Struktura pracovních a manažerských činností zdravotních sester v řídících funkcích

Thesis deals with structure of working and managerial activities of department and head nurses in Czech hospitals. As the managers in the first line and middle level they have a huge influence on the total level and quality of nursing health care. Therefore their level of managerial activities is very important. The aim of the thesis is to create and apply a methodology of monitoring of the work structure independent on the founder, hospital size and department/clinic orientation. For these reasons, the author cooperated with the Faculty hospital of St. Anna in Brno, Faculty hospital Brno, Army hospital Brno, Faculty hospital Ostrava, Hospital Trebic and Hospital Znojmo. There is used auto screening method, in the thesis. Measurements were proceeded for all department nurses and head nurses in Hospital Znojmo and Army hospital Brno. In order to achieve the aim of the thesis there were used the above-mentioned auto screening method and analysis of internal hospital documents, questionnaire research and interviews. The questionnaires were filled-in before beginning the auto screening and they investigated own estimation of working time consumptions of department and head nurses. Results were compared with statistical results of auto screening. The statistical results, proposals and recommendations were given to the deputy-managers for health care. In praxis we can often hear the statement, that nurses in the Czech Republic are overloaded with administrative tasks. The analysis of structure of working and managerial activities should either confirm or falsify this statement. The hypotheses were postulated to confirm or falsify correlation of variable load of administrative/managerial activities with the variables number of subordinates, total length of health care praxis or praxis in a managerial function.

Možnosti využití kompetencí sestry pro komunitní péči v oblasti péče o zdraví v ČR / Possible utilize of nurse's competenicies in community health care in Czech Republic

The philosophy of community care is deeply rooted in an opinion that states that one of the main missions in a cultivated society is helping the people who, due to their disease, are not able to to meet their own demands and their needs vitally necessary for life. Community care means providing an intervention and support at the right level and in the right form for clients, so that they can reach the highest possible level of self-sufficiency and control over their own lives. Recently, there have been many changes in the health care system. The goal of community care is to provide thourough care in clients' home environments and thus enable them to stay with their community as long as possible. The major task is providing phychical, psychological, paliatic and spiritual services. Families are the vital part of the care and a fundamental root of the whole team cooperation within the system of community care. The community teams consist of health care and social workers, community volunteers, the sick or the disabled. Nurses are important members of multidisciplinary teams. They take an active part in community assessment and they also help ensure a complex care for citizens. The research survey combined a quantitave and a qualitative survey. Firstly, a set of questionnaires for nurses and non-professional public were worked out. Next, an interview was prepared for nurses. The research sample group was made of 153 non-professional respondents and 121 nurses working in different medical facilities all over the Czech Republic. Nine nurses were interviewed. In my thesis two goals were set up. The first goal was to find out where in the community care nurses could implement their competences within the legislative frames. The second goal was to find out what opinion on community nurses non-professional public have. I worke out four hypotheses on the grounds of my goals. The first hypothesis is worded as: The non-professional public is interested in having nurses working for the community care. The second hypothesis is worded as: Nurses between 36-45 of age are willing to work as community care nurses. The third hypothesis is worded as: Nurses' interest in working for the communty care depends on their finished education. The fourth hypothesis is worded as: Nurses working 21-30 years in the health care system are interested in being active as community nurses in home care agencies. On the grounds of the qualitative research survey and five research questions it is possible to set up two hypotheses. The first hypothesis is worded as: Nurses do not know who a community nurse is. The second hypothesis is worded as: Nurses are not interested in working as community nurses. Community care has had a long history. The foundations of community care have been laid in Great Britain. In the Czech Republic community care has not been a novelty. However, we can claim that so far, no community care system has been created.

Postoj ošetřovatelského managementu k výzkumu v ošetřovatelství / Attitude of Nursing Management to Nursing Research

A nursing research is an integral part of the nursing. An implementation of research activities in this field makes a basis for an improving of the nursing care and a development of the discipline itself. For nurses working in a clinical department who should carry out research investigations, there is important a knowledge how to conduct investigations and a support from the hospital management. Without this support they could not put the research results into practice which is currently based more on standard procedures. A theoretical part of the thesis deals with the nursing research, research concepts and its historical development. Then it also discusses ethical principles of the nursing research, nurses´ fundamental roles, evidence-based activities, opportunities of the research funding in the Czech Republic and an evaluation of a scientific work. An aim of the thesis was to establish the awareness, participation and interest of the primary and secondary level management in the nursing research. We have identified four hypotheses which we verified by a quantitative survey. H1: More general nurses with a higher education participate in the nursing research than the nurses who are graduates of the secondary medical school. H 2: General nurses-managers who completed a university education are better prepared for an implementation of the nursing research than nurses-managers with the secondary education. H3: The primary and secondary level nursing management of Hospital in České Budějovice participate in the nursing research more than the management from other hospitals in the South Bohemian Region. H 4: The nursing management which is involved in the nursing research apply the research results into practice. The quantitative survey was made by a questionnaire technique. The research sample consisted of nurses working on the positions of the secondary and primary level managers in the inpatients departments of hospitals in the South Bohemian Region. The investigation was conducted in eight hospitals in the South Bohemian Region. It is possible to say, that the awareness of the nursing research is on a good level, nearly half of the nurses monitor and apply the information and the research results in their clinical practice as well as most nurses are ready to implement either a simple research survey or to participate in the research under a guidance or to prepare and implement an independent research project. We believe that the nurses should be interested in the nursing research, but not as a part of their normal duties which they have in their workplace. For a development in the nursing field it is necessary to increase a further awareness of the nursing research and to encourage the nurses so that they should be interested to conduct this research. Results of the survey will be sent to the primary nurses/directors of the nursing in all of the addressed medical facilities. They may also serve as a basis for further research investigations. The lecture on the topic: ?The importance of the nursing research in the nurse´s work? will serve to raise the awareness of the nursing research.

Podíl staničních a vrchních sester na kvalitě ošetřovatelské péče / Contribution of sisters and ward sisters to the nursing care quality

This degree work concerns the role of sisters and ward sisters and their share in the nursing care quality. The theoretical part of the work is divided into three sections. Section one presents sisters and ward sisters as active members of the nursing management. Section two deals with the managements functions of the sister staff and their impact on the nursing care quality. Section three reviews the basic issues of the nursing care quality. The purpose of section one was to review the scope of activities of sisters and ward sisters with respect to the nursing care quality. The purpose of section two was to identify the key difficulties preventing sisters and wards sisters from increasing the nursing care quality. The purpose of section three was to analyse the working relationships between sisters and ward sisters with respect to the nursing care quality. While mapping out the situation in the selected hospital (the Faculty Hospital in Plzeň), we asked the following six research questions: 1. What are the areas where the hospital sisters may affect the nursing care quality? 2. What obstacles the sisters can see preventing them from taking part in continuous improvements in the nursing care quality? 3. How do the hospital sisters co-operate with the respective ward sisters concerning the nursing care quality? 4. What are the areas where the ward sisters may affect the nursing care quality? 5. Are there any obstacles preventing the ward sisters from taking part in continuous improvements in the nursing care quality? 5. How do the ward sisters co-operate with the respective sisters concerning the nursing care quality? The questions were asked during informal discussions with ten sisters and ten wards sisters at the Faculty Hospital in Plzeň. The information and data collected were processed using the framework analysis method whereby the answers to the research questions were derived. The research identified the following main problems preventing the hospital sisters and ward sisters from increasing the nursing care quality: inefficient time management on the side of ward sisters, insufficient competencies of ward sisters regarding staff remuneration, insufficient application of the existing nursing-care quality improvement tools by both sisters and ward sisters, limited engagement of the hospital sisters and ward sisters in creation of internal standard procedures, and reluctant attitudes of the hospital staff to changes. Based on the shortcomings identified through our research we have worked out proposals on practical improvements.

Vypracování standardu výživy dětí do tří let / The Development of Standaros of Proper Nutrition for Children to The Age of Three

The target of the thesis was to find out the way of alimentation of the child up to the age of three years in the family, if the nurses inform the mothers of the children up to the age of three years about the principles of the correct alimentation of their child and to create the education and information in the field of principles of correct nutrition of the child up to the age of three years. These targets were achieved. In the quantitative research examination, the research complex consisted of a general and children nurse, and mothers of children up to the age of three years from all 123 consulting rooms of physicians for the children and young people (pediatrists) in the South Bohemian Region. 123 questionnaires for nurses and 1230 questionnaires for mothers of children up to three years of age were distributed. 85 questionnaires for nurses and 360 questionnaires for mothers were used for the research. It follows from the results of the research examination that mothers are well informed about the correct way of alimentation of their child in the consulting rooms of practical physicians for children and young people and they observe the recommended principles of the correct nutrition of their child. It follows from the research examination of nurses that there are not unified opinions in introducing some kinds of food into the nutrition of sucklings and toddlers, first of all concerning the pasteurised cow milk, fat and cholesterol. The answers of nurses and mothers in the research examination were the base for elaboration of the educational manual for the parents ? especially for mothers and a standard for the nutrition of children up to there years.

Adaptační proces anesteziologických sester z pohledu školitele / The trasition period of anesthesiologist nurses from the point of view of mentor

Introduction: The transitional process is completed by all new employees with a view to the fastest and the most effective involvement in a complete work process of an organization. Also new employees, who start working in the anesthesiology department, go through specific adaptation. The research of this diploma thesis is focused on the anesthesiology department and brings a view of mentors - anesthesiologist nurses on the adaptation transition of new employees. Methodology: This is a qualitative study, data collection was conducted by semi-structured interviews with anesthesiologist nurses and mentors from various fields of anesthesia. Data analysis was performed in open coding steps followed by categorization. A total of 9 anesthesiologist nurses had participated in the research survey. Results: Findings of this research shows important aspects of successful adaptation transition of anesthesiologist nurses. These important aspects are mentor's experiences from both of their transitional process and his previous workplace, also mentoring that is represented by an experienced trainer, as well as cooperative and helpful work environment and collective, and at last, it is a quality feedback for mentor as well as new employer. Problematika adiktologického klienta ve věznicích České republiky z pohledu sestry / Issue of addictologic patient in the Czech prisons the nurse point of view

Issue of addictologic patient in the Czech prisons - the nurse point of view Abstract The aim of the work: The aim is to define the needs of addictologic patient when entering prisons in the Czech Republic and to map how care about this patient is provided. Another aims defined the aspects which influence work of nurses and found out the nurses´ possibilities in care of those particular patients in prisons in the Czech Republic. Methodology: The qualitative survey was used to elaborate the thesis. The method of questionning was realized by semi-structured interviews. The research file was consisted of 15 nurses of the Prison Service and 15 clients entering the execution of sentence or custody and who were observed and checked because of their addiction to narcotics. The results were processed into 16 cathegories with the method of open coding and pencil and paper method. Results: The research investigation showed that the nurses are aware of the clients´ worries and needs when entering the prison. They try to saturate their somatic and mental needs. Care of those patients is provided according to the regulations which lead up to the timely revelation of possible health complications and starting the needed intervention. Work of nurses who care of addictologic patients is influenced by non-cooperation of clients, their bad adaptation to the new surroundings, but also higher rate of administration and insufficient number of medical staff. Benefits of the work: The benefit was to reveal the nurses´ possibilities in care of addictologic patients in prisons. Nurses should focus on appropriate communication, psychological support and motivate the client to cooperate. Knowledge gained from the research could be used to improve the care of addictologic patients in prisons in the Czech Republic.

Současný stav oboru Dětská sestra a jeho dopady do praxe / The current status of pediatrics nurse field and its impacts on practice

This thesis: "The current status of pediatrics nurse field and its impacts on practice" deals with actual problem and it means education of pediatrics nurses. There were set two targets: 1. Changes in pediatrics nurses field in time. 2. Detections of problems in education of pediatrics nurses that are caused by new changes in the study program. These targets were specified by four research points and three hypothesis yet. Implementation of research was carried by a combination of qualitative and quantitative research methods. Five head nurses participated to the research and then one hundred nurses from children's unit in hospital as well. Our research was divided in three parts. In the first there were made interviews with head nurses. We used semi - structured form. In second phase we spread our written survey in pediatrics nurses who are working in children's unit. It was created an online survey mainly for students of medical disciplines for comparison. In last phase there were analyzed documents of work load of nurses from children's unit. Results show that in practice there we are able to see many problems. Pediatrics nurses who completed their education in high school grow old. New ones are missed and it means that like pediatrics nurses work general nurses as well. In our survey we found out that nurses with different length of practice are the same opinion on education. Then that pediatrics and general nurses, who are working in children's unit perceive the problems equally. But nurses with different length of practice are different opinion on actual long of study. Final results of this thesis will be published in professional journal.

Vnímání bezpečí v ošetřovatelské praxi / The Perception of Safety in Nursing Practice

The theme of this thesis is "The Perception of Safety in Nursing Practice". While most of literaure of focused on the psyhisal aspects of safety, this thesis is focused on the psychological ones. This thesis has two goals altogether. Its firt goal is to find out the possibilities of securing patient´s and nurse´s feeling of safety from nurse´s outlook. The second goal is to find out nurses´s perception of their own feeling of safety while at work. The research questions are 1) How do patients perceive their safety during nursing care? 2) What does patient´s safety mean to nurses? 3) How does the personnel perceive their own safety during nursing care? The theoretical part describes safety, needs, communicaton, trust and the possibilities of securing psyhical and psychological safety of patients and nurses alike. The practical part consists of nurses and patient interviews about safety and factors, that may influence it. The data were collected by semi-structured interviews with nurses and patients located in hospitals of south and west Bohemia. The research involved 20 respondents, 10 nurses and 10 patients, of deliberate choosing and by the form of snowball sampling. The data were collected since May through to July 2019 and the collection was concluded at the moment of theoretical saturation. The interviews covered themes like safety, communication, trust, quality of care, EBN, teamwork and what do nurses find lacking while ensuring safety of their patients and their own. The results show that each person perceives their safety differently and it is necessary to fit the care to patient´s specific needs. The respondents agree on the importance of proper communication, of trust between patient and nurse and between personnel alike, and of informedness. Most of the nurses also responded that they miss more time to spend with their patients to build mutual trust and to develop their feeling of safety. Usually the blame lies on their ward being understaffed. This thesis mapped out the perception of safety of nurses and patients including some of the shortcomings involved in the current nursing practice. It can serve as a reference material for studying.

Nejčastější konflikty v nemocnici z pohledu sestry ve spolupráci s lékařem / The most usual conflicts in hospital from the point of nurses view during cooperation with the physician

To keep good and non-conflict relationships between doctors and nurses is not an easy task their stressful working environment. Despite the fact that both doctors and nurses have a common goal, and that is to provide the best care of their patients, there are situations leading to conflicts between doctors and nurses. Good relationships between nurses and doctors are a guarantee of a harmonized cooperation in the entire medical team. That is why it is important to avoid conflicts and, if any conflict has already occurred, it is important to use it for the benefit of the whole team. The goal of our work was to define the most frequented conflicts in hospitals from a point of nurse's view in a cooperation with doctors, and what impact these conflicts have on nurses' work. To process the data in the first part of my thesis, the quality method for a research survey was used. The data collection was implemented by interviews. The research sample group was composed of eight nurses working in bed departments of different hospitals. On the grounds of the quality survey I set four hypotheses which I tried to verify in the quantity survey. To collect the data in the quantity part a questionnaire survey was used and the research sample group was also composed of nurses working in bed departments. The data were being collected in the period of March through May. To assess the results 175 questionnaires were used. The hypothesis 1 - The most frequented conflicts between nurses and doctors root in a wrong communication, has been proven on the grounds of our findings. The hypothesis 2 - Conflicts between nurses and doctors result from increasing competences for nurses has been proven, too, as well as the hypothesis 3 - Conflicts between nurses and doctors result from misunderstanding nurses' and doctors' roles. The hypothesis 4 - Conflicts between nurses and doctors have a negative impact on nurses' work was proven, too. The results have also revealed that the second most frequented reasons for conflict situations represent personal qualities of doctors, which was not included in the hypotheses. In the summary of the thesis I incorporated some recommendations how to handle conflict situations effectively. This work should be a foundation for a potential follow- up research not only in this field, but it can also serve management teams in medical institutions. They should take actions to improve the situation to some extent.
