Nutritionens betydelse för sårläkning : en litteraturstudie

<p>Syftet med denna litteraturstudie var att via litteraturen beskriva nutritionens betydelse för sårläkning. Studien genomfördes som en deskriptiv litteraturstudie där metoden bestod av att söka vetenskapliga artiklar i databaserna Medline, Cinahl och Academic Search Elite. Sjutton artiklar valdes ut. Resultatet visade att patienter med bensår eller trycksår i många fall inte når upp till de näringsrekommendationer som finns. Det visade sig vara vanligt att dessa patienter ligger vid gränsen för att vara undernärda. Det upptäcktes att det är sällan som kostintaget täcker energibehovet. Framför allt var zinknivåerna genomgående väldigt låga och det visar även att det är brist på en hel del andra ämnen. Ämnen som visat sig speciellt fördelaktiga för sårläkning är protein, vitamin C, zink samt arginin. För patienter med brännskador däremot har det visat sig att saltet ornithine α-ketoglutarate minskar proteinkatabolismen samt förbättrar proteinsyntesen, vilket är en förutsättning för sårläkning. Slutsatsen med föreliggande studie var att nutritionen är viktig och kan ha en avgörande betydelse för sårläkning.</p> / <p>The purpose with this literature study was that through literature describe the nutrition’s significance for healing wounds. Seventeen articles were chosen. The result showed that patients with leg ulcers or pressure sores in many cases don’t attain the recommendations for nutrition that are available. It showed that it is normal that these patients are on the border of being undernourished. It was discovered that the nutrient intake rarely covers the energy needs. Above all the zinc levels were throughout very low and that shows that there is also lack of some other substances. The substances that have been shown especially good for healing wounds are protein, vitamin C, zinc and arginine. However, for patients with burn injuries the salt ornithine α-ketoglutarate has been shown to reduce protein catabolism and also improve protein synthesis, which are essential to healing wounds. The conclusion from this study was that nutrition may be an important but also a determining factor to healing wounds.</p>

Fecal near-infrared reflectance spectroscopy calibrations for predicting diet quality and intake of donkeys

The objective of these studies was to develop near-infrared reflectance spectroscopy calibration equations from diet-fecal pair datasets to predict the diet quality and intake of donkeys. One hundred-forty diet-fecal pair samples were generated from two independent in vivo feeding trials conducted in the United States (N = 100) and Africa (N = 40). At each site, ten female donkeys were fed mixed diets blended from 25 forage and crop residues. The modified partial least square model (MPLS) was used to develop calibration equations for crude protein (CP), digestible organic matter (DOM), dry matter digestibility (DDM) and organic matter digestibility (OMD), for the US, Africa and US/Africa combined datasets, and dry matter (DM) and organic matter (OM) intake calibrations from the US datasets. Crude protein (CP) equations were developed with standard error of calibration (SEC) < 1.0 and coefficient of determination (R2) > 0.90, (SEL = 0.5). The US, US/Africa and Africa CP equations had SEC value of 0.77, 0.97 and 0.88 with corresponding R2 of 0.97, 0.95 and 0.88, respectively. Validation of the US CP equation resulted in a standard error of prediction (SEP) of 1.79 with corresponding coefficient of correlation (r2) of 0.82 and slope of 0.84 indicating high accuracy of prediction. In vivo derived DOM equations were also developed for the US, Africa and US/Africa datasets with SEC values of 2.58, 4.91 and 3.52, and R2 of 0.60, 0.81 and 0.84, respectively. In addition, the SEC and R2 values were 3.25 and 0.72 for US OMD, 3.28 and 0.79 for US DDM, and 4.2 and 0.85 for US/Africa OMD, and 4.3 and 0.87 for US/Africa DDM equation, respectively. Calibration equations for predicting DMI and OMI have resulted in SEC values of 3.45 and 3.21 (g/kgw0.75) and R2 values of 0.89 and 0.84, respectively. The present study explored the relationship between DMI and diet quality attributes. Crude protein and digestible organic matter to crude protein ration (DOM/CP) with r2 values of 0.60 and 0.39, respectively, have shown good correlations with intake. The present studies have confirmed the potential for the fecal NIRS profiling for predicting CP, DOM, DDM, OMD, DMI and OMI of donkeys. Both calibration and validation results have indicated that the present donkey equations were comparable to previously developed equations for ruminants; they have the capability for accurate prediction of diet quality and intake, and can be a useful tool for monitoring the nutritional well-being of donkeys with acceptable accuracy. Research works to further expand the present calibration equations with additional diet-fecal samples particularly from Africa that did not meet the required accuracy level is recommended.

Administrating poverty : Studies of intake organization and social assistance in Sweden

The general purpose of this dissertation is to study the causes and the consequences of the formal structure of intake of potential social assistance clients at Swedish social welfare offices. The focus lies on the social welfare offices, their organizational framework and routines concerning intake. A focus on the formal structure of the intake may provide information about the importance of organization for people seeking help but also for the municipalities themselves. The data used in the analyses comes from two surveys of welfare offices augmented with register data. Study 1 examines the very first contact between social assistance inquirers and the social welfare offices. Telephone intake and first personal visits are documented and a considerable variation in the share of inquirers who received an appointment for further assessment was found. Considerable variation was also found in the share who were granted social assistance after the assessment during the personal visit. An examination of the offices’ intake routines and organization provided some indications that the unequal priority given to intake is an important explanation behind the variation. Study 2 analyzes the link between intake organization and the degree of selection taken place at telephone intakes by focusing on those inquirers not becoming clients. The relationship between intake organization and the social workers’ reasons for selection is examined and several selection strategies could be found. The results confirm the fact that Swedish municipalities have great autonomy in designing the social services and in addition show that offices within the same municipality may choose different organizational solutions. The aim of study 3 is to investigate how intake of social assistance inquirers is organized in Swedish municipalities and what factors determine intake organization. The results show that there are three different intake types. One of them, called special intake units, is distinct from the other two in that the intake staff has relatively high qualifications. Examining factors likely to affect the creation of special intake units, the results show that mainly professional and organizational factors related to the organization of work within the whole social assistance unit are important. The purpose of study 4 is to examine the connection between organizational factors and local social assistance expenditures in Swedish municipalities. The organization of the social assistance unit, in particular to the intake of social assistance inquirers, and its potential implication for local social assistance costs are emphasized. The results show a cost reducing effect of special intake units first when analyzed together with additional specialization and taking account for staff resources. Thus, specialized intake organization by itself does not play a cost reducing role, but in combination with certain other factors describing internal organization. In an introductory part the studies are located in a broader framework starting with a historical description of different strategies and classification systems traditionally used when distributing poor aid. The expansion of the welfare state changed the importance of a last safety net. Specific characteristics of public organizations administering social assistance are described and the legal framework is outlined focusing on the right to apply for social assistance is outlined. Different aspects of intake are then discussed: stages of the intake process, the organization of intake in Sweden and elsewhere, intake organization as a fashion, and possible functions of the intake. Finally, implications of the four studies with regard to access to benefit and the issue of specialization as well as further research are discussed.

Caregivers’ Attitudes Toward Milk Fat Type and Milk Consumption Among WIC Participants: An Exploratory Study

Factors such as parental/caregiver influences and socioeconomic status have been shown to impact food-related attitudes and behaviors. Consequently, these attitudes and behaviors affect health outcomes. The purpose of this study was to assess, using the Social Cognitive Theory (SCT), attitudes toward milk fat type and milk consumption among Texas WIC participants. Few studies, using this theoretical framework, have examined milk intake specifically among this population. Four hypotheses were proposed according to the theoretical model. The inclusionary criteria used for this study yielded a subset sample of 2,115; all cases included were Texas WIC participants. The results of this study show that caregivers' attitudes toward drinking and offering milk fat type are related. Caregivers' attitudes toward drinking milk fat type and the type of milk they drank were proven to be statistically significant. Similarly, caregivers' attitudes toward offering milk fat type and the milk fat type their children drank were proven to be significant. Caregivers' milk intakes were positively associated with children's milk intakes. It is evident that parental/caregiver modeling influences children's dietary habits. Parental/caregiver behaviors are important influences to consider when implementing nutrition education programs or intervention efforts, especially for participants of WIC. Improving caregivers' attitudes toward low-fat or fat-free milk intake can also contribute to healthier food-related choices.

The Relationship between Omega-3 Fatty Acid Intake and Muscle Soreness in Collegiate Rowers

The primary objective of this study was to determine if the intake of omega-3 fatty acids (O3FA), from self-selected foods and supplements, was associated with a reduction in muscle soreness in collegiate rowers. Several groups of collegiate rowers were identified as potential subjects and, using an IRB-approved protocol, 61 volunteers from this subject pool agreed to participate in the study. At the end of the first week of fall practice, subjects were asked to provide information on age, weight, height, ethnicity, rowing level, sun exposure, exercise regimen, pre- and peak-soreness and exertion levels, hydration status, and food/supplement intake. A Talag scale was used to determine the severity of muscle soreness, and a modified Borg scale was used to determine the exertion intensity level. A food frequency questionnaire using a 10-point Likert scale was used to assess food and supplement intake, with a special focus on O3FA. Subjects with a higher intake of EPA/DHA rich bottled supplemental fish oil, taken by spoon, had significantly higher soreness levels than those with lower intakes (p = 0.028). Higher soreness was also significantly associated with higher intake of EPA/DHA rich fish oil capsules (p < 0.001). Lower soreness was significantly associated with higher fruit intake (p = 0.030). These findings suggest that high intakes of O3FA from fish oils, regardless of liquid or capsule form, may be contraindicated for collegiate rowers wishing to reduce muscle soreness. Recommending fruit intake may be warranted for rowers wishing to reduce muscle soreness.

Between a rock and a hard place : a qualitative investigation of the experience of accessing counselling

Clients experiences in attempting to access counselling help have rarely been studied. As such, a full and clear understanding of clients experiences with accessing services, as well as identification of the barriers encountered by clients and clients ideas about what would make accessing more comfortable, have eluded researchers. Typically, the research focus has been quantitative investigations of the no-show phenomenon, whereby clients fail to arrive at pre-booked counselling appointments. Such studies have left 24% of the variance in the unexplained "other" category. More recently, researchers have begun to question whether or not mental health stigma impacts counselling attendance. The present study sought to describe the experience of individuals who self identified as having booked a counselling appointment within the previous 8-months and then had failed to attend. A basic interpretive qualitative research design (Merriam, 2002) was employed for the purpose of describing and understanding this phenomenon, with special emphasis given to identifying possible barriers to accessing counselling. Interviews with four middle class Caucasian adults aged 27-33; two of whom had accessed counselling previously and two of whom had not, were conducted. Transcripts were analyzed in terms of a shared meaning and descriptive categories (Kearney, 2001). Findings revealed that stigma, self-stigma, several fears, and some counselling practices functioned as barriers. However, participants also expressed positive emotions associated with reaching out and accessing counselling help and a desire for information about what to expect in counselling, whether they had accessed previously or not. The beginnings of a pattern associated with accessing counselling are discussed and implications for counselling practice and future research are described.

Olanzapine-induced Weight Gain: An Animal Model

Introduction: Using an animal model, we examined weight gain in rats exposed to olanzapine, as well as whether increased weight was associated with food intake, visceral fat and/or locomotion. Methods: Sprague-Dawley rats were chronically treated with olanzapine while being offered diets including standard chow, a high fat (60% fat) diet, and a high fat/high carbohydrate (42% fat; 42.7% carbohydrate) diet. Body weight, food intake, visceral fat and locomotor activity were measured. Results: Our findings related to weight gain are in line with other reports indicating that while olanzapine-induced weight gain can be observed, it does not mirror what is observed in humans on two levels: (i) it is not of the same magnitude, and (ii) it is more gender specific i.e., females greater than males. Conclusions: These data confirm that chronic treatment with olanzapine has varying effects on weight gain, food intake, visceral fat and locomotor activity.

Energy metabolism in the weanling pig : effects of energy concentration and intake on growth, body composition and nutrient accretion in the empty body

Dietary energy is the largest single cost in pork production. Accurate and current understanding of energy metabolism is crucial to production efficiency. The overall objective of this thesis was to evaluate the effects of dietary energy concentration and energy intake on growth, nutrient deposition rates and energy utilization in weaned pigs. In experiment 1, the optimum total lysine:DE ratio for weaned pigs was estimated at 4.27 and 4.46 g/Mcal for pigs growing from 7.5 to 12.8 kg and 7.5 to 22.5 kg BW, respectively. Experiment 2 determined if a more predictable growth, nutrient deposition and energy utilization in the weaned pig is achieved with NE or with DE. ADG either remained similar or was depressed with increased NE compared to the control (P < 0.05). Empty body protein content and deposition (PD) declined relative to the control (P < 0.05) and lipid content and deposition (LD) tended to increase (P < 0.10). Body composition and nutrient deposition rates were more correlated with determined NE concentration and intake compared with DE. The results of Experiment 3 indicated that amino acid intake impaired the growth of pigs when an energy intake restriction greater than 30% occurred. Experiment 4 investigated the interaction of dietary NE concentration and feeding levels (FL) on body weight gain, tissue (protein, lipid, ash, water) accretion rates and ratios. Growth performance was not affected by NE (P > 0.05) but increased with feeding level (P < 0.001). Energy intake increased with NE and FL (P < 0.001), but the efficiency of energy utilization for growth declined (P < 0.05). Empty body protein content declined (P < 0.05) while lipid content increased with NE (interaction, P < 0.05). Empty body PD was not affected by NE (P > 0.05) but both LD and LD:PD ratio increased (interaction, P < 0.001). These data suggest that when amino acid:energy ratio is optimal, increasing dietary energy concentration increased energy intake but does not improve PD and overall body weight gain of weaned pigs. However, body lipid content and LD were increased. Finally, NE offers an advantage over the DE in predicting the body composition and nutrient deposition rates rather than in overall BW gain.
