Potřebnost hospicové péče na území obce s rozšířenou působností Jindřichův Hradec / The need for hospice care in Jindřichův Hradec, the municipality with extended competence

An essential part of high-level medicine of every state is hospice-based and palliative care. Throughout the world and throughout western Europe there is a fully established network of mobile and residential hospices. Meanwhile, the Czech Republic is in the early stages, making efforts to improve the situation and to firmly embed hospice care into its social and health service provision. The year 2016 is one of great expectations. Firstly there is an ongoing pilot programme of grants supporting mobile palliative care, which will compare the "cost of dying" at home with the cost spent by institutions. Secondly, a decision by the Ministry of Work and Social Services is expected with respect to proposed changed in law no. 108/2006 of the Collection of Laws on social services. If passed, this would mean a hospice would turn into an official social and health institution, thereby markedly improving the problem of financing the care provided within one, as it is a combination of social and health services. The aim of this thesis is to provide a more integrated briefing of current information related to the given issues. Therefore the theoretical part covers not only hospice and palliative care itself, but space is given to other issues such as cancer, population ageing with its impal on the economy and to new forms of care for senior citizens, dying and death. Furthermore the thesis deals with the question of financing hospice care, with particular emphasis on community planning. Lastly the theoretical part of the thesis covers the characteristics of Jindřichův Hradec, the municipality with extended competence and a description of the need for hospice care from the perspectives of both the Town Jindřichův Hradec and the Southern Bohemian region. The Hospital Jindřichův Hradec, plc and the Town itself are aware of the need for palliative care in the given region. Both organizations have therefore already undertaken important steps, based on which the Southern Bohemian region has included support for hospice and palliative care into planning and health development for the coming years. The aim of the thesis is to map out the situation concerning the provision of hospice care in the area of Jindřichův Hradec, the municipality with extended competence and to establish the requirements for this care by considering the interest of the citizens from the given area. The experimental part of the thesis uses two approaches to establish the need for hospice care. Firstly, this was content analysis of data materials provided by social and health service providers, and secondlya qualitative investigation using interviews with eight respondents of a panel on community planning of social services in the area of Jindřichův Hradec, the municipality with extended competence. Included are also statistical data from the Hospital Jindřichův Hradec plc, with emphasis on oncology patients, because these form the major group receiving palliative care. The result of the research is not surprising. The population is ageing demographically, cancer-related illnesses are the nightmare of our time and the social and health care in Czech Republic must adapt to this fact. The research confirmed that hospice care in Jindřichův Hradec, the municipality with extended competence is needed, that the families of patients are requesting it and that services currently offered are deficient. It became evident that the overwhelming support is for the development of a residential hospice or for establishment of palliative beds in the hospital. In Southern Bohemia there is only one residential hospice in Prachatice, which is little used due to the long distance and thus the families have no opportunity for respite care, which the carers are also interested in. The research confirmed, that the creation of a residential form of hospice type in the area of Jindřichův Hradec, the municipality with extended competence is supported by general practitioners, specialist doctors

At the beginning of my thesis, the risk of floods in the territory of the Czech Republic is generally characterized which is followed by basic legal documents governing flood protection. The characteristics of the territory on which this thesis focuses is presented in the next section, which is describes in detail the administrative district of the municipality with extended powers (MEP) of Týn nad Vltavou, the basic description of its hydrological basin and waterways and, of course, the flood profile of the individual municipalities. The following section explains the term of flood, classification of floods and a detailed description of flood risks within detail the administrative district of the MEP of Týn nad Vltavou emphasizing the risk of formation a special flood. This is followed a list of complex activities and measures aimed at preventing and eliminating the threats to the health, lives and property of the citizens, society and the environment during floods. It also discusses planning in the field of flood protection and the classification of flood plans. Emphasis is placed on the documentation of flood protection of the administrative district of the MEP of Týn nad Vltavou that is associated with the activities of flood protection authorities. The section discusses in detail their activities during floods as well as when there are no floods. The function of flood committee of the MEP Týn nad Vltavou is mentioned detailing the description of activities of its individual members. The objectives of this thesis were aimed at finding out about the level of prepared flood protection documentation processed, analysing the activities of flood protection authorities and examining their knowledge about the issue of flooding. All the objectives were directed to the administrative district of the MEP Týn nad Vltavou and its flood protection authorities. Through a sociological survey among the flood protection authorities of the administrative district of the MEP Týn nad Vltavou that was conducted in the form of an anonymous questionnaire testing their knowledge in the field of flood protection. The results obtained were subjected to statistical evaluation, which, together with graphical representation, is of part of the thesis.

Optimizing embryo culture conditions and spent culture media analysis as predictors of embryo quality and pregnancy

Philosophiae Doctor - PhD / The aim of this thesis is first, to evaluate various culture conditions to improve embryo development, and secondly, to analyze spent culture media for any biomarkers that may be predictive of embryo health. Single-step and sequential culture media were compared in both Planer and EmbryoScope™ incubators. Single-step media resulted in better blastocyst development compared to sequential media and the EmbryoScope™ incubation system showed slight improvements in embryo development than the Planer system. The benefits of supplementing the culture medium with either insulin or insulin-like growth factor 1 (IGF-1) or culturing in a 2% O2 environment, using two different strains of mice (hybrid and C57), as well as the suitability of these strains for quality control were compared. In insulin, hybrid embryos were slower to blastulate and had a lower blastocyst rate, whereas C57 embryos were slower to the morula and faster to blastocyst stages, and lower blastocyst rate than the controls. IGF-1 showed no difference in time-lapse morphokinetics (TLM) or blastocyst rates compared to controls in both hybrid and C57 embryos. Under 2% O2, hybrid embryos showed no significant difference in TLM up to the 8-cell stage, but slowed down afterwards, resulting in blastocysts with significantly lower cell counts than the 6% O2 group. The C57 embryos were slower to reach morula and expanded blastocyst, and had lower blastocyst rates in 2%O2 vs 6%O2. The C57 strain had significant slower overall embryo development for all time points than hybrid embryos in insulin, IGF-1 and ultra-low O2, as well as lower blastocyst rates. Measurement of growth differentiation factor 9 (GDF-9) and oxidation-reduction potential (ORP) in spent media as markers for embryo health were evaluated. Day 5 human blastocysts yielded higher pregnancy rates and GDF-9 levels in spent media compared to Day 6 blastocysts, but TLM parameters showed no impact on pregnancy outcome. In Day 6 blastocysts, the non-pregnant group showed significantly faster embryo development compared to the clinically pregnant group up to the 8-cell stage and start of blastulation. GDF-9 did not show any significant differences between non-pregnant and pregnant groups of Day 5 or Day 6 embryo transfers. ORP in spent media from good quality Day 3 embryos that developed into blastocysts were significantly higher than from those that did not, with no difference in control medium ORP. Spent media from arrested embryos showed lower ORP than their corresponding controls. Arrested embryos had slower development at syngamy, morula, blastulation and blastocyst stages. The single step medium in the EmbryoScope™ is the preferred choice for embryo culture. Insulin or IGF-1 media supplementation or 2% O2 culture did not provide any benefit to embryo development. The C57 mouse strain is more sensitive and may be better to detect changes in culture conditions, and therefore better model for quality control assays. GDF-9 values decrease from Day 5 to Day 6 which gives new insight to understanding the role of GDF-9 during embryogenesis. ORP in spent media indicate that embryos that developed into blastocysts did not contribute to ROS, but maintained ORP balance.

This thesis focuses on the quality of life in dimension of health at the regional level in the Czechia for the period 2010-2019. The main aim of this work is to describe and evaluate the spatial differentiation and spatio-temporal variability of the concept quality of life in dimension of health at the regional level of administrative districts of municipalities with extended powers. The assessment is based on objective indicators mapping the health status of the population, lifestyle, and the availability of primary health care. The theoretical part describes definitions quality of life, the characteristics of whole concept, health, its determinants, and inequalities that arise in health and can resonate at the regional level. This work also describes the types of prevention, as an effective tool in reducing health inequalities and the possible promotion of public health, in the form of improving health literacy. The analytical part is based on factor and cluster analysis and methods of spatial autocorrelation. In the Czech Republic, significant regional differentiation of quality of life in health dimension has been found.
