Disfunção erétil, auto-referida e segundo o Índice Internacional de Função Erétil, em doadores de sangue / Self-reported erectile dysfunction and according to the International Index of Erectile Function in blood donors

INTRODUÇÃO: Inúmeros estudos epidemiológicos realizados nos mais diversos países sugerem alta prevalência de disfunção erétil (DE), mas não há consenso acerca de sua exata magnitude. MÉTODOS: Foi conduzido um estudo para estimar a prevalência de DE e investigar seus fatores associados em homens com idade entre 40 e 60 anos, considerados sadios. Tal estudo de corte transversal com doadores de sangue, heterossexuais, com parceira estável há pelo menos seis meses, incluídos entre janeiro de 2006 e julho de 2007, investigou a presença da DE por meio do Índice Internacional de Função Erétil (IIFE) e também com pergunta direta, para auto-avaliação. Foram investigados fatores sócio-demográficos, história clínica, características do relacionamento afetivo e sexual, qualidade de vida, presença de sintomas depressivos e/ou ansiosos e de stress. RESULTADOS: A amostra incluiu 300 sujeitos, dos quais 12 foram excluídos da análise porque faziam uso de medicação oral para ereção, metade dos quais sem orientação médica. Dos 288 sujeitos analisados, 92 (31,9 %; IC95%: 26,6% a 37,7%) foram classificados pelo IIFE como tendo algum grau de disfunção erétil: 87 (30,2%; IC95%: 25,0% a 35,9%) indivíduos possuíam disfunção leve, quatro (1,4%; IC95%: 0,4% a 3,5%), disfunção moderada e um (0,3%; IC95%: 0,01% a 1,9%), disfunção completa. Entretanto, apenas nove (3,1%; IC95%: 1,4% a 5,8%) referiram não se sentirem potentes sexualmente. A concordância entre DE detectada pelo IIFE e DE referida foi baixa (kappa = 0,11; p < 0,001). Dos nove indivíduos que não se sentiam potentes sexualmente, apenas dois (22,2%) procuraram tratamento. Os fatores associados à presença da DE detectada pelo IIFE, após regressão logística multivariada, foram: inatividade profissional (OR = 3,3; IC95%: 1,4 a 7,8; p = 0,005); suspeita de transtorno depressivo e/ou ansioso (OR = 2,6; IC95%: 1,1 a 6,3; p = 0,033); desejo sexual bom (não excelente) (OR = 2,5; IC95%: 0,9 a 7,0; p = 0,082), ou baixo/moderado (OR = 5,5; IC95%: 1,2 a 25,1; p = 0,027) e referir DE (OR = 7,5; IC95%: 0,8 a 71,7; p = 0,080). CONCLUSÕES: A prevalência da DE, segundo o IIFE, foi alta, embora inferior à observada em outros estudos conduzidos com voluntários e usuários de serviços de saúde. A concordância entre DE, segundo o IIFE e auto-referida, foi baixa, possivelmente porque as duas formas de investigação avaliam diferentes dimensões da função erétil (aspectos funcionais versus aspectos subjetivos). Há grande interação entre os fatores biológicos, culturais e psicológicos no surgimento e manutenção da DE. É importante que os profissionais de saúde estejam atentos para a saúde sexual dos homens, especialmente após os 40 anos de idade, e para as possíveis condições que podem acompanhar a DE (doenças sistêmicas, sofrimento psíquico e outros aspectos psicossociais). / INTRODUCTION: Several epidemiological studies from different countries indicate high prevalence of erectile dysfunction (ED) but there is no consensus about its actual magnitude. The objective of the present study was to estimate the prevalence of ED and describe its associated factors in healthy men. METHODS: Crosssectional study comprising a sample of heterosexual blood donors, aged between 40 and 60 years, with a steady partner for at least six months, was carried out between January 2006 and July 2007. ED was assessed using the International Index of Erectile Function (IIEF) as well as direct inquiring for self-assessment. Data was collected on sociodemographic characteristics, medical history, affective and sexual behavior, quality of life, presence of depression and/or anxiety symptoms and stress. The study sample comprised 300 subjects, of which 12 were excluded from the analysis since they were taking oral medication for erectile dysfunction, and half of them took this medication without any prior medical advice. RESULTS: Of 288 subjects analyzed, 92 (31.9%; 95% CI: 26.6% 37.7%) had any degree of erectile dysfunction according to IIEF: 87 (30.2%; 95% CI: 25.0% 35.9%) had mild; four (1.4%; 95% CI: 0.4% 3.5%) had moderate; and one (0.3%; 95% CI: 0.01% 1.9%) had severe dysfunction. Only nine subjects (3.1%; 95% CI: 1.4% 5.8%) reported not feeling sexually potent. The concordance between ED assessed using IIEF and self-reported ED was low (kappa = 0.11; p<0.001). Of the nine subjects who reported not feeling sexually potent, only two (22.2%) sought treatment. The factors associated to ED assessed using IIEF in the multivariate logistic regression analysis were: professional inactivity (OR = 3.3; 95% CI: 1.4 7.8; p = 0.005); suspected depressive and/or anxiety disorder (OR = 2.6; 95% CI: 1.1 6.3; p = 0.033); satisfactory (but not strong) sexual desire (OR = 2.5; 95% CI: 0.9 7.0; p = 0.082) or low/moderate (OR = 5.5; 95% CI: 1.2 25.1; p = 0.027); and self-reported ED (OR = 7.5; 95% CI: 0.8 71.7; p = 0.080). CONCLUSIONS: ED prevalence according to IIEF was high though lower than that found in other studies conducted in voluntary health service users. The concordance between ED according to IIEF and selfreported ED was low, probably because these two approaches assess different dimensions of the erectile function (functional versus subjective components). Biological, cultural and psychological factors strongly interact in the development and maintenance of ED. Health providers should give special attention to sexual health of men, particularly those over 40, and to potential conditions accompanying ED (systemic diseases, psychic distress, and other psychosocial conditions)

Foi avaliado o conhecimento de enfermeiros intensivistas sobre métodos e medida da Pressão Arterial (PA) em UTI-A, além de ser verificada a percepção desses sujeitos quanto ao que sabem sobre medida da PA. Participaram do estudo 54 enfermeiros que responderam a um questionário com 65 questões que abordavam os diversos aspectos de medida da PA, assim como aspectos de auto-avaliação. O questionário foi dividido em três partes. A primeira era composta de dezenove questões que abordavam a caracterização do sujeito. A segunda continha quarenta questões, divididas em seis domínios relacionados à medida da PA com os diferentes métodos de medida encontrados em UTI-A. Nesta parte, cada questão correta possuía um escore de 0,25. A terceira parte continha seis questões destinadas à auto-avaliação. Ao responder a segunda parte, os sujeitos obtiveram escore médio de 4,6 (correspondente ao acerto de 18 a 19 questões). A maior taxa de erros concentrou-se no domínio métodos indiretos: apenas 7,8% dos sujeitos obteve um percentual de acertos de 60%. A maior taxa de acertos ocorreu no domínio método de medida direta da PA, em que 75,1% da amostra atingiu um percentual de acertos acima de 50%. Quanto à auto-avaliação dos sujeitos sobre seu conhecimento sobre medida da PA, 40 (74,1%) sujeitos se referiram como bons antes de responder ao questionário, e apenas 04 (7,4%) mantiveram este conceito após realização do teste de conhecimento. Nenhum sujeito se sentiu totalmente satisfeito em relação ao que sabe sobre PA, quatro deles (7,4%) consideraram-se satisfeitos, 27 (50%) mostraram-se pouco satisfeitos, 14 (26%) insatisfeitos e seis (11,1%) totalmente insatisfeitos. Três sujeitos (5,5%) não responderam a esta questão. Apesar dos sujeitos possuírem aproveitamento insuficiente no teste, eles referiram que este assunto é importante para atuação prática em UTI. O estudo revela lacunas no conhecimento dos enfermeiros intensivistas abordados sobre aspectos importantes envolvidos na medida da PA com os três métodos de medida (canulação arterial, oscilometria e auscultatório/palpatório) assim como retrata a consciência desses sujeitos com relação a este déficit. Estratégias de atualização são sugeridas para revisar a técnica com os diferentes métodos de medida da PA disponíveis nas UTI-A estudadas, assim como aspectos gerais sobre PA em educação continuada, visto que enfermeiros de terapia intensiva necessitam da segurança na realização desta técnica para orientar sua equipe, assim como guiar condutas diante de valores de PA no cuidado ao paciente grave / Abstract: This cross sectional descriptive study was conducted in three adult intensive care units (ICU-A), environment with highly skilled and able professionals to care for clients in critical condition. We assessed the knowledge of nurses about methods and measurement of blood pressure (BP) in ICU-A and verified their satisfaction about what they know about BP measurement. Study participants were 54 nurses who answered a questionnaire with 65 questions that addressed various aspects of BP measurement, as well as aspects of self-evaluation. The questionnaire was divided into three parts. The first part was composed of nineteen questions which addressed the characterization of the subject. The second one contained forty questions, divided into six domains related to the BP measurement with different methods present in ICU-A. In this part, each question had a correct score of 0.25. The third part contained six questions related to self-evaluation. In answering the second part, the subjects had a mean score of 4.6 (corresponding to 18 to 19 correct answers). The highest rate of errors occurred on indirect methods area: only 7.8% of subjects obtained a percentage of correct responses of 60%. The highest percentage of correct answers occurred in the domain method of direct measurement of BP, where 75.1% of the sample achieved a percentage of correct responses above 50%. As self-evaluation of the subjects about their knowledge of BP measurement, 40 (74.1%) subjects reported they were good before answering the questionnaire, and only 04 (7.4%) maintained this concept after completion of the test. No subject felt completely satisfied in relation to what it knows about BP, four (7.4%) considered themselves satisfied, 27 (50%) were little satisfied, 14 (26%) dissatisfied and six (11.1 %) completely satisfied. Three subjects (5.5%) did not answer this question. Despite the subjects' inadequate performance on the test, they reported that this issue is important for practical application in the ICU-A. This study reveals gaps in knowledge of nurses about important issues involved in measuring BP with the three methods of measurement (arterial cannulation, oscillometry and sphygmomanometry) and reflects the conscience of these individuals related to this shortfall. Update strategies are suggested to review the technique with the different methods of BP measurement available in the ICU-A investigated, as well as overall aspects of BP in continuing education, because nurses from intensive care must be capable to perform this technique correctly and safely to guide their team, as well as to decide conducts related to BP values in the care of critically ill patients / Mestrado / Enfermagem e Trabalho / Mestre em Enfermagem

Rating is made using free accessible tools, which are offered by selected practice and which offer rating process. Result is established according to consultation with people involved in project and their rating using mentioned tools.

This study investigates Swedish secondary and upper secondary school teachers' knowledge and attitudes with respect to implementing self-assessment activities in teaching of Spanish as a foreign language. The purpose of the study has been to get an overview of the state of the implementation of self-assessment in Swedish schools. In order to achieve this objective, a survey method has been used. The results of this study, like the findings of previous studies, indicate that there is a deficiency in the implementation of self-assessment in teaching. Additionally, the results suggest some possible explanations for this deficiency. However, in order to be able to draw a definite conclusion regarding the matter, more detailed information and further studies would be needed. / Este estudio investiga conocimientos y actitudes de profesores suecos de secundaria y de bachillerato acerca de la implementación de actividades de autoevaluación en la enseñanza de español como lengua extranjera (ELE). El propósito del estudio ha sido obtener una imagen general sobre el estado de la implementación de la autoevaluación en los institutos suecos. Para lograr este objetivo, se ha recurrido a una técnica de encuesta. Los resultados de este estudio, similares a los de los estudios anteriores, indican que existe una deficiencia en la implementación de la autoevaluación en la enseñanza. Los resultados obtenidos sugieren además algunas explicaciones posibles para esta deficiencia. No obstante, para poder sacar alguna conclusión definitiva, habría que profundizar más en el tema y realizar nuevos estudios.

I am the Greatest Driver in the World! : -Does self-awareness of driving ability affect traffic safety behaviour?

This simulator study aims to investigate if there is a relationship between self-awareness of driving ability and traffic safety behaviour. Self-awareness in this study is accurate self-evaluation of one’s abilities. By letting 97 participants (55-75 years old) drive the simulator and answering the Driver Skill Inventory (DSI; Warner et al., 2013) as well as the Multidimensional locus of control (T-loc; Özkan &amp; Lajunen, 2005). A measure of self-awareness was computed using the residuals from regression line. Furthermore, this measure could show if a participant over-estimated or under-estimated their ability. Four self-awareness measures were made. The self-awareness measures were compared to traffic safety behaviour. Three different traffic safety measures were computed using specific events in the simulator scenario. The self-awareness measures were grouped into three groups; under-estimators, good self-awareness and over-estimators. These groups were then compared to each other with respect to traffic safety. A multivariate ANOVA was made to test for differences between the self-awareness groups but no significant main difference was found. The results showed no difference in traffic safety behaviour given the different levels of self-awareness. Furthermore, this could be a result of the old age of the sample group as self-awareness may only be relevant in a learning context. The conclusion of the study is that the analysis shows that there is no difference between over-estimators and under-estimators of driving ability, at least not in experienced older drivers.

This thesis is focused on research of school undiscipline (misbehaviour) by using the anchoring vignettes method. The results obtained of qualitative research (focus groups and in-depth interviews) were used for the compilation of the vignettes and questionnaire. In its theoretical part, the thesis introduces with multi-dimensional concept of school discipline and with works of the other authors. Further the thesis presents an overview of research methods for measuring of school discipline. The practicle parts presents the results of the author's own survey. These results have led for construction of the questionnaire with anchoring vignettes. These ones were compiled by using results of focus groups with 13 - 15 years old pupils and in-depth interviews with their teachers. The final questionnaire will be standardized within author's doctoral studies a thesis. Even so, it appeals that the anchoring vignette method takes its place in educationas scienses. The diploma thesis is of a theoretical-research nature. The theoretical part deals with an innovative school assessment approach which is required by the current conception of the primary education. The attention is paid to the innovative forms of assessment, especially to the self-assessment and the pupil's portfolio. The aims, the research questions and the methods of the exploration are formulated in the research part. There is a presentation of the results of the research, which was focused on the pupil's portfolio as a means of assessment and self-assessment in the conception of several primary education teachers. The options as well as the difficulties of the portfolio assessment are presented. The final product of the thesis is a summary of recommendations for primary education teachers who would like to start to apply the pupil's portfolio in their teaching practice. Self-assessment of managerial competencies of nurse managers in South Africa - identifying the skills gaps

Magister Commercii - MCom / Broad access to healthcare services is a key factor of human development in any country. The current health care situation in South Africa can be diagnosed as critical. The hospitals are understaffed, over-occupied and the diseases like Human Immunodeficiency Virus (HIV) give health care workers additional challenges. The demand for management skills in the health sector including those for nurse managers is high. A recent World Health Organization (WHO) study of nurses working in maternal health services identified good management as more important than salary, unless the remuneration was dramatically higher. In South Africa, little empirical research exists about the management skills of nurse managers, even though proper management of human resources is vital to achieve better outcomes and access to health care around the world. In South Africa, a greater focus on human resource management in health care and more research is needed to develop new policies that will help to address the skills gap of nurse managers. The object of this research project was to identify the gaps between required and existing management skills of senior nurse managers in South Africa in private and public hospitals. Once identified, this skills gap assessment can be used by employers and policy-makers to define the management education that nurse managers require. The research is based on a survey of nurse managers in private and public hospitals using a questionnaire. The survey instrument was based on prior research of hospital managers' competencies in South Africa, and a review of the related theoretical literature. / South Africa

Evaluation of sociocultural competency training in enhancing self-efficacy among immigrant and Canadian-born health sciences trainees

The study was to investigate the effectiveness of Sociocultural Competency Training (SCCT) as an intervention in enhancing self-efficacy among trainees in the health care profession. The purposes of the study were threefold: (a) to evaluate the effectiveness of the training in enhancing the trainees' self-efficacy and behavioural performance; (b) to examine their personal experiences in the learning ofthe sociocultural competencies, and (c) to identify the factors that contribute to effective outcomes. A sample of 84 participants in the Health Sciences program at the Vancouver Community College was recruited. There were 26 local born Canadians and 32 immigrants in the experimental group, whereas 11 local born Canadians and 15 immigrants were in the control group. Experimental group participants took part in an 18- hour training over a 6-week period as part of their regular Human Relations Skills course curriculum. The control group also took the same training course after post-test data collection. This study used both quantitative and qualitative methods. Self-efficacy and behavioural performance were assessed quantitatively by the results from the General Self-Efficacy Scale (GSE), Situational Social Avoidance Scale (SSA), Social Self-Efficacy Scale (SSE), and Interpersonal Skills Checklist (ISC-33). Qualitative data was collected through written feedback from 28 participants and semi-structured interviewing with 24 volunteer interviewees in the experimental group. The results of this study supported the hypotheses that the Sociocultural Competency Training was effective in improving the interpersonal skills and lowering the social avoidance tendency among participants in the experimental group when compared to individuals in the control group. The hypothesis that there would be more significant change in participants' social self-efficacy was also supported. The Sociocultural Competency Training offered effective ways of helping people develop positive self-efficacy and behavioural competencies. Participants reported the training enabled them to have the sociocultural competencies to conduct their professional career in a multicultural community. The thesis deals with the possibilities of the quality information system assessment. The theoretical part of the thesis describes the information systems, explains the term quality in information systems and describes the methods of the IT processes maturity assessment. The implementation of selected methods of the IT process maturity assessment is the content of the practical part of the thesis. A questionnaire survey based on the selected methods was conducted in the SAINT-GOBAIN ADFORS CZ s.r.o. company. The survey was analyzed, the level of the Problem Management process maturity was defined and possible improvements of this process were suggested.
