To determine the effectiveness of the cell block technique for immunocytochemical
diagnosis by comparing cytomorphologic preservation and immunocytochemistry (ICC)
stains in paired cell block and conventional fine needle aspiration (FNA) samples.
Study Design
This was a prospective study. Material for both conventional smears and cell blocks were
collected simultaneously during fine needle aspiration of 50 lesions comprising lymph
node, lung and liver masses. Grading of cellularity, morphological preservation,
architectural preservation, immunocytochemical staining intensity and presence of
background staining were compared on paired FNA smears and cell block samples
derived from the same case. Each arm of the paired analysis was performed blindly
without knowledge of the grading outcome of the other. The Kappa statistic (Κ) was used
to measure inter-rater agreement.
Results
The fifty samples evaluated included FNAs from the lung, 24/50 (48%); liver, 23/50
(46%) and lymph node, 3/50 (6%). The immunocytochemistry stains consisted of 44/50
(88%) CK7, 44/50 (88%) CK20, 18/50 (36%) TTF1, 10/50 (20%) synaptophysin, 10/50
(20%) Hepar-1 and 7/50 (14%) AE1/3. There was no overall agreement in preservation
of cytomorphological detail and ICC staining between the two methods. The
Papanicolaou stained conventional FNA smears fared better then cell block for the

evaluation of nuclear and cytomorphologic characteristics; cells in the cell block were
poorly preserved in many cases. The ICC stains worked better on the cell block samples
due to lack of background and aberrant staining.
Conclusion
Conventional FNA smears and cell blocks complement each other. Our results indicate
that it would be optimal to use both modalities in the diagnostic work-up of mass lesions
amenable to FNA diagnosis; the former to assess morphology, and the latter for optimal
immunocytochemistry results. In resource constrained settings, the cost implications of
performing both conventional and blocked smears on all FNA material warrants further
evaluation.

Há poucos estudos sobre a associação do resultado de citologia cérvico-vaginal alterado em mulheres gestantes e mulheres com sangramento genital brasileiras. Além disso, ainda permanece controverso se as gestantes deveriam ser submetidas ao exame de citologia cérvico-vaginal como rotina do pré-natal e se a citologia cérvico-vaignal pode ser utilizada como método único de exclusão de neoplasia do colo do útero para as mulheres com informação clínica de sangramento genital. OBJETIVOS: Estudar os fatores associados ao resultado do exame de citologia cérvico-vaginal alterado para mulheres gestantes e não-gestantes (idade, idade de início da atividade sexual, intervalo entre exames); e para mulheres com informação clínica de sangramento genital. MÉTODOS: Estudo observacional analítico que avaliou os resultados dos exames de citologia cérvico-vaginal encaminhados ao laboratório de Citopatologia Dr José Aristodemos Pinotti do Centro de Atenção Integrada à Saúde da Mulher da Universidade Estadual de Campinas durante o período de Janeiro de 2000 a Dezembro de 2009 (10 anos) oriundos de mais de 70 municípios da região de Campinas, São Paulo - Brasil. O resultado do exame de citologia cérvico-vaginal foi reportado de acordo com o Sistema Bethesda. A partir dos formulários próprios da Instituição foram extraídos dados clínicos, citopatológicos e sociodemográficos necessários para a realização do presente estudo. patients.
Methodology: A case detection transversal study took place in a leprosy reference center in the Amazon State.
Results: The study included 48 leprosy patients being 15 multibacilary (MB) cases and 33 paucibacilary (PB) cases. The diagnosis of MB patients was confirmed through
bacteriological examination and PCR of the skin slit smears, while, 16 (33%) of PB patients were positive in PCR and none in bacteriology examination of the skin smears. Four PB
patients with negative PCR skin smears were PCR positive in the periodontium and saliva, 1 positive in saliva and 2 in the periodontium. No sample of periodontium and saliva had
positive bacteriological results. A prevalence of 58,3% of periodontal disease (DP) was observed.
Conclusion: there was no correlation between DP and the presence of M. leprae; bacteriological examination did not proved to be an efficient technique for analysis of saliva
and periodontium; the skin smears PCR technique proved to be most efficient method for to diagnosis PB patients than bacteriological examination; PCR positivity for detection of M.
leprae in PB patients can be increase by collecting slit skin smears, periodontium and saliva. / Objetivo: verificar através da Baciloscopia e da Reação em cadeia da polimerase (PCR) a presença do M. leprae no periodonto, saliva e raspados intradérmicos de pacientes com
hanseníase.
Metodologia: realizou-se um estudo transversal do tipo detecção de casos numa instituição referência de hanseníase no Amazonas.
Resultados: O trabalho foi composto por 48 pacientes sendo 15 multibacilares (MB) e 33 paucibacilares (PB). Os pacientes MB tiveram o diagnóstico confirmado pela baciloscopia e
PCR de raspados intradérmicos, enquanto que, 16 (33,3%) dos PB foram positivos na PCR e nenhum na baciloscopia. Quatro pacientes PB negativos na PCR de raspados intradérmicos foram positivos no periodonto e na saliva, 1 positivo na saliva e 2 no periodonto. Nenhuma amostra do periodonto e da saliva foi positiva na baciloscopia. Verificou-se uma prevalência de doença periodontal (DP) de 58,3%.
Conclusões: não houve relação entre a DP e a presença do M. leprae; a baciloscopia não mostrou ser uma técnica eficiente para análise da saliva e periodonto; a técnica de PCR de
raspado dérmico mostrou ser um método mais eficaz no diagnóstico dos PB do que a baciloscopia; a positividade da PCR para detecção do M. leprae nos PB pode ser aumentada
coletando raspado intradérmico, periodonto e saliva.

With a career spanning over four decades, Henry Fillmore earned wide recognition as an important and prolific composer of marches and trombone smears in the wind band medium. While he contributed significantly to chamber music, solo literature, vocal settings, and arrangements for band, his compositions for wind band have arguably provided his most universal acclaim. Fillmore’s published marches are unique in that scores rarely had performance markings and contained little more than notes and repeat signs. Fillmore conducted most of his marches and altered march strains, changed orchestration, and added stylistic markings that were not indicated in the original printed score.
This research is focused on preparing and presenting historically accurate performances of the marches and smears of Henry Fillmore with appropriate march style. More specifically, the author wishes to provide an approach to the interpretation of Fillmore’s music that directly portrays the historical performance practices of Fillmore’s performances of his own works and interpretative methodology. Those who wish to consider this historically accurate approach may apply this document’s analysis of primary source recordings of Fillmore conducting his music in addition to new included performance editions of specified marches and smears.
