drawn on in data collection and then a documental analysis was done. The study was carried out in Chapeco, Western region of the State of Santa Catarina. The oldest Family Health Unit was selected in the most socially vulnerable urban area, with a complete team according to the proposal and rules of the Ministry of Health.The criteria used to make the assessment are based on the respective ministerial publications: The National Plan for Inclusion of Mental Health actions in Basic Care, the manual of the physical structure of the basic units of health: family health, the Basic Care Booklet ? Family Health Program; and the creation of a family health unit (booklet 1), Practical Guide to Family Health Program. The data analysis was divided into three subjects; structure assessment, process assessment and outcome assessment. In the structure assessment, important limitations on the offering and management of the quality of the patient care were observed but, despite those problems, the integraity was not damaged. The process assessment was made from the Analytic Flowchart. This flowchart showed the process used by the interviewer in the health sector in the town, allowing us to observe the problems and potencialities in the health professionals\' work process. The outcome assessment was done by accessing user satisfaction. Through observation, we could obtain an assessment of the care offered and how the user benefits from the service organization to meet their needs. The bond, the relation between team and commnunity, the care for the family, the home visits, the health professional\'s attentive look in the physical examination and the number of users waiting for an appointment were reported as reasons for satisfaction by the users. The assessment in Health, in modern times, contemplates a dynamic and innovative process both to the service organization and to the research. Thus, we could verify the interplay that guides the assessment process, pursuing the transformation and the improvement of policy practices and knowledge in mental health.

No. of bitstreams: 0 Previous issue date: 2010-04-28 / Trata-se de um estudo quantitativo-descritivo que objetivou avaliar a percepção dos usuários cadastrados nas equipes da Estratégia Saúde da Família da zona urbana de Montes Claros, Minas Gerais, relacionados às dimensões da satisfação dos usuários quanto à tangibilidade, confiança,responsabilidade, segurança e empatia; bem como descrever o perfil da população entrevistada. A fundamentação teórico-metodológica apoiou-se em referenciais de autores que se aprofundaram no assunto. O método SERVQUAL foi utilizado. A amostra foi obtida por um processo probabilístico, por meio da amostragem aleatória simples, utilizando-se o Software Minitab for Windows. Os Softwares Microsoft Excel 2002 for Windows XP e o SPSS 15.0 subsidiaram a análise dos dados, servindo à construção de tabelas e gráficos, além deste último proporcionar a realização do teste do qui-quadrado x2. Para a coleta de dados, empregou-se um questionário com questões fechadas para a construção do perfil da população, e com afirmativas, cujas opções de respostas foram graduadas, de acordo com a Escala de Likert, de “discordo integralmente” a “concordo integralmente”. Os dados foram coletados nos meses de outubro e novembro de 2008. A análise foi realizada por meio da estatística descritiva e associações estatisticamente significantes (p<0,05) entre algumas variáveis. Os dados indicaram que a maioria dos entrevistados são do sexo feminino (80,6%), faixa etária de 18 a 30 anos (38,2%), possuíam Ensino Médio completo (32,3%), renda familiar de 1 a 3 salários mínimos (76,8%), moram nos bairros há mais de 10 anos (52,6%). Na Dimensão Empatia, verificou-se que apesar da maioria dos usuários afirmarem que os profissionais da ESF oferecem Atendimento Individual, alguns demonstraram que quanto ao Entendimento das Necessidades Específicas não estão satisfeitos. Foi possível identificar também que os idosos possuem uma percepção mais positiva do serviço da ESF em todos os quesitos. Espera-se que este estudo possa proporcionar reflexões aos profissionais da ESF frente aos serviços oferecidos e subsidiar ações de adequação dos serviços prestados a essa população, podendo melhorar o planejamento do trabalho das equipes. / TEDE / BV UNIFESP: Teses e dissertações
