Abandono em Psicoterapia Psicanalítica : estudo qualitativo

Esta tese teve como objetivo geral analisar o fenômeno do abandono em psicoterapia psicanalítica (PP) através de metodologia qualitativa. Para tanto, três artigos foram realizados identificando características de inicio e término de tratamento de pacientes adultos classificados por seus psicoterapeutas como pacientes que abandonaram a PP, em um serviço de atendimento da cidade de Porto Alegre/Brasil, cujo objetivo principal é a formação de especialistas em PP. Foi utilizado em todos os estudos o método de Bardin (1995) para analisar o conteúdo das entrevistas iniciais de tratamento, encontradas no arquivo do serviço de atendimento, e das entrevistas pós-tratamento realizadas pela autora da tese. O primeiro artigo apresenta a análise do tratamento de seis mulheres que abandonaram a PP. Objetivos pouco claros de tratamento, fraca disposição para mudar, sinais precoces de transferência negativa e resistência, e ausência de reconhecimento da própria participação nos problemas são fatores que surgiram no início da psicoterapia. Ganhos terapêuticos, insatisfação e resistência durante o processo psicoterapêutico pareceram estar associados ao abandono. O segundo artigo revela os achados dos tratamentos de cinco pacientes que abandonaram a PP e de cinco que a completaram. Pacientes que abandonaram a PP apresentaram no início do tratamento: objetivos e expectativas focalizadas, fraca disposição para mudar, capacidade de insight diminuída, percepção negativa dos tratamentos anteriores, e manifestações significativas de transferência negativa e resistência. Por outro lado, pacientes que completaram a PP possuíam metas e expectativas de psicoterapia relacionada com aspectos mais amplos da vida, foram menos resistentes para começar o tratamento, apresentaram maior disposição de mudar, transferência mais positiva, e níveis mais elevados de percepção e de satisfação com o tratamento anterior. Durante o tratamento, pacientes que completaram a PP foram menos resistentes e estavam mais satisfeitos com a psicoterapia, referiram benefícios mais eficazes e alcançaram maior capacidade de continuar trabalhando em problemas psicológicos, em comparação com os pacientes que abandonaram a PP. E o terceiro artigo, mostra os dados encontrados nos tratamentos de pacientes que abandonaram a PP em diferentes momentos da psicoterapia. Sete pacientes de tempo de abandono médio (AM- dois a 11 meses após o início da psicoterapia) comparados com sete pacientes de tempo de abandono tardio (AT- mais de um ano após o início) foram identificados como aqueles que iniciaram o tratamento mais por indicação de terceiros do que por conta própria, apresentando maior resistência, com expectativas de mais apoio, menor transferência positiva, mais queixas depressivas e experiências negativas com tratamentos anteriores. Na entrevista pós-tratamento revelaram mais resistência durante o processo de psicoterapia. Abandonaram a psicoterapia com menor capacidade de insight, avaliaram mais negativamente o tratamento tanto nos aspectos gerais como nos específicos. Embora distinções tenham sido observadas, entende-se que a diferenciação das características dos grupos de AM e AT é tênue e necessita de mais investigações. Esta tese oferece algumas hipóteses ou explicações para o complexo fenômeno do abandono da PP. Sugere que as decisões de iniciar, abandonar ou completar a psicoterapia dependem de múltiplos fatores, tais como: definição de metas e objetivos estabelecidos em conjunto pela dupla paciente/psicoterapeuta, disposição para empreender mudanças, capacidade de insight que implica em reconhecimento da condição psíquica e da participação nos problemas, resistência, transferência e experiência vivenciada em tratamento anterior. Os resultados obtidos nesta tese são exploratórios necessitando mais estudos nessa área. / This thesis had as general objective to analyze the phenomenon of dropout in psychoanalytic psychotherapy (PP) through qualitative methodology. To do so, three articles were written identifying characteristics of beginning and end of treatment of adult patients who were classified by their psychotherapists as patients who dropped out the PP, in a service of attendance in the city of Porto Alegre/Brazil, whose main objective is the formation of specialists in PP. On the other hand, patients who completed the PP had goals and expectations of psychotherapy related to wider aspects of life, were less resistant to begin the treatment, presented a bigger disposition to change, more positive transference and higher levels of perception and satisfaction concerning the previous treatment. During the treatment, patients who completed the PP were less resistant and were more satisfied about the psychotherapy, referred more effective benefits and reached a bigger capacity to continue working in psychological problems if compared to patients who dropped out the PP. And the third article shows the data found in treatments of patients who dropped out the PP in different moments of the therapy. Seven patients of medium time of dropout (MD - two to eleven months after the beginning of the psychotherapy) compared to seven patients of late time of dropout (LD - more than one year after the beginning) were identified as the ones who started the treatment by indication of others more than by their own, presenting more resistance, with expectations of more support, less positive transference, more depressive complains and negative experiences about the previous treatments. In the post-treatment interview they revealed more resistance during the process of psychotherapy. They dropped out the psychotherapy with minor capacity of insight, evaluated the treatment in a more negative way concerning its general aspects as well as the specific ones. Although distinctions have been observed, it is understood that the differentiation of the characteristics of the groups of MD and LD is tenuous and it needs more investigation. This thesis offers some hypothesis or explanations for the complex phenomenon of dropout of PP. It suggests that the decisions about initiate, dropping out, or completing the psychotherapy depend on multiple factors, such as: definition of marks and objectives established in partnership (patient/psychotherapist), disposition to undertake changes, capacity of insight which implies in recognition of the psychic condition and the participation of problems, resistance, transference and experience that were experienced in previous treatment. The results which were obtained in this thesis are exploratory and it is necessary to study more in this area.

Claims arising from invalid employment termination Abstract in English language The goal of this Master thesis is to discuss and analyse the law and case law surrounding invalid employment termination and claims arising from such disputes. An unseparable part of this issue is the distinction whether such an action taken by a party is valid, invalid, or putative. In the introducion i describe the goals and themes of this Master thesis in more detail. In the first chapter I describe employment itself, it's features and it's importance in the context of Labour Code. I also discuss the difference between it and other types of labour contracts, and I try to find an answer to why there is such a few cases of parties pursuing legal action in response in invalid termination of these contracts. The second chapter contains the ways employment can be lawfully terminated, where I focus primarily on unilateral termination. I describe the relatively uncomplicated mutually agreed employment termination, dismissal both from the employer's end and the employee's end, including all legal grounds for dismissal. Then I briefly describe severance pay, an important claim of the employee. It also deals with methods used to review the formal security of cryptographic protocols from a mathematical point of view. It analyse the tools used to automatic and semi-automatic evaluation of the safety of cryptographic protocols. It describes the process of working with these tools and finally test the security of protocols Kerberos, EKE and Unilateral authentication using symmetric cryptography, HMAC function and hash function. These tests are in tools AVISPA, ProVerif and Scyther. At the end is comparison of results.

Tensions between Sovereignty and Self-Determination Principles in the UN : UN's Ambiguity in relation to the West Papua Self-Determination Claims

Sovereignty and self-determination are two principles accepted by UN in the UN Charter and resolutions. The aim of this thesis is to analyze the tensions between sovereignty and self-determination principles in the UN and to increase understanding of how these tensions might have led to ambiguity in UN policy toward the West Papua case. The thesis identifies that there are tensions between those two principles in the UN resolutions. The tensions cause ambiguity in the UN when they are involving in self-determination cases outside the classical colonial context. The argument will be strengthened by conducting a single case study analysis on West Papua self-determination claim. As one of the self-determination claims outside the classical colonial context, the UN role when being involved in the case is argued to be lack of response and ambiguous. It concludes that the ambiguity of the UN when involved in the West Papua self-determination claim is resulted from the tensions between sovereignty and self-determination principles in the UN resolutions.

Relationship Between Corrective Saccades and Measures of Physical Function in Unilateral and Bilateral Vestibular Loss

Objectives:Following the loss of vestibular function, some patients functionally improve and are minimally bothered by their loss of peripheral function while others remain more symptomatic and are unable to return to their activities of daily living. To date, the mechanisms for functional improvement remain poorly understood. The purpose of the present study was to examine the association between corrective saccades and measures of handicap, dynamic visual acuity, gait, and falls. Design:A retrospective chart review was performed to identify patients who were diagnosed with unilateral or bilateral vestibular hypofunction and who also completed a baseline vestibular rehabilitation evaluation. A total of 82 patients with unilateral vestibular hypofunction and 17 patients with bilateral vestibular hypofunction were identified. The video head impulse test results for each patient were grouped based on the type of presenting saccades. Specifically, the saccade grouping included the following: (1) covert, (2) overt, or (3) a combination of both types of saccades. Results:The results show that covert saccades are associated with better performance on measures of dynamic visual acuity, gait, and balance in patients with unilateral vestibular hypofunction. Patients exhibiting overt saccades or combination of both covert and overt saccades were more often found to have an abnormal gait speed and be characterized as being at risk for falls using the Dynamic Gait Index. We observed no differences in physical function for those patients with bilateral vestibular hypofunction as a function of saccade grouping. Conclusions:When comparing saccade groups (covert, overt, or combination of both), patients with unilateral vestibular hypofunction and covert saccades demonstrated better performance on standard baseline physical therapy measures of dynamic visual acuity and gait and balance. We did not observe any significant associations between saccade group and physical function in patients with bilateral vestibular hypofunction; however, additional studies are needed with adequate sample sizes. Our findings may suggest that corrective saccade latency in patients with unilateral vestibular hypofunction is related to measures of physical function. The extent to which saccade latency has the potential to be a useful target for vestibular rehabilitation is still to be determined and may be promising target to improve functional outcomes.

Muscular Strength Training Modifies Regulation of Bone Remodeling: Inferences From Serum Biomarkers in Young Women

Biochemical markers of bone turnover allow inference of the events occurring at the bone tissue level and may detect changes in bone cell activity earlier than densitometric technologies. Serum concentrations of receptor activator for nuclear factor kappa-beta ligand (RANKL), osteoprotegerin (OPG), osteocalcin, and N-telopeptide (NTx) were measured in women aged 20 + 1.5 years (mean + SD) who underwent 32 weeks of unilateral isokinetic concentric or eccentric muscular strength training. Changes in serum biomarkers were compared with changes in arm and leg flexor and extensor muscle strength. Dual X-ray absorptiometry (DXA) measures of bone mineral density (BMD) and bone mineral content (BMC) of the total forearm, total tibia, and total body also were assessed. The mean serum OPG concentration increased from 4.6 + 1.9 pmol/L to 5.2 + 2.1 pmol/L (â 14.9 %, mean + SD; p = 0.05, n = 20) following long-term isokinetic exercise training that also increased elbow extensor and knee flexor muscular strength (p < 0.05) and total forearm BMD (p = 0.04). The ratio of OPG/RANKL also increased over the course of the study (p = 0.045). Serum concentrations of other measured bone biomarkers did not change during training. Serum concentrations of OPG, a suppressor of osteoclastogenesis, increased with high-load muscular strength training that led to local increases in muscle strength and BMD. These adaptations may represent an exercise-mediated suppression of osteoclast differentiation and activity. The central role of the RANKL-OPG cytokine system in the regulation of bone cell biology is well established. Further research is needed to confirm the efficacy of using serum OPG and RANKL as biomarkers of bone cell metabolism in healthy populations undergoing long-term exercise interventions. / Master of Science

Reconstruction of the alveolar process in cleft patients

Background. The treatment of patients born with cleft lip and palate has been gradually modified over the years as the surgical procedures have developed and improved. Multidisciplinary team care has evolved and provided improved care with enhanced results. Clefts in the alveolus can be reconstructed by alveolar bone grafting or by periosteoplasty. The main goal is to repair and close the alveolar cleft and create a continuous alveolar processes so that the teeth can erupt. Aims. This thesis has several aims: to investigate the impact of dental status and initial cleft width on the outcome of Secondary alveolar bone grafting (SABG) in patients born with unilateral cleft lip and palate (UCLP) at the 10-year follow-up (Studies I and II); to compare the outcomes of primary periosteoplasty (PPP) with those of SABG in patients born with unilateral cleft lip and alveolus (CLA) (Study III); to evaluate clinical and radiographic conditions and identify factors important for the final treatment outcomes after SABG ( Study IV); to evaluate two radiographic methods, i.e. occlusal radiographs and cone beam tomography (CBCT)) for assessing alveolar bone height ( study IV). Results. In UCLP patients, SABG achieved excellent results in terms of bone height; tended to reduce with time, correlated with dental status and dental restoration factors. Occlusal radiographs correspond well with the CBCT, for evaluating alveolar bone height in cleft area. The width of the initial cleft does not seem to affect the success of SABG. Finally, patients with CLA treated with PPP at the time of lip repair have inferior bone formation outcomes in the cleft area compared with patients treated with SABG at the time of mixed dentition. Conclusion.  Poor dental status and malpositioning negatively affect the long-term survival of bone in the alveolar cleft. The initial cleft width affects certain dental status factors. In adults with UCLP, the alveolar bone height in the cleft was correlated to the presence of gingival inflammation and restorations at 20 years follow-up. Specially designed maintenance therapy is beneficial, after complex dental restorations in the cleft area. SABG is preferred to PPP for the reconstruction of alveolar clefts.
