Evaluación de la hipersensibilidad dentinaria post terapia quirúrgica a colgajo periodontal

La asociación entre la terapia periodontal de cirugía a colgajo y la disminución de la hipersensibilidad dentinaria fue estudiada en 100 piezas dentarias de pacientes con periodontitis, que fueron atendidos en los consultorios de la Sección de Periodoncia de la Rama de Salud Oral del Hospital de la Fuerza Aérea del Perú, a los cuales se les realizó dos evaluaciones pre quirúrgica y post quirúrgica después de los 14 días de cicatrización. La hipersensibilidad dentinaría se determinó por pruebas de estimulación al frío y al sondaje. La prevalencia de hipersensibilidad en pacientes con periodontitis fue de 38%. De 38 casos con hipersensibilidad pre quirúrgica, 26(68.4%) no la presentó en la evaluación post quirúrgica; y de 62 casos sin hipersensibilidad pre quirúrgica, sólo 5(8.1%) la presentó en la evaluación post quirúrgica. Se reportó una asociación significativa entre la terapia periodontal de cirugía a colgajo y la disminución de la hipersensibilidad dentinaria. El promedio de nivel de exposición radicular fue de 1.04mm, el índice de higiene dental aumentó en la evaluación post quirúrgica. No hubo diferencia significativa en la distribución de la hipersensibilidad según clasificación anatómica de piezas dentarias, sexo y edad. / The association between the periodontal therapy of flap surgery and the décrease of the dentine hypersensitivity was studied in 100 teeth of patients with periodontitis from the Section of Periodoncía of the Oral Health Branch office of the Air Force Hospital in the city of Lima -Perú , they were examinated by two evaluations pre surgical and post surgical after the 14 days of scaring. The dentine hypersensitivity was determined by stímulation tests to the cold and the probe, The dentine hypersensitivity in patíent wíth periodontitis was of 38%. Of 38 cases with hypersensitivity pre surgical , 26(68.4%) didn't have it in the evaluation post surgical; and of 62 cases without hypersensitivitY pre surgical only 5(8. 1 %) had it in the evaluation post surgical. A significant association was reported between the periodontal therapy of flap surgery and the decrease of the dentine hypersensitivity. The average of level of root exhibition was of 1.04mm, the index of dental hygiene íncreased in the evaluation post surgical. There was not significant dífference in the distribution of the hypersensitivitY according to teeth anatomical classification, sex and age.

Periodontitis crónica asociada a placa bacteriana en pacientes con enfermedad cardiovascular-aterosclerosis

Existen diversos estudios que relacionan a las Enfermedades Cardiovasculares relacionadas a Aterosclerosis con la Infección que producen las Enfermedades Periodontales, entre ellas la Periodontitis Crónica, estos se dan por diversos mecanismos inflamatorios, de reacción cruzada y de entrada directa de bacterias orales al torrente sanguíneo. El presente estudio tuvo la finalidad de identificar la frecuencia y porcentaje de aquellos pacientes que tienen Periodontitis Crónica asociada a Placa Bacteriana en una muestra de pacientes con Enfermedad Cardiovascular con el uso de una fuente secundaria (Historias Clínicas). Se consultaron 172 Historias Clínicas Médicas con el debido diagnóstico médico de Angina de Pecho, Infarto de Miocardio y Enfermedad Coronaria Crónica, todas ellas del Departamento de Cardiología del Centro Médico Naval “Cirujano Mayor Santiago Távara”, con esta muestra se consultaron las Historias Clínicas Odontológicas en la cual se observaron los siguiente resultados: 33,7% presentaban un diagnóstico de Periodontitis Crónica y 66,3% no presentaba ningún diagnóstico periodontal, este resultado se comparó con el resultado obtenido por Lipari y col. (45%) mediante la Prueba de Proporciones resultando una diferencia de resultados al 95% de confianza. Dentro de los factores de riesgo para padecer Enfermedad Cardiovascular relacionada a Aterosclerosis podemos mencionar que la Hipertensión Arterial es la principal enfermedad que tiene una cantidad mayor de pacientes con periodontitis seguidos de las Dislipidemias y la Diabetes. Según los resultados obtenidos en el presente estudio se concluye que existe una baja prevalencia de periodontitis crónica en pacientes con enfermedad cardiovascular relacionada a aterosclerosis. / There are several studies related to cardiovascular disease related to atherosclerosis with infections that cause periodontal diseases, including chronic periodontitis, these are given by various inflammatory mechanisms, cross-reactive and direct entry of oral bacteria into the bloodstream. This study was intended to identify the frequency and percentage of patients with chronic periodontitis associated with bacterial plaque in a sample of patients with cardiovascular disease using a secondary source (Medical Histories). 172 were consulted medical history with proper medical diagnosis of angina pectoris, myocardial infarction and chronic coronary disease, all from the Department of Cardiology, Naval Medical Center "Cirujano Mayor Santiago Távara" with this show were consulted Dental Medical Histories in which we noted the following results: 33.7% had a diagnosis of chronic periodontitis and 66.3% no periodontal diagnosis, this result was compared with the result obtained by Lipari et al. (45%) using the proportions test results resulting in a difference of 95% confidence. Among the risk factors for cardiovascular disease related to atherosclerosis may be mentioned that hypertension is a major disease with a greater number of patients with periodontitis followed by dyslipidemia and diabetes. According to the results obtained in the present study concludes that there is a low prevalence of chronic periodontitis in patients with cardiovascular disease related to atherosclerosis.

Matrix Metalloproteinase-7 Degradation of Fetuin Blocks Fetuin-mediated Inhibition of Mineralization

Mechanisms underlying the association between atherosclerosis and periodontitis are not defined. Matrix metalloproteinases (MMPs) are increased in untreated periodontitis. Fetuin, a serum protein, inhibits vascular mineralization in humans. I hypothesized that MMP-7 affects fetuin function. In vitro mineralization was conducted based on methods developed by Hunter and colleagues. Mineralization was quantified using absorbance measurements (at 540 nm) of alizarin red bound to nascent crystals. Mineralization assays included intact MMP-3 and MMP-7-treated human and bovine fetuin (0-2 μM).The inhibition of mineralization mediated by fetuin (2 μM) was maximal at 3.5-4 hours. Transmission electron microscopy and electron diffraction analysis of crystals formed revealed the presence of hydroxyapatite. SDS-PAGE demonstrated that MMP-7 is more effective in digesting fetuin than MMP-3. The inhibitory effect of fetuin on mineralization was more reduced by MMP-7 digestion compared to MMP-3. Fetuin inhibits mineralization in vitro and this effect is reduced by MMP-7 degradation.
