Associações entre níveis séricos de fatores de crescimento, insulina e leptina com fatores prognósticos de câncer do mama

Dentre os fatores contribuintes para o desenvolvimento do câncer de mama estão o sedentarismo, a alta ingesta calórica, o sobrepeso e a obesidade, entre outros. O objetivo do presente estudo foi avaliar a relação entre os níveis séricos de IGF-1, IGFBP3, insulina e leptina com fatores prognósticos clínicos e patológicos do câncer de mama. Foi realizado um estudo transversal com 77 pacientes com diagnóstico recente de câncer de mama e indicação de quimioterapia, atendidas no Hospital Fêmina, em Porto Alegre, de agosto de 2009 a setembro de 2010. Foram coletados dados antropométricos e revisados os prontuários para a obtenção de dados clínicos e anátomo-patológicos. Foi realizada uma coleta de sangue antes da administração do primeiro ciclo de quimioterapia, para realizar dosagens de IGF-1, IGFBP3, insulina e leptina. Os níveis de IGFBP3 foram maiores em pacientes cujos tumores expressavam receptores hormonais, em comparação àquelas cujos tumores não expressavam tais receptores (p=0,004 e p=0,005, respectivamente). Não houve diferenças nos níveis de IGF-1 e IGFBP3 em relação às classes do índice de massa corporal (IMC). Houve correlação positiva entre níveis de insulina e leptina com IMC. Houve correlação negativa entre níveis de IGF-1 e relação cintura-quadril (RCQ) (correlação de Pearson = - 0,348; p=0,007). Houve correlação positiva entre níveis de insulina e leptina com circunferências abdominal, umbilical, da cintura e do quadril. Neste estudo, foram encontradas correlações positivas entre os níveis de fatores de crescimento e leptina com medidas antropométricas em pacientes com câncer de mama, sugerindo um papel de tais fatores no sobrepeso e na obesidade. Níveis mais elevados de IGFBP3 em pacientes com expressão de receptores hormonais no tumor sugerem que o estado hormonal possa contribuir para o comportamento deste fator de crescimento e de sua cascata, embora ainda não se conheça o significado clínico de tal associação. / Introduction: There is growing evidence that sedentary lifestyle, high caloric intake, overweight, obesity and its related growth factors have an important role in the risk of developing breast cancer. Objectives: to evaluate the relation among serum levels of insulin, IGF-1, IGFBP3, leptin and VEGF with clinical and pathologic breast cancer prognostic factors. Material and methods: a transversal study was performed at a public hospital in Southern Brazil, from August 2009 to September 2010. Body composition measurements were performed, pathologic review of specimens were done and blood samples were taken before chemotherapy to evaluate insulin, IGF-1, IGFBP3 and leptin. Results: 77 patients entered the study. There were higher levels of IGFBP3 in ER-positive and PR-positive tumors when compared to those ER-negative and PR-negative (p=0.004 and p=0.005, respectively). There were no differences in the levels of IGF-1 and IGFBP3 according to BMI classification. There was a positive correlation of insulin and leptin levels with BMI classification. There was a negative correlation of IGF-1 levels and waist-to-hip (WTH) ratio (Pearson’s correlation= - 0.348; p=0.007). There was a positive correlation of insulin and leptin levels with waist, hip, abdominal and umbilical circumferences. Conclusion: we found positive correlations of growth factors and leptin with anthropometric measurements in patients with breast cancer, suggesting a role of these factors in overweight and obese patients. Higher levels of IGFBP3 were found in patients with hormone receptor-positive breast cancer when compared with those with no expression of hormone receptors. Although we need further research on the clinical significance of this finding, we suggest that hormonal status may contribute to the behavior of IGFBP3 and its pathways.
