Age-Related Alterations of Active Pumping Mechanisms in Rat Thoracic Duct

Objective: To evaluate the age-related changes in active pumping in thoracic duct (TD) from 24-month-old Fisher-344 rats comparing with TD pumping in 9-month rats. Methods: Lymphatic diameters, contraction amplitude and frequency, ejection fraction, and fractional pump flow were determined in isolated TD preparations. Western blot analyses were performed to evaluate relative levels of eNOS and iNOS in 9- and 24-month-old TD. Results: Stretch-dependent regulation was altered in aged TD especially at higher levels of pressure: the negative inotropy, negative chronotropy and diminished minute pumping (2- to 3-fold decrease) were observed. Physiological NO/imposed-flow-dependent inhibition was completely abolished in aged TD, yet NO-synthase blockade by l-NAME (10-4 M) increased pumping in a flow-independent manner. Western blot analyses indicated that the relative levels of eNOS were decreased ∼ 7-fold in the 24-month-old TD when compared with 9-month-old TD; whereas iNOS levels were increased ∼ 10-fold in 24-month-old TD. Conclusions: These data provide the first evidence that stretch- and imposed-flow-dependent regulatory mechanisms are greatly altered in aged TD. These alterations of active pumping mechanisms in TD appear to be related with age-related disturbances in NO-dependent regulatory pathways, and may reflect diminished lymphatic muscle contractility as well as altered lymphatic endothelium function.

Peeling away the layers to anemia

Anemia is a significant public health issue that affects a great number of people in developed and developing countries. The World Health Organization states when the Hb value is/dL in an adult male and/dL in a nonpregnant female, the individual is considered as anemic. Iron deficiency is one of the most common causes of anemia. Inadequate intake of iron, chronic blood loss, and/or a combination of both factors typically lead to iron deficiency anemia. In developed countries, chronic blood loss from gastrointestinal, genitourinary, and gynecological sources are the most common etiologies of iron deficiency anemia. Although there are reports of iron deficiency anemia leading to self-inflicted skin excoriation, there are few cases of chronic blood loss from skin excoriation resulting in severe iron deficiency anemia. We present a 49 year old female with significant past medical history of depression, anxiety and chronic back pain who presented after she was found to have profound anemia with hemoglobin of 4.1g/dl. During interviewing, she denied hematuria, hemoptysis, hematochezia, and had been without menstrual cycles for the past year. Urinalysis was negative for blood as well as two documented negative fecal occult blood tests. Iron studies completed showed severely reduced iron levels. Upon further interviewing, the patient reported a supposedly self-diagnosed keratin disorder. For the past ten years she has been self-treating the keratin disorder by applying topical tretinoin and then wrapping it in saran wrap. She would then peel off areas of skin over multiple areas of her body including all extremities and her face. Multiple pictures of bloody piles of tissue were shown. The patient required 3 units of packed red blood cells and was started on iron supplementation. Gastroenterology was consulted and agreed there was no GI source of her blood loss. Psychiatry further evaluated the patient and diagnosed her with obsessive-compulsive disorder with somatic delusions. The prevalence of anemia among chronic psychiatric patients is more frequent than general population. This coexistence deteriorates the quality of life of the patients, prolongs the psychiatric treatment period, and could even cause an increase in morbidity and mortality. Treatment-related factors, drugs taken, physical conditions, negative lifestyle habits, and nutritional disorders are the reasons for anemia among chronic psychiatric patients. Even though iron deficiency anemia in developed country is most often caused by chronic blood loss from gastrointestinal, genitourinary, and gynecological causes, it is important to evaluate for other factors when none of these are present. Psychiatric causes when warranted from history including somatic delusions from obsessive-compulsive disorder should be considered on the differential when other etiologies are less clear.

Title: The effect of manual lymphatic drainage on the musculus soleus muscle tension in patients with chronic venous insufficiency. Aims: The aim of this thesis is to summarize theoretical knowledge about chronic venous insufficiency, muscle tension and manual lymphatic drainage and then to perform a pilot study to determine whether people suffering from chronic venous insufficiency have changed muscle tone of musculus soleus and to verify whether the implementation of a single manual lymphatic drainage of the lower limbs results in a change of muscle tension of musculus soleus. Methods: There were three patients who participated in the experiment. Anthropometric examinations of circuits of both lower limbs on all patients was carried out, using measuring tape and then a muscle tone of musculus soleus was measured using a device called myotonometer. After that the single manual lymphatic drainage of one lower limb (more affected lower extremity) was performed. The second, untreated leg was considered to be the control limb. There was also performed basal treatment of the neck, before every manual lymphatic drainage of lower limb. Lymphatic vessel function in atherosclerosis

L'athérosclérose est une maladie inflammatoire chronique caractérisée par l'accumulation de cholestérol dans la paroi artérielle et associée à une réponse immunitaire anormale dans laquelle les macrophages jouent un rôle important. Récemment, il a été démontré que les vaisseaux lymphatiques jouent un rôle primordial dans le transport inverse du cholestérol (Martel et al. JCI 2013). L’objectif global de mon stage de maîtrise a été de mieux caractériser la dysfonction lymphatique associée à l’athérosclérose, en étudiant de plus près l’origine physiologique et temporelle de ce mauvais fonctionnement. Notre approche a été d’étudier, depuis l’initiation de l’athérosclérose jusqu’à la progression d’une lésion athérosclérotique tardive, la physiologie des deux constituants principaux qui forment les vaisseaux lymphatiques : les capillaires et collecteurs lymphatiques. En utilisant comme modèle principal des souris Ldlr-/-; hApoB100+/+, nous avons pu démontrer que la dysfonction lymphatique est présente avant même l’apparition de l’athérosclérose, et que cette dysfonction est principalement associée avec un défaut au niveau des vaisseaux collecteurs, limitant ainsi le transport de la lymphe des tissus périphériques vers le sang. De plus, nous avons démontré pour la première fois l’expression du récepteur au LDL par les cellules endothéliales lymphatiques. Nos travaux subséquents démontrent que ce défaut de propulsion de la lymphe pourrait être attribuable à l’absence du récepteur au LDL, et que la dysfonction lymphatique observée précocement dans l’athérosclérose peut être limitée par des injections systémiques de VEGF (vascular endothelial growth factor) –C. Ces résultats suggèrent que la caractérisation fonctionnelle de la capacité de pompage des vaisseaux collecteurs serait une condition préalable à la compréhension de l'interaction entre la fonction du système lymphatique et la progression de l'athérosclérose. Ultimement, nos travaux nous ont amené à considérer de nouvelles cibles thérapeutiques potentielles dans la prévention et le traitement de l’athérosclérose. / Atherosclerosis is driven by the accumulation of cholesterol in the arterial wall, which triggers an inappropriate immune response in which macrophages play an important role. It has now been shown that the lymphatic vessels play an important role in reverse cholesterol transport (Martel et al. JCI 2013). The overall objective of my Master internship was to better characterize lymphatic dysfunction associated with atherosclerosis, studying closely the physiological and temporal origin of this pathological feature. Our approach was to study, from the initiation of atherosclerosis to the progression of the atherosclerotic lesion, the physiology of the two main components that form the lymphatic vessels: the lymphatic capillaries and collectors. Using a mouse model that closely resembles human atherosclerosis (Ldlr-/-; hApoB100+/+) we have demonstrated that lymphatic dysfunction is present before the onset of atherosclerosis, and that this dysfunction is primarily associated with a defect in the collecting vessels, thereby limiting the lymph transport from peripheral tissues to the blood. In addition, we have clearly demonstrated, for the first time to our knowledge, the presence of the LDL receptor on lymphatic endothelial cells. Our subsequent work shows that this reduction in lymph flow could be due to the absence of the LDL receptor, and that lymphatic transport can be restored by systemic injections of VEGF (vascular endothelial growth factor) –C. These results suggest that the functional characterization of the pumping capacity of the collecting vessels would be a prerequisite for the understanding of the interactions between the function of the lymphatic system and the progression of atherosclerosis. Altogether, our work unveils new potential therapeutic targets for the prevention and treatment of atherosclerosis.

Este estudo avaliou a densidade vascular linfática (DVL) em relação a expressão do fator de crescimento endotelial vascular-C (VEGF-C) pelas células tumorais, bem como verificou a associação com variáveis clínicas e microscópicas de pacientes portadores de carcinoma espinocelular de boca (CEC). Oitenta e sete pacientes com CECs primários, clinicamente T1N0M0 ou T2N0M0, com metástase linfonodal oculta (pN+) e sem (pN0), foram analisadas após tratamento no Hospital do Câncer A.C. Camargo, São Paulo, Brasil. Amostras dos tumores emblocados em parafina foram seccionadas e coradas com os anticorpos primários anti-podoplanina e anti-VEGF-C humanos. Os vasos linfáticos foram contados nas regiões intratumoral (DLI) e peritumoral (DLP) em vinte campos microscópicos (400X) sequenciais e em cinco campos microscópicos (400X) por hot spot de cada tumor. As médias das densidades linfáticas, intratumoral e peritumoral, foram obtidas isoladamente e usadas como pontos de corte da amostra, dividindo-a em tumores com alta densidade linfática e baixa densidade linfática. As associações entre DLI / DLP e a expressão de VEGF-C ou em relação aos parâmetros clínicos e microscópicos avaliados foram obtidas pelo teste do qui-quadrado ou teste exato de Fisher. As taxas de sobrevidas, acumuladas em cinco e dez anos, foram calculadas pelo método de Kaplan-Meier e comparadas pelo teste log-rank (=5%). Não houve associação estatisticamente significativa entre a expressão de VEGF-C nos CECs de boca e os parâmetros clinicopatológicos avaliados. Os pacientes com alta DLP apresentaram maiores taxas de sobrevida específica por câncer (p=0,044) e sobrevida livre de doença (p=0,016). Houve associação estatística significativa entre a alta DLI e a maior ocorrência de recidiva regional nos pacientes da amostra (p=0,047). Entretanto, a alta DLI não foi suficiente para influenciar a sobrevida livre de doença desses pacientes do ponto de vista estatístico. No statistically significant difference was found between VEGF-C expression in OSCC in regard to clinicopathological parameters. An association between the ILVD and regional recurrence was found (p=0.047) and the PLVD showed associations with disease-specific survival (p=0.044) and disease-free survival (p=0.016). These findings indicate that high ILVD may influences the regional recurrence in neck lymph nodes of patients with early OSCC, however this data was not enough to influence the disease-free survival of these patients with a statistically significant association. PLVD or ILVD did not show predictive value for occult lymph-node metastases in the early stages of OSCC.
