Assessing Condom Use among Navajo Men in the Southwest

The Navajo tribe reports extraordinarily high number of cases of sexually transmitted diseases (STDs), such as gonorrhea, chlamydia, and human immunodeficiency virus (HIV) among men who have sex with men (MSM) and heterosexual populations. The purpose of this qualitative study was to explore the cultural value of adil' idli (self-respect) and how this cultural practice might influence health behaviors in sexual activity, condoms use, and acquiring of STDs among Navajo men. The Health Belief Model (HBM) was the theoretical foundation for providing a deeper understanding of the social, environmental, and cultural factors of condom use among Navajo men. Research questions focus on understanding whether condoms affected sexual activity, its protective role against STDs/HIV, and acceptability concerns pertaining to adil' idli (self-respect). A purposeful criterion-based sampling was used to select and interview 20 Navajo men ages 20 to 39 who lived in or near Shiprock, New Mexico and Gallup, New Mexico. I used a grounded approach and categorizing strategy to code and analyze the transcripts. Key findings revealed that the positive components of adil' idli (self-respect) have influenced Navajo men to protect themselves by wearing condoms. Recommendations include identifying strategies to address condom errors and failures and to develop tactful approaches to promote correct condom use in order to decrease the rates of STDs and HIV among Navajo men. The positive social change implications include health professionals' use of findings to improve STD and condom use prevention behavior among Navajo men by integrating the cultural beliefs of adil' idli (self-respect), specifically emphasizing the positive aspects of staying healthy in health messages.

Substance Abuse, Intimate Partner Violence and Risk for HIV among a Community Sample of Hispanic Women

Among the health disparities affecting the U.S. Hispanic population today are those relating to risky behaviors such as substance abuse, intimate partner violence (IPV) and HIV/AIDS. However, few studies have examined how these conditions may impact this population. The purpose of this dissertation was to explore the experiences that Hispanic women in South Florida have with regard to substance abuse, IPV and risks for HIV/AIDS, to describe how these conditions may be related, and to develop a model that can be used to guide research and interventions targeting this population. This dissertation uses data collected in Project DYVA (Drogas y Violencia en las Americas- Drugs and Violence in the Americas), a pilot research study that utilized both qualitative (Phase I) and quantitative (Phase II) research methods to describe the experiences of Hispanic women in South Florida between the ages of 18 and 60 with regard to substance abuse, violence and risky sexual behaviors. Three studies were conducted as part of this dissertation. The first study utilizes data collected during the qualitative phase of Project DYVA. During this phase eight focus groups were conducted and analyzed using qualitative content analysis (N = 81). The second and third studies utilize data collected during the second phase of Project DYVA. In this phase cross-sectional questionnaires collecting information regarding demographics, acculturation, self-esteem, depression, substance abuse, IPV and risks for HIV, were administered to 82 Hispanic women. Univariate and multivariate statistics were used to explore the relationships between substance abuse, IPV and risk for HIV (study 2) and between resource availability, IPV and depression (study 3). The findings from this dissertation suggest that substance abuse, IPV and risk for HIV are closely related intersecting health issues. IPV, the condition that emerged as the most salient of the three, also appears to be closely associated with resource availability (i.e., self-esteem and income) and depression. Additional individual, cultural, relationship and socio-environmental factors that may play a significant role in shaping the experiences that Hispanic women have with regards to these intersecting conditions were also identified and organized into a model.

The Chi-square test was used to assess the significance where a p-value of &lt / 0.05 was considered statistically significant. In conclusion, Primary school pupils in Meru district engage in several risky sexual behaviors including substance use, sexual coercion, early sexual debut and engaging in various sexual practices. This calls for a more comprehensive approach in the fight against HIV among primary school children in Tanzania, which could include life skill training at an early age, behaviour change communication interventions, advocacy activities to influence policy formulation, condom promotion and incorporating key stakeholders in the rollout of school based HIV programmes such as parents, community leaders and faith-based organizations leaders.</p>

Use of HPV Vaccination for Cervical Cancer Prevention in African American Women

Despite the availability of the human papillomavirus (HPV) vaccine, African American women are still at risk for contracting HPV. This is significant because HPV is one of the main risk factors for cervical cancer. The purpose of this quantitative study was to explore the relationship between the use of HPV vaccination for cervical cancer prevention and personal history of HPV, reduced access to healthcare, and risky sexual behaviors in African American women. The theoretical framework used for this study was the health belief model (HBM). Two hundred twenty-nine (n=229) African American women living in the United States, ages 18-49, who participated in the 2013-2014 National Health and Nutrition Examination Survey (NHANES) were the sample under study. A univariate analysis was performed to describe the population and obtain frequencies and percentages for all covariates. A bivariate analysis was conducted to determine whether there was an association between any of the independent variables and the dependent variable. A multivariable logistic regression was conducted to build a predictor model for use of HPV vaccination. Women between the ages of 18 and 34 were 7.22 times as likely to receive the HPV vaccine as women aged 35-49 years, and this was statistically significant (OR: 7.22; 95% CI: 2.36 - 22.13). This study can contribute to positive social change within the community and public health profession through an increased awareness and knowledge of HPV and cervical cancer, especially for African American women.

A possibilidade de obtenção de informações relevantes através domonitoramento hormonal não-invasivo foram fatores determinantes para o delineamento deste projeto, que possibilitou um estudo detalhado dos aspectos comportamentais, reprodutivos e endócrinos do sagüi-detufo-preto (Callithrix penicillata), mantido em cativeiro. Utilizamos6 casais desagüi-de-tufopreto (Callithrix penicillata) mantidos cativos e em pares no Centro de Reabilitação de Animais Silvestres - CRAS, São Paulo. Todos os animais tiveram seus comportamentos sócio-sexuais registrados pelo método de amostragem focal por intervalo de tempo com registros a cada 30 segundos em sessões de 30 minutos, totalizando 60 horas para cada animal. Foram coletadas amostras fecais matinais, seis vezes por semana durante 6 meses, a fim de serem extraídos e dosados os metabólitos fecais de estradiol, progesterona e testosterona pelo método de enzimaimunoensaio (EIE). O objetivo deste trabalho foi descrever e analisar os comportamentos sócio-sexuais e suas respectivas associações com as concentrações de metabólitos fecais de esteróides sexuais em machos e fêmeas de sagui-detufo- preto (Callithrix penicillata), nas diferentes fases do ciclo ovariano.Os resultados mostraram a duração do ciclo ovariano de 24,3 ± 4,1 dias e as maiores concentrações de metabólitos fecais de estradiol e testosterona das fêmeas na fase lútea. A ocorrência dos comportamentos sócio-sexuais se mostrou mais frequente na fase peri-ovulatória do que nas fases folicular e lútea. Não houve diferença significativa destes comportamentos entre as fases folicular e lútea. Six couples of captive black tufted marmoset (Callithrix penicillata) were housed at the Centro de Reabilitação de Animais Silvestres - CRAS, São Paulo. Socio-sexual behaviors of all animals were registered using the method of focal interval sampling with observations performed each 30 seconds in 30 minutes sessions, in a total of 60 hours per animal. Fecal samples were collected during the morning, six times a week, for 6 months, in order to extract and measure fecal metabolites of estradiol, progesterone and testosterone by enzyme immunoassay (EIE). The objective of the present study was to describe and analyze the socio-sexual behaviors and correlate them with the fecal concentrations of sexual steroids metabolites in paired male and female black tufted marmoset (Callithrix penicillata) in different moments of the ovarian cycle. Results demonstrated that helengthofovariancycleis24. ± 4.1 days. Also, higher concentrations of fecal metabolites of estradiol and testosterone in females are observed during the luteal phase.The occurrence of socio-sexual behaviors were more frequent during the periovulatory phase when compared to the follicular and luteal phases. No differences were found when comparing the follicular and luteal phases. In the males, fecal metabolites of testosterone were similar throughout the ovarian cycles of the respective females.

Long-term associations between childhood sexual/physical violence experience, alcohol use, depressive symptoms, and risky sexual behaviors among young adult women

Current literature lacks longitudinal understandings of the association between childhood sexual/physical violence, alcohol use, depressive symptoms, and indiscriminant sexual behaviors among young women, as well as the racial/ethnic differences in these associations. Therefore, using the 1994-2008 National Longitudinal Study of Adolescent Health, this study examined a) heterogeneous growth trajectories of problem alcohol use during the transition from adolescents to young adulthood and the impact of childhood sexual/physical violence on drinking trajectories, b) the long-term impact of childhood sexual/physical violence on alcohol use and depressive symptoms, and c) the structural associations between childhood sexual/physical violence and indiscriminant sexual behaviors by examining alcohol use and depressive symptoms as mediators between White and African-American women. First, with 1,702 women, LCGM was used to identify trajectories of problem alcohol use using the first three waves. Four trajectories of problem alcohol use emerged: stable abstainers; decliners (moderate-low); incliners (low-moderate); and rapid incliners (low-high). From the bivariate level analyses, in reference to stable abstainers, White women who experienced childhood sexual/physical violence were more likely to be rapid incliners (low-high). Second, with 1,756 women, autoregressive cross-lagged path models were performed to test longitudinal associations between childhood sexual/physical violence, problem alcohol use, and depressive symptoms of White and African-American women. Both groups demonstrated significant association between childhood sexual/physical violence and subsequent development of depressive symptoms, while only White women demonstrated significant association with subsequent problem alcohol use. Third, with 1,388 women, SEM and multigroup SEM were used to test pathways between childhood sexual/physical violence and indiscriminant sexual behaviors for White and African-American women. SEM indicates that problem alcohol use and depressive symptoms mediated the proposed relationship. Multigroup SEM indicates that, for White women, both problem alcohol use and depressive symptoms mediated the association between childhood sexual/physical violence and indiscriminant sexual behaviors, while only depressive symptoms mediated the proposed association for African-American women. These findings highlight the importance of designing and providing effective prevention and treatment programs for women who experienced childhood sexual/physical violence to interrupt subsequent problem alcohol use, depressive symptoms, and indiscriminant sexual behaviors. / text
