Upplevelser under döendeprocessen hos patienter med cancersjukdom som vårdas palliativt : en litteraturöversikt / Experiences during the process of death among patients with cancer disease in palliative care : a literature review

Bakgrund Cancer är den näst främsta orsaken som leder till döden bland människor runt världen. Cancer beräknas vara orsaken till att drygt tio miljoner människor avled världen runt under 2018. Inom palliativ vård är cancer den främsta sjukdomen. Det beror på att det finns över hundratals olika cancertyper. Cancersjukdomen växer ständigt globalt och är ett stort individ, samhälls- och globalt problem. Denna sjukdom belastar individer, närstående, samhället och hälsovården i hög grad fysiskt och psykiskt och det krävs ständiga resurser för att bekämpa cancersjukdomar. SyfteSyftet med studien var att belysa upplevelser av döendeprocessen hos patienter med cancersjukdom i palliativ fas. Metod En litteraturöversikt gjordes genom att söka i databaserna PubMed och CINAHL. Det var 15 vetenskapliga artiklar som granskades och analyserades utifrån vilken kvalitet artiklarna innehar och dokumenteradesi en matris. Dataanalysen för arbetet gjordes med hjälp av Kristenssons analysmodell. Resultat Kategorierna som uppstod var lidandet ur flera olika dimensioner och hanteringsstrategier under döendet och visas som överrubriker, och relaterade subkategorier visas som underrubriker till ovannämnda kategorier. Subkategorierna som uppstod från lidandet ur olika dimensioner var att känna sig som en börda, ensamhet och isolering, osäkerhet kring sjukdomsförloppet, längtan efter att dö, och överväganden om dödshjälp. Subkategorierna som uppstod från hanteringsstrategier under döendet var patientens självbestämmande under döendet, förberedelser inför döden, förändrad livssyn och acceptans inför döden. SlutsatsLidandet var något gemensamt för alla patienter med cancersjukdom som vårdades palliativt, men var det skiljde sig var hur patienterna hanterade det lidandet. Genom en djupare insikt om patientens upplevelser under döendet i en kontext som cancersjukdom, kan sjuksköterskor ge en bättre omvårdnad som är formad efter patientens behov. Den mänskliga upplevelsen är något subjektivt och i konstant förändring till miljön som hen befinner sig i. Genom fortsatt forskning inom ämnet och vidare utbildning kan vårdpersonal möta nya utmaningar som uppstår inom den palliativa vården.

Factors that Influence Under-represented Minority Students in Science, Technology, Engineering, and Mathematics attending a Predominately White Institution, the Role of Retention

The quality of STEM in higher education is widely recognized as an important issue. There is tremendous amount of concern around persistence, retention, and attrition for under-represented minority students (URMs) majoring in STEM disciplines. Retaining this population of students in the different STEM discipline is also a major concern for educators and policy makers in the United States. Student interest is there, however, problems arise when course work becomes challenging. As a result, students tend to lose confidence and change their majors. The purpose of this study was to discover factors that are related to persistence in STEM majors for a group of URMs who participated in a summer bridge program between 2008- 2014 at a Predominately White Institution of higher learning. In this study the researcher used archived data and quantitative research methods to identify different factors. Data analysis was conducted to answer the eight research questions that guided the study. To answer research question 1, descriptive statistics were used to analyze data that provided a comprehensive description of the students’ high school and college academic performance. One way ANOVAs were computed to analyze data for research questions 2-6, whereas Chi –Square Tests of Independence were used to analyze data for research questions 7 and 8. The researcher examined data for 232 URMs who participated in a summer bridge program during 2008-2014 academic years. The findings for research question 1 revealed that URMs represented in this study were college ready according to their average ACT scores and high school GPAs. An examination of two gatekeeper courses (Calculus and Chemistry) revealed that majority of the students passed with a letter grade of C or below. The findings for research questions 2-6 revealed statistically significant differences for URM students who persisted in STEM majors. They included ACT composite scores, ACT subscale scores all expect for reading, first semester GPA, first year GPA, sophomore year GPA, and number of credits earned through sophomore year. The findings for research questions 7 and 8 revealed that persistence was not influenced by gender or STEM major.

One of the main issues concerning my musical career is the difference in quality, con- viction, and ease between my playing in a practice room where I am alone, and my performance in a concert stage crowded with an audience. In this thesis I will focus on researching different practice methods and working routines to prepare successfully for a performance onstage and overcome this difficulty.  Through a self-analysis I have come to the conclusion that any standard performance will broadly face the following three issues: memory, technical ease and conveyance of a musical plan. I will call these the three pillars of a successful performance. Through this project I will research on how to develop these three pillars in the best way possi- ble towards a performance.  This process will consist of three steps. Firstly, I will make a review of previous scien- tific research about how the pressure of a performance onstage can generate mental and physical burdens in the playing. With the information acquired I will focus on finding appropriate working methods for technique, memory, and music making, in order to overcome those burdens. I will then implement these methods into my prac- tice and consequently write a performance diary describing the outcome.  By the end of the project, I hope to have found better working methods mentally and physically, and to have better acknowledgment of my body and mind as a performer and as an artist. I would also wish that this project will be of help to any future readers that are experiencing the same issue.

Galopperande genom ungdomslitteraturens sidor : En komparativ textanalys av hästintressets förändring under 1900-talet genom ungdomslitteraturen

Syftet med denna uppsats har varit att undersöka hur hästintresset och relationen till hästar har utvecklats genom ungdomslitteraturen under 1900-talet och därmed dess koppling till utvecklingen av hästintresset i samhället. För att uppnå detta syfte har sex böcker från 1900-talet av olika svenska författare som kan klassas som hästungdomsböcker lästs och analyserats. De sex böckerna visar på kärleksfulla relationer mellan huvudpersonen och deras hästar, där relationen spelar en stor roll för karaktärernas identitet. Hästarnas betydelse ökar även i omfattning genom böckerna, från att ha en begränsad roll till att bli den mest framträdande delen av böckerna. En tydlig skiftning kan ses vid 1970-talet, då hästarna både tar mer plats i samhället och därmed också i böckerna. Det finns även en tydlig påverkan av författarnas kunskaper och hur korrekta böckerna är. Desto senare in på 1900-talet vi kommer, desto mer kunniga är författarna. Slutsatsen är att författarna har fått större tillgång till hästkunskaper och därmed kan förmedla detta på ett mer korrekt sätt genom sina böcker.

Friend/foe Identification Accuracy And Shooting Performance: Effects Of Prior Task Loading And Time Pressure

The current dismounted soldier and the soldier of the future will be "loaded" with more information processing tasks while they perform shooting tasks. It is conceivable that some increased level of cognitive tasking may be performed simultaneously with required shooting tasks. The effect of cognitive load on shooting performance has been previously examined (Scribner and Harper, 2001). This study concentrated on the effect of various cognitive workload demands on a friend-foe discrimination shooting task in a single- and dual-task scenario. In light of this, it is imperative that the soldier not be overburdened mentally, which may result in decreased survivability and lethality. Specifically, this study was designed to examine the ability of the soldier to perform friend-foe target discrimination and shooting accuracy, with varying target exposure times, friendly target signatures, and varying cognitive load demands (working memory recall task). Using the Small Arms Simulator Testbed (SAST) we examined the effects of manipulations of working memory load and sustained information transfer, on shooting performance (as measured by target acquisition and friend/foe discrimination indices). Additionally, we investigated subjective measures of workload and stress. A secondary task, administered aurally, was given to subjects to attend to while they performed shooting (friend/foe discrimination task) scenarios: working memory recall task. Each type of task consisted of three levels of difficulty. Analysis of variance revealed significant differences for the memory recall task during shooting and non-shooting conditions. Furthermore, results showed that workload increased as a function of task demand, with associated decreases in shooting performance.

Design and Control of Two Under-Actuated Upper Body Exoskeletons for Augmenting Human Capabilities in Lifting

Exoskeletons are getting popular day by day due to their abilities in helping people. Exoskeletons can be used to help people gain motor senses through rehabilitation. It can also help healthy people to augment their abilities. These exoskeletons need to be strong yet light, so that the human body can support the exoskeleton, while the exoskeleton can support the activity that is being performed. This calls for under-actuated systems, which help in avoiding unnecessary mass due to additional actuators, while providing the same movement capabilities. This thesis describes in detail about two such under-actuated upper body exoskeletons which can be used for lifting loads. The design of such exoskeletons and novel control techniques for comfortable usage is discussed in detail. / Master of Science / Exoskeletons are assistive devices which can help people in several ways. An exoskeleton can help people who are affected with stroke by enabling them to walk through rehabilitation and physiotherapy. It can also help people to perform beyond their capacity in terms of physical activities. This could be to lift more load than possible, run faster than usual. This thesis describes the design and working two such exoskeletons which can be attached to the upper body. These exoskeletons can be used by people to lift loads which would require a lot of effort and muscle activity. The addition of these exoskeletons potentially reduce the muscle activity on the user and helps avoiding injuries in long term. Such exoskeletons have to be light weight so that they do not defeat the purpose of reducing muscle activity. This problem is solved by using under-actuated systems, because a significant mass of the exoskeleton is taken by the actuators such as the motors. Using under-actuated systems help in lowering the mass of the exoskeleton, while still being able to perform the same kinds of motion. This thesis also talks about how these exoskeletons can be controlled such that the load is being lifted with minimal efforts, and being aware of the loads it is lifting to provide the correct amount of torque, above or below which can lead to the motor shooting up or down causing muscular discomfort and injuries in the arm.

ABSTARCT [sv]  Introduktion: Sjuksköterskestudenter finns runt om i olika vårdverksamheter i samhället och verksamhetsförlagd utbildning är ett krav för att få legitimation som färdigutbildad sjuksköterska.För att garantera en god och patientsäker vård handleds studenten av en legitimerad sjuksköterska inom vårdenheten under den verksamhetsförlagda utbildningen. Syfte: Syftet var att beskriva patienters upplevelser av att vårdas av sjuksköterskestudenter. Metod: Litteraturstudie med deskriptiv design. För att besvara syftet valdes 11 artiklar ut från databaserna PubMed, CINAHL och SwePub. Innehållsanalys för dessa artiklar genomfördes med Fribergs (2022) analysmetod genom att dela in i huvudkategorier och subkategorier. Resultat: Patienters upplevelser av att vårdas av sjuksköterskestudenter delades in i två huvudkategorier vilka var ”det personcentrerade mötet” och ”lärandeprocessen”. I resultatet beskriver patienterna samhörighet med studenterna. Patienterna kände sig väl omhändertagna då studenterna hade mer tid över och var mer lyhörda än ordinarie personal vilket bidrog till ett ökat välbefinnande under vårdtiden. Vid några tillfällen kunde patienterna uppleva en osäkerhet då studenten inte var redo för ett större ansvar och sårbara situationer kunde uppstå. Slutsats: Patienterna fann glädje i att följa studenter genom lärandeprocessen och följa deras utveckling. De gav gärna återkoppling till studenterna på saker som var bra eller dåliga under lärandeprocessen. Patienter kunde dock utsättas för sårbara situationer i den studentledda vården men merparten av patienterna ansåg att vården höll god kvalitét då god kommunikation mellan student och patient fanns. / ABSTRACT [en] Introduction: Nursing students can be found all over health care-based facilities in society, with onsite internship training being a key requirement to be able to obtain their qualification to be identified as a qualified nurse. During the work-based training, the student is expected to be supervised by an experienced registered nurse within the health care unit, this is to ensure that safe and professional care is given to all patients Aim: The aim was to describe patients’ experiences of being cared for by nursing students. Method: Literature study with a qualitative approach. To respond to the aim of the study 11 articles were selected from databases PubMed, CINAHL and SwePub. Content analyses were made according to Friberg´s (2022) method by dividing into main categories and subcategories. Results: Patients experiences of being cared for by nursing students was divided into two main categories which were: “the person-centered meeting” and “the learning process. In the results, the patients describe a sense of belonging with the students. The patients felt better cared for as the students had more time to give and were more responsive than the regular staff, which contributed to increased well-being during the treatment period. In some cases, patients couldexperience uncertainty as the student was not ready for the responsibility and vulnerablesituations could arise. Conclusion: The patients found joy in accompanying the student in the learning process and following their development. The patients were happy to give feedback to the students on things that were good or bad during the learning process. Patients may be exposed to vulnerable situations in the student-led care, but most of patients felt that the care was of good quality when there was good communication between student and patient.
