Situation Awareness in LPNs: a Pilot Study

Purpose: The purpose of this pilot study was to describe situation awareness (SA) among licensed practical nurses (LPNs) working in direct patient care. Specific Aims: The specific aims for this study are 1) to examine SA scores, as measured by the Situation Awareness Global Assessment Technique (SAGAT), in LPNs working in direct patient care and compare to published data on SA in registered nurses (RNs), 2) to examine the relationship between SA scores and years of LPN experience, 3) to examine differences in SA scores by type of workplace setting and 4) to describe the relationship between levels of satisfaction with simulation, as measured by the Satisfaction with Simulation Experience Scale (SSES) and SA scores among LPNs. Framework: Situation Awareness Theory, as described by Endsley, was used as the framework for this study. Design: A cross-sectional, descriptive design using the Situation Awareness Global Assessment Technique was used to gather data from a convenience sample of LPNs. Results: LPNs (N=24) participated in the study and achieved an average SAGAT score of 72.6%. There were no differences in scores between those LPNs enrolled in an RN program and those who were not enrolled. Individual scores on the SAGAT were comparable or better than scores in a similar study of RNs. Conclusion: LPNs in this study demonstrated adequate situation awareness. Den här avhandlingen handlar om vårdlärares arbete med simulering av arbetsuppgifter i metodrum Ett metodrum kan ses som en hybrid mellan klassrum och sjukhussal. I denna skolmiljö övas arbetsuppgifter som förekommer inom vård och omsorg. Det saknas patienter att öva på och ibland sjukvårdsmaterial, vilket innebär en utmaning för vårdläraren att skapa en autentisk miljö. Syftet med studien är att undersöka hur vårdlärare arbetar med simulering av arbetsuppgifter i metodrumsundervisning för vuxna elever. Avhandlingen grundar sig på ett sociokulturellt perspektiv och har analyserats med hjälp av kvalitativ innehållsanalys. Tre metodrumslektioner från tre olika skolor ljudinspelades digitalt : venprovtagning, urinkatetrisering och såromläggning. Resultatet visar att vårdlärarna arbetar med tre olika simuleringsaktiviteter: Tekniskt utförande, Tolkande och Bemötande. Simuleringsaktiviteternas utförande varierar med lektionsinnehållet. Studien visar vilka redskap i metodrumsundervisningen som är vanligt förekommande och hur de används. Ingela Leibring har bakgrund som barnmorska och arbetar för närvarande som vårdlärare inom kommunal vuxenutbildning i Karlstad samt är utbildningsledare för Yrkeshögskolans utbildning till barnspecialiserad undersköterska. / This study centres on clinical skills laboratory teaching in municipal adult education. This type of teaching takes place in a school setting and is hybrid of a nursing and healthcare environment. Future professional tasks are simulated and practised here. Three different schools were visited and three clinical skills laboratory lessons were recorded with a digital audio recorder. The following tasks were included in the study: venipuncture, urinary catheterisation, and wound dressing. The analysis was performed as a qualitative content analysis informed by a sociocultural perspective. The aim of the study is to extend the knowledge of how teachers approach the simulation of tasks in clinical skills laboratory training. The study identifies three simulation activities and their application: technical performance, interpretation, and treatment.  The main teaching tools used in the lessons were the teacher’s body/hands, different physical tools specific to the respective laboratory session and the teacher’s language. All the simulation tasks – venipuncture, urinary catheterisation and wound dressing ­– were included in the sessions. A similarity was that technical performance on the whole was the most common feature in all the laboratory lessons. A difference emerged when the clinical skills laboratory lesson was divided into the components: introduction, students practise themselves, and conclusion. The lesson introduction included treatment to nearly the same extent as technical performance. Differences related to lesson content were evident in the activities interpretation and treatment. A common pattern was that the teachers shifted between everyday language and professional language when they worked with simulation. They named their patients and gave the students immediate response.

This is a panel study of 148 students between the ages of 16 and 19 who are enrolled in a three year practical nursing education programme. The aim of the study is to investigate the socialisation process into the nursing world by studying specific attitudes towards nursing, elderly people and general life values and beliefs. Another aim is to locate this groups position in the nursing education context. Part of the study is a description of differences between registered and practical nurses. Some of these are their professionalism, their attitudes to knowledge, their respective trade union actions and the development of a hospital culture. The development of a separated and a coordinated education route for registered and practical and nurses is also analysed. Data for the panel study were collected every year by questionnaires and groups discussions. The results show that the majority of the students are empathetic towards patients. The majority have also had difficult experiences during their student nursing practice associated with their encounter with death. Two out of five students spend a considerable amount of time with elderly. Most respondents experience a sense of security from the elderly and feel that what they have to say is important. Approximately 30 percent of the students give helping the elderly as the reason for entering the practical nursing programme. However, only about 11 percent want to work with the elderly in their future jobs as practical nurses. The majority of the students feel that life is meaningful and almost half of them regard their future prospects as bright. The only apparent connection between different attitudes is the one between religious belief and wanting to work with old people. A larger share of religious students are prepared to work in old age care. <p>Abstract</p><p>This essay is an investigation where I try to gain an increased knowledge of the three assistants’ (in the mentally disability profession) opinion about their education and their first years in their profession. I have also tried to answer the questions whether the course mentally disability/functional gives the students a good start in their profession, if there are any flaws or if something is missing in the course or if they can think of any improvements to make the course better.</p><p>This investigation is performed as a qualitative interview study where three women were interviewed about their education and their first years as assistants’ (in the mentally disability profession). All the interviewed women have been working in the care activity for two or three years since they graduated. The investigation shows that there is a great need for practical training for students on the mentally disability/functional disability course, since it leads to a better start in their profession. Rural Community Colleges and the Nursing Shortage in Severely Distressed Counties

The United States is in the middle of a gripping nursing shortage; a shortage that is putting patients' lives in danger. This study determined the impact community and tribal colleges in severely economically distressed counties of the United States have on the nursing shortage faced by health care facilities serving these areas. The initial sample of 24 institutions selected in the Ford Foundation's Rural Community College Initiative (RCCI) (1995-2000). Data were collected from the Fall 1998 National Study of Post Secondary Faculty to obtain characteristics of faculty and from the 2003 Integrated Postsecondary Education Data System (IPEDS) to obtain characteristics of students, both at all publicly-controlled community colleges, all tribal colleges, and the 24 RCCI colleges that included 18 community and six tribal colleges. A survey was sent to the directors/deans/chairs of the nursing programs to ascertain issues related to the nursing program, nursing faculty, and nursing students. Respondents were asked to identify the healthcare facilities used for students' clinical experiences. A survey was then sent to each of these facilities asking about rural health, and source of nursing staff. Findings: 1) 87% of these these rural healthcare facilities are experiencing a significant shortage of nurses, and they are challenged to recruit and retain nursing staff; 2) Nursing programs, including both Licensed Practical Nursing and Associate's Degree Nursing are important to these rural community and tribal colleges, have seen growth over the past 5 years and expect to continue growth (86%); 3) Financial aid for nursing students is critically important; 4) Students are predominantly white and female; minorities are significantly under-represented; 5) Lack of subsidized public transportation and child care for nursing students even at tribal colleges are barriers that impact program completion; and 6) A shortage of nursing faculty exists at rural community and tribal colleges that negatively impacts student enrollment in these programs, thus reducing the rural nursing workforce pipeline. It is the rural community and tribal college nursing programs help provide severely economically distressed counties of the United States with the nursing workforce needed to decrease the nurse to patient ratio.
