The neural correlates of perinatal OCD: An exploratory investigation into serotonin risk genes and cortical morphology

Introduction: Obsessive-compulsive disorder (OCD) is a complex disorder that is associated with significantly impaired functioning. The current prevailing model of OCD implicates dysfunction of the serotonergic neurotransmitter system and fronto-striatal neural networks, but challenges in replicating findings within OCD samples are often attributed to clinical heterogeneity. OCD symptoms that develop or worsen within the perinatal period appears to reflect a distinct subtype of the disorder, but the genetic and neurobiological factors that contributes to its presentation in women is poorly understood. In this dissertation, we aimed to review the literature on the genetic architecture of OCD, identify potential gene candidates for perinatal OCD and analyze one serotonin system gene according to OCD and possible subtypes using meta-analytic techniques. Based on these findings, we then tested the association of serotonergic candidate gene polymorphisms with the presence of infant-related obsessive-compulsive symptoms (OCS). Lastly, we investigated the cortical morphological features associated with perinatal OCD and OCS symptom severity in postpartum mothers. Results: From prior reports in the literature and our own meta-analytic investigation, polymorphic variants in genes coding for the serotonergic transporter and serotonin 2A receptor subtype (SLC6A4 and HTR2A, respectively) appear to be candidates for perinatal OCD due to their association in female samples. However, upon investigation in our perinatal sample (n=107), we found no evidence to support the association of the 5-HTTLPR polymorphism of SLC6A4 with perinatal-related OCS, but larger samples are needed to confirm this finding. Due to technical challenges, the HTR2A polymorphism remains to be tested. Our novel whole-brain explorations revealed distinct cortical morphology associated with symptom worsening across the perinatal period, irrespective of diagnosis. Cortical parameters were not able to differentiate mothers with and without OCD; however, OCD mothers displayed positive correlations between cortical surface area and symptom severity in widespread regions, including the frontal, parietal, temporal and occipital cortex. Conclusions: Overall, this body of work aimed to fill the gap in the literature by exploring the possible genetic and cortical correlates of perinatal-related OCS and OCD. While 5-HTTLPR or HTR2A are candidates for perinatal OCD, it is not yet clear whether they increase susceptibility for the development of infant-related OCS in the perinatal period. Distinct cortical alterations in surface area appeared alongside OCS exacerbation in the postpartum period in regions that extend beyond the frontoparietal network. This suggests that additional neural networks may be contributing to symptom severity and that the cortical plasticity that occurs across the perinatal period may predispose women for risk of OCD. Future studies should continue to use a multiple perspective approach, that utilizes genetic and neurobiological techniques, in order to provide greater insight into the etiology of perinatal OCD. / Dissertation / Doctor of Philosophy (PhD) / Women are at greater risk for the development of mental illness in the time surrounding pregnancy and postpartum, known as the perinatal period. In the case of perinatal obsessive-compulsive disorder (OCD), mothers may experience unique worries in regard to their parenting or fears that their baby may be harmed. While these worries are common, they can become disruptive when persistent and impact the mother’s mood and ability to bond with the infant. Our current understanding of OCD includes the influence of genetic factors and brain changes, but little is understood about what factors may increase risk for OCD in the perinatal period. In this thesis, we aimed to review whether certain alterations within DNA segments, known as gene variants, may be linked to the development of OCD in females and if these gene changes, as well as differences in brain structures in postpartum mothers, are associated with OCD symptoms during the perinatal period. The genes we examined are important for regulating a chemical signaling substance in the brain known as serotonin. Based on our results, we did not find a relationship between serotonin gene variants and OCD symptoms in perinatal women. We also found no differences when comparing the cortical brain structures between mothers with OCD and healthy mothers; however, we observed that measures of surface area across several cortical brain regions were related to symptom worsening from pregnancy to postpartum, and also with symptom severity in postpartum mothers with OCD. These results suggest that there are widespread brain changes during the postpartum period that may increase a mother’s risk for developing OCD. Overall, the work in this thesis provides the first glimpse into potential risk factors for perinatal OCD.

An analysis of antidepressant noncompliance in the private health sector of South Africa / Francois Naude Slabbert

The main aim of the thesis was to measure antidepressant (AD) non-compliance, to determine which factors are closely associated with AD non-compliance and the consequences of prolonged AD non-compliance in the private health sector of South Africa. The empirical study followed an observational, prospective, cohort study using longitudinal medicine claims data provided by a nationally representative Pharmaceutical Benefit Management company (PBM) from 1 January 2006 to 31 December 2011. Failure to respond to AD treatment and achieving remission has severe neurobiological and clinical consequences. The clinical consequences include increased social and functional impairment, higher risk for recurrence and relapse of a depressive episode, a weak treatment outcome, significant increase in treatment cost, over-utilization of health care systems, and ultimately an increased suicide risk. However, the neurobiological consequences are much more far reaching. One of the more serious yet under-recognized neurobiological complications of AD non-compliance is the development of antidepressant discontinuation syndrome (ADS), which is the result of non-compliance or the abrupt discontinuation of AD treatment. Altered serotonergic dysfunction appears central to ADS so that how an antidepressant targets serotonin will determine its relative risk for inducing ADS and thereby affect later treatment outcome. Low ADS risk with agomelatine versus other antidepressants can be ascribed to its unique pharmacokinetic characteristics as well as its distinctive actions on serotonin, including melatonergic, monoaminergic and glutamatergic-nitrergic systems. After the first four months only 34% (n=12 397) of patients were compliant. What’s more a statistically significant association was found between active ingredient consumed and compliance (p < 0.0001). Only 26.2% of patients who received amitriptyline-containing products were complaint compared to 38.8% and 38.7% in the cases of venlafaxine and duloxetine, respectively. The current study found that females have a significantly higher prevalence of MDD and HIV/AIDS when compared to males. The co-morbidity between HIV/AIDS and major depressive disorder (MDD) had a significant effect on AD treatment compliance as patients diagnosed with both HIV/AIDS and MDD (74.43. ± 32.03, 95%Cl: 71.51-77.34) displayed a lower compliance vs. MDD patients (80.94% ± 29.44, 95%Cl: 80.56-81.33). Noteworthy, observations were that 75% (p < 0.0217; Cramer’s V = 0.0388) of venlafaxine and 28.6% (p < 0.0197; Cramer’s V = -0.0705) of the paroxetine items were compliant in patients diagnosed with both HIV/AIDS and MDD. The overall compliance (35.19% acceptable compliance; n = 42 869) of patients taking both ADs and GDs was weak. In the group receiving both AD and GDs, an increased AD treatment period was associated with a significant increase (p < 0.0001) in AD compliance (406.60 days; 95%Cl: 403.20 – 409.90 vs. 252.70 days; 95%Cl: 250.20 – 255.20). In this cohort amitriptyline (29.57%), mirtazapine (31.36%) and fluoxetine (32.29%) were associated with the lowest levels of compliance, while duloxetine (40.67%) was found to have the highest compliance. Lastly, ADs with highest non-compliance were associated with an increase use in GDs. Alprazolam (n = 10 201) and zolpidem (n = 9 312) were the most frequently dispensed GDs in combination with AD treatment. In conclusion the current study confirms that AD non-compliance is as big an obstacle in developing countries as it is in developed countries. Antidepressant treatment non-compliance has far reaching consequences especially with the development of ADS which further complicates MDD and might be a precursor for the development of TRD. The temporal dynamics of volitional emotion regulation

Happiness, anger, surprise, irritation… if we note down the emotions that we go through on a given day, the list will most probably be quite long. A surge of studies on the bidirectional interaction between emotion and cognition suggests that we need emotional appraisals in order to lead a successful life and maintain our personal, social and economic integrity (Bechara, 2005; Damasio, 1994; Fox, 2008; Gross &amp;amp; Thompson, 2007; Walter, 2005). And yet, we seldom ‘just’ experience emotions, but often try to influence them to best fit our current goals. Based on the assumption that emotional reactions entail changes on various levels, and that these changes happen in- or outside of our awareness, affective science has adopted emotion regulation as one of its major research topics (Beauregard, Levesque, &amp;amp; Paquette, 2004; Gross, 1999; Ochsner, 2007). In fact, neural (e.g. amygdala activation) and behavioral (e.g. feeling of negativity) correlates of emotional reactions are effectively reduced by top-down processes of explicit and implicit control (Drabant, McRae, Manuck, Hariri, &amp;amp; Gross, 2009; Levesque, et al., 2003; Ochsner, Ray, et al., 2004). Furthermore, evidence from studies investigating voluntary thought control suggests that control strategies may have lasting and paradoxical consequences (Abramowitz, Tolin, &amp;amp; Street, 2001; Wegner, 2009). In a very recent investigation, lasting effects of regulation were also shown after the cognitive control of emotions: the activation timecourse of the amygdala was significantly increased immediately following regulation, and this difference was also related to the activation of the amygdala to the same stimuli a few minutes later (Walter, et al., 2009). Aside from these contextual or qualitative influences, emotional processing also differs between individuals: genetic variation within the serotonergic system for instance is known to affect emotional reactivity both on the behavioral and on the neural level (Hariri, et al., 2005; Hariri, et al., 2002; Lesch, et al., 1996). In the present work, the temporal dynamics of volitional emotion regulation were investigated in three studies. It was hypothesized that both the subjective experience of negativity and the amygdala activation can be attenuated by the detachment from negative emotions, which in turn leads to an immediate neural aftereffect after the offset of regulation. Furthermore, volitional emotion regulation was expected to be capable of reducing or even obliterating genetically mediated amygdala hyperreactivity to negative emotional cues. Similar to previous investigations (Walter, et al., 2009), pictures of aversive or neutral emotional content were presented while participants were instructed to react naturally to half of the pictures, and to regulate their emotional response upon the other half of the stimuli. The first two studies of the present work were designed to further characterize the immediate aftereffect of volitional regulation in the amygdala: Study 1 included behavioral ratings of negativity at picture offset and at fixation offset in order to provide behavioral measures of experiential changes, while in Study 2, participants continued to experience or regulate their emotions during a “maintain” phase after picture offset. The primary goal of Study 3 was to evaluate whether volitional emotion regulation can reduce genetically mediated amygdala hyperreactivity to aversive emotional material in individuals with the short variant of the serotonin transporter genotype (Hariri, et al., 2005; Hariri, et al., 2002), and whether the immediate aftereffect is also influenced by the serotonin transporter genotype. In all three studies, the amygdala was significantly activated by aversive versus neutral stimuli, while cognitive emotion regulation attenuated the activation in the amygdala and increased the activation in a frontal-parietal network of regulatory brain regions. This neural effect was complemented by the behavioral ratings which show that the subjective experience of negativity was also reduced by detachment (Study 1). Also in all three studies, an immediate aftereffect was observed in the amygdala following the end of regulation. Moreover, the preoccupation with the previously seen pictures after the scanning session varied across the experimental conditions (Studies 2 and 3). Volitional regulation proved effective in reducing amygdala activation to negative stimuli even in 5-HTTLPR short allele carriers that show an increased reactivity to this type of cue. At the same time, functional coupling of the ventrolateral and medial orbital prefrontal cortex, the subgenual and the rostral anterior cingulate with the amygdala was higher in the s-group. However, in Study 3 the immediate aftereffect was found only in l/l-homozygote individuals following the regulation of fear. Taken together, the results of the three studies clearly show that volitional regulation is effective in reducing behavioral and neural correlates of the experience of negative emotions (Levesque, et al., 2003; Ochsner, Bunge, Gross, &amp;amp; Gabrieli, 2002; Ochsner, Ray, et al., 2004), even in the case of a genetically mediated hyperreactivity to such materials. Thus, it seems reasonable to assume that conscious will can effectively counteract genetic determinants of emotional behavior. Moreover, the present results suggest that the temporal dynamics of volitional emotion regulation are characterized by a paradoxical rebound in amygdala activation after regulation, and that the immediate aftereffect is a marker of the efficiency of the initial and the sustained effects of emotion regulation (Walter, et al., 2009). In summary, the successful replication of the immediate aftereffect of emotion regulation in all three studies of this dissertation opens up exciting new research perspectives: a comparison of the short- and long-term effects of different regulatory strategies, and the investigation of these effects also in positive emotions would complement the present results, since the neural mechanisms involved in these processes show some characteristic differences (Ochsner, 2007; Staudinger, Erk, Abler, &amp;amp; Walter, 2009). A comprehensive characterization of this neural marker and its implications for emotional experience might also be useful with respect to clinical applications. The detailed examination of the various time scales of emotional regulation might for instance inform the diagnostic and therapeutic interventions in affective disorders that are associated with emotional dysfunctions (Brewin, Andrews, &amp;amp; Rose, 2000; Johnstone, van Reekum, Urry, Kalin, &amp;amp; Davidson, 2007). Ultimately, we might thus come to understand the neural underpinnings of what the feelings we have today have to do with the feelings we had yesterday – and with the feelings with might have tomorrow.

Genetic Variation and Shared Biological Susceptibility Underlying Comorbidity in Neuropsychiatry

Genetic factors underlying alcoholism, substance abuse, antisocial and violent behaviour, psychosis, schizophrenia and psychopathy are emerging to implicate dopaminergic and cannabinoid, but also monoaminergic and glutamatergic systems through the maze of promoter genes and polymorphisms. Candidate gene association studies suggest the involvement of a range of genes in different disorders of CNS structure and function. Indices of comorbidity both complicate the array of gene-involvement and provide a substrate of hazardous interactivity. The putative role of the serotonin transporter gene in affective-dissociative spectrum disorders presents both plausible genetic variation and complication of comorbidity. The position of genetic variation is further complicated through ethnic, contextual and social factors that provide geometric progressions in the comordity already underlying diagnostic obstacles. The concept of shared biological susceptibilty to two or more disorder conditions of comorbidity seems a recurring observation, e.g., bipolar disorder with alcoholism or schizophrenia with alcohol/substance abuse or diabetes with schizopsychotic disorder. Several lines of evidence seem to suggest that the factors influencing variation in one set of symptoms and those affecting one or more disorders are observed to a marked extent which ought to facilitate the search for susceptibility genes in comorbid brain disorders. Identification of regional genetic factors is awaited for a more compelling outline that ought eventually to lead to greater efficacy of symptom-disorder arrangements and an augmentation of current pharmacological treatment therapies.

THE ASSOCIATION OF THE 5-HTTLPR POLYMORPHISM WITH PERINATAL ONSET OBSESSIVE-COMPULSIVE DISORDER AND DISTINCT BRAIN ACTIVATION PATTERNS: A GENETIC NEUROIMAGING STUDY / PERINATAL OBSESSIVE-COMPULSIVE DISORDER

Obsessive-compulsive disorder (OCD) is heterogeneous. Clinical presentation of OCD differs by sex and age-of-onset and evidence supports classification based on these subtypes. The prevalence of OCD in the general population is 2%. However, it has been established that women tend to experience onset and exacerbation of OCD during reproductive milestones. In particular, the prevalence of postpartum OCD is between 4 to 9%. This study seeks to examine the effects of past childhood maltreatment and S/Lg-allele status of the 5-HTTLPR polymorphism on perinatal obsessive-compulsive symptoms and aberrant resting state functional connectivity in the postpartum period. Forty women participated in the first visit and sixteen women have been followed up with in the postpartum period. 5-HTTLPR genotype was determined from whole blood samples via polymerase chain reaction and a restriction fragment length digest. We used the Yale-Brown Obsessive-Compulsive Scale and Perinatal Obsessive-Compulsive scale to measure symptom severity. Resting state functional connectivity was determined from functional magnetic resonance imaging data. Obsessive-compulsive symptoms during late pregnancy are significantly predicted by 5-HTTLPR genotype, past history of total childhood maltreatment or childhood emotional neglect and trait anxiety symptoms. Whereas obsessive-compulsive symptoms during the postpartum period are predicted by poor sleep quality and childhood emotional maltreatment or 5-HTTLPR genotype, childhood emotional maltreatment and trait anxiety symptoms. Seed to region-of-interest analysis was employed to evaluate resting state functional connectivity differences between OCD patients and healthy controls in the postpartum period. Compared to healthy controls, OCD patients show greater connectivity between the caudate nucleus with the orbitofrontal cortex, the pars triangularis and the cingulate area. The insular cortex shows decreased connectivity between the right and left, the dorsal anterior cingulate area and the pars opercularis. The amygdala has increased connectivity with the cingulate area, the calcarine fissure, the supramarginal gyrus and decreased connectivity with the gyrus rectus. The above clinical and neuroimaging findings are in line with past work. However, this is the first study to show both 5-HTTLPR genotype and history of childhood maltreatment predict obsessive-compulsive symptoms in a perinatal population. Further, the resting state data replicates findings in the OCD literature but the study is the first to show this in postpartum women. This study serves as a platform for future work to further investigate both gene-environment interactions and distinct neuroimaging correlates in perinatal OCD. / Thesis / Master of Science (MSc)

Stereotypical behaviour in the deer mouse (Peromyscus Maniculatus bairdii) : a pharmacological investigation of the frontal–cortico–striatal serotonergic system / Wolmarans D.

Obsessive–compulsive disorder (OCD) is a psychiatric condition that is characterized by two main symptom cohorts, namely recurrent inappropriate thoughts (obsessions) and seemingly purposeless repetitive motor actions (compulsions). In 70% of cases, the condition only re–sponds to chronic, but not sub–chronic, high dose treatment with the selective serotonin reup–take inhibitors (SSRIs), such as fluoxetine and escitalopram. This indicates a role for hyposero–tonergic functioning in the primary brain areas involved in OCD, namely the components of the cortico–striatal–thalamic–cortical (CSTC) circuit which include the prefrontal cortex, the basal ganglia, and the thalamus. A number of studies have demonstrated a lower serotonin trans–porter (SERT) availability in OCD patients compared with healthy controls, supporting the hy–pothesis of a hyposerotonergic state in OCD. The current study focuses on the validation of the deer mouse (Peromyscus maniculatus bairdii) model of OCD and builds on previous work done in our laboratory. Deer mice that are bred and housed in confinement naturally develop two main forms of stereotypical behaviour, namely vertical jumping and pattern running. Furthermore, these behaviours can be catego–rized into various levels of severity, namely high (HSB), low (LSB) and non–stereotypic (NSB) cohorts. The seemingly purposeless and repetitive nature of these behaviours mimics the com–pulsions that characterize human OCD and constitutes the basis for the face validity of the model. However, although these two forms of stereotypy seem equally repetitive and persis–tent, stereotypical pattern runners do not complete the required number of cage revolutions per 30 minutes compared to the amount of jumps executed by stereotypical vertical jumpers. As only one set of criteria for the appraisal of the different topographies of deer mouse stereotypy has been applied in previous studies, the matter of whether pattern runners do in fact generate stereotypical behaviour of the same persistent and severe nature as opposed to the behaviour expressed by vertical jumpers, is problematic. Therefore, the first objective of the current study was to develop a new classification system for the appraisal of the different forms of behavioural topographies of deer mice and subse–quently to evaluate whether pattern runners can indeed be categorized into non–, low– and high stereotypical cohorts. After an eight–week behavioural assessment period, deer mice express–ing the two different behavioural topographies could be classified into non–, low– and high stereotypical cohorts (NSB, LSB, and HSB respectively), applying different criteria for each be–havioural topography. Based on the weekly mean stereotypy count generated during three 30–minute intervals of highest stereotypical behaviour over the course of a 12–hour assessment period, HSB pattern runners were found to execute on average 296 cage revolutions per 30 minutes, while HSB vertical jumpers executed an average of 3063 jumps per 30 minutes. This discrepancy between the generated numbers of the different topographies of stereotypy indi–cates that one classification system for the appraisal of both behavioural topographies is indeed inappropriate, and hence requires re–evaluation and validation. As patients with OCD present with a lower central SERT availability compared to healthy controls, the second objective of the study was to determine whether a decrease in SERT den–sity could be demonstrated in HSB animals compared to the NSB and LSB controls. After eight weeks of behavioural assessment, animals were sacrificed and frontal–cortical and striatal SERT binding was performed. HSB deer mice presented with significantly lower striatal, but not fron–tal–cortical SERT availability compared to the [NSB/LSB] control animals (p = 0.0009). Although previous studies undertaken in our laboratory demonstrated that deer mouse stereotypy is attenuated after chronic (21–day) fluoxetine administration, OCD only responds to chronic, but not sub–chronic treatment with the SSRIs. The lack of response of deer mouse stereotypy to sub–chronic treatment has not been established and therefore the third study ob–jective was to assess the behavioural effects of sub–chronic (7–day) and chronic (28–day) SSRI treatment on expression of deer mouse stereotypy. Chronic, but not sub–chronic treatment with oral escitalopram (50 mg/kg/day) significantly increased the number of intervals over a 12–hour assessment period during which no stereotypical behaviour were expressed by HSB deer mice (p = 0.0241) and decreased the number of intervals during which high–stereotypical be–haviour were executed (p = 0.0054). Neither chronic, nor sub–chronic treatment significantly affected the behaviour of animals in the [NSB/LSB] cohort. The fact that the model demon–strates a lack of response to sub–chronic treatment with high dose SSRIs, positively contributes to the predictive validity of the deer mouse model of OCD. The results from the current study therefore strengthens the construct and predictive valid–ity of the deer mouse model of OCD and confirm the model’s status as a prominent animal model of OCD. Not only is hyposerotonergic functioning in the CSTC circuitry implicated in the behaviour of HSB animals, but the model also demonstrates selective response to chronic SSRI–treatment - two core characteristics of human OCD. / Thesis (M.Sc. Stereotypical behaviour in the deer mouse (Peromyscus Maniculatus bairdii) : a pharmacological investigation of the frontal–cortico–striatal serotonergic system / Wolmarans D.

Obsessive–compulsive disorder (OCD) is a psychiatric condition that is characterized by two main symptom cohorts, namely recurrent inappropriate thoughts (obsessions) and seemingly purposeless repetitive motor actions (compulsions). In 70% of cases, the condition only re–sponds to chronic, but not sub–chronic, high dose treatment with the selective serotonin reup–take inhibitors (SSRIs), such as fluoxetine and escitalopram. This indicates a role for hyposero–tonergic functioning in the primary brain areas involved in OCD, namely the components of the cortico–striatal–thalamic–cortical (CSTC) circuit which include the prefrontal cortex, the basal ganglia, and the thalamus. A number of studies have demonstrated a lower serotonin trans–porter (SERT) availability in OCD patients compared with healthy controls, supporting the hy–pothesis of a hyposerotonergic state in OCD. The current study focuses on the validation of the deer mouse (Peromyscus maniculatus bairdii) model of OCD and builds on previous work done in our laboratory. Deer mice that are bred and housed in confinement naturally develop two main forms of stereotypical behaviour, namely vertical jumping and pattern running. Furthermore, these behaviours can be catego–rized into various levels of severity, namely high (HSB), low (LSB) and non–stereotypic (NSB) cohorts. The seemingly purposeless and repetitive nature of these behaviours mimics the com–pulsions that characterize human OCD and constitutes the basis for the face validity of the model. However, although these two forms of stereotypy seem equally repetitive and persis–tent, stereotypical pattern runners do not complete the required number of cage revolutions per 30 minutes compared to the amount of jumps executed by stereotypical vertical jumpers. As only one set of criteria for the appraisal of the different topographies of deer mouse stereotypy has been applied in previous studies, the matter of whether pattern runners do in fact generate stereotypical behaviour of the same persistent and severe nature as opposed to the behaviour expressed by vertical jumpers, is problematic. Therefore, the first objective of the current study was to develop a new classification system for the appraisal of the different forms of behavioural topographies of deer mice and subse–quently to evaluate whether pattern runners can indeed be categorized into non–, low– and high stereotypical cohorts. After an eight–week behavioural assessment period, deer mice express–ing the two different behavioural topographies could be classified into non–, low– and high stereotypical cohorts (NSB, LSB, and HSB respectively), applying different criteria for each be–havioural topography. Based on the weekly mean stereotypy count generated during three 30–minute intervals of highest stereotypical behaviour over the course of a 12–hour assessment period, HSB pattern runners were found to execute on average 296 cage revolutions per 30 minutes, while HSB vertical jumpers executed an average of 3063 jumps per 30 minutes. This discrepancy between the generated numbers of the different topographies of stereotypy indi–cates that one classification system for the appraisal of both behavioural topographies is indeed inappropriate, and hence requires re–evaluation and validation. As patients with OCD present with a lower central SERT availability compared to healthy controls, the second objective of the study was to determine whether a decrease in SERT den–sity could be demonstrated in HSB animals compared to the NSB and LSB controls. Kombinierte Analyse funktioneller PET/MRT Veränderungen des zentralnervösen Noradrenalin-/Serotonin-Netzwerkes und deren Einfluss auf das emotionale Wohlbefinden bei Adipositas

Die grundlegenden neurobiologischen Mechanismen für das Zusammenwirken eines pathologisch erhöhten Körpergewichts und der gewichts-assoziierten emotionalen Belastung sind bisher noch wenig erforscht. Die vorliegende Arbeit untersucht gezielt Abweichungen der regionalen Transporter-Verfügbarkeiten mittels Positronen-Emissions-Tomographie (PET) mit hochselektiven Marker für den Noradrenalin- (NET) sowie den Serotonin-(5-Hydroxytryptamin-)transporter (5-HTT) und funktioneller Magnetresonanztomographie (fMRT) sowie damit verbundene Alterationen der neuronalen Ruhe-(resting-state-)Aktivität konnektierter Hirnregionen. Die Ergebnisse der kombinierten PET/fMRT Analyse wurden mit zwei neuropsychologischen Scores zur Erfassung allgemeiner und gewichtsabhängiger emotionaler Veränderungen korreliert. Insgesamt 48 Teilnehmer (Body-Mass-Index [BMI]: 19 - 50 kg/m2) erhielten eine fMRT und eine PET mittels NET-selektivem [11C]MRB (n = 20) beziehungsweise 5-HTT-selektivem [11C]DASB (n = 28). Die PET ergab im Hypothalamus eine tendentielle, BMI-abhängig verminderte Verfügbarkeit des NET, nicht jedoch des 5-HTT. Zusätzlich fand sich bei steigendem BMI innerhalb beider Neurotransmitternetzwerke in Abhängigkeit zur jeweiligen Transporter-Verfügbarkeit eine teils geschlechtsspezifisch verstärkte funktionelle Konnektivität zwischen dem Hypothalamus und Hirnregionen der Verarbeitung und Bewertung von Nahrungsreizen. Korrelationen der lokalen resting-state Aktivitäten mit den neuropsychologischen Scores lassen vermuten, dass diese Regionen zudem auch mit langfristigen, negativen Veränderungen des gewichtsabhängigen emotionalen Wohlbefindens assoziiert sind. Insgesamt spielen diese beiden zentralen Neurotransmitter-Systeme eine wichtige Rolle in der Modulation von Netzwerken zur Regulation des gewichtsabhängigen emotionalen Wohlbefindens und könnten somit wichtige Anhaltspunkte für neue pharmakologische Ansätze bereitstellen.
