Changes of the immune system in the pathogenesis of primary Sjögren's syndrome / Imuninės sistemos pokyčiai pirminio Sjögreno sindromo patogenezėje

There are many studies done to determine factors what can cause susceptibility to Sjögren's syndrome. Despite intensive research of the immune system, the model of the Sjögren's syndrome pathogenesis is not completely clear. Lymphopenia is a common symptom found in the pSS patients. Numerous studies are performed in order to determine the causes of lymphopenia, but there is a lack of detailed studies to reveal which cell population counts increase or decrease. Scarce studies are done to associate the changes in the immune cell population and the expression of humoral factors in the peripheral blood of pSS patients. The aim of dissertation work was to investigate the changes of the components of the systemic immune response in the peripheral blood of patients with primary Sjögren's syndrome (pSS) with different manifestations of the disease. In this study, a comprehensive analysis of B, NK and T cell populations and humoral factors in the peripheral blood of pSS patients was performed. For the first time the expression of CD57 and CD27 markers on CD8+ T cell population was analyzed. For the first time Th17/Th1-like cells in the peripheral blood of pSS patients were identified and imbalance in the distribution of T helper cell population was revealed. It can be explained by the significant increase of Th17/Th1-like lymphocyte population. Étude du lymphocyte B au cours du rejet d'allogreffe rénale

Le rejet d'allogreffe représente un obstacle majeur en transplantation rénale humaine. Le lymphocyte B (LB) joue un rôle lors de cette réaction contre le greffon, mal défini à ce jour. Notre objectif a été de caractériser et identifier son implication dans le rejet humoral chronique (cABMR) et le rejet cellulaire aigu (ACR). Dans une première partie, la caractérisation phénotypique des LB par cytométrie en flux chez ces patients a mis en évidence d'importantes différences dans la distribution des sous-populations de LB uniquement chez les patients cABMR. Chez les patients ACR, dont la distribution des LB n'est pas altérée, l'analyse de coupes de biopsies rénales a permis de mettre en évidence un infiltrat cellulaire constitué de lymphocytes B et T. Dans une seconde partie, l'activité fonctionnelle et régulatrice des LB issus de patients cABMR et ACR a été évaluée à l'aide d'un modèle in vitro de coculture entre des LB et des LT. Cette étude a révélé que les LB, issus des patients cABMR uniquement, sont dépourvus d'activités régulatrices sur la fonction des LT autologues. Cette étude a aussi mis en exergue que les LB des patients cABMR présentaient une déficience dans la sécrétion de molécules immunosuppressives telles que le TGFβ et l'indoleamine 2,3-dioxygénase (IDO). Ce défaut conduit à une incapacité à générer des lymphocytes T régulateurs. Finalement, notre étude a clairement démontré le rôle du LB dans les mécanismes physiopathologiques conduisant au rejet. Ces travaux ont donc permis de générer d'éventuelles perspectives pour définir de nouvelles stratégies thérapeutiques dans la lutte contre le rejet d'allogreffe.

Magister Scientiae - MSc (Medical BioSciences) / In the past three decades there has been a huge increase in the use of herbal medicine globally. The active principles of these herbal medicines are mostly unknown with supportive evidence for safety and efficacy very rare. SeptilinTM is a phytopharmaceutical formulation which is recommended for the treatment and management of various infections. It has been claimed to have immunomodulatory actions that potentiates the body's immune response. The immunomodulatory activity of SeptilinTM has not been well investigated via appropriate in vitro models. Therefore this study was undertaken to investigate the in vitro effects of SeptilinTM on biomarkers of specific immune pathways by using WBC. Stimulated and unstimulated WBC were incubated with the product. Enzyme linked immunosorbent assays were used to screen for IL-6, IL-10, and IFN? as biomarkers for inflammation, humoral immunity, and cell mediated immunity, respectively. Results show that the presence of SeptilinTM in LPS stimulated WBC has no effect on the release of IL-6 and IFN? production but stimulated IL-10 production. SeptilinTM in unstimulated WBC has no effect on the release of IL-10 and IFN? production but stimulatory effects on IL-6 production.

Estudos anteriores neste importante hospedeiro da cadeia epidemiológica de N. caninum demonstram que as respostas de anticorpos IgG são tardiamente detectadas e que a infecção clínica é de difícil indução. Desta forma, este trabalho objetivou o estudo da imunidade de cães frente à infecção oral por N. caninum. Os resultados obtidos a partir de análises de diversos animais experimentalmente infectados indicam que os cães apresentam uma prolongada fase aguda da infecção, com eliminação de oocistos associado à queda nos níveis de linfócitos T CD4+ e CD8+ e diminuição de MHC de classe II por células apresentadoras de antígeno. Adicionalmente, os animais apresentam soroconversão instável durante o mesmo período, sendo que somente IgG1 e IgG3 foram detectados em adultos e filhotes, respectivamente, entre o 2o e 3o mês de infecção. De forma concomitante, observa-se uma predominância da expressão de citocinas imunomoduladoras como TGF 1, IL-4 e IL-10. Após dois meses de infecção, o perfil da resposta se inverte, sendo observado picos de produção dos marcadores CD4 e CD8 de linfócitos T e citocinas próinflamatórias como IFN , IL-6 e IL-12, além do aumento nos títulos de anticorpos, principalmente IgG1 e IgG4 nos cães jovens. Com base nestes resultados, conclui-se que os cães apresentam uma relação de equilíbrio com N. caninum, a qual induz nesta espécie uma modulação da resposta imunológica durante a fase de merogonia. / Neospora caninum is an Apicomplexan parasite firstly described as the cause of encephalomyelitis in puppies serologically negative to Toxoplasma gondii. Previous reports on the parasite s definitive host indicate a late IgG antibody response and that clinical disease is difficult to be induced. The aim of this study was to investigate canine immunity during N. caninum oral infection. The results obtained from the analysis of infected animal s samples indicate that dogs present a protracted acute phase, with oocyst shedding correlated to a drop in CD4+ and CD8+ T cell levels, and low MHC class II expression by antigen presenting cells. Additionally, the dogs presented an unstable seroconversion pattern in the same period, with only IgG1 and IgG3 being detected in adult dogs and puppies, respectively, between the second and third months of infection. Concomitantly, dominant Th2 cytokine expression was observed, with peak expression levels of TGF 1, IL-4 and IL-10. After 2 months of infection, the immunity profile shifts towards a Th1 response, with high levels of CD4 and CD8 lymphocytary marker production and pro-inflammatory cytokine expression (IFN , IL-6 and IL-12), besides of the raise in antibody levels, especially IgG1 and IgG4 in puppies. Based in the results presented herein, we may conclude that dogs present a balanced host-parasite relationship, modulating the host immune response during N. caninum merogony.

Each inoculation was done with intervals of 30 days. In the 30th and 34th days after the 3th inoculation, it was applied in each animal, 4000 larvaes of R. (B.) microplus, free of blood parasites and also, it was inoculated a sample of Babesia bovis UFV1 9th passage with a concentration of 1x106 babesias/mL, respectively. According to the results, the animals inoculated with the Group II obtained greater clinic response after challenging with both parasites. Besides, these animals showed high levels of total IgG specific to the SBbo23290, beginning one week after 1st inoculation, with titles of 1,06 ± 0,2486. The titles with high affinity of total IgG to SBm7462 was presented from the 8th week, with levels of 1,75 ± 0,1849 and 1,13 ± 0,2819 to Groups I and II, respectively. Both Group showed high levels of IgG1 over IgG2 isotopes with high affinity to SBbo23290. The Group I showed titles of total IgG and IgG1 with high affinity to SBm7462 and low efficiency (EF) to control the ticks, different of the obtained in the Group II, where it was found high titles of total IgG, low levels of IgG1 and EF of 46,80 %. These results suggest that, intraclonal competition was present after presentation by APCs (DC or WC1.1+) to CD4+ and lymphocytes B in the germinal centers, with major affinity by specific clones to SBbo23290 over SBm7462. It is possible that these changes were determined by antigen solubility. / Avaliou-se a resposta imunológica conferida pelos peptídeos SBbo23290 e SBm7462 para o controle de Babesia bovis e Riphicephalus (Boophilus) microplus, ao serem aplicados simultaneamente na apresentação individual (Group I) e em associação (Group II). Foram realizadas três aplicações em cada grupo, via subcutânea, com intervalos de 30 dias. O desafio foi feito 30º e 34º dias após a 3ª inoculação, colocando-se 4000 larvas de R. (B.) microplus, cepa livre de hemoparasitas, e inoculando-se a cepa de B. bovis UFV1 9ª passagem numa concentração de 1x106 parasitas/mL, respectivamente. De acordo com os resultados obtidos, os animais do Grupo II, apresentaram uma melhor resposta clínica frente ao desafio, acompanhado de altos níveis de IgG total específicos para o peptídeo SBbo23290, a partir da 1ª semana após a inoculação, com títulos de 1,06 ± 0,2486. Os títulos de IgG total específicos para SBm7462 estiveram presentes a partir da 8ª semana com níveis de 1,75 ± 0,1849 e 1,13 ± 0,2819 para Grupo I e II, respectivamente. Ambos os tratamentos apresentaram maiores níveis de isotipos IgG1 sobre IgG2 específicos para SBbo23290. O Grupo I, apresentou títulos de IgG total e IgG1 específicos para SBm7462 e baixa eficácia (EF) no controle dos carrapatos, diferente do obtido no Grupo II, onde encontrou-se altos títulos de IgG total, níveis baixos do IgG1 e uma EF de 46,80 %. Sugere-se que estes resultados foram determinados por competição intraclonal em populações após a apresentação por APCs (DC ou WC1.1+) a CD4+ e linfócitos B nos centros germinais, com maior afinidade por clones específicos para SBbo23290 sobre SBm7462, talvez determinado por solubilidade antigênica.

A literatura mostra um número substancial de trabalhos que descrevem a influência do treinamento físico sobre o remodelamento ventricular em pacientes no contexto do pós-infarto agudo do miocárdio (IAM). Entretanto, essas publicações têm apresentado resultados conflitantes. O presente estudo teve como objetivo avaliar os efeitos do treinamento físico aeróbico de moderada intensidade, realizado em pacientes pós-IAM, sobre o remodelamento ventricular, e sua correlação com a ativação neuro-humoral. Foram avaliados 14 pacientes, de ambos os gêneros, idade média de 55,1 ± 10,8 anos, acometidos por um único IAM de parede anterior, divididos em dois grupos: grupo treinado (GT) (n=07) e grupo controle (GC) (n=07). O período de seguimento para esse estudo foi de 12 semanas. Antes e após o período de seguimento, os pacientes realizaram exames laboratoriais, avaliação antropométrica, coleta da freqüência cardíaca (FC) de repouso, teste cardiopulmonar e ressonância magnética cardíaca. O GT realizou treinamento físico aeróbico supervisionado em esteira ergométrica, três vezes por semana, com intensidade definida pela FC atingida no limiar de anaerobiose ventilatório. A análise estatística foi realizada através do teste não paramétrico da Soma de Postos de Wilcoxon (análise intragrupo) e teste não paramétrico de Mann-Whitney (análise intergrupo), com nível de significância de 5%. Não houve alteração estatisticamente significante no peso, índice de massa corporal, perfil lipídico e glicemia de jejum pré e pós-intervenção tanto no GC quanto no GT. Houve redução estatisticamente significante da FC de repouso no GT, sem alteração no GC. Observou-se aumento estatisticamente significante do consumo de oxigênio no limiar de anaerobiose e no pico do esforço, bem como, no incremento do pulso de oxigênio apenas no GT. Houve redução estatisticamente significante do volume diastólico final, e tendência a significância no volume sistólico final do GC. Não se observou mudanças estatisticamente significantes nos volumes cavitários pré e pós-intervenção no GT. Não houve mudança estatisticamente significante na fração de ejeção tanto no GT quanto no GC. Houve redução estatisticamente significativa do fragmento N-terminal do proBNP na condição basal e no pico do esforço tanto no GT quanto no GC. Concluímos que o treinamento físico aeróbico, de moderada intensidade, propiciou um aumento da aptidão física e cardiorrespiratória, manutenção dos volumes cavitários e da função cardíaca, e comportamento satisfatório do status neuro-humoral no GT durante o período do estudo. / The literature shows a substantial number of studies describing the influence of physical training on ventricular remodeling in patients in the context of post-acute myocardial infarction (AMI). However, these publications have produced conflicting results. The present study was to evaluate the effects of aerobic physical training of moderate intensity, performed in patients after MI on ventricular remodeling, and its correlation with neurohumoral activation. We evaluated 14 patients of both genders, mean age 55.1 ± 10.8 years, affected by a single anterior wall AMI were divided into two groups: trained group (TG) (n = 07) and control group ( CG) (n = 07). The follow-up period for this study was 12 weeks. Before and after follow-up period, patients underwent laboratory tests, anthropometric measurements, collection of heart rate (HR) at rest, cardiopulmonary exercise testing and cardiac MRI. The TG performed supervised aerobic exercise training on a treadmill three times a week, with intensity defined by HR reached at ventilatory anaerobic threshold. Statistical analysis was performed using the nonparametric Wilcoxon Sum of stations (intragroup analysis) and nonparametric Mann-Whitney test (intergroup analysis), with a significance level of 5%. There was no statistically significant change in weight, body mass index, lipid profile and fasting glucose and post-intervention in both the CG and TG. There was a statistically significant reduction in resting HR in TG, no change in CG. There was a statistically significant increase in oxygen uptake in the anaerobic threshold and peak effort, as well as an increase in oxygen pulse only in the TG. There was a statistically significant reduction in end-diastolic volume, and a tendency to significance in end-systolic volume of the CG. There was no statistically significant changes in cavity volume before and after intervention in TG. There was no statistically significant change in ejection fraction in both the TG and GC. There was a statistically significant reduction of the N-terminal fragment of proBNP at baseline and at peak exercise in both the TG and CG. We conclude that aerobic exercise training of moderate intensity, provided an increase in cardiorespiratory fitness and, maintenance of the cavity volume and cardiac function, and satisfactory performance of the neuro-humoral status in TG during the study period.
