The influence of the hidden curriculum on professional socialisation of student nurses in a military nursing context

The South African Military Health Service (SAMHS) Nursing College offers a four-year integrated nursing programme leading to registration as a professional nurse at the South African Nursing Council (SANC). Student nurses assume a dual role when entering the SAMHS to commence with nurse training – that of a soldier and a nurse. Because student nurses have to assume dual roles, hidden aspects of military culture may influence the professional socialisation of student nurses in one way or another. With a view to determining whether the military environment does indeed impose any influence on student nurses’ professional socialisation, this study set out to explore the multifaceted context in which these students find themselves. Given the organisational and locational complexity of the SAMHS Nursing College, its campuses and the three military hospitals in South Africa, the population was narrowed down to an accessible target population comprising nurse educators and student nurses of the SAMHS Nursing College (Main Campus). Included in the two samples were nurse educators at the SAMHS Nursing College who had at least three years’ experience as nurse educators and who were registered with the SANC as nurse educators, and student nurses registered at the SANC for the fouryear Integrated Nursing Programme and who were in their fourth year of training. A qualitative constructivist grounded theory study was conducted based on the researcher’s philosophical assumptions. The researcher made use of focus groups and critical-incident narratives to collect data. In keeping with the constructivist paradigm adopted for this study, Charmaz’s (2014) data-analysis approach was followed. Concepts derived from the qualitative data were used to develop a substantive model to create an awareness of the existence of a hidden curriculum, to guide role players through the impact of the hidden curriculum on students’ professional socialisation and to help them to understand how their contribution could improve the outcome of the professional socialisation process / Health Studies / D. Compassion Fatigue Among U.S. Military RNs Post Overseas Deployment

Purpose: Describe the meaning of compassion fatigue (CF) as experienced by the U.S. military registered nurse (RN) post-deployment from Iraq and Afghanistan. Background: CF is characterized by deep emotional and physical exhaustion, and may resemble posttraumatic stress disorder (PTSD). Often this causes a shift in confidence and clouds RN perceptions. Symptoms include difficulty concentrating, intrusive imagery, hopelessness, exhaustion, and irritability leading to profound alterations in one's view of the world, patients, family, and friends. Outcomes include depersonalizing patients, poor coping mechanisms, lowered standards, clinical errors, and blurring boundaries, all can contribute to a toxic work environment and RNs leaving the profession. Method: Hermeneutic phenomenological methodology facilitated a description of CF in words and meaning expressed by U.S. military RNs (i.e., Army, Navy). The sample was obtained through the snowball method and aided by key informants. Data were collected through interviews and observations with each active duty or reservist RN (N = 8) on three occasions. Participants described their military and nursing backgrounds and experiences of CF through hermeneutic interview. Analysis was ongoing during the interview process and included continual questioning, reflecting, and validating. This process allowed for understanding through engagement of text (e.g., dialogue, transcriptions). Journaling and self-reflection assisted with trustworthiness. Findings: Participants shared many experiences. While some had unique experiences, their feelings and perceptions resonated with other participants and informed the emergence of four shared meanings and shared concerns: (a) the term CF does not fit me, manifested by expressions of the meaning of CF; (b) compassion fatigue as all encompassing, manifested by physical, emotional, relational, and spiritual experiences; (c) compassion fatigue will not interfere with my military duty, manifested by the pervasive military lens that the meaning of the experiences are filtered, and (d) compassion fatigue affects people long after deployment . . . gone, but not forgotten, manifested by the lasting effects of CF. Implications: While CF among military RNs has many similarities with the general nursing population, the practice environment appears to create additional triggers and manifestations. This study provides an understanding of the progression of CF in this population. Implementing interventions before and after trauma exposure can preserve the care in military caregivers.

Este estudo, de cunho histórico-social, tem como objeto a inserção de enfermeiras como oficiais da Força Aérea Brasileira (FAB) por meio do pioneiro Quadro Feminino de Oficiais (QFO). O marco inicial do estudo refere-se ao início do Estágio de Adaptação militar, em 02 de agosto de 1982 no Centro de Instrução Especializada da Aeronáutica (CIEAR), localizado na cidade do Rio de Janeiro. O marco final do estudo diz respeito ao término do período inicial obrigatório de dois anos de cumprimento de serviço ativo dessas enfermeiras, que culminou com a promoção das mesmas ao posto de 1Tenente (1984). Os objetivos do estudo são: descrever as circunstâncias de inserção das enfermeiras no processo seletivo do QFO, analisar o processo de incorporação do habitus militar durante o Estágio de Adaptação, e discutir as estratégias de luta das enfermeiras militares para ocuparem seus lugares devidos nos hospitais da FAB. A técnica de coleta de dados utilizada foi a entrevista e ocorreu no período de abril a maio de 2009 em hospitais da FAB da cidade do Rio de Janeiro. Foram entrevistadas cinco enfermeiras militares da primeira turma do QFO. O estudo foi cadastrado no SISNEP e aprovado pelo Comitê de Ética da FAB. Todos os sujeitos assinaram o Termo de consentimento livre e esclarecido e o Termo de doação de depoimento oral. O método utilizado foi o da História oral temática o referencial teórico do estudo foi baseado no pensamento do sociólogo francês Pierre Bourdieu, cujos conceitos de poder simbólico, habitus, campo, espaço social e violência simbólica sustentaram a construção desta dissertação. Para a análise e interpretação dos dados, seguimos os passos propostos por Maria Cecília Minayo de ordenação de dados, que compreendeu a transcrição na íntegra dos depoimentos; classificação cronológica e temática dos documentos escritos; classificação dos dados e a análise final. Evidenciou-se que diversos motivos incentivaram as enfermeiras a almejarem sua inserção na FAB como a boa remuneração, estabilidade financeira, progressão profissional, desbravamento de um novo campo de trabalho, clientela diferenciada, aposentadoria com salário integral e pioneirismo na FAB. O objetivo do Estágio de Adaptação militar foi inculcar do habitus militar nas candidatas a partir de ensinamentos baseados na hierarquia, disciplina, ética, dever e compromisso militar. Ao se inserirem nos hospitais da FAB, as enfermeiras receberam diversos cargos e funções, galgando um poder simbólico sobre a equipe de enfermagem. As inevitáveis lutas simbólicas dessas enfermeiras ocorreram com os médicos militares, com a equipe de enfermagem, com as enfermeiras civis e com a própria administração do hospital, e revelaram aspectos característicos de violência simbólica desencadeada por lutas de gênero e pela manutenção do poder, visto que as enfermeiras, dotadas de status de chefe e de militar, se inseriram num campo eminentemente masculino. / This study, historical-social, has as its object the inclusion of nurses as officers of the Brazilian Air Force (FAB) by the pioneer Table Female Officers (QFO). The first milestone in the study refers to the start of Stage Adaptation military on August 02th, 1982 in Center for Specialized Instruction Air Force (CIEAR), located in Rio de Janeiro. The final conquest of the study relates to the expiration of the initial mandatory two years of active service with these nurses, which led to the promotion of same to the rank of 1st Lt. (1984). The study aims to describe the circumstances of the nurses integration in the selection process of the QFO, analyze the process of incorporation of military habitus during Stage Adaptation, and discuss strategies to fight of the military to occupy their proper places in the FAB hospitals. The technique of data collection used was the interview and took place between April and May 2009 in hospital of FAB in the city of Rio de Janeiro. Five nurses from military first group of QFO were interviewed. The study was registered in SISNEP and approved by the Ethics Committee of FAB. All subjects signed the informed consent and donation term oral testimony. The method used was thematic oral history of the theoretical study was based on the thought of French sociologist Pierre Bourdieu, whose concept of symbolic power, habitus, field, social space and symbolic violence claimed the construction of this dissertation. For the analysis and interpretation of data, we follow the steps proposed by Maria Cecilia Minayo sort of data, which comprised the full transcript of testimony, chronological and thematic classification of the written documents, data classification and the final analysis. It was found that various reasons have encouraged nurses to aim their insertion in the FAB as good pay, financial stability, professional development, clearing a new field, distinguished clientele, retirement with full pay and pioneer in FAB. The goal of the adaptation was to inculcate the military habitus in military candidates from teachings based on hierarchy, discipline, ethics, duty and military commitment . When they entered in FAB hospitals, the nurses received a variety of positions and functions, climbing a symbolic power of the nursing staff. The inevitable symbolic struggles of nurses occurred with military doctors, with the nursing staff, with nurses and civilians, with proper administration of the hospital, and showed the characteristic features of symbolic violence triggered by gender struggles and maintaining power, as nurses, with their status as military chief and, inserted in a predominantly male field.

Well-being in Military Reserve Health Care Providers Post Deployment or Mobilization

Purpose: To describe well-being in military reserve health care providers post deployment or mobilization Background: A comprehensive literature review exhibited that during a deployment or mobilization, military health care providers endure stressful and uncomfortable situations, spend time away from their family, friends and usual home comforts leaving them feeling completely changed. These circumstances can negatively affect a provider’s well-being. Moreover, although the literature outlines military and psychological symptoms including combat stressors, mental health concerns such as depression and anxiety, job stress, work performance and resiliency, there is a gap in the literature regarding well-being among military health care providers post deployment or mobilization. Methods: This project used a qualitative descriptive methodology with face-to-face interviews to describe the phenomenon of well-being among three United States reserve military health care providers post deployment or mobilization. Data was collected using semi-structured, open ended questions, allowing the participants an opportunity to discuss and further elaborate on their experiences, perspectives and feelings. Each interview began with pre-established questions and further questions and discussions were guided by the participants’ earlier responses. Interviews were audio recorded and transcribed into text, allowing me to identify commonalities of experiences, perspectives and feelings among the participants. Results: Analysis of the interview data revealed information associated with transitions and can be grouped into five categories. The categories that impacted the participants’ well-being post deployment or mobilization include: separation from family and friends, austere living conditions, exhaustion from long work hours, consecutive work days without days off and being unprepared for what was to come. Implications: This project describes well-being in military reserve health care providers post deployment or mobilization. Findings from this small project may be used to develop research studies to describe and understand the concept of well-being among military health care providers. Armed with better understanding, we could then develop interventions to prepare our military health care providers to deliver high quality care during overseas deployment or mobilization while also maintaining their physical and mental well-being.

Omvårdnadsdiagnoser hos militärpersonal i relation till militärt etos – En litteraturöversikt / Nursing diagnoses of military personnel in relation to military ethos – A Literature Review

Background The International Council of Nurses (ICN) states in it’s ethical code that the nurse practitioner is responsible for giving each patient correct and culturally adapted care. Military culture is unique and comes with its own challenges and needs of nursing care. Aim The aim is to find the prominent nursing diagnoses in military personnel in relation to the military ethos. Method A literature review of twenty articles found through searches in PubMed, Cinhal and Summon. Results The prominent nursing diagnoses were risk-prone health behavior together with pain issues and sleep disturbances. Two thirds of military personnel consumed high amounts of alcohol, pain issues was the most prevalent, up to half of all personnel experienced sleep disturbances and willingness to seek care was low. Stress, depression, consequences of pain and effects on the family process was also found. Result of military duty could also be infections, wounds, dermatological problems, hearing impairment and diarrhea. Conclusion Military culture is unique and reports health differently than the civilian population. Because of this it is important to know the prominent diagnoses to be able to give this patient group appropriate nursing interventions and competent care. / Bakgrund I enlighet med International Council of Nurses (ICN) etiska kod har sjuksköterskan ett ansvar för att varje person ska få en korrekt, tillfredsställande och kulturellt anpassad information som grund till sin vård och behandling. Militärkulturen ger upphov till en patientgrupp med unika behov och detta skall styra vården. Syfte Syftet är att genom en litteraturstudie förstå vilka omvårdnadsdiagnoser som är framträdande hos militärpersonal med relation till militärt etos. Metod En litteraturstudie av artiklar (n=20) genom sökningar i PubMed, Cinhal och Summon. Resultat Riskfyllt hälsobeteende var största temat följt av smärta och sömnproblematik. Hos militärpersonal var alkoholkonsumtion hög hos två tredjedelar, smärta hade högst prevalens, påverkan på sömnen återfanns i upp till hälften av personalen och viljan att söka vård var nedsatt. Stress, depression, smärtkonsekvenser samt påverkade familjerelationer och processer återfanns. Följder av tjänstgöringen var infektioner, hudskada, hudåkommor, hörselnedsättning och diarré. Slutsats Militärkulturen är unik och underrapporterar ohälsa, därför behövs det en förståelse för de framträdande omvårdnadsdiagnoserna för att kunna ge denna patientgrupp god vård.
