Im Zentrum des forschungsleitenden Interesses stand die Fragestellung, ob es Unterschiede bezüglich des Auftretens und der Intensität von Angst bzw. Depression sowie körperlichen Beschwerden zwischen den Patientengruppen mit unterschiedlichen chronischen Lebererkrankungen (HBV, HCV, AIH) gibt. Die Bestimmung der Korrelation zwischen dem Fortschreiten der Lebererkrankung (von Hepatitis über Fibrose zu Zirrhose) und dem Ausmaß depressiver und ängstlicher Symptomatik stand hierbei für alle drei Patientengruppen im Fokus des Interesses. Alle Patienten waren zum Erhebungszeitpunkt ohne spezifische medikamentöse Behandlung aufgrund ihrer Lebererkrankung. Als sekundäres Studienziel sollte die Forschungsfrage beantwortet werden, ob die vier angewendeten verschiedenen psychometrischen Erhebungsinstrumente (HADS-D, SCL-90-R, IIP-C und SF-36) hier zu unterschiedlichen Ergebnissen führen oder ob die Verfahren größtenteils in ihren Ergebnissen übereinstimmen. Auf der Grundlage der histologischen Daten konnte in der Subgruppe der Hepatitis-C-Patienten ein Zusammenhang zwischen Progression der Leberfibrose, depressiver Symptomatik und verminderter körperlicher Gesundheit dargestellt werden. Nur bei Patienten mit chronischer Hepatitis-C-Infektion konnte ein signifikant positiver Zusammenhang (r = 0,205) zwischen der Zeitdauer seit Mitteilung der Primärdiagnose und dem Ausmaß der depressiven Symptomatik festgestellt werden. Alle drei chronischen Lebererkrankungen waren dagegen mit erhöhten Angstwerten assoziiert: bei der Autoimmunhepatitis kam es in 21,4 % der analysierten Fälle, bei der Hepatitis B in 13,9 % und bei der Hepatitis C in 11,8 % zu einer Überschreitung des Cutoff-Wertes mit klinisch relevanter Symptomausprägung. Dieser Prozentsatz fällt in der Allgemeinbevölkerung mit 5,7 % deutlich geringer aus. Es fiel auf, dass die Angst mit zunehmend besseren Therapiemöglichkeiten der Grunderkrankung abnimmt. Lediglich in der Subgruppe der HBV-Infizierten war eine signifikant positive Korrelation der Variablen Angst bzw. Ängstlichkeit und Zeitraum seit Mitteilung der Diagnose zu verzeichnen (r = 0,388). Das eingesetzte psychiatrische Screeningverfahren (SCL-90-R) vermochte nicht zwischen den drei chronischen Lebererkrankungen zu differenzieren, zeigte jedoch eine deutlich erhöhte psychische Gesamtbelastung im Vergleich zur Allgemeinbevölkerung. Die Gesamtbelastung war größer als beispielsweise bei Patienten nach Myokardinfarkt. Dagegen scheinen interpersonale Probleme gemäß den vorgestellten Studienergebnissen bei keiner der drei hepatologischen Erkrankungen eine Rolle im Sinne einer Belastung mit Krankheitswert zu spielen. Bezüglich der erhobenen Lebensqualitätsparamter (SF-36) zeigte sich ein deutlich messbarer Unterschied zwischen der psychischen und der physischen Gesamtbelastung. Hierbei war die psychische Belastung eindeutig im Vordergrund. Alle drei Patienten-Subgruppen zeigten in diesem Zusammenhang durchaus vergleichbare Resultate. Die Hepatitis-B-Gruppe gab insgesamt die größte mentale Belastung an, bei gleichzeitig größten körperlichen Schmerzen innerhalb der drei Gruppen. Zusammenfassend kann man sagen, dass die gemessene psychische Gesamtbelastung der drei Erkrankungen deutlich über der der Allgemeinbevölkerung liegt und vergleichbare Muster aufweist. Bei der chronischen HCV-Infektion zeigte sich ein signifikanter Zusammenhang von Fibroseprogression, länger dauerndem Krankheitsbewusstsein und erhöhten Depressionswerten. Alle drei Erkrankungen führten zu psychischen Auffälligkeiten (z.B. bezüglich der erhobenen HADS-Scores): Während die Depression bei den HCV-Patienten überwog, war die Angst das dominierende Problem der AIH- und der HBV-Patienten. Bei letzteren wären mit Blick auf den weiterführenden Forschungsbedarf erneute Studien anzustreben, die die Angst in den Fokus der Betrachtung stellen, da deren Auswirkung auf die Therapie nicht zu unterschätzen ist. Für die Klinik bedeutet dies, dass es wünschenswert wäre, den neu erworbenen Wissensbestand bezüglich Depression und Angst der Patienten in Schulungen des klinischen Fachpersonals handlungsleitend zu integrieren und auf diesem Wege unter anderem einen Beitrag zu leisten, den Stigmatisierungstendenzen im Umgang mit den Erkrankungen entgegenzuwirken und die klinischen Betreuung weiter zu optimieren. / In the center of the research-leading interest the question was located whether there is a difference in the occurrence and the intensity of fear and/or depression as well as physical complaints between the groups of patients with different chronic liver illnesses (HBV, HCV, AIH). The focus of the interest stood here in the determination of the correlation between the progressing of the liver illness (hepatitis over fibrosis to cirrhosis) and the extent of depressive and fearful symptomatology. At the collection time all patients were without specific medicamentous treatment due to their liver illness. As a secondary study goal the research question should be answered whether here the four applied different collection instruments (HADS-D, SCL-90-R, IIP-C and SF-36) to lead to different results. On the basis of the histological data a connection between progression of the fibrosis, depressive symptomatology and decreased physical health could be represented in the group of the hepatitis C patients. Only the patients with chronic hepatitis C infection showed a correlation (r = 0.205) between the length of time since report of the primary diagnosis and the extent of the depressive symptoms. All three chronic liver illnesses were associated with increased fear values. The SCL-90-R showed similar results for the psychological stress by all three chronic liver illnesses. Who was larger than by patients after myocardial infarction. In summary one can say that the measured psychological total cost of the three illnesses is higher as in the general population. With the chronic HCV infection a significant connection showed up of fibrosisprogression, longer lasting disease consciousness and higher depression values. While the depression outweighed with the HCV patients, the fear was the dominating problem of the AIH and the HBV patients.

Modification to the immunodominant loop of hepatitis B virus core protein to enhance vector properties of virus-like particles

Gene therapy has shown potential in alleviating a wide range of diseases, ranging from viral infections to autosomal diseases. One of the obstacles to gene therapy reaching its full potential is the inadequacy of methods to deliver therapeutic nucleic sequences. Current delivery of gene therapy entails use of viral and non-viral vectors. Viral vectors are however associated with drawbacks such as potential toxicity, high cost and labour-intensive production. Thus non-viral delivery alternatives are being developed in an attempt to overcome difficulties associated with nucleic acid delivery for gene therapy. Virus-like particles are potentially very useful delivery vehicles as their production is simple and cost effective, the particle surface is amenable to modification and the capsid interior can be altered to accommodate a variety of cargoes. One such particle is the recombinant HBV capsid, which comprises a single species of protein and is tolerant of amino acid substitutions on the surface exposed immunodominant loops. This study aimed to enhance the vector-like properties of the HBV virus-like particle by including amino acid substitutions on the particle surface. These substituted residues in turn provided a conjugation site for tropic and immuno evasive moieties. It was found that lysine substitutions resulted in poor conjugation to the capsid surface, whereas substituted cysteine residues resulted in almost all protein-moiety conjugates forming. In order to introduce lysine and cysteine substitutions, a novel method of cloning into the HBV was generated. In doing so, complicated procedures associated with cloning into the immunodominant loop of the HBV capsid have been alleviated. Ligands containing galactose were utilised on the surface of both the HBV capsid and liposomes to confer hepatotropism. The presence of the galactose moieties on the surface of the HBV capsid prevented indiscriminate cellular uptake in cultured cells, however did not improve hepatotropism. Galactose ligands on the surface of liposomes did improve transfection efficiency, however they required a short linker distance between liposome surface and galactose group. The inclusion of galactose in liposome formulations also provided a means to deliver siRNA to the liver of transgenic HBV mice. It is believed that with alterations to the ligand structure, it is possible to provide HBV capsids with hepatotropism in future experimentation. This study demonstrated that the exposed external surface of the HBV capsid is amenable to convenient conjugation, which potentially facilitates immune evasion and conferring of defined tropism.

As hepatites virais continuam sendo uma preocupação em nível de saúde pública no Brasil e no Mundo, tanto pelo número de indivíduos atingidos, como pela possibilidade de complicação das formas agudas e crônicas. Segundo a Organização Mundial de Saúde (OMS), 170 milhões de pessoas são portadoras crônicas de hepatite C e 350 milhões portadoras crônicas de hepatite B. No Brasil, a estimativa de portadores de hepatite B crônica é de aproximadamente 600 mil pessoas e de hepatite C crônica, 1,5 milhão. Quanto à hepatite aguda A foram confirmados no país, em 2010, 5943 casos. O objetivo deste estudo foi conhecer a prevalência de marcadores sorológicos dos vírus das hepatites A, B e C em crianças e adolescentes matriculados em creches e escolas de ensino infantil e fundamental da rede municipal na cidade de Santos; conhecer os aspectos moleculares dos vírus das hepatites B e C, identificando o genótipo dos dois agentes e estudar modo de aquisição nos casos com sorologias positivas. Tratou-se de um estudo transversal realizado no período de 28 de Junho a 14 de Dezembro de 2007 onde foram coletadas 4680 amostras de sangue colhidas através de punção capilar e ao mesmo tempo aplicado um questionário nos familiares das crianças e adolescentes. Os exames sorológicos foram realizados utilizando a técnica de ELISA. O estudo molecular foi realizado pela técnica de reação em cadeia de polimerase \"in House\". A idade da população estudada variou de 7 meses a 18 anos e 1 m. A prevalência geral do anti-HVA IgG reagente foi de 9,7% e desses 74,6% foi anti-HVA IgM reagente. A prevalência de anti-HVA IgG foi maior entre as crianças mais velhas, meninas, aquelas que brincavam em córregos, sem esgoto em sua moradia, de pais com baixa instrução, de baixa renda familiar e aquelas que não eram moradoras da Orla. A prevalência de anti-HVA IgM, não foi diferente entre as diferentes categorias, exceção feita à faixa etária (pico no primeiros anos e posterior queda) e morro e Zona Noroeste foi mais baixa. A prevalência geral do anti-HBc reagente foi de 0,1%, do AgHBs de 0,02% e do anti-HVC foi de 0,02%. Conclui-se que a prevalência geral em crianças dos marcadores sorológicos para hepatites A, B e C na cidade de Santos foi baixa, quando comparada com os dados de literatura. Apesar dos nossos dados confirmarem uma mudança no perfil epidemiológico da hepatite A, as medidas preventivas atuais quanto ao saneamento, grau de instrução, habitação, ainda permanece com uma deficiência em Santos, cidade balneária com o maior porto do Brasil. A vacinação para hepatite B foi altamente eficaz com a baixa prevalência encontrada dos marcadores sorológicos. A utilização do papel de filtro em estudos epidemiológicos para hepatite A foi eficaz. It has been confirmed in the country, in 2010, 5943 cases of acute hepatitis A. The The aim of this study was to learn the prevalence of serological markers of hepatitis A, B and C virus in children and teenagers enrolled at the municipal education network in the city of Santos, to learn molecular aspects of hepatitis B and C, identifying the genotype of the two agents and to study the acquisition mode in cases with positive serology. Cross-sectional study carried out over the period from June 28 to December 14, 2007, in which 4680 fingerprick blood samples were collected; at the same time, a survey questionnaire was applied to the family members of the children and teenagers. Age of the population studied ranged from 7 months to 18 years and 1 month. . There was higher prevalence among older children, females, those who used to play in streams near their home, the absence of a sewage system in home, parents with low education, low household income and among those who did not live in the seashore. The prevalence of anti-HAV IgM was not different between the categories, except for the age (peak in the early years and subsequent fall) and lower on the Hills and Northweast Zone. The general prevalence of anti-HBc reagent was 0,1%, AgHBs was 0,02% and anti-HCV was 0,02%. It is concluded that, in children, the general prevalence of serological markers for hepatitis A, B and C in the city of Santos was low when compared with literature data. Although our data confirm a change in the epidemiological profile of hepatitis A, the current preventive measures regarding sanitation, education level and housing still remain with a deficiency in Santos, the coastal city with the largest harbor in Brazil. Vaccination for hepatitis B was highly effective because it found a low prevalence of serological markers. The use of filter paper in epidemiological studies for hepatitis A was effective. However for the vírus of hepatitis C still requires comparative studies using venous blood because the prevalence of infected children was very low in the city of Santos.

Development of novel vaccine strategies for duck Hepatitis B virus infection.

Hepatitis B virus (HBV) is a life-threatening pathogen with major economic significance. Acute infection in adults is common, albeit usually self-limiting. Importantly, infection in infants typically results in chronic infection and increased incidence of hepatocellular carcinoma (HCC). Furthermore, the infectious carrier state is perpetuated in chronically infected individuals. Successful immuno-therapeutic vaccination would reduce the incidence of chronic infection and of HCC as well as reduce transmission of the disease. Recovery from acute and chronic HBV infection typically occurs in the presence of robust antigen-specific humoral and cellular immune responses (CMI), whereas these responses are low or absent in chronically HBV-infected individuals. Therefore, it was hypothesised that effective stimulation of both humoral and CMI responses, in conjunction with currently available antiviral therapies, may contribute significantly to development of vaccines for treatment of chronic HBV infection. The duck hepatitis B virus (DHBV) model of HBV infection was used to test novel vaccine strategies that could complement existing antiviral therapeutic approaches to treat chronically HBV-infected humans. To this end, three separate vaccine studies were conducted to investigate potential therapeutic regimes. Methods to assess the efficacies of the vaccine strategies included immunoperoxidase detection of viral antigen and immune cell markers within the liver and development of sensitive assays to monitor levels of DHBV DNA, duck hepatitis B virus surface antigen (DHBsAg), antibodies to duck hepatitis B core (anti-DHBc) and surface antigens (anti-DHBs) in serum were developed and validated which allowed monitoring of the kinetics of the humoral immune response following vaccination and the course and outcome of experimental DHBV infection. The first vaccine study tested the protective efficacy of DNA vaccines encoding either the small form of DHBsAg (DHBs) protein or the larger antigen (DHBpre-S/S), These were administered to ducks at day 4 and 14 of age. On the same day as the second vaccination, ducks were challenged intravenously with DHBV. Immunoperoxidase staining of biopsy tissue collected at day 4 p.i. showed significant decreases in the number of DHBV infected hepatocytes in ducks receiving the DNA vaccines compared to the mock-vaccinated control ducks. Significant protection against development of chronic DHBV infection was observed in ducks vaccinated with DNA vaccines expressing either pre-S/S or S protein. Although anti-DHBs antibodies were not detected prior to DHBV challenge, the decrease in the percentages of DHBV-infected hepatocytes at day 4 p.i is suggestive that neutralisation of the inoculum by low-level anti-DHBs antibodies in cohort with CMI responses induced by vaccination were the most probable mechanisms of action. The second vaccine study examined the protective efficacy of a novel whole-cell vaccine that expressed the DHBV core antigen (DHBcAg). Ducks were vaccinated on day 4 and 14 of age and DHBV challenge was administered 4 days later. Detectable anti-DHBc antibodies were generated as soon as 4 days after the initial vaccination suggesting that this regimen elicited increased immunogenicity than vaccination with DNA vaccines alone. In contrast to the first vaccine study with DNA vaccines expressing DHBsAg, no significant differences in the percentage of DHBV-infected hepatocytes were observed in biopsy tissue collected at day 4 p.i.This finding is confirmation that anti-DHBc antibodies were not neutralising to the initial DHBV inoculum. However, significant protection against development of chronic DHBV infection was observed in the whole-cell vaccinated ducks suggesting that the mechanism of protection was consistent immune-mediated killing of DHBV-infected hepatocytes following CMI responses to determinants of DHBcAg. The final vaccine study involved a combination strategy of antiviral drug Entecavir (ETV) and prime-boost vaccination with DNA vaccines and recombinant fowlpoxvirus (rFPV) expressing DHBV antigens. Immediately following DHBV infection, ducks were dosed by oral gavage with the antiviral drug Entecavir (ETV) and at the same time received the priming DNA vaccines encoding DHBV antigens. Seven days later the boosting vaccination consisting of recombinant fowlpox viruses (rFPV) also expressing DHBV antigens was administered. Development of novel vaccine strategies for duck Hepatitis B virus infection.

Hepatitis B virus (HBV) is a life-threatening pathogen with major economic significance. Acute infection in adults is common, albeit usually self-limiting. Importantly, infection in infants typically results in chronic infection and increased incidence of hepatocellular carcinoma (HCC). Furthermore, the infectious carrier state is perpetuated in chronically infected individuals. Successful immuno-therapeutic vaccination would reduce the incidence of chronic infection and of HCC as well as reduce transmission of the disease. Recovery from acute and chronic HBV infection typically occurs in the presence of robust antigen-specific humoral and cellular immune responses (CMI), whereas these responses are low or absent in chronically HBV-infected individuals. Therefore, it was hypothesised that effective stimulation of both humoral and CMI responses, in conjunction with currently available antiviral therapies, may contribute significantly to development of vaccines for treatment of chronic HBV infection. The duck hepatitis B virus (DHBV) model of HBV infection was used to test novel vaccine strategies that could complement existing antiviral therapeutic approaches to treat chronically HBV-infected humans. To this end, three separate vaccine studies were conducted to investigate potential therapeutic regimes. Methods to assess the efficacies of the vaccine strategies included immunoperoxidase detection of viral antigen and immune cell markers within the liver and development of sensitive assays to monitor levels of DHBV DNA, duck hepatitis B virus surface antigen (DHBsAg), antibodies to duck hepatitis B core (anti-DHBc) and surface antigens (anti-DHBs) in serum were developed and validated which allowed monitoring of the kinetics of the humoral immune response following vaccination and the course and outcome of experimental DHBV infection. The first vaccine study tested the protective efficacy of DNA vaccines encoding either the small form of DHBsAg (DHBs) protein or the larger antigen (DHBpre-S/S), These were administered to ducks at day 4 and 14 of age. On the same day as the second vaccination, ducks were challenged intravenously with DHBV. Immunoperoxidase staining of biopsy tissue collected at day 4 p.i. showed significant decreases in the number of DHBV infected hepatocytes in ducks receiving the DNA vaccines compared to the mock-vaccinated control ducks. Significant protection against development of chronic DHBV infection was observed in ducks vaccinated with DNA vaccines expressing either pre-S/S or S protein. Although anti-DHBs antibodies were not detected prior to DHBV challenge, the decrease in the percentages of DHBV-infected hepatocytes at day 4 p.i is suggestive that neutralisation of the inoculum by low-level anti-DHBs antibodies in cohort with CMI responses induced by vaccination were the most probable mechanisms of action. The second vaccine study examined the protective efficacy of a novel whole-cell vaccine that expressed the DHBV core antigen (DHBcAg). Ducks were vaccinated on day 4 and 14 of age and DHBV challenge was administered 4 days later. Detectable anti-DHBc antibodies were generated as soon as 4 days after the initial vaccination suggesting that this regimen elicited increased immunogenicity than vaccination with DNA vaccines alone. In contrast to the first vaccine study with DNA vaccines expressing DHBsAg, no significant differences in the percentage of DHBV-infected hepatocytes were observed in biopsy tissue collected at day 4 p.i.This finding is confirmation that anti-DHBc antibodies were not neutralising to the initial DHBV inoculum. However, significant protection against development of chronic DHBV infection was observed in the whole-cell vaccinated ducks suggesting that the mechanism of protection was consistent immune-mediated killing of DHBV-infected hepatocytes following CMI responses to determinants of DHBcAg. The final vaccine study involved a combination strategy of antiviral drug Entecavir (ETV) and prime-boost vaccination with DNA vaccines and recombinant fowlpoxvirus (rFPV) expressing DHBV antigens. Immediately following DHBV infection, ducks were dosed by oral gavage with the antiviral drug Entecavir (ETV) and at the same time received the priming DNA vaccines encoding DHBV antigens. Seven days later the boosting vaccination consisting of recombinant fowlpox viruses (rFPV) also expressing DHBV antigens was administered. Mudança na suscetibilidade à hepatite A em crianças e adolescentes de Porto Alegre na última década

Objetivos: Estimar a prevalência de anticorpos contra hepatite A (anti-HAV) em grupo de crianças e adolescentes de laboratório público e privado em Porto Alegre e comparar com estudo realizado na década anterior. Métodos: Entre 2007 e 2008 foi realizado estudo transversal onde foram incluídas, consecutivamente, 465 amostras de soros de crianças e adolescentes entre 1 e 19 anos de idade para determinar a prevalência de anticorpos anti-HAV total. As amostras foram fornecidas por laboratório público (Grupo 1), que atende somente Sistema Único de Saúde, e por laboratório privado (Grupo 2), representando os estratos socioeconômicos mais baixo e mais alto, respectivamente. O teste foi realizado em único laboratório (eletroquimioluminescência - Roche Diagnostics). Resultados > 20 UI/L foram considerados positivos. Resultados: A soroprevalência de anti-HAV no grupo 1 foi de 37,6% e o percentual de positividade aumentou conforme a idade, variando de 19,4% entre 1-4 anos a 54,1% entre 15- 19 anos. No grupo 2, a frequência de anti-HAV foi de 46,1% e foi inversamente relacionada à idade, caindo de cerca de 50,0% nas faixas etárias menores para 29,1% aos 15-19 anos. Houve diminuição significativa na prevalência do anti-HAV nas crianças de 5-9 anos do grupo 1 (P=0,03), quando comparadas com estudo realizado na década de 1990. Conclusões: Os resultados sugerem queda na endemicidade da hepatite A em Porto Alegre na última década e indicam maior suscetibilidade à doença em crianças e adolescentes, principalmente no estrato socioeconômico mais baixo. / Objectives: To estimate the prevalence of anti-hepatitis A virus (anti-HAV) antibodies in serum samples from children and adolescents obtained at two clinical pathology laboratories (one public, one private) in the city of Porto Alegre, state of Rio Grande do Sul, Brazil, and compare findings to those of a study carried out in the 1990s. Methods: In this cross-sectional study conducted between 2007 and 2008, 465 serum samples obtained from children and adolescents (1-19 years) were consecutively tested to determine the prevalence of total anti-HAV antibodies. Samples were provided by a public laboratory (Group 1) that serves the Unified Health System exclusively, meant to represent the lowest socioeconomic strata, and by a private laboratory (Group 2), meant to represent the higher socioeconomic classes. Tests were performed at a single laboratory using commercially available electrochemiluminescence kits. Antibody levels ≥ 20 UI/L were considered positive. Results: The seroprevalence of anti-HAV in Group 1 was 37.6%. The percentage of anti- HAV reactivity increased with age, ranging from 19.4% in the 1-to-4-year group to 54.1% in the 15-to-19-year group. In Group 2, the overall frequency of anti-HAV positivity was 46.1% and was inversely correlated with age, declining from roughly 50% in the youngest age groups to 29.1% in the 15-to-19 group. Comparison of sample findings to those reported in a 1990s study showed a significant reduction in anti-HAV prevalence among 5-to-9-year-olds in Group 1 (p = 0.03). Conclusions: The results suggest that the endemicity of hepatitis A in Porto Alegre has been declining over the past decade, and that children and adolescents, particularly those in the lowest socioeconomic strata, are more susceptible to the disease.
