Krizová připravenost Nemocnice Jihlava na vybrané mimořádné události / Crisis readiness of the hospital Jihlava relating to selected extraordinary events

There are currently many emergencies that may affect the security environment of the Czech Republic. No extraordinary event chooses time or place to intervene. Among the most vulnerable people we can rank people in social facilities or health facilities because these people are dependent on the help of others. Healthcare facilities have their own specificities, which need to be given increased attention, especially in the event of emergencies. Currently, patients' entitlement to health care is high. Care should be provided in its entirety and at a high level. The aim of this thesis is to increase the readiness of Jihlava Hospital for selected emergency events. A research question has been set for this work: What is the readiness of Jihlava Hospital for selected emergencies? Data acquisition will take several forms. The research and analysis of legal norms and documents related to the issue will be crucial. Analysis of synthesis, document induction and environmental observation will be used. Subsequent interviews with employees of Jihlava Hospital and other relevant people will also play an important role. To evaluate the readiness of Jihlava Hospital, a risk analysis will be performed using the KARS method. The result will be a Plan of Emergency Preparedness, which will be given to the hospital. Thesis can also serve research purposes for hospitals to increase their preparedness for emergency situations.

The purpose of this thesis is to examine the experience of nurses with burnout syndrome and its prevention. In order to this, I have been examining a number of nurses who have experienced burnout syndrome, the level of intensity they experienced and the nurses' opinion on the prevention of this psychical state. The theoretical part will focus on the nurses' job description and psychological stresses related to their duties. Furthermore I will focus on burnout syndrome - its risk factors, causes, stages and prevention. I will inform as well about daily course and operations at the Intensive Care Unit and the standard inpatient department. The empirical part focuses on comparing the level of burnout syndrome of nurses working in the Intensive Care Unit versus those working in the standard inpatient department. Furthermore I am examining the experience of nurses with the burnout syndrome and its prevention. I will use a questionnaire as the research method.

Zajišťování požární ochrany ve vybraných nemocnicích v hlavním městě Praha / The provision of fire protection in selected hospitals of the Capital City of Prague.

The aim of this work is to map fire safety in selected hospitals in the capital city of Prague. The main aim is to answer whether is the fire protection by the corporate fire brigade unit considering the vastness of the area and the number of beds measurable benefit for the fire safety of medical facility. Custom work is also examined whether the unit is reduced by the presence of the corporate fire brigades in the hospital requirements for fire protection using dedicated kinds of firefighting equipment. Another part of the research is dedicated to the fact whether is the load of the fire rescue service (FRS) of the capital city of Prague reduced by the presence of the corporate fire rescue service in the hospital and thus has to adjust the area coverage? In the first part of the thesis is summary of the regulations in the area of fire prevention. Above the Law on Fire protection and the Ministry of Interior Decree on fire prevention. There are described in detail obligations of legal entities and individuals, competence and training of fire protection. From the perspective of repression is described by the distribution of fire protection units, and also their rights and obligations. For purposes of this thesis is defined by the fire and method of propagation of fire. This will take the necessary measures to prevent its spread. Solution is also the fire safety of buildings according to the CSN 73 0802 Fire protection of buildings - nonproductive objects. Specifically resistance of structures and the use of fire safety equipment. It is important to define the hospital as a medical facility and its basic characteristics and distribution. They are described in detail in terms of medical facilities and fire safety through technical standard CSN 73 0835 Fire safety of buildings - Buildings for health care facilities. Rated objects are the Motol University Hospital, the University Hospital of Royal Vinohrady and Thomayer Hospital, all with headquarters in Prague. The main criteria for the selection of the hospital's number of beds, number of patients treated and the site area. Another criterion is the number of employees and the difficult conditions for intervention. At first, the comparison parameters hospitals for fire prevention, such as the number of professionally qualified persons, the number of trainees, preventive fire patrols. In the Motol University Hospital are installed sprinkler system and a special spraying equipment and also there is an entity of its own corporate fire rescue brigade. The next section analyzes the number of incidents in each hospital complexes or the number of fires. We found a relatively large number of fires in the Motol University Hospital. And with an average of 5.7 fires per year in the past decade, compared to 0.3 fires in the Thomayer Hospital and 1.0 of fire in the University Hospital of Royal Vinohrady. Space is analyzed with the most complex variant of fire, especially the requirements for forces and resources. The results are compared to their inclusion in areal coverage depending on the degree of danger cadastral municipality. In all three cases, these conditions suit. The result of the research is that the selected areas of hospitals in the capital city of Prague in terms of fire protection are at a very high level. Whereas there is the unit with its own fire protection or not. Its presence is a matter of fire protection concepts during the design of the hospital complex. Establishment of fire protection units in hospitals, where it currently is, would only increase the comfort of visitors and hospital patients on the one hand and financial wastage on the other. Functioning FRS in the Motol University Hospital is an objective benefit for the fire safety of the hospital complex. Analyses of the current state of the recommendation by the FRS of the capital of Prague conduct more tactical exercises, mainly in the hospital where they have their own fire protection unit.

Etické problémy na odděleních psychiatrické nemocnice / Ethics at the wards of psychiatric hospital

Thesis deals with the ethical problems at the wards of psychiatric hospital. The environment of a psychiatric hospital in the context of its target group psychiatric patients is a source of ethically important situations, which must be solved by the medical social worker. This puts high demands on the worker as he is the/an executive tool of medical social work.The goal of the thesis is to identify ethically important situations, to suggest their solution and eventually to reflect these solutions with the ethical theories, ethical codes, inner rules of the psychiatric hospital and other statutory norms. The structure of the thesis is adjusted to this goal: first, I define the environment of the hospital, describe the target group and medical social worker profession. Secondly, I describe ethical problems identified before, suggest their solutions and reflect them. Finally, in the last part of the thesis, in the discussion, I contemplate about preliminary ethical understanding as a basis for identification of the ethical problems and I ponder other factors that matters in reaching the remedies of ethical problems.

This diploma thesis is aimed at assessing the provision of emergency electric power supply to hospitals in South Bohemia. The chapter titled Present State briefly characterises the generation of electric power, the history of power plants in the Czech Republic, the power distribution system, it lists the basic elements of the power distribution system, including its stability and security. Among other things, it presents the term of blackout and its potential impact on the society. Furthermore, this chapter deals with the infrastructure in relation with the critical infrastructure and its history. At the end of this chapter, the functioning of health service is mentioned in relation to emergency situations, including the emergency sources used by hospitals. The research made use of the qualitative method of data collection and the data was acquired through the technique of controlled interviews and a secondary analysis of data from competent South Bohemian hospital staff who is responsible for or deal with the issue or providing emergency power supplies.

In 2011 and 2012, dramatic changes were made to the health care reform in the Czech Republic. As a consequence, almost the complete legislation related to traumatological planning was changed. This reform primarily introduced two new types of operating divisions; a department of emergency preparedness, as for the emergency medical service providers, and a contact point, from the aspect of emergency inpatient care providers. Emergency preparedness primarily processes a concept of the traumatological plan for the emergency medical service and introduces potential changes. The contact point, as a department of the emergency inpatient care with continuous operation, receives calls from the emergency medical service, especially in the event of emergencies and crisis situations. The given Master's Diploma Thesis entitled "Cooperation of the Trauma Centre of the Hospital České Budějovice and Medical Operation Center of the Emergency Medical Service of the South Bohemian Region in the Event of Mass-Casualty Incident" develops a key issue of the interconnection of traumatological plans established by health service providers within the South Bohemian Region and the level of their cooperation. Then the work analyses three mass-casualty incidents which happened in the past within the area and the level of involvement and cooperation of the aforementioned departments, i.e. the Hospital's trauma centre and Medical Operation Centre upon the Medical Emergency Centre of the Region of South Bohemia. The Diploma Thesis proceeds from the qualitative research which is divided into two parts. The first one defines the V1 research question specifying the role of the Traumatological Plan of the South Bohemian health care during the cooperation with the trauma centre under the Hospital České Budějovice and medical operation centre of the Medical Emergency Service of the South Bohemian Region in the event of a mass-casualty incident. By using a comparative method, which assessed the three traumatological plans, the level of cooperation did not exceed more than 20 % owing to the fact that the Traumatological Plan of the South Bohemian health care is out-dated. As a consequence, the answer to the V1 research question was following: The role of the Traumatological Plan of the South Bohemian health care with the cooperation of the trauma centre under the Hospital České Budějovice and Medical operation centre of the Medical Emergency Service of the South Bohemian Region in the event of a mass-casualty incident is insignificant. The second part develops the V2 research question: whether the failure in communication between the aforementioned trauma centre and medical operation centre could have any impact on the injured in the event of a mass-casualty incident. The qualitative research with the use of a method of analysis implemented during the previous mass-casualty incidents and interviews with four operators working for the medical operation centre and four doctors working at the accident, emergency and plastic surgery department provided the second answer to the V2 research question: Failure in communication between the two aforementioned departments could have a significant impact on the injured in the event of a mass-casualty incident. In the end, the thesis refers to the dramatic difference between the ideal traumatological planning and the current situation pointing out the Traumatological Plan of the South Bohemian healthcare which has not been updated for three and a half years since the reform. The given work also describes the issue of the ideal solution to the establishment of contact points in hospitals and compares it with the current situation at the Hospital České Budějovice where the contact point was established only in order to comply with the legislation, i.e. the contact point is based in the operating centre of medical transport.

Problematika kvality pitné vody v nemocnici Prachatice / The quality of drinking water in the Hospital Prachatice.

This diploma thesis deals with the drinking water quality in the Prachatice Hospital. In every healthcare facility, it is essential to maintain and monitor the quality of drinking water. The diploma thesis has the following objectives: Evaluation of the quality and quantity of drinking water and hot water at the Prachatice Hospital in certain time periods, comparison between another hospital in the South Bohemian region with the occurrence of Legionell in the given period, the possibility of solving crisis situations in accidents in the healthcare facility and possibilities of rainwater use in the Prachatice Hospital. The thesis is divided into theoretical and practical parts. The theoretical part of the thesis describes basic concepts related to the quality of drinking water, indicators of drinking water and a section is devoted to a water accident in the Prachatice Hospital in the autumn of 2015. The practical part was processed by the method of secondary analysis of data. The necessary data for the analysis was obtained directly at the Prachatice Hospital and at the Regional Hygiene Office in České Budějovice. For the analysis, data from January 2011 to January 2017 was used and processed in Microsoft Excel into tables and charts. The results show the overall drinking water supply situation in the Prachatice Hospital. The work could draw attention to the problems and issues associated with emergency situations, for example. Hospital management and professionals in drinking water technology department can get a comprehensive overview of the hospital's drinking water supply from its own well, public well and the public water supply.

Zmapování indikátorů kvality ošetřovatelské péče v českobudějovické nemocnici / Quality Indicator Mapping of Nursing Care in Hospital České Budějovice

Quality nursing care is today the essential goal of contemporary nursing. Therefore it is necessary to introduce in sanitary facilities a quality improvement program, whose part is tracking quality indicators of care provided. Within the practical part we set three goals. The first of them is to define tracked quality indicators of nursing care in Hospital České Budějovice, a.s. The second goal is to evaluate the selected quality indicators of nursing care in Hospital České Budějovice. The third goal is to inform nurses on quality indicators of nursing care in Hospital České Budějovice. Based on these goals we introduced six hypotheses. The first hypothesis says: Hospital České Budějovice, a.s. tracks at least 15 quality indicators of nursing care within the tracked period. The second hypothesis says: Hospital České Budějovice, a. s. realized at least four nursing care audits per year. The third hypothesis says: nurses of Hospital České Budějovice are satisfied with working conditions. The fourth hypothesis says: clients of Hospital České Budějovice, a.s. are satisfied with the provided nursing care. The fifth hypothesis says: Nurses of Hospital České Budějovice know at least four quality indicators of nursing care in Hospital České Budějovice being tracked in their wards. The sixth hypothesis says: Prevalence rate of decubitus in Hospital České Budějovice decreases. The research ran over under collaboration with staff nurse of Hospital České Budějovice, a.s.; one of forms of the research was also a nursing audit; further data collection method by means of questionnaires was used. The research file involved nurses and clients of Hospital České Budějovice, a.s. In the course of the research we fulfilled the set goals; the first, fourths and the fifth hypotheses were confirmed and the second, third, and the sixth hypotheses were not confirmed. The research results have been offered to the hospital top management as a data source concerning the nursing care quality. The work should also provide nurses with information on that monitoring and evaluating nursing care quality through specific indicators is at present a necessity and indispensable fact and that its goal is not individual disciplinary decisions of individual nurses, but it concerns finding out system errors, their assertion and resolution.

Optimization of the number of nursing staff at selected workplaces of the České Budějovice Hospital As the title shows determination of the optimum number of non-physician health care staff members at surgical departments of České Budějovice Hospital is the main question this thesis deals with. The topic is up-to-date mainly for the lack of funds in health care and for the permanent pressure from the general public on the need of health care quality improvement. The works of Karla and Otakar Pochylý, who published their first book dealing with these issues in 1999, were the basic inputs for the research part of the thesis. We set the following hypotheses before commencement of the research: 1 ? The numbers of nursing staff at standard health care units are in line with K. Pochylá?s methodology. This hypothesis was partly confirmed. The average numbers of nursing staff workloads are in line with K. Pochylá?s methodology at two surgical departments only. 2 ? Qualification representation of nursing staff at the individual health care units complies with the recommended methodology of K. Pochylá. This hypothesis was not confirmed. Representation of the nursing staff qualifications is based on the individual needs of the particular departments. 3 ? The number of nursing staff members at the intensive care unit corresponds with the recommended methodology of K. Pochylá. This hypothesis was not confirmed. The number of nurses is lower by 2.23 in average at the intensive care unit. The content of the work has more informative character and should actually serve as an insight into the existing situation at the particular departments. It might also serve as a guide helping top managers with efficient utilization of human resources in health care facilities. This is why the results will be provided to the surgical department management of the České Budějovice Hospital.

The thesis deals with the issue of the formation of the ABC model in healthcare facilities. The aim of this work is the formation of the ABC model on the example of the Infectious Department of the selected teaching hospital. The work is divided into theoretical and practical part. In the framework of the theoretical part are summarised findings from literature review regarding the Activity-Based Costing method and its application in medical organisations. In the practical part are already constructs of the ABC model itself, based on an annual data basis using a procedure adapted to the medical facilities.
