Die Adipositasprävalenz hat in den letzten Jahren in den Industrieländern signifikant zugenommen. Fettleibigkeit ist ein unabhängiger Risikofaktor für das Auftreten verschiedener Erkrankungen wie Schlaganfall, Lungenembolie, Myokardinfarkt und verschiedene Tumoren. Da kardiovaskuläre Erkrankungen in Industrieländern die häufigste Todesursache darstellen, stellt die Adipositas ein wachsendes Gesundheitsproblem dar und sollte Schwerpunkt intensiver Forschungsarbeit sein. Da die Adipositas in der Mehrzahl der Fälle von einer Hyperleptinämie begleitet ist, war das Ziel der vorliegenden Arbeit, die mögliche Rolle von Leptin als Thrombozytenagonist auf verschiedenen Ebenen zu analysieren. Die durchgeführten Experimente konnten eine Leptin-induzierte Aktivierung des Integrins αIIbβ3, dem thrombozytären Fibrinogenrezeptor, und eine Steigerung der Adhäsion von Thrombozyten an immobilisiertem Fibrinogen zeigen. Leptin ist weiterhin in der Lage, die ADP-induzierte Thrombozytenaggregation signifikant zu steigern. Das Adipokin führt ebenfalls zu einer Steigerung der Thromboxan-B2-Freisetzung aus Thrombozyten. Neben der Untersuchung des Einflusses von Leptin auf die Thrombozytenfunktion sollte in der vorliegenden Studie ebenfalls die thrombozytäre Signalkaskade unter Wirkung des Proteohormons beleuchtet werden. Zentrales Molekül in diesem Signalweg ist JAK2. Für diese Tyrosinkinase konnte eine biologische Relevanz mit Hilfe von in vitro Versuchen zu Adhäsion und Aggregation von Thrombozyten unter Einfluss von Leptin demonstriert werden. Auch STAT3 wird zeit- und dosisabhängig durch Leptinstimulation in Thrombozyten phosphoryliert, wobei seine Bedeutung für die Thrombozytenaktivierung noch ungeklärt ist. Weitere an der Signaltransduktion in Thrombozyten unter Wirkung von Leptin beteiligte Kinasen sind die Phosphatidylinositol-3-Kinase, AKT, Phospholipase Cγ2 und Proteinkinase C sowie die p42/44-MAP-Kinase, die p38 MAP-Kinase und die Phospholipase A2. Für die Aktivierung von PLCγ2, AKT und der p42/44-MPAK konnte eine Abhängigkeit von der Tyrosinkinase JAK2 gezeigt werden. Für diese intrazellulären Mediatoren konnte ebenfalls unter Verwendung von verschiedenen Tyrosinkinase-Inhibitoren die Bedeutung bei der Thrombozytenadhäsion an immobilisiertem Fibrinogen bzw. der Aggregation nachgewiesen werden. Die in dieser Studie aufgedeckten Kaskaden zeigen somit biologisch relevante Leptin-vermittelte Signalwege in Thrombozyten auf und sollen zu einem besseren Verständnis des Zusammenhangs von Hyperleptinämie und erhöhtem Thromboserisiko bei Adipositas beitragen.

Chronic intermittent hypoxia is used an experimental animal model to simulate the hypoxia occurring in sleep apnoea patients. The aim of this study was to measure plasma insulin and leptin concentrations and hypothalamic-pituitary-axis activity in rats exposed to either intermittent hypoxia (CIH) or sham hypoxia (SH) for fourteen days. To induce CIH plexiglass cylinders were flushed with 100% nitrogen for nine seconds every 90 seconds, seven hours/day. The rats were weighed each day during the exposure period. Venous blood samples for insulin and leptin were collected on days one, three, five, eight and fifteen. Faecal samples were collected to measure glucocorticoid metabolites. There was no significant difference in the daily change in body weight between the rats exposed to CIH compared to the rats exposed to SH (unpaired t-test). Plasma insulin concentrations were not affected by CIH. In both groups of rats plasma leptin concentrations were significantly higher on day fifteen compared to day five (p=0.03, unpaired t-test). Glucocorticoid metabolites were significantly increased in the intermittent hypoxia group on day two (p=0.003 one-way ANOVA). In conclusion, exposing normal weight rats to CIH for fourteen days resulted in a transient iv increase in HPA axis activity on day two and an elevation in plasma leptin levels, in both groups of rats, at day fifteen.

Appetite regulation by leptin

Obesity is a prevalent problem in modern society, which requires the upmost attention in the biomedical sciences. A leading cause of obesity related diseases is due to overeating, especially in industrialized countries. Leptin is the hormone that is secreted by fat cells responsible for communicating body nutritional status to the brain. Leptin interacts with other bodily systems such as the cognitive, digestive, neuronal, and endocrine systems. Leptin acts mainly on the Ob-Rb receptor in the arcuate nucleus of the hypothalamus and largely suppresses food intake and increases energy expenditure by activating Proopiomelanocortin and Cocaine- and amphetamine-regulated transcript (anorexigenic signals) neurons and by suppressing Neuropeptide Y and Agouti-related peptide (orexigenic signals) neurons, among other chemical signaling pathways. In both rodent and human studies, exogenous leptin administration resulted in elevated plasma leptin concentrations. When researchers tried to use leptin for weight reducing medical treatments in humans, the results show difficulty in establishing clinical efficacy. However, for diseases such as congenital leptin deficiency, obesity related leptin resistance, and lipodystrophy, medical treatments involving exogenous leptin have been relatively successful. The goal of this thesis is to give readers an understanding of leptin’s role in regulating appetite and the different leptin associated diseases. Leptin’s role is still continuing to be developed and more research is needed to fully utilize leptin for therapeutic benefit.
