Title of the diploma thesis: Use of promotive therapy and stimulation in hospice care Aim of the thesis: Possible use of promotive therapy and stimulation in therapy of patients in hospice care is discussed in this diploma thesis. The aim of thesis is to compare use these therapies in hospice facilities that provide accommodation for clients or in home hospice care in the Czech Republic and the Slovak Republic and to analyze way of its use in praxis. For purpose of this thesis following promotive therapies and stimulation were selected: music therapy, art therapy, bibliotherapy, basal stimulation and concept Snoezelen. Methods of work: Quantitative and qualitative investigation was used for research. Quantitative investigation in form of questionnaires provided to hospice facilities with accommodation for clients was utilized as a method for data acquisition. Observation of practical promotive therapies processes in hospice facilities with accommodation for clients was included in methodology of qualitative investigation. Results: Research deals with death and dying which is still quite often taboo theme in our conditions. This fact is perceptible in results of research investigation. Nerovnováha financování lůžkové péče v ČR a její dopady / Imbalance in financing of bed care in the Czech Republic and its impacts

This master thesis deals with the question of medical treatment financing focusing mainly on analysis of health insurance companies reimbursement rates and their comparison with costs of aftercare beds. Secondary goal of the thesis is comparison of acute care beds costs and aftercare beds costs. The opening chapter discusses relationships between economics and health service and describes structure of health care funding compared to insurace companies financing strategies. Payment mechanism mostly used in the aftercare field is the pay-per-day system, this mechanism is therefore later explained in detail including particular effects on after care beds financing. The second part of the thesis compares insurance reimbursement rates with real costs of one particular facility. The analysis proves that health insurance companies reimbursement rates are wrongly set and reimbursement legislation is being violated.

Sdílená péče o seniory a její dopad na psychické a sociální problémy neformálních pečujících / Mixed forms of elderly care and its impact on informal carers' mental health and social problems

The diploma thesis deals with mixed forms of elderly care in the Czech Republic. The aim is to identify the advantages and potential disadvantages of mixed forms of elderly care in connection with informal carers' mental health and social life. There are 2 groups of informal carers distinguished within the research - those, who get professional care help and those, who care alone or get just a little professional care help. Mixed methods design, which combines quantitative and qualitative research, was used to study the world of informal carers. The quantitative part is based on data from a research Podpora neformálních pečovatelů, which were selected by STEM/MARK agency in 2014. Qualitative research is based on 10 in-depth interviews with informal carers. Both pieces of research are integrated in a phase of data analysis by the explanation method. Informal carers' mental health problems and social problems and its intensity in connection to mixed forms of elderly care are formulated in the results. The thesis also presents factors, which influence mental health problems and social problems. The results showed that using professional care help simultaneously with informal elderly care has a positive impact on informal carers' mental health and social life. This thesis deal swith the specifics of nursing care for patiens with a resected tongue in the oral surgery department. The objectives of this work include focusing on the basics of care for patiens after a tongue resection and the most common problems nurses may encounter hen caring for these patients. The work consists of two parts: the theoretical and empirici parts. The theoretical part describes the field of dentistry and oral surgery, anatomy and physiology of the oral cavity. It also deals with orofacial surgery and the actual tongue resection procedure with a focus on nursing care for patiens after wards. In other chapters of the theoretical part we can read about the most common nursing problems in patiens after a tongue resection. In addition, the twelve patterns of health in Marjory Gordon's nursing model will be described for the empirical section. The empirical part is devoted to the research investigation. The thesis describes the metodology and organization of the research investigation. This was a qualitative research, the method of data collection was to create three case reports or case studies with follow discussion. Podpora kojení v rámci systému zdravotní péče ČR / Breastfeeding support in the Czech health care system

Abstact The diploma thesis is focused on forms and dimensions of breast-feeding support regarding different levels of women and child care in the health care system of the Czech Republic from the perspective of a woman, a mother, who is the health care recipient in the context of the legal framework of Czech legislation and relevant international documents. The findings are based on empirical research results gained from ten semi-structured in-depth interviews. Ten individual research participants gave birth in the same medical institution with the title Baby Friendly Hospital during the last three years, which is the unifying element of the participants. The perspective of a woman, based on a thematic analysis of interviews , shows a lot of perceived dimensions of the provided health care. This work identifies the most obvious women themes: their perception of breastfeeding in pregnancy, importance of breastfeeding for woman, the mother's contact with the baby after the childbirth, medical staff attitude and quality of provided care, aspects of pediatric care and support of breast-feeding out of the health care system. In my thesis I focus on integration of the palliative care in the retirement home describing it through the practical cooperation of two organisations - The Homecoming and Domov Sue Ryder. In the theoretical part I deal with the term of palliative care first, and I also introduce the specifications of the geriatric palliative care. In the subsequent part I describe the residential institutions of social care and professions that are employed in them, as well as, the multi-disciplinary team offering specialised palliative care. I also present the term of shared care existing abroad together with the foreign models of palliative care integration in the retirement homes already used in Austria and Great Britain. The conclusion of the theoretical part explores the actual project of palliative care integration called that is currently taking place in the Czech Republic. Disertační práce se zabývá problematikou seniorského věku a snaží se poukázat na některé základní přístupy v souvislosti s porozuměním této velmi heterogenní a diverzifikované skupině, těžiště přitom klade do vzájemného vyváženého poměru speciálně pedagogických přístupů a teologicko-pastorální impulsů. Toto spojení však vyžaduje hledání společného dialogu mezi vědami a umožňuje tak najít kvalitní základnu pro porozumění seniorům v jejich integrovaném životním příběhu a zasadit se tak o kvalitní, ale i efektivní péči navazující na poznatky humanitních, medicínských i společenských věd. Zvolená metoda vyžaduje multidisciplinární přístup a vzdělání, které přesahuje hranice vymezení oborů. Práce využívá jako zdroj kvalitativní výzkum, který byl zaměřen na širokou oblast kvality života seniorů a zohledňoval též spirituální otázky. Péče o seniory v denním stacionáři / Adult day care center

5 Abstract This thesis is focused on the research of adult day care centers. Its aim is to answer the question how the the day care centers define the care from the point of view of different stakeholders; how are the centers specific to other social services and what are the possible problems and obstacles. Theoretical part describes the current situation of elderly care and deals with topics such as the demographic development of the Czech Republic, the media image of the elderly, new trends in the area of care, active aging, social services etc. Empirical research deals with two day care centers from the points of view of different stakeholders. The research, which results in two case studies and their comparisons, is carried out using qualitative methods - semi-structured interviews, analysis of documents and survey. Data analysis shows that the main function of day care centers is "relief service" for families who provide care to the frail seniors. The distinctive feature of day care centers is emphasizing differnces with other social services for elderly, especially the residential ones.
