Vulnerable China¢ÎStudies¡XProfessor Jean-Pierre Cabestan and CEFC

Pai, Ting-Ting 13 July 2010 (has links)
The emergence of French Center for Research on Contemporary China (CEFC) in 1992, one of the 27 overseas research centers under the direction of French Ministry of Foreign Affairs, shows that the French government is eager to understand contemporary China. Because of the location of CEFC, it can provide a headquarter for researchers from France or European countries to spread their research activities and forms a local academic network, then let them exchange ideas with others. Then a new way of China studies with diversity in the research issues, methods and fields takes shape. This research introduces the history of CEFC and the distinguished features of its periodical ¡§China Perspectives¡¨ through the information from some manuscripts of interviews and the researches of the former director Jean-Pierre Cabestan as a representative to show the character of researches of CEFC. Comparing to some China studies in other dominant countries, e.g. USA, Japan and other European countries, CEFC evokes a Vulnerable China Studies that means a softer and more flexible way. Every researcher through CEFC can use their own way to go into China, discuss China and understand the latest changes in China. Then they can shape the characteristic research approach that differs from the past.

Diabetes Tele-Health Program for Hispanic Veterans: Program Evaluation

Eisenbise, David Alan January 2015 (has links)
Background and Rationale: Rural Hispanic Veterans in the Southern Arizona region with type 2 diabetes mellitus (T2DM) are at risk for diabetes related complications. The Southern Arizona Veterans Administration Health Care System (SAVAHCS) care coordination home tele-health (CCHT) program serves as a useful tool in T2DM management in addition to services through primary care. CCHT has not been evaluated to determine effectiveness in this uniquely vulnerable demographic - rural Hispanic Veterans with T2DM in the Southern Arizona region. Purpose: The purpose of this DNP project was to evaluate the effectiveness of a nurse managed and nurse ran program known as the SAVAHCS CCHT T2DM program in achieving successful diabetes self-management and related biological measures among male and female Hispanic veterans who reside in rural Arizona counties and receive their health care through SAVAHCS. Project Aims: Aim I: Evaluate the effects of the SAVAHCS CCHT T2DM program among rural Hispanic veterans on the biological measure of glycosylated hemoglobin A1C (HbA1C) at three different time periods, initially at admission, three months after admission, and six months after admission. Aim II: Evaluate the effects of the SAVAHCS CCHT T2DM program among rural Hispanic veterans on the other quality measures of diabetes management including Body Mass Index (BMI), blood pressure (BP) and low density lipoprotein (LDL) cholesterol at three different time periods (see Aim I). Methods: A descriptive study design was used to evaluate the SAVAHCS CCHT program. The Centers for Disease Control and Prevention (CDC) Framework for Program Evaluation in Public Health was used to examine the effectiveness of the tele-health program. Data were collected from the Veterans Health Administration data warehouse. Biological measures were collected and analyzed at three different time points. Results: Descriptive data analysis of the veterans (n=12) demonstrated minimal intervention improvement of 0.1 units on A1C (8.3% of veterans), BP (16.7%) and BMI (16.7%). Due to missing data the results may not be due to the CCHT program but to structure and process issues related to collection of reliable data. A more robust program evaluation is recommended.

Spanish Influenza in the City of Vancouver, British Columbia, 1918-1919

Buchanan, Sarah 21 September 2012 (has links)
During the last year of World War I (1918), a second deadly foe was causing mortality around the world. Spanish Influenza killed an estimated 50-100 million people worldwide, including 50,000 people in Canada during the 1918-1919 pandemic. This thesis examines the impact of Spanish Influenza on people living in Vancouver, British Columbia, Canada between June of 1918 and June of 1919. Statistical analysis with SPSS was used to determine the association between influenza-caused deaths and socio-demographic characteristics such as age, gender, immigration status, and employment. In Vancouver, those who were between the ages of 19 to 39, and those who were employed, showed higher odds of dying from influenza during the epidemic. / Graduate

Medicaid's Postpartum Tubal Sterilization Policy's Effect on Vulnerable Populations

Turner, Katherine 09 January 2015 (has links)
After the forced sterilizations of low-income and minority women were exposed in the 1970’s, new Medicaid policies were put into place in order to protect vulnerable populations. The revised policy included a mandatory consent form and a waiting period of 30 days between consent and procedure, as well as a presentation of the form at time of procedure. Although these policies were enacted to protect vulnerable populations, research has shown they are ineffective and act as barrier to women receiving the post-partum tubal sterilization that they desire. The policy has been shown to have a disproportionate detrimental impact on minority populations, and it has created a two-tiered health care system in terms of sterilization. The unfulfilled requests lead to many inadvertent consequences, including higher rates of unintended pregnancies, abortions, loss of self-efficacy, and higher costs for the Medicaid system. In order to ensure equitable treatment of Medicaid patients in regards to tubal sterilization, the 30-day waiting period should be rescinded. Additionally, to confirm that patients are fully knowledgeable of the implications of the tubal sterilization, the form and any ensuing consent should be rewritten to meet literacy standards for the target demographic. This analysis will include a history of the issue, an examination of relevant research, a policy analysis and recommendations to enhance healthcare equity.

Human trafficking and models of governing security

Varghese, Ashley Daniel January 2015 (has links)
This thesis seeks to address the complex forms assumed by the problem of human trafficking in the contemporary world and in particular to analyse and overcome certain identified shortcomings in existing models of intervention. It begins by locating the problem historically through its foundations in slavery, and explores the incomplete nature of the abolition of that institution. These observations provide the context for the emergence of contemporary forms of exploitation. The complexity of the problem of human trafficking is reflected in the variety of approaches to intervention thus far attempted and in the lack of consensus among stakeholders concerning the effectiveness of competing models. The tendency towards institutional failure in current approaches, and the consequent growth of the trafficking phenomenon are compounded by the ability of criminal networks to infiltrate and corrupt the very systems that are meant to safeguard vulnerable populations. This thesis argues that the shortcomings of existing models are themselves evidence that one of the foundational reasons for the emergence of human trafficking as a networked global crime is the security deficit created by the replacement of the structures of state with criminal networks embedded within communities. These observations are supported by empirical evidence in the form of i) a detailed review of records relating to a number of prosecutions and ii) first-hand observations and interviews with officials and civil society organisations in India, the United Kingdom and the Netherlands. I argue, in conclusion, that the evolution of models of intervention, leading to the creation of a preferred multidisciplinary model, show that this security deficit is best countered by emergent partnerships between the structures of the state, communities and civil society groups. Such innovations prefigure new hybrid structures, which offer a degree of hope for holistically tackling common global problems and networked crimes such as human trafficking.

Early detection and treatment strategies for vulnerable atherosclerotic plaques

Pham, Tuan A. 12 March 2016 (has links)
Atherosclerotic plaque ruptures have been determined as the most common underlying cause of acute coronary syndromes and stroke. Currently, the standard of care for plaque rupture risk is based on the amount of luminal stenosis presented in a particular vessel; however, X-ray angiographic studies have shown that plaques at risk of rupture generally show <50% luminal narrowing. These findings explicate the need for other, more accurate methods of identifying problem lesions prior to the rupture event. Unfortunately, the study of thrombotic events and vulnerable plaque lesions in humans is difficult due to the spontaneity of rupture and the lengthy time course of disease progression. To further the understanding of plaque rupture risk in light of vulnerability detection, a rabbit model of atherothrombosis was used in conjunction with magnetic resonance imaging (MRI). MRI has been validated as a suitable imaging modality for in vivo, non-invasive detection of atherosclerosis and has provided quantitative predictors of plaques at risk of rupture. Additionally, the rabbit model has been shown, histologically, to present 6 of the 8 human plaque types classified by the American Heart Association. The first portion of this dissertation work focuses on using MRI to serially image rabbits undergoing the atherosclerotic protocol in order to assess rupture risk at the various time points. Previous work has determined that an increase in the vessel remodeling ratio (which hides a large plaque in the vessel wall) and contrast uptake (which indicates inflammation) are both characteristics of increased rupture risk. By obtaining these parameters at various time points in the disease progression, it was possible to determine when a certain plaque displays a heightened risk of rupture. The second portion of this work tested the efficacy of a pro-resolving molecule, lipoxin (an endogenous molecule), in reducing atherosclerotic disease state, specifically rupture with a luminal thrombus. Using chronic administration of this molecule in the same rabbit model of atherosclerosis yielded a faint reduction in atherosclerotic severity based on the parameters of decreased vessel lipid content and decreased thrombotic events presented in the treated group.

Bajotto, Aletheia Peters January 2015 (has links)
Introduction: By the year 2025, Brazil will be the sixth country with the highest number of older adult worldwide. Bioethics discusses the vulnerability because the vulnerability exposes the subject to exploitation, but exploitation is morally wrong once it presupposes a relationship where one party is facing disadvantage. For the year 2050 it is estimated that this age group represents 20% of world population, approximately 2 billion people. Of these, over 60% will be living in developing countries. Goals: Search for the characterization of vulnerability in the context of aging, comparing groups of elderly people and adults in different situations of care in general hospital and community. Methods: The research is characterized as transversal, quantitative and qualitative (mixed method) when comparing three groups of older people (1 -outpatient care, 2 - inpatient unit, 3 - physically active) with 3 adult groups (4 - outpatient care, 5 - inpatient, 6 - physically active) assessed by the instruments quality of life (QOL), Moral Psychological Development (MPD) , functional capacity (FC), research coercion (RC) and assistance coercion (AC) . Qualitatively, we analyzed the speech of patients according Bardin, through a semi-open interview. Results: The sample was composed predominantly of women in all groups (p = 0.002). The level of education revealed higher in groups 3 and 6, and the group 3 enjoyed a level of even greater statement that the group 6. Both adults and elderly achieved the 5th level of MPD and the level of coercion in research showed significant difference in group 6 (p <0.001). The occupation when compared adults and the elderly showed a higher proportion of retired (59.7%) and 28.6% of adult unskilled workers (p <0.001). When the sample was compared in the three situations, showed a significant difference between in older people, with p=0.008. The functional capacity reveals independent participants mostly, and in the groups of patients (1,2,4 and 5) is where we find participants with a greater degree of dependence for ADL. The quality of life showed overall average of 64.91 ± 13.35 in older adults and 62.30 ± in 14.29 in adults, with no significant difference. The area with the highest average for seniors was the psychological domain and for adults, the social relationship. The domain "environment" show significant differences between the large group, with p = 0.002. By comparing elderly and adults considering the collection site, elderly practitioners of physical exercise and community adults had the highest average overall, with p <0.001 and p = 0.019, respectively, in the elderly and adults. Qualitative research take out from the speech of participants categories, whose four most significant categories were: 2 Health and disease-with 25% of participants; 8 Behavior- with 20%, 1 Autonomy - 17% and 7 Fragility - totaling 15% of the inferences. Conclusion: The environment and the health situation in which the sample was taken, seems to say more about the vulnerability of the age criterion: the vulnerability is more related to injuries that the human being experiences - that is dynamic. The participants express the vulnerability related to the disease or health, to a moral judgment, autonomy and frailty.

Incentivos e barreiras para a realizaÃÃo do teste de HIV entre homens que fazem sexo com homens / Incentives and barriers to HIV testing among men who have sex with men

EugÃnia Marques de Oliveira Melo 30 April 2012 (has links)
This work aims to explore the contexts of vulnerability that interfere on the (non) realization of the HIV test among men who have sex with other men. In a more specific way, it aims to explore which individual, social and programmatic factors may be established as encouragement and hurdles for the realization of the HIV test, outlining the vulnerability framework of the Men Who Have Sex with Men (MSM). It also explores how the recommendations of the Ministry of Health for the realization of the HIV test are extended and experienced by the MSM. The adopted theoretical reference was constructionist and the vulnerability framework was the reasoning used as a basis for the whole research. The study brings a general panorama of the aids epidemics among the MSM on a national and international level, as well as the international recommendations policy that have influenced the Brazilian responses to HIV/aids, and the building of national public policies that refer to the HIV test. This research has an exploratory character, qualitative approach, and used the methodology of Rapid Assessment, which has anthropological presuppositions. Forty semi-structured interviews were done with MSM who were at least 18 years of age, with both positive and negative serology for HIV, and who had never done the test, living in the city of Fortaleza. The interviews were done in many places around the city, aiming to get a wider sample of information, being a constant for the research only a NGO and a Service of Specialized Attention of Reference to HIV/aids. The information was analyzed through the technique of content analysis that was category-based. The results show that the reasons for the realization of the HIV test among the MSM were: they perceived they were at risk, a demand from the partner, self-care and fear of the disease, blood donation, encouragement from the healthcare professionals, prevention workshops and LGBT events. On the other hand, the barriers to the test were: poverty, lack of general knowledge about HIV prevention and transmission, prejudice against the non-heterosexual sexualities, stigma in relation to the homoerotic practices and to aids itself, homophobia, discrimination referring to aids and low or no perception of the risk. The pragmatic barriers were: lack of knowledge about the places, the procedure and reliable time to do the test, discredit for the test recommendation from the Ministry of Health, discredit for the efficiency of the quick test, homophobia, lack of preparation to deal with questions of the homoerotic sexuality and emotional reasons deriving from the positive results among the healthcare professionals, lack of and inefficient governmental campaigns for the stimulation to look for the diagnosis and lack of identification with the HIV/aids policies created for MSM when in everyday life there is no such identification. The results also show that these reasons have a direct impact on the non-following of the recommendations of testing, making the MSM population from Fortaleza vulnerable to the epidemics. Based on the wide concept of health, public policies must be elaborated taking into consideration the concept of integrality in such a way that this population can be assisted in their rights, which will have a consequence in the reduction of the vulnerability to the HIV/aids.
