Nurses perceptions of the factors contributing to the spread of tuberculosis in a clinic in the Odi Moretele sub district of Gauteng

Introduction: Despite being one of the most preventable diseases, TB still remains a serious and largely neglected disease. Nurses as compared to the general population are at greater risk of acquiring nosocomial TB. This study was conducted to describe the perceptions of nurses on the underlying contributory factors that may lead to the spread of TB in the clinics treating TB patients. Methods: Quantitative, non – experimental, descriptive, exploratory and cross sectional design was used. A structured and pretested questionnaire was used. Findings: The key contributory factors identified were insufficient TB training for staff and lack of knowledge on the TB legislative framework and TB policy directives. Conclusion: The findings indicate the need for a comprehensive TB infection prevention and control policy, with associated standards for provision and practice. / Health Studies / M.A. (Public Health)

Manifestations of corruption in the City of Tshwane Metropolitan Municipality

This study was conducted as a result of the various media reports, various law enforcement investigation reports, Public Service Commission, reports of the Auditor General South Africa and the Public Protector South Africa on corruption and the various forms in which corruption manifests itself in the City of Tshwane Metropolitan Municipality and the effectiveness of the various law enforcement agencies and the government in tackling the scourge of corruption in public and private institutions. While South Africa has various anti-corruption strategies, they all seem to be ineffective and have thus far not achieved their desired results. A qualitative research approach was used to investigate this problem. A thorough literature study and documentary analysis was used to gather all the relevant data related to this topic. It has been found that the most common manifestations of corruption that take place in the South African public service are fraud, bribery, extortion, nepotism, conflict of interest, cronyism, favouritism, theft, graft, embezzlement and abuse of power. Other forms of corruption that raise concern are, for example, influence-peddling, insider trading/abuse of privileged information, bid-rigging and kickbacks. South Africa has, on the other hand, seven anti-corruption institutions and seventeen pieces of legislation which are intended to combat corruption in the public and private sector. These pieces of legislation give South African law enforcement and investigators in the public and private sectors a clear mandate with a view to investigating the high levels of corruption. It is the submission of the Public Service Commission (2011:vi) that corruption has become a global concern that seriously hampers development in any country and diverts its resources from where they are needed the most. Community service nurses' experiences regarding health care services at Tshwane district public hospital

Text in English / The aim of this study was to gain understanding of community service nurses’ experiences of health care services at Tshwane district public hospital. This qualitative study followed an Interpretative Phenomenological Analysis (IPA) approach to explore the community services nurses’ experiences of health care services at Tshwane district public hospital. Data were collected from 11 purposively selected community service nurses using a semi-structured interview format. Data was analysed using Interpretative Phenomenological Analysis framework for data analysis. Four super-ordinates emerged from data analysis: (1) Resources, (2) Work environmental relations, (3) Supervision and support and, (4) Impact of community service experiences. The study revealed that the health care services rendered at the hospital studied are substandard. Community service nurses reported several challenges experienced during their placement in the hospital under study. Lack of human and material resources, supervision and support contributed to hindrance of smooth acquisition of their clinical skills and experience. These challenges resulted in the psychological and emotional drain of the participants. There is a need for development of guidelines to ensure constant and adequate support to all the community service nurses placed at Ratanang Hospital. / Health Studies / M.A. (Nursing Science)

Implementation of waste management policy in the City of Tshwane

The City of Tshwane is the capital city of South Africa and the administrative seat of government. The status of the city as a capital creates high expectations on the level of basic service delivery on municipal services like waste management, water, electricity and health. The city is currently facing challenges in relation to the delivery and implementation of waste management services. In an attempt to find solutions to the waste management challenges of the city a study on the implementation of waste management services is undertaken by the researcher. The study identifies and then analyses the underlying reasons for the challenges faced by the City of Tshwane in the implementation of waste management services. This aim of this study was to investigate and to analyse the implementation of waste management services in the City of Tshwane’s historically disadvantaged areas with particular focus on Region 01, 02, 05 and 07. In order to investigate these factors, a descriptive research design and qualitative methodology was used which related to convenient and purposive sampling of officials and data collected from fifteen (15) respondents using semi structured interviews and observations. The study also utilised document analysis to interpret the challenges and solutions related to the research topic. It emerged from the study that a lack of capacity and resources to perform efficient waste management services underpinned many of the challenges experience by the city. The failure of the city to ensure community participation and involvement is another reason for the challenges the city faces in waste management. Furthermore, the lack of policy implementation and enforcement is an element that the city needs to deeply consider. When policy is crafted with input of residents and when there is a social contract as to the roles and responsibilities of each party, it becomes easier to enforce. Some of the challenges that the city faces with regards to waste management can also be attributed to political interference and institutional deficiencies. Beyond issues like capacity, institutions and others, the city will continue to face challenges if it does not seriously invest in innovation and new technologies that address its generic and specific conditions in relation to the management of waste. The marginalisation and selective enforcement of by-laws on the informal recyclers and reclaimers are some of the findings of the study that demonstrate the inability of the city to find specific solutions to specifics regions on waste management .These challenges resulted in the peri urban regions like region 01, 02, 05 and 07 not receiving quality and consistent waste management services. The study recommends some interventions to address the waste management problems identified in the highlighted regions and entire City of Tshwane. Some of the recommended interventions include, assessing the unique characteristics of the communities and regions with a view of identifying waste management solutions that will be relevant for the circumstances and profile of such regions, ensure that proper and adequate resources, infrastructure and capacity is deployed to such areas to improve the waste services in those areas. Furthermore aggressive education and awareness campaigns conducted in partnership with communities will be critical to change people’s attitude towards waste management and a clean environment. This can be achieved through a consultative process led by the City of Tshwane in partnership with its communities and enforced through a progressive and incentive driven by-law system. / Public Administration and Management / M.P.A. Perceptions of the R48 primary health care trained nurses regarding the implementation of expanded programme of immunization in Tshwane District

PURPOSE: The purpose of the study was to explore the perception of the R48 trained professional nurses with regard to implementation of the EPI in Tshwane District Gauteng Province. METHODOLOGY: A qualitative research design was followed. Data were collected through in-depth qualitative interviews from a purposive sample of eight (8) R48 trained professional nurses. Eight steps of Tech’s inductive, descriptive open coding technique was followed. FINDINGS: The study established that EPI programme is important in R48 training. The implementation of integrated approach was viewed as beneficial leading to the reduction of diseases. Evidence-based guidelines to promote the health and safety of health care workers in selected public hospitals in the Tshwane health care district in Gauteng, South Africa

Text in English / The purpose of this research was to investigate occupational health and safety challenges and their impact on health care workers (HCWs) in selected public hospitals from the Gauteng Province, South Africa. Method: A quantitative descriptive cross-sectional survey was conducted among HCWs in the study sites. A two-staged sampling that include purposive sampling of study sites and census sampling of 2000 HCWs was used. Self-administered questionnaires were used to obtain data from HCWs. In addition, two different checklists were used to conduct retrospective records reviews to assess occupational health and safety (OHS) policy compliance and occupational injuries and diseases occurrence. The SAS Release 9.3 was used to analyse data. The Fischer Exact test and Chi-square were also used to determine the association of variables and P-value was set at <0.05 to indicate significant association. Results: A total of eight public hospitals and 926 (46.3%) HCWs who were all females nurses participated in this survey. Major occupational health hazards reported by the participants include: needle-stick injuries 275 (54.67%), slips trips and falls 67 (13.32%) and splashes 57 (11.33%). The analysis of open-ended responses indicated increased workloads, long hours of work and shift work as the most reported psychosocial hazards among HCWs. The reviewed records indicated that back injuries 22 (4.37%), tuberculosis (TB) 17 (3.38%) and asthmatic reactions 8 (1.59%) were the commonly reported occupational injuries and diseases among the HCWs. The records review also revealed a lack in the conducting of adequate medical surveillance among participants. The results showed poor compliance with the OHS policy and a negative impact of biological and psychosocial hazards on the HCWs. Conclusion: There was a high risk of exposures to biological hazards whilst providing care to patients, thus warranting the implementation of robust preventive measures. As a result, the guidelines were developed to promote the health and safety of HCWs with a view to promoting policy compliance and preventing the occurrence of occupational injuries and diseases as well as their impact among HCWs. / Health Studies / D.Litt et Phil. Integration experiences and challenges of immigrant students : a case study of a private secondary school in the Tshwane North district in Gauteng

The research sought to study the integration experiences and challenges of immigrant students in a private secondary school in the Tshwane North district in Gauteng. The study focused on investigating the experiences of immigrant learners when integrating with the local learners at the private secondary school. Integration denotes how various cultural groups interact within the bounds of a single society. Scholars argue that the dominant group in a society sets the tone and determines the nature of the integration activities and objectives. However, in other instances integration is facilitated by both the immigrants and the host people. Literature review helped the researcher to gain a broader and deeper knowledge base of the concept of migration and experiences faced. Literature review indicated the sociological and economic explanations for migration. Literature covered integration of immigrants, assimilation and acculturation. Sources of stress for immigrant students and cross-cultural adaptation were also outlined. The investigation was a case study research. Mixed methods were used to collect data. The researcher used questionnaires, interviews and observations. Questionnaires were issued to 28 immigrant learners who participated in the study. The 28 immigrant learners were also interviewed in order to obtain richer data. All indications show that the integration process at the school was positive. The results of the study showed that most immigrant learners have friends among local learners. The immigrant learners disclosed that local learners were friendly to them. Use of the biological body-fluid detection dog for investigation of rape cases

Serious and violent crime in South Africa (SA) has been on the rise in the last few years. The SAPS has been stretched to the maximum in its efforts of trying to stabilise the situation, and has applied various tactics and strategies to eradicate crime. This included, among other things, changes to the basic training programme and reintroduction of specialised units. The success or failure of any criminal investigation will still often depend on the detection and analysis of physical evidence found on the crime scene. Crimes such as rape will always leave behind physical evidence in the form of body-fluids. The detectives need not be experts in order to detect or analyse this physical evidence, but should be experienced enough to know which experts or investigative aids must be used to ensure maximum recovery of the evidence. The purpose of this study was to determine how the Biological Body-fluid Dog (BBFD) can assist detectives in the investigation of rape cases. The researcher has chosen a unique investigative aid in the form of man’s best friend, namely “The Police K9” (canine). The literature shows that trained police dogs have achieved outstanding success in numerous fields around the world, i.e. from narcotic busts, detection of explosives, to countering terrorist threats, to the most chilling search-and-rescue operations. The SAPS has sent its search-and-rescue dogs to many countries abroad to assist in natural disasters. Dogs have been trained by various police agencies for various purposes, but the BBFD dog is unique to the SAPS, and is trained to detect only human blood and semen. The use of K9s in the complex forensic science environment can never be doubted or overlooked. The main problem facing the Booysens SAPS was the low arrest and conviction rate in rape cases, due to a lack of evidence. The researcher hopes to broaden the detectives’ knowledge and skills regarding the objectives of crime investigation, with special focus on detection of physical evidence at rape crime scenes. The experiences of adult children who grew up in female-headed families

Female-headed families have become a major component of society, globally and locally. South Africa has also witnessed an increase in the number of female-headed families. Children raised in female-headed families have been reported to be disadvantaged in several ways. Many are said to be performing poorly at school, having low self-esteem, experiencing early sexual activity, and displaying adverse behaviour. Hence, the views of adult children who grew up in female-headed families were explored to inform social work practice. A qualitative, explorative, descriptive, and contextual study was undertaken with 12 participants whose ages ranged from 25 to 35 years. The goal of this study was to gain insight into the experiences of adult children who grew up in female-headed families. Data was gathered through semi-structured interviews and analysed following Tesch’s (in Creswell, 2009) framework. Data was verified using Lincoln and Guba’s model of trustworthiness. The major findings highlighted first, a deep-seated need to understand reasons for being abandoned by their fathers. Second, the resources inherent within female-headed families, often overlooked, revealed the strengths that these families possess. Third, spirituality as the foundation of their resilience during difficult times dominated the participants’ accounts of being raised in female-headed families. Fourth, various support structures emerged as fundamental components, requisite for the optimal functioning of female-headed families.
