Pracovní zátěž a životní spokojenost u zdravotníků (se zaměřením na všeobecné sestry) / Occupational load and life satisfaction of medical staff (focusing on general nurces)

The topic of this master thesis is focused on occupational workload and life satisfaction of medical staff members, specifically general nurses. Profession of nurse is very physically and mentally demanding and the workload can lead to disturbance of nurse's health and well-being, thus also life satisfaction, which is a cognitive part of well-being. The aim of the theoretical part of the thesis is to be knowledgeable in thematic literature and summarize gained information into meaningful content. The first chapter is dedicated to the profession of nurse, its content, what education must be completed and what personality assumption should be met. The following chapters are focused on the occupational workload and stress of nurses, connected problems and coping options and also life satisfaction with related concepts is discussed. Aim of the empirical part is to analyse how selected Czech general nurses from inpatient wards scored in the Questionnaire of Life Satisfaction and the Meister's Questionnaire of Workload Rating, explore dependence of the outcomes on selected factors and causality between the outcomes. A general nurse may, without clinical supervision, administer medicinal products with the exception of intravenous injections (IV) or infusions in newborns and children under 3 years of age and with the exception of radiopharmaceuticals. This activity is regulated in Decree No. 55/2011 Coll. by Ministry of Health of the Czech Republic. The preparation and administration of the medicinal product is an integral part of the everyday practice of a l nurse; however, the dimensions of this procedure are very broad, and it does not start and end with the administration of the prescribed medicinal product. In the wider context, this is a multi-disciplinary process, in which other health professionals can participate with their well-founded roles. Determining individual roles and collaboration in the multi-disciplinary team composed of a doctor, a nurse and a clinical pharmacist has an effect on the correct pharmacological treatment of the patient. Of all professions, nurses spend time with patients. 40 % of their time administering medications; therefore, they play a key role in the reduction of medication errors. (Miller et all, 2016). One of the key factors influencing the quality of nursing care practice is the research applied within nursing processes. Therefore it is vital for theory to be closely linked with practice. To perform research and apply its results in practice is one of the conditions which lead to the development of evidence-based practice. Research applied in nursing care is a key strategy resulting into the unification of the human educational and scientific aspects of nursing care. This thesis focuses on the nursing care research and its application in practice together with the role of senior nurses within this process. The aim was to investigate whether and how the nurses are actually involved in nursing care research plus to examine whether ordinary nurses are willing to accept any changes associated with the introduction of the results of nursing care research into nursing practice. Besides this, also to determine whether nurses are encouraged to implement the results of nursing research into nursing practice, to observe nursing care deputies and leading nurses experience in respect to the implementation of the nursing care research results into practice in different types of hospitals plus to determine whether and how nurses introduce the results of nursing care research into practice. The final objective of this thesis was to determine whether the head nurses manage to introduce the results of the nursing care research into their nursing practice. In quantitative survey performed, the research sample consisted of nurses who work in inpatient departments at clinics and regional hospitals - public limited companies of the Central, South, West Bohemia, and Prague. 350 questionnaires were distributed. The research involved 186 questionnaires. Qualitative research data collection involved 12 respondents, out of which there were 3 head sisters employed at clinics and 3 nursing care deputies plus 3 head nurses from regional hospitals - public limited companies. All interviews were entirely anonymous. According to the survey findings, most of the nurses replied that they are not actively involved in the nursing care research. Nurses generally follow published results of nursing care research, but on the other hand, it is obvious, that they follow nursing care research outcomes randomly and without particular focus. Nurses not only that they refuse any changes, but they do not see any benefits of the implementation of research findings into the nursing care practice. The qualitative survey performed met objectives and all questions were answered. Any of the responses can serve for further quantitative research, especially when the results of the implementation of the nursing care research into practice at clinics and regional hospitals are compared.

Role školící sestry v adaptačním procesu sester na vybraném pracovišti / The role of preceptor nurse in the transition period at the selected workplace

Introduction: A nurse preceptorship is essential in the transition period (TP) of novice nurses. Especially in intensive care workplaces, where training for highly specialized and demanding care is provided. TP thus becomes a sensitive period in which the employee decides whether to remain in the workplace or even in the profession. Methodology: This is qualitative study with the main focus of the survey on the specific experience of preceptors with TP management. Data collection was performed using semi-structured interviews with preceptor nurses in one selected workplace of intensive care. Data analysis was performed in 4 steps, open coding and sorting of data into categories and subcategories were used. Results: Six main topics were identified: Preceptors' experience from their own transition, TP organization attributes, Evaluation and feedback, The role of the preceptor nurse, Critical moments and Factors influencing the TP. The experience of trainers from their own TP is a significant predictor for their individual way of training and leading novice nurses. Evaluation and feedback have a culture and should be performed regularly in cycles to achieve the purpose. Information and education of senior patients in the frame of health care. This thesis focuses on informations providing by health care professionals to the seniors age patients/clients. I describe the contemporary situation in this field in the Czech republic, and define main attributes and conditions according to the historical, social, cultural and moral context in the theoretical part of thesis. I introduce the transmission and the acceptation of the information during the dialogue between a layperson and an expert in two basic levels: level of rules, especially an Inform consent, and level of an ethical discourse. I compare an atmosphere created by law and rules with human reactions, either as medical professionals, or as a patient. For the empirical section of the thesis, I describe and evaluate experiences of patients with information-giving by health care professionals. I have chosen qualitative method for the main body of my empirical research, which consists of semi-structured interviews with 20 patients. The collected data were cathegorised, analysed and then quantified and prepared for interpretation. The topic of this diploma thesis is the compliance with legislation in an area of competencies of general nurses and special nurses working in intensive care units (ICU). The aim of the investigation was to map out, how nurses in the ICU comply with legislation in the area of their competencies and whether their work is influenced by anything. The results showed two trends - situation within a specific unit, that means whether nurses contravene their competencies; whether nurses are aware about their competencies and about competencies of doctors and in addition why nurses are contravening their competencies and what is the opinion of doctors on compliance of competencies for medical professionals, what is the position of management towards contravention of competencies by nurses . Furthermore, the situation overall, that means existence of further regulatory measures, which impact on the conduct of nurses within units. On the basis of those findings the following recommendations are made for leaders of department. The specialty of Intensive Care deals with acute conditions from all medical branches within the entire age spectrum of adults. This field has its own distinctive characteristics, which determine the character of not only medical personnel communication with families of hospitalized patients, but the overall quality of nurses' cooperation with relatives. In Czech professional literature in the area of intensive care there is minimum publications on this issue up to date. The task of the current thesis is to map a view of paramedical staff (nurses) on the process of communication and cooperation with family members of patients hospitalized in the intensive care units. The empirical part of the research is processed as a qualitative research by the method of semistructured interviews with nurses from the Intensive Care workplaces. As the result of this work we found inadequate professional readiness of nurses in the fields of communication strategy and approach to the families of patients hospitalized in the intensive care environment.
