Cardiovascular diseases are the leading cause of death in the Czech Republic. An increasing number of its inhabitants become cardiac patients who require a nursing intervention. Nursing in cardiology has an irreplaceable position in the cardiovascular care provision. The health care costs are rising, nursing has been undergoing many changes. Training of cardiology nurses gets changed, as well as the image and role of cardiology nurses in society. Cardiology nurses are much more independent, educated and organized. In cardiovascular facilities nursing care is being standardized and the quality of care is improving. In this thesis, focus is given to nursing in cardiology. In the introduction the issue of cardiovascular diseases is outlined, the major part is devoted to the concept of Czech nursing in cardiology. Individual stages of the system of care for cardiac patients in the Czech Republic and the status of cardiology nurses involved in the system are surveyed. Attention is also paid to the structure of cardiovascular facilities and expert companies in our country and abroad. The thesis has the title: Survey of continuity of care for cardiac patients in the Czech Republic with focus on nursing care. In total five goals, seven research questions and four hypotheses were stated. We surveyed the cardiology nurses´ attitude to lifelong learning, if they are building their own specific image, and if they ensure the continuity of care. Our objective was also to identify the role of nurses in education of cardiac patients and the level of cardiac patients´ knowledge. The research section is elaborated on the basis of a qualitative and quantitative investigation. The qualitative part was carried out by semi-structured interviews with cardiology nurses working in comprehensive cardiovascular centers. The quantitative part of the survey was conducted with patients of comprehensive cardiovascular centers. Seventy domestic and foreign sources of literature devoted not only to the issueof cardiovascular diseases were quoted. The research results are supposed to serve as a basis for coordinated efforts of all participating nurses and institutions to build a well-functioning and technically and economically efficient system of nursing care for cardiac patients in CR.

The role of the nurse is to aid in the improvement of the quality of life for children who have been through meningitis. In recent years, the issue of quality of life has become of interest in many disciplines and the general public. In health care, the quality of life is known as HRQoL - Health Related Quality of Life - loosely translated as ?the quality of life related to health?. The term of ?diagnostic quality of life in children and adolescents? is primarily used for the area of professional health care and it is an important indicator of disease severity and the outcomes of provided care. The objective of this thesis was to assess the quality of life of children who have been through meningitis, to identify the main areas in which the quality of life of these children is reduced, and to suggest nursing interventions aimed at improvements. The data collection took place in the period November 2009 - June 2010 at the Clinic for Childhood Infectious Diseases, University Hospital Brno. A qualitative research used the technique of studying written documents and, subsequently, non-standardized in-depth interviews with respondents were carried out. Prior the beginning of the qualitative survey, five research questions were established. Question 1: What kind of care and what interventions do children expect from nurses? Question 2: What kind of care and what interventions do parents expect from nurses? Question 3: How to evaluate subjectively the quality of children?s? life after meningitis as compared to the state prior the disease? Question 4: How do parents see the quality of life of their child after meningitis as compared to the state prior the disease? Question 5: In which areas is the quality of children?s life reduced most? Question 6: What nursing interventions can help nurses to improve the children?s lives? All of the questions were answered using a quantitative research method involving a standardized questionnaire named ?HEALTH AND SATISFACTION QUESTIONNAIRE?. This questionnaire is used in the project ?QOLOP? ? Quality of Life Longitudinal Study of Paediatric Oncology Patients. Hypothesis H 1 was defined prior the start of the quantitative research: The quality of life of children who have been through meningitis is lower than in healthy children. This hypothesis was not confirmed. It is clear, from what has been mentioned above, that the objective of this thesis has been met. This project will be useful in practice. The generated ?Algorithm of Initial Care for a Child with Meningitis? and ?Subsequent Care of a Child with Meningitis? should help nurses orientate and identify the basic needs of children with this disease. Moreover, this thesis can be used as a theoretical basis for the education of nursing students.

The dissertation objective on the theme {\clqq}Interpersonal relationship development between children's nurse and mother in the neonatology department by means of the H. E. Peplau model`` is to chart individual phases utilization in the process, professional relationship development between children´s nurse and mother according to the H. E. Peplau model. Further through created documentation to find out differences at the approaches during professional relationship connecting between children´s nurse and mother in the neonatology department in the Hospital JSC in České Budějovice. To meet these objectives the dissertation is divided into theoretical part which focuses on differences at the care of developed and underdeveloped infant, nursing model H. Peplau including its application into practice. The second, practical part is divided into quantitative and qualitative parts. The quantitative part concentrates on factors finding out which influence professional relationship between children´s nurse and mother in compliance with the H. E. Peplau model and concurrently specifies individual nurse roles changing during professional relationship children´s nurse and mother. The data of the quantitative research part have been learned by means of questionnaire disquisition. By the dissertation results summarizing I found out facts regarding H. E. Peplau model individual phases utilization at the children nurse {--} mother relationship establishing in neonatology. Hypothesis 1 {\clqq}Children nurses with own children establish faster the professional relationship with mother`` {--} has come true. Hypothesis 2 {\clqq}Children nurse practice length influences the ability to establish the professional relationship with mother`` {--} has come true. Hypothesis 3 {\clqq}Children nurses undertake the role of the mentor in most cases`` {--} hasn´t come true. The data of the qualitative research part were identified by observation, conversation with children nurse, creating of the documentation own proposal at the mothers of prematured children hospitalized by ROOM IN system and its application in practice. Mothers were divided into two groups, the common way of the interpersonal relationship establishing with mother was used in the first group, the way of the interpersonal relationship establishing with mother according to the H. E. Peplau model was applied in the second one. At the quasiexperiment it has been identified the interpersonal relationship establishing with mother proceeds better at the nursing H. E. Peplau model utilisation. The dissertation knowledge can be partly utilised by neonatology department of the Hospital JSC in České Budějovice, in particular by children nurses to master the abilities for professional relationship establishing with mother and to provide subsequently mother and child with the complex nursing care. Further to students and other interested people in this issue.

The topic of the thesis is: Specifics of nursing care of a child patient with oncologic pain. Children{\crq}s pain is a phenomenon surrounded by many myths and false information. It is also not sufficiently assessed and treated and often bagatelized by both nurses and doctors. Nurses play irreplaceable role in pain treatment, they are with the child patient more frequently then the doctor and are in close touch with the child{\crq}s parents. The care of a suffering child is a serious task in nursing. Treatment of a patient in pains requires a complex, holistic approach. Nurses, even if they diagnose pain properly, do not set an adequate nursing care plan and do not carry it out. Nurses have to defend interests of the sick child and must not let him/her suffer. The data was obtained by quantitative and qualitative research. The quantitative research was carried out by the technique of anonymous questionnaire. The questionnaire was made for the parents of children with oncologic disease. The qualitative research was carried out by the technique of semi-structured in-depth interview. The research sample consisted of nurses working at the Department of children{\crq}s oncology One of the aims of this thesis was to outline the problems in nursing care of a child patient with oncologic pain from the point of view of the parents. We observed if the parents had been informed about their child{\crq}s pain, if they know what symptoms they should watch in their child, if they know all the aspects of pain to be assessed and treatment methods to relieve the pain and also how nurses respond to their child{\crq}s pain. For this aim, a following hypothesis has been set forth: The parents of children patients with oncologic pain are educated about the possibilities of nursing care focused on relieving the pain. This hypothesis was confirmed by statistic testing. Another aim of this thesis was to find out the specifics of nursing care of a child patient with oncologic pain. Nurses think that the specifics of nursing care of a child patient with oncologic pain are not different from any other care of a child in pains. They just state that an oncologic disease is associated with pains caused by the disease itself or by diagnostic or therapeutic procedures. Therefore they consider a specific feature the fact that the pain is observed more intensively and the staff is more precise in relieving the pain. It is obvious from the research that it would be effective for both parties, both for the nurses and the parents, to use educational materials on pain intended for them. I suppose that if there were such materials available, the parents would not need to contact nurses repeatedly. There was also a positive finding in the research that 92.5% of parents stated that nurses respond to children{\crq}s pain within 10 minutes. Further, that nurses believe that the child has pains, which was stated by 98.2% of the parents.

Abstract We most often see a family as a social group characterised by interaction within itself. A family is linked through numerous relations and it is also affected by a child. This applies to any other family even a family with a disabled child. In the world, there are about five million people who are affected by Down syndrome. It is the most widespread of all forms of mental retardation. In the Czech Republic, approximately seventy children with Down syndrome are born each year. In this paper, I worked on ``Collaboration of nurses caring of children with Down syndrome{\crqq}. The diploma paper was to give comprehensive information on specialized care of children with Down syndrome, including information on diseases and problems that might occur in the phylogenesis and life of such a child. The purpose of the paper is also proper understanding these issues, linkages with nursing care, therapeutic and diagnostic procedures as well as but finding the right approach to a family, education of families, and communication with parents. As a result, the paper should provide information about the collaboration of nurses and parents in influencing health and development of a child with Down syndrome. The objectives of the paper to map the problem of collaboration of nurses and parents raising a child with Down syndrome and to map the impact of nurses{\crq} collaboration with parents on complex care of children with Down syndrome have been met. Quantitative research was conducted using a questionnaire for nurses in surgeries of paediatricians and general practitioners for adolescents in the South Bohemian and Pilsen the Region. The hypothesis that nurses educate the parents of children with Down syndrome in the field of nursing care has been refuted on the basis of evaluated questionnaires. For qualitative research the following question was specified: ``What is the role of nurses in complex nursing care of children of children with Down syndrome.{\crqq} The answer was assessed based on interviews with parents bringing up a child with Down syndrome. The role of a nurse is one of a care provider, educator, adviser and protector. The findings of this diploma paper could serve as feedback for self-reflection of nurses in paediatric surgeries and to improve collaboration between parents and nurses in complex care of children Down syndrome.

Nursing care of restless and aggressive patients, despite the unstoppable progress, can hardly do without the use of restrictive measures in all cases. In order to provide quality nursing care and to ensure the patient´s safety even with the application of restrictive measures, it is necessary to determine the level of professional quality, or the introduction of nursing standards at all wards where restrictive measures are used. The aim of this thesis was to verify if the restrictive measures in psychiatric inpatient units and inpatient wards of other medical disciplines are being used in accordance with the standard of nursing care of patients who are restrained in bed, and analyze the current standards of nursing care determining the use of restrictive measures. Another objective was to find out nurses´ attitudes to application of restrictive measures and to survey their feelings while applying restrictive measures. On the base of the objectives hypotheses were formulated with regard to data in literature and my own clinical practice, which also includes the use of restrictive measures. The nursing research was conducted through a questionnaire at the wards where restrictive measures are applied. The respondents were nurses working in these units. The results provided clear evidence that not only worldwide, throughout Europe, but also within medical facilities of the Czech Republic, the care of psychiatric patients is inconsistent in terms of restrictive measures, and therefore, standardization of this care is highly desirable. A component part of this thesis is the created Standard of nursing care for application of restrictive measures in medical inpatient facilities, which will be used in practice in České Budějovice Hospital, Inc.

Mentor v ošetřovatelství - vyhodnocení a inovace kurzu / Mentor in Nursing {--} Evaluation and Innovation of the Course.

The Diploma thesis is focused on evaluation and innovation of a mentor course. Based on the analysis of 11 educational programmes in certificated courses we found out that the courses are not unified. We chose one of the analysed mentor courses called ``Supervisor of clinical practice`` at the Faculty of Health Care Studies of the University of West Bohemia in Pilsen. The main goals of our research were: 1. to find out the opinions of the mentor course gradutes on the course, 2. to identify possible shortcomings of the courses for clinical practice mentors, 3. to suggest innovation of the current courses. The empiric part of the Diploma thesis was worked on a quantitative research technique to find information required. The data collected through semi-structured interviews were formed into casuistries and subsequently categorised in the tables. Respondents praised the length, the number of participants and the overall structure of their course but they considered the practical part of the course to be insufficient in the extent and the lack of experienced clinical mentors. So a draft proposal of the course was prepared. Our research found some shortcomings of the courses and resulted into a course innovation. The analysis used by the Ministry of Health should help unify educational programmes of mentor courses.

The master thesis named {\clqq}Differences in the relationships between nurse mentors and Bachelor´s and Master´s degree students`` is divided into the theoretical and practical part. In the practical part it was discovered with the aid of the qualitative research, that the nurse mentors does not see differences in the relationships between Bachelor´s and Master´s degree students. The hypothesis stating that the students of Master´s degree programmes enjoy better relationships with mentors than the students of Bachelor´s degree programmes has been disconfirmed. The interviews with the mentors showed that one of the factors affecting the relationships of the nurse mentors to the Master´s and Bachelor´s degree students is the lack of time they have for the students. The results of the questionnaires confirmed the hypothesis stating that the most frequent factor which affects the relationships of students to the mentor is the mentor{\crq}s attention given to them.

Edukační proces pro sestry v pediatrické primární péči a rodiče dětí s epilepsií / The education process for nurses in pediatric primary practice and parents of children with epilepsy

The epilepsy occurs in the whole world without exception of age, race, geographical location and socio-economic structure. The incidence of epilepsy depends on age and influences each time the quality of life of the child and its next relatives. Our children and young people perceive, experience and evaluate their life usually from different points of view than adults including the parents. For this reason the main target of the nurse effort should be the education and involving the family into the process of healing. Also the preventive and educational care is connected with this. The target of thesis was to map if the education by the nurse concerning the regimen measures has influence on knowledge extent of the family of an epileptic child. The work focused, as for quantitative examination, on nurses in the surgeries of physicians for the children and young people (pediatrists) in South Bohemia. By the technology of the questionnaire the possibilities of provided primary and secondary care for children with epilepsy by the nurse and the scope of the co-operation of the nurse with the clubs of epileptics were mapped. Within the qualitative examination, based on the talks with parents of the children with epilepsy, the contribution of the knowledge of regimen measures handed on through the education by nurses in the primary pediatric care was examined. It follows from the results of the quantitative examination that the nurses in the outpatient departments of pediatrists do not devote attention to primary or secondary prevention of epilepsy and simultaneously they do not co-operate with the clubs of epileptics. It was impossible to verify all three hypotheses. It followed from the results of the quantitative examination that nurses in outpatient departments of physicians for children and young people do not sufficiently educate the parents of children with epilepsy. If the education by the nurse in the pediatric primary care has been performed, it was a contribution for parents each time. The results of the thesis lead us to reflection concerning the role of the nurse as educator and simultaneously as defender of the client- child in the outpatient departments of pediatrists. The results of the thesis: I would like to address the nurses in primary care, that they should assert themselves more, educate themselves and get involved into the care for children with epilepsy and hereby help to children to improve their childhood in the way necessary for their healthy growth and development.
