Adolescents agresseurs sexuels : l’alliance thérapeutique et la motivation au changement en contexte d’autorité

La littérature scientifique s’intéressant aux adolescents agresseurs sexuels démontre qu’il s’agit d’une population qui apparait souvent résistante au changement (Rich, 2003). Il importe ainsi d’identifier les facteurs qui favorisent ou compromettent le déroulement de l’intervention auprès de cette clientèle. L’alliance thérapeutique et la motivation au changement figurent d’ailleurs parmi ces facteurs ayant démontré leur influence positive. Toutefois, considérant le caractère dynamique de ces dernières, peu d’études ont documenté les particularités de leur développement respectif, spécialement dans un contexte d’autorité tel que celui des adolescents agresseurs sexuels. La présente recherche permet donc de mieux comprendre la contribution de caractéristiques individuelles et environnementales sur le développement de l’alliance thérapeutique et le processus de changement chez cette clientèle juvénile. Basés sur une collecte de données quantitatives auprès de 166 adolescents agresseurs sexuels suivis dans différents centres de traitement spécialisés en délinquance sexuelle au Québec, les résultats obtenus à partir d’analyses corrélationnelles et multivariées illustrent de nombreuses relations significatives entre un ensemble de caractéristiques individuelles et environnementales et le développement de l’alliance thérapeutique et de la motivation au changement. En effet, différents traits de personnalité, les distorsions cognitives, les habiletés sociales, la détresse post-traumatique et les stratégies de coping sont plus ou moins associés à l’établissement d’une relation d’aide et d’une motivation à changer de comportement. De plus, les caractéristiques de l’environnement institutionnel et la perception du soutien social reçu se veulent des facteurs qui influencent généralement de façon positive les variables à l’étude. L’utilisation du Modèle transthéorique du changement permet de nuancer les résultats en ce sens que les facteurs explicatifs de la motivation peuvent différer d’une étape à l’autre dans le cycle du changement. Ainsi, les services offerts aux adolescents ayant commis une agression sexuelle doivent tenir compte du degré de motivation puisque celui-ci semble être associé à des caractéristiques individuelles et environnementales spécifiques. Finalement, il est important de mentionner que les conclusions de cette étude ne permettent pas d’établir des relations causales entre les variables à l’étude. Néanmoins, les résultats indiquent l’importance de porter attention aux facteurs de changement, qui peuvent différer d’une étape à l’autre dans le cycle du changement, et ceux associés au développement de l’alliance thérapeutique. / The scientific literature pertaining to adolescent sexual aggressors indicates that it is a population which often seems to be resistant to change (Rich, 2003). It is important to identify the reasons that promote or compromise the intervention with this clientele. The therapeutic alliance and the motivation to change are among the factors that have demonstrated a positive influence. However, taking into consideration the dynamic nature of the latter, few studies have documented the particularities of their respective development, especially when the adolescent sex offenders are placed in a context of authority. The current study makes it possible to better understand the contribution of individual and environmental traits on the development of a therapeutic alliance and the process of change with this youthful clientele. Based on a quantitative data from 166 adolescent sex offenders tracked in different centers specialized in sexual delinquency in Quebec, the results from correlational and multi-varied analysis show a significant relationship between a set of individuals and environmental characteristics, and the development of the therapeutic alliance and motivation to change. Indeed, different personality traits, cognitive distortions, social skills, post-traumatic stress and coping strategies are more or less linked with the establishment of a helping relationship and motivation to change behaviour. In addition, the characteristics of the institutional environment and the perception of social support received, appear to be factors that generally positively influence the variables under study. The Transtheoretical Model of Change enlightens the results in such a way that the factors explaining the motivation may differ from one step to another in the cycle of change. Thus, the services available to youth who have committed a sexual assault must take into account the degree of motivation, since it seems to be associated with specific individual and environmental characteristics. Finally, it is important to note that the findings of this study do not establish causal relationship between variables in the study. Nonetheless, the results show the importance of awareness of the changing factors, which can differ from one step to the next in the cycle of change and those associated with the development of a therapeutic alliance.

Organizational Change: Evaluating the Effect of Motivational Interviewing on Readiness to Change

Failure accompanies most organizational change efforts. Change agents' efforts focus on employee resistance or readiness to change without considering employee ambivalence. Motivational interviewing (MI) may reduce ambivalence and improve the success rate of organizational change initiatives. The purpose of this experimental research was to evaluate the effectiveness of MI to increase readiness to organizational change, to assess the influence of MI on change-related beliefs, and to investigate the relationship between beliefs and readiness to change. The theoretical framework was the transtheoretical model of change, the theory of planned behavior, and social cognitive theory. Through random assignment, 56 employees of a company undergoing change and located in the Midwest region of the United States populated the experimental and control groups. Members of the experimental group participated in 3 motivational interviewing sessions over a 30-day period. Participants indicated their readiness and underlying change-related beliefs by completing the Job Change Ladder and the Organizational Change Recipients' Belief Scale. Within and between group differences from a mixed ANOVA revealed that MI significantly increased readiness to change. There was not a significant difference between the beliefs of both groups as indicated by the results of the MANOVA test. Participants' beliefs explained readiness to change as evidenced by the results from the use of multiple regression. The findings indicate that leaders of organizational and societal change initiatives could incorporate MI to prepare individuals and groups to embrace the change process, thereby improving the chances that the change initiative will be successful.

Influences of Nutritional Food Label Understanding in African-American Women with Obesity

Nutritional food label understanding (NFLU) in African American Women (AAW) is a philosophy that addresses obesity. Public health efforts have implemented nutritional and caloric information to packaged and restaurant foods to improve nutrient and calorie literacy. Research suggest NFLU might have a minimal effect on reducing obesity. However, it is not known how obese AAW born during the baby-boom era (51 to 64 years of age) perceive NFLU in relation to their dietary behaviors. The purpose of this phenomenological study was to understand the lived experience of obese AAW regarding NFLU from a cognitive and behavioral perspective. Twelve, AAW answered 21-semi-structured questions that were audio recorded and transcribed verbatim. Findings of the study revealed the limited appreciation of NFLU in relation to healthy nutrition behaviors. Reasons for not applying nutritional food label (NFL) information centered on self-help perception, the time to read and understand the content on the NFL, skills required for effective NFL usage (math, organization of content), barriers to overcome while grocery shopping and motivational interest to change their behavior. In addition, the interviews of obese AAW revealed a lack of interest in NFL information while dining out despite understanding the perceived health benefits of knowing such information. Positive social change implications for obese AAW include improved nutrition literacy and nutritional behavior using NFLU as the guide to healthier dietary choice. From an individual, community, societal and nation level, reversing the trajectory of obesity through nutritional health literacy needs further improvement and individual adoption to possibly assist with obesity self-management.

With this bachelor thesis we want to research the correlation between motivation and coerced addiction treatment among clients. We want to know more about what the clients think about coerced addiction treatment, what possibilities there are to create motivation to change and what the clients experiences of the motivational work is. Coerced addiction treatment has been a part of Swedish history for a long time. Even though it has changed over the years there is still the question regarding how ethical it is and also whether coerced treatment is the most effective way to help people with an addiction problem. When a person is taken against their will to enter addiction treatment it is most likely done as a last resort trying to save a life. There are no written guidelines on how each treatment-institution should work regarding treatment and motivation, it is up to every single institution itself to decide. To get a better understanding of this we have done a literature study of two doctoral dissertations that has focused on clients opinion regarding coerced treatment and motivational work. Our findings showed us that motivation during coerced treatment is very complex because there are a number of factors that play important roles. Even though the opinion from the clients regarding their own coerced treatment and the motivational work at LVM-institutions varied, it gave us the understanding that there needs to be change. There needs to be more research on how to best help and support people with addiction problems during coerced treatment.
