Introdução: O Papilomavirus Humano ( HPV) é um vírus de DNA que inclui 118 genótipos, sendo o tipo 16 responsável por 80% dos casos de câncer cervical nas mulheres. Os homens são um importante reservatório de HPV e os principais responsáveis pela transmissão às suas parceiras. Objetivo: Detectar HPV DNA e determinar a prevalência de HPV-16, 18, 6 e 11 em amostras de urina de homens infectados pelo HIV-1. Material e Métodos: Homens adultos infectados pelo HIV proveniente de clínica de Urologia/doenças sexualmente transmissíveis e clínica de HIV foram convidados a participar da pesquisa. O estudo foi conduzido entre março de 2006 e abril de 2008. Cerca de 20 ml de urine foi coletada em uma sala específica. As amostras foram submetidas a um PCR Real Time utilizando-se Sybr Green® com primers degenerados PGMY09/11 para detecção de HPV DNA. As amostras positivas, então, foram submetidas a uma PCR convencional utilizando-se primers específicos para cada tipo de HPV. Resultados: Um total de 223 homens infectados pelo HIV-1 foi testado, sendo que 81% deles utilizavam HAART. Sessenta e nove (30.9%) homens apresentaram positividade para HPV DNA na urina pelo método de PCR Real Time. Vinte e dois (31.9%) deles foram positivos para o HPV-16, pelo método da PCR convencional. Dezoito (26.5%) homens apresentaram o DNA do HPV-11; quatro (5.8%) apresentaram o HPV-6 e cinco (7.2%) apresentaram o HPV-18. Vinte (29%) apresentaram DNA de algum tipo de HPV que não aqueles que foram propostos nos objetivos do trabalho. Os homens HIV/HPV mostraram atividade sexual precoce menor que os não infectados por HPV (33.4% vs 40.3%, p=0.22), porém alto número de parceiros sexuais no último ano (40.6% vs 36.4%, p=0.50) do que os homens HPV negativos. A média de carga viral de HIV foi de 10663 cópias/mL e 14104 cópias/mL (p=0.0002), para HPV positivo e HPV negativo, respectivamente. A contagem média de células TCD4+ foi de 441 cels/ml e 517 cels/ml (p=0.30), respectivamente. Conclusões: Foi detectada alta prevalência (30.9%) de HPV na urina de homens assintomáticos, infectados pelo HIV-1. Um terço deles foi infectado pelo HPV-16. O alto número de parceiros sexuais foi um fator importante para a aquisição de HPV nesta população. Previous issue date: 2017-12-13 / Population aging is one of the greatest challenges for mordern public health. As a reflection of
the enhancement in quality of life, there is a maintenance of the entire exercise of sexuality in
the elderly and, with it, the risks of contracting Sexually Transmitted Infections (STIs), among
them Human Papillomavirus (HPV) infection. The persistence of high-risk HPV infection
associated with low tracking of the female population are the main factors responsible for the
development of cervical carcinoma, which represents a significant cause of morbimortality in
this age group. This research intent to characterize the profile of elderly women in Gama -
Federal District, with emphasis on the preventive aspects of cervical cancer, comparing the
profile and attitude of the elderly women before the colpocitological examination, regarding to
their expertise of the exam. It is an evaluative study of the type inquiry knowledge, attitude and
practice. The data collection was through a survey , under a direct hearing, addressing aspects
concerning Knowledge, Attitude and Practice (KAP) of the elderly women in relation to the
cervical cancer preventive exam (CCPE). The KAP surveys were applied to 300 elderly women,
aged 60 and 93, passers-by at Gama in places such as public fairs, shopping malls and cultural
and leisure centers for the elderly. The majority were married (43.3%), had elementary
education (60.3%), resident near the health unit (83.7%) and had coitarche after 18 years
(70.3%). The data collected displayed: as to Knowledge, 4.7% responded adequately; As for
Attitude, 93.0% were considered adequate; and in practice, 50.7% were adequate. As far as
knowledge was concerned, no significant correlation was observed with sociodemographic
factors, whereas in relation to Attitude there was a statistically significant correlation with
schooling, being higher in those with higher education. In the practice question there was a
significant association with the age of the consulted women, being more appropriate in those
with age group of 60 to 64 years. Evaluating the identification of the perception regarding the
preventive examination of cervical cancer of the women interviewed, knowledge needs to be
enhanced in order to develop the practice, which is below the desired level in this City. At the
district level, the understanding of cervical cancer tracking is a relevant problem that requires
the attention of the authorities in order to perform actions aimed at reducing the incidence of
this disease and promoting early detection. Health promotion for the elderly is an important item
in public policies, respecting the dignity of the elderly and avoiding unnecessary examinations,
which onus public capital. / O envelhecimento populacional constitui um dos maiores desafios para a saúde pública
contemporânea. Como reflexo da melhoria na qualidade de vida, há manutenção do pleno
exercício da sexualidade na terceira idade e, com ele, os riscos de contrair Infecções
Sexualmente Transmissíveis (IST’s), dentre elas a infecção por Papilomavírus Humano (HPV).
A persistência da infecção pelo HPV de alto risco associada ao rastreamento deficiente da
população feminina são os principais fatores responsáveis pelo desenvolvimento do carcinoma
de colo uterino, que representa causa expressiva de morbimortalidade nesta faixa etária. Esta
pesquisa visou caracterizar o perfil de idosas residentes na Região Administrativa do Gama -
Distrito Federal (DF), com destaque nos aspectos preventivos do câncer de colo uterino,
comparando-se o perfil e atitude das idosas frente ao exame colpocitológico, atrelados ao seu
nível de conhecimento acerca do referido exame. Trata-se de estudo avaliativo do tipo inquérito
conhecimento, atitude e prática. A coleta de dados foi realizada por meio de um questionário,
sob entrevista direta, abordando aspectos concernentes a Conhecimento, Atitude e Prática
(CAP) das idosas em relação ao exame preventivo do câncer de colo uterino (EPCCU). Os
inquéritos CAP foram aplicados a 300 idosas, com idade de 60 e 93 anos, transeuntes em locais
de livre circulação do Gama-DF como feiras públicas, shoppings e centros de cultura e lazer
para idosos. A maioria era casada (43,3%), possuía ensino fundamental (60,3%), morava
próximo à unidade de saúde (83,7%) e teve coitarca a partir dos 18 anos (70,3%). Os dados
coletados mostraram: quanto ao Conhecimento, 4,7% responderam adequadamente; quanto à
Atitude, 93,0% apresentaram esta considerada adequada; e quanto à Prática, 50,7% foram
adequadas. No que tange ao conhecimento não foi observada correlação significativa com os
fatores sociodemográficos, enquanto em relação à Atitude houve correlação estatisticamente
significativa com a escolaridade, sendo maior naquelas com ensino superior. No quesito Prática,
houve associação significativa com a idade das idosas entrevistadas, sendo mais adequada
naquelas com faixa etária de 60 a 64 anos. Avaliando-se a identificação da percepção frente ao
exame preventivo do câncer de colo uterino das mulheres entrevistadas, o conhecimento precisa
ser aprimorado para que se tenha uma melhoria na prática, que se encontra aquém do desejado
nesta Região Administrativa. Em nível regional, a compreensão do rastreamento do câncer de
colo uterino apresenta-se como um problema relevante que necessita da atenção das autoridades,
a fim de executar ações voltadas à redução da incidência desta moléstia e à promoção de uma
detecção precoce. A promoção à saúde dos idosos é item importante das políticas públicas,
respeitando a dignidade da pessoa idosa e evitando realização de exames desnecessários, que
oneram os fundos públicos.

Avaliações econômicas do uso da vacina contra o Papilomavírus Humano (HPV) em meninas adolescentes: uma revisão sistemática / Economic evaluations of the use of HPV vaccine in adolescent girls: a systematic reviewCarlos José Coelho de Andrade 30 September 2010 (has links)
O câncer de colo do útero persiste como um importante problema de saúde em todo o mundo, em particular nos países em desenvolvimento. Duas vacinas contra o papilomavirus humano (HPV) encontram-se atualmente disponíveis e aprovadas para uso em meninas adolescentes, antes do início da vida sexual: uma bivalente, contra os sorotipos 16 e 18 e outra quadrivalente, contra os sorotipos 6, 11, 16 e 18. Estes imunobiológicos têm por objetivo induzir uma imunidade contra o papilomavírus e, desta forma, atuar na prevenção primária do câncer do colo de útero. As avaliações econômicas podem ser um elemento que auxiliem nos processos de tomada de decisão sobre a incorporação da vacina em programas de imunização nacionais. Estas avaliações foram o objeto central deste trabalho, que teve como objetivo sintetizar as evidências procedentes de uma revisão sistemática da literatura de estudos de avaliação econômica da utilização da vacina contra o HPV em meninas adolescentes e pré-adolescentes. Foi realizada uma busca na literatura nas bases MEDLINE (via Pubmed), LILACS (via Bireme) e National Health Service Economic Evaluation Database (NHS EED) ate junho de 2010. Dois avaliadores, de forma independente, selecionaram estudos de avaliação econômica completa, que tivessem como foco a imunização para HPV em mulheres com as vacinas comercialmente disponíveis direcionada à população adolescente. Após a busca, 188 títulos foram identificados; destes, 39 estudos preencheram os critérios de elegibilidade e foram incluídos na revisão. Por tratar-se de uma revisão de avaliações econômicas, não foi realizada uma medida de síntese dos valores de relação incremental entre custos e efetividade. Os 39 artigos incluídos envolveram 51 avaliações econômicas em 26 países. Predominaram estudos de custo-utilidade (51%). Do ponto de vista da perspectiva da análise, predominou o dos sistemas de saúde (76,4%). A maioria dos trabalhos (94,9%) elegeu meninas, com idade entre 9 e 12 anos, como sua população alvo e desenvolveu simulações considerando imunidade para toda a vida (84,6%). Os modelos utilizados nos estudos foram do tipo Markov em 25 análises, de transmissão dinâmica em 11 e híbridos em 3. As análises de sensibilidade revelaram um conjunto de elementos de incerteza, uma parte significativa dos quais relacionados a aspectos vacinais: custos da vacina, duração da imunidade, necessidade de doses de reforço, eficácia vacinal e cobertura do programa. Estes elementos configuram uma área de especial atenção para futuros modelos que venham a ser desenvolvidos no Brasil para análises econômicas da vacinação contra o HPV. / The cervical cancer persists as a major health problem worldwide, particularly in developing countries. Two vaccines against human papillomavirus (HPV) are currently available and approved for use in adolescent girls before the onset of sexual behavior: a bivalent against serotypes 16 and 18 and other quadrivalent against serotypes 6, 11, 16 and 18. These biopharmaceuticals are intended to induce immunity against papillomavirus and thus act in the primary prevention of cervix cancer. The use of vaccine in population programs with the definition of guidelines depends on an elaborate decision making process based on a careful Health Technology Assessment (HTA). The economic evaluations are part of this process. These assessments were the object of this work, which aimed to synthesize the evidence coming from a systematic literature review of studies on the economic evaluation of the use of human papillomavirus vaccination in adolescent girls and pre-teens. We performed a literature search in MEDLINE (via Pubmed), LILACS (via BIREME) and National Health Service Economic Evaluation Database (NHS EED) until June 2010. Two readers independently selected full economic evaluation studies that have focused on immunization for HPV in women with commercially available vaccines targeting the adolescent population. After the search, 188 of these titles were identified, 39 studies met the eligibility criteria and were included in the review. As a review of economic evaluations we did not perform a synthesis of the values of relationship between incremental cost and effectiveness. The 39 articles included 51 economic evaluations in 26 countries. Cost-utility studies predominate (51%). From the standpoint of the perspective of the analysis, there were predominant views of health systems (76.4%). Most studies (94.9%) chose girls, aged between 9 and 12 years as its target population (94.9%) and developed simulations considering immunity for life (84.6%). The models used in the studies were Markov in 25 analysis, transmission dynamics in 11 and hybrid models in 3. The sensitivity analysis revealed a number of important elements of uncertainty and that influenced ICER, a significant part of which related to aspects vaccine: the vaccine costs, duration of immunity, the need for booster doses, vaccine efficacy and program coverage. These elements make up an area of special attention for future models that may be developed in Brazil for economic analysis of vaccination against HPV.

in adolescence worldwide. The cervical cancer, related to this infection, is the leading cause
of death in Maranhão. The biomolecular diagnosis used to HPV detection is the polymerase
chain reaction (PCR). It is proposed to investigate the frequency of the HPV genotypes in
high school students from a public school in Sao Luis by different diagnostic methods. We
conducted a cross-sectional study, descriptive at adolescents in high school, totaling 39
female students aged 15 to 19 years in the period July 2008 to June 2009. A questionnaire
covering socio-demographic and sexual behavior was used. It was carried out cytopathology,
colposcopy, cervical biopsy and genotyping by Nested PCR in each sample. The inclusion
criteria were presence of sexual activity and we excluded women who had no sexual activity,
those with positivity for HIV and pregnant. The data were transferred to a spreadsheet in the
software Excel 2007 and evaluated using Epi-Info 6.04. The socio-demographic data showed
that most female adolescents studied was 15 to 17 years old, they lived in urban area and they
were full-time students, they were white and had schooling from 12 to 14 years. In 25
(64.1%) adolescents, the first sexual intercourse occurred before age 16 (p = 0.240), half said
to have among one and four sexual intercourses per week (p = 0.082), one partner (51.3%),
and two to three partners (38.5%), 82% reported ever having had STDs (p = 0.704). Three
reported abortion (p = 0.463). One reported having given birth to 17 years. As for gynecologic
complaints, the main were leukorrhea (79.5%, p = 0.016) and pruritus (23.1%, p = 0.928).
Eight (20.5%) reported family history of cancer. At colposcopy, there were no vulvar or
vaginal lesions and we diagnosed minor injury in seven adolescents (17.0%). The Pap smear
result showed inflammation in 37 (94.9%) and LSIL / HPV in a test. Histopathology was
performed in 15 students (38.5%), and seven had changes suggestive of HPV. Genotyping
was positive in 29 students (74.3%), predominantly to 14 high-risk genotypes in 21 (72.4%)
adolescents, namely: 16, 18, 31, 35, 39, 45, 51, 52, 56, 58, 59, 66, 68, 82, distributed in
multiple coexisting and simple infections. In assessing the accuracy of Nested PCR, positive
predictive value was 85.7% (true positives), and negative predictive value was 12.5% (true
negatives), Nested PCR provides useful diagnostic information to the study of human
papillomavirus, as multiple and simple infections diagnosis. / O papilomavírus humano (HPV) é o agente da infecção sexualmente transmissível mais
comum na adolescência em âmbito mundial. O câncer do colo do útero, relacionado a esta
infecção, é a principal causa de morte no Maranhão. O diagnóstico biomolecular usado é a
reação em cadeia de polimerase (PCR) para detecção do HPV. Propõe-se investigar a
frequência dos genótipos de HPV em adolescentes do ensino médio de uma escola pública de
São Luís-MA, por diferentes métodos diagnósticos. Realizou-se um estudo transversal,
descritivo, em adolescentes, cursando o ensino médio, totalizando 39 alunas com idade entre
15 a 19 anos no período julho de 2008 a junho de 2009. Utilizou-se questionário abordando
variáveis sócio-demográficas e comportamento sexual. Realizou-se citopatologia,
colposcopia, colpobiópsia e genotipagem por PCR-Nested em toda amostra. O critério de
inclusão compreendeu presença de atividade sexual e os de não inclusão foram mulheres que
não tiveram atividade sexual, positividade para HIV e gravidez. Os dados foram avaliados e
transferidos para uma planilha no software Excel 2007 e Epi-Info 6.04. Os dados sóciodemográficos
mostraram que a maioria das adolescentes estudadas tinha de 15 a 17 anos, era
de procedência urbana e estudantes em tempo integral, cor branca e escolaridade entre 12 e 14
anos. Em 25 (64,1%) adolescentes, a sexarca ocorreu antes dos 16 anos (p=0,240), a metade
disse ter de uma a quatro relações sexuais semanais (p=0,082); um parceiro (51,3%), e dois a
três parceiros (38,5%); 82% disseram nunca ter tido DST (p=0,704). Três relataram
abortamento (p=0,463). Uma informou parto aos 17 anos. Quanto às queixas ginecológicas, as
principais foram leucorréia (79,5%; p=0,016) e prurido (23,1%; p=0,928). Oito (20,5%)
relataram antecedentes de câncer familiar. À colposcopia, nenhuma apresentou lesões
vulvares ou vaginais, tendo sido diagnosticado lesão menor em sete adolescentes (17,0%). O
exame citopatológico apresentou resultado inflamatório em 37 (94,9%) e LSIL/HPV em um
exame. A histopatologia foi realizada em 15 alunas (38,5%), sendo que sete apresentaram
alterações sugestivas de HPV. A genotipagem foi positiva em 29 alunas (74,3%), com
predomínio para 14 genótipos de alto risco em 21 (72,4%) adolescentes, sendo eles: 16, 18,
31, 35, 39, 45, 51, 52, 56, 58, 59, 66, 68, 82, distribuídos em infecções múltiplas coexistentes
e simples. Na avaliação da acurácia por PCR - Nestad o valor preditivo positivo foi de
85,7% (verdadeiros positivos), e o valor preditivo negativo foi de 12,5% (verdadeiros
negativos), técnica que fornece informações diagnósticas úteis ao estudo do papilomavírus
como infecção múltipla e simples.

Introdução: A infecção genital pelo papilomavírus humano (HPV) é um fator necessário para o desenvolvimento do câncer cervical. Vacinas para prevenir a infecção pelos tipos de alto risco HPV 16 e 18 foram desenvolvidas e idealmente devem ser administradas antes da exposição ao HPV através do contato sexual. As variações na prevalência do HPV e na de seus tipos específicos em diferentes populações podem influenciar as recomendações da vacina contra o HPV em diferentes locais. A vacinação após o primeiro parto poderia ser uma estratégia em potencial para atingir mulheres jovens e saudáveis, dependendo da proporção de mulheres desse grupo ainda não infectadas pelos tipos de alto risco, HPV 16 e 18. Objetivos: O objetivo principal deste estudo foi determinar a prevalência genital do DNA de tipos específicos do HPV e avaliar a associação dessa infecção com fatores de risco selecionados em mulheres após o primeiro parto, usuárias de uma maternidade pública. Métodos: Esse estudo transversal foi realizado no Hospital Maternidade Leonor Mendes de Barros (HMLMB), uma das maiores maternidades públicas da cidade de São Paulo. Durante junho de 2006 até fevereiro de 2007, 301 primíparas de 15-24 anos, cujos partos ocorreram no referido Hospital, foram incluídas no estudo entre 43 e 60 dias após o parto. Na detecção de DNA do HPV extraído das células cervicais esfoliadas foi utilizado protocolo padrão da Reação em Cadeia por Polimerase (PCR), utilizando primers PGMY09/11. Para estimar a associação da infecção por HPV com fatores de risco selecionados, foi calculada a Razão de Prevalência (RP) e o intervalo de 95% de confiança [IC]; o ajuste foi realizado utilizando-se o Modelo Linear Generalizado (MLG) com distribuição binomial e função de ligação logarítmica. Resultados: O DNA do HPV foi detectado em 58,5% (IC 95% 52,7%-64,0%) das jovens mulheres. Os tipos de HPV mais comumente encontrados foram: HPV 16, HPV 51, HPV 52, HPV 58 e HPV 71. A prevalência dos tipos de HPV incluídos nas vacinas profiláticas foi: HPV 16 - 12,0%, HPV 18 - 2,3% e HPV 6+11 - 4,3%. Os tipos de alto risco de HPV foram encontrados em 133 (44,2%) mulheres, enquanto 43 delas (14,3%) apresentaram somente tipos de HPV de baixo risco. Cento e duas mulheres (33,9%) foram positivas para apenas um tipo de HPV; entretanto, 43 (14,3%) apresentaram dois tipos, e 31 (10,3%) apresentaram três ou mais tipos virais. A análise multivariada revelou que somente a idade (p=0,020) e o hábito de fumar (p <0,001) foram fatores de risco independentemente associados com a infecção por HPV. Conclusões: Essas adolescentes e jovens primíparas apresentaram elevada prevalência de infecção genital por tipos de alto risco do HPV, mostrando que constituem um grupo de risco para o desenvolvimento de câncer cervical. Contudo, apenas 17,3% apresentaram pelo menos um dos quatro tipos virais presentes na vacina quadrivalente (HPV 6, 11, 16 ou 18), 13,3% apresentaram infecção pelos tipos HPV 16 ou 18, e somente 1,0% apresentou concomitantemente infecção por esses dois tipos virais de alto risco presentes nas vacinas. Portanto, esse estudo indica que a grande maioria dessas jovens primíparas poderia ainda se beneficiar da imunização (catch-up) contra o HPV e constitui um grupo que deve ser alvo de programas efetivos de prevenção primária e secundária para o câncer cervical / Introdution: Genital infection by human papillomavirus (HPV) is a necessary factor in the development of cervical cancer. Vaccines to prevent infection by high risk HPV genotypes 16 and 18 were developed and ideally should be administered before exposure to HPV through sexual contact. Variations in HPV prevalence in different populations and of specific HPV types could affect vaccine recommendations in different settings. Vaccination after first delivery could be a potential strategy for reaching healthy young women depending on the baseline prevalence of high risk genotypes 16 and 18 in this target group. Objectives: The main objective of this study was to determine genital type specific HPV DNA prevalence and selected risk factors associated with HPV infection after the delivery of the first child among young women in a public maternity. Methods: This cross-sectional study was carried out at Hospital Maternidade Leonor Mendes de Barros (HMLMB), one of the largest public maternity hospitals in Sao Paulo. During June 2006 to February 2007, 301 primiparous women aged 15-24 years, who gave birth at that hospital, were included in the study between 43 and 60 days after delivery. Detection of HPV DNA in cervical specimens was performed using a standardized polymerase chain reaction (PCR) protocol with PGMY09/11 primers. To estimate the association of HPV infection with selected risk factors, prevalence ratios (PR) and 95% confidence interval [CI] were estimated using a Generalized Linear Model (GLM) with binomial distribution and log link function. Results: Any HPV DNA was detected in 58.5% (95% CI 52.7%-64.0%) of the enrolled young women. Most common types of HPV found were: HPV16, HPV51, HPV52, HPV58 and HPV71. The overall prevalence of HPV types targeted by the HPV prophylactic vaccines was: HPV16 - 12.0%, HPV18 -2.3% and HPV 6+11- 4.3%. High-risk HPV types were found in 133 (44.2%) women, whereas 43 women (14.3%) had only low-risk HPV types. One hundred and two women (33.9%) were positive for one HPV type only; however, 43 (14.3%) had two types, and 31 (10.3%) had three or more types detected. The multivariate analysis revealed that only age (p for trend =0.020) and smoking habits (p <0.001) were risk factors independently associated with HPV infection. Conclusions: These adolescents and young primiparous women had high cervical HPV prevalence, suggesting that this is a high risk group for cervical cancer development. Nevertheless, 17.3% were positive to any of the four HPV types included in HPV vaccines (HPV6, 11, 16 or 18), with 13.3% positive for HPV 16 or 18, and only 1.0% of them had both vaccine related oncogenic HPV types. Thus, this study supports that the most part of young primiparous women could benefit from catch-up HPV vaccination, and represents a target group for effective primary and secondary cervical cancer prevention programs

This dissertation thesis touches very current and among public and experts often discussed topic. It concerns particularly matter of papillomaviral infections and vaccination against them. The research proved vaccination-coverage level of respondents and also their knowledge level. The vaccination-coverage level of girls is very high (almost 80%). A lot of girls also used possibility of full vaccination in the age of 13. This dissertation thesis should serve as a source and summary information about HPV viruses, diseases caused by them, about ways of transmission and epidemiological measures. Furthermore it also informs about cervical cancer, risk factors, diagnostical possibilities, therapy and at last but not lest about prevention of cervical carcinoma.
