Young adult smoking cessation: What predicts success?

Background: Across North America, smoking prevalence is highest among young adults (YAs). Understanding the cessation behaviours of YAs is critical given their higher smoking rates; however, there is a paucity of prospective studies on YA cessation from a population-based sample of smokers. Objectives: This study characterizes younger and older adult smokers as well as identifies the rate of making a quit attempt (QA) and smoking cessation among a representative sample of younger and older adult smokers. Guided by the Social Cognitive Theory (SCT), this study also examines the interpersonal predictors of moving toward smoking cessation among YAs. Methods: Self-report data on 592 YA and 2777 older adult smokers were compiled from the Ontario Tobacco Survey baseline and six-month follow-up interviews. Smoking cessation behaviour was measured as an ordinal variable: made no attempt to quit, made a QA that lasted for less than 30 days, and successfully quit for 30 days or longer. Design-based analyses examined the characteristics of young and older adult smokers. Making a QA and smoking cessation during a six-month follow-up period were modeled according to the SCT constructs while accounting for the complex study design. Results: Young adult smokers were more likely to be men with lower levels of addiction but greater self-efficacy and an intention to quit than their older counterparts. While YA smokers were more likely to make a QA than their older counterparts (25% vs. 17%, respectively), they were no more likely to succeed (14% vs. 10%, respectively). Having an intention to quit smoking and having made two or more lifetime number of QAs predicted making a QA; the use of smoking cessation aids or resources and having knowledge that stop smoking medications make quitting a lot easier also contributed to making an attempt to quit. Self-efficacy, use of smoking cessation aids or resources and having someone to support one‘s QA were positive predictors of quitting whereas having high levels of addiction was a negative predictor of cessation. Conclusions: Young and older adults are distinct types of smokers with different personal and smoking characteristics. Different factors predicted making a QA and smoking cessation among YAs. Smoking cessation interventions for YAs should provide social support and skills to build and maintain self-efficacy to quit. It is critical to ensure YA smokers have effective smoking cessation aids and services that are easily accessible and appropriate for this population. Future research is needed to understand long-term smoking cessation and relapse in this vulnerable population.

Exploring popular theatre in education: A participatory project in an alternative education program for pregnant teens and young mothers

To shift the emphasis with regards to what is important in education, beyond achievement tests, it would be helpful to consider other possibilities for engaging students. Popular theatre as form of popular education is explored as an option for providing this better way in. I considered both critical and feminist pedagogies while being guided by the following questions: What are the possibilities and limitations of a popular theatre project implemented within an alternative school context? Why should we make space for such processes in schools? Through a participatory study using popular theatre within an alternative program for young mothers and pregnant teens, ethnodramatic scenes are presented, depicting themes that arose within the program. There were successes and failures; however, what was most notable were the constraints of the school system that may not allow for such processes to exist within current educational structures. Suggestions for future projects are proposed.

A value proposition for lunar architectures utilizing on-orbit propellant refueling

In 2004, President Bush addressed the nation and presented NASA's new vision for space exploration. This vision included the completion of the International Space Station, the retirement of the Space Shuttle, the development of a new crew exploration vehicle, and the return of humans to the moon by 2020. NASA's Exploration Systems Architecture Study (ESAS) produced a transportation architecture for returning humans to the moon affordably and safely. This architecture requires the development of two new Shuttle-derived launch vehicles, an in-space transportation vehicle, a lunar descent and landing vehicle, and a crew exploration vehicle for human transportation. The development of an in-space propellant transfer capability could greatly improve the performance, cost, mission success, and mission extensibility of the overall lunar architecture, providing a more optimal solution for future exploration missions. The work done in this thesis will analyze how this new capability could affect the current NASA lunar architecture, and will outline the value proposition of propellant refueling to NASA. A value proposition for propellant refueling will be provided to establish why an architecture that utilizes propellant refueling is better equipped to meet the goals of the Vision for Space Exploration than the current baseline design. The primary goal addressed in this research is the development of a sustainable and affordable exploration program. The value proposition will outline various refueling strategies that can be used to improve each of the architecture Figures of Merit. These include a decrease in the Life Cycle Cost of both the lunar and Mars exploration campaigns, the ability to more than double the mission payload that can be delivered to the lunar surface during cargo missions, improving the probability of successfully completing each lunar mission, decreasing the uncertainty, and therefore risk, experienced during the development process, and improving the extensibility of the exploration architecture by utilizing a greater portion of the lunar program for future crewed mission. The ability to improve these Figures of Merit provides NASA with a more valuable architecture because NASA is able to achieve a greater return on its large initial investment.

What does it mean to be a global citizen? : A qualitative interview study with Indian and Nepalese young adults concerning their perceptions of global citizenship

Today's increasingly interconnected world creates new challenges related to the use and understanding of the concept of citizenship. The idea of a global citizenship is not new; however, in recent years there has been an evolution of increasing research leading to the expansion of interest with regard to the exploration of the concept and how it could be implemented in practice. This study attempts to deepen the understanding of the qualitatively different ways of perceiving this concept amongst a number of Indian and Nepalese young adults. The study is based on a phenomenographic research approach, where the data material was collected through semi-structured interviews. The results of the study show that global citizenship can be perceived as being related to equality, cultural diversity, global responsibility, global communication and cosmopolitan governance. Furthermore, the study demonstrates that the young adults perceive themselves either only as national citizens, or as both national and global citizens. Finally, the respondents believe that formal education can contribute to the promotion of global citizenship by including the concept in the educational curriculum and by encouraging cultural exchange. The role of formal education in promoting global citizenship is also perceived to be unclear due to terminological confusion.

Health-Related Quality of Life (HRQOL) is an important outcomes measure in cancer and there are specific issues depending on the site, stage, treatment and patient age. Although numerous instruments are available for cancer HRQOL, most are designed for adults, some for children, but none for adolescents and young adults (AYA) who have special age-specific concerns and poor improvement in survival compared with other age groups. An existing HRQOL instrument was modified to ensure its suitability for AYA, its validity, reliability and sensitivity were tested in Australians aged 16 to 25 years old diagnosed with cancer or a blood disorder. Varni’s PedsQLTM Measurement Model (13-18 year olds) was selected, modified then administered to families recruited from haematology/oncology clinics and wards at three Sydney Metropolitan Hospitals in person or by telephone. The Memorial Symptom Assessment Scale was used to categorise participants into groups reflecting sensitivity of symptom severity (slight, moderate and severe). The instruments demonstrated excellent internal consistency reliability, making them suitable for both group and individual comparisons. Clinical validity, construct validity, and discriminant validity were demonstrated by “known-groups” analysis, exploratory factor analysis and correlations, respectively. These new versions of the PedsQL Generic Core and Cancer Module are reliable, valid and sensitive measures of HRQOL in patients aged 16-25 years diagnosed with cancer or a blood disorder. The measures will soon be available for use as outcome measures in clinical trials and clinical practice with this age cohort in Australasia and internationally.

Rare and tragic: Young women diagnosed with advanced breast cancer; a discourse analysis

Recent research into advanced breast cancer has suggested that young women in general tend to have more aggressive disease, present at a later stage of disease progression and suffer many more issues and concerns than their older counterparts. Whilst breast cancer in women in general has been the target of a vast amount of research and public attention, values and beliefs surrounding advanced breast cancer have not been a focus of concern. The aim of this thesis is to explore scientific journals, the media and to listen to the young women themselves in order to identify the understandings of advanced breast cancer in young women and the ways in which these understandings are perpetuated and sustained over time. The goal is to illuminate the various discourses that are currently being drawn upon to understand this life-limiting illness and the impact these discourses have on the lives of young women concerned. Poststructuralism is the theoretical perspective within which this thesis is located. This approach allowed for a focus on language, power and text. Discourse analysis of three data sets was used. These data sets were drawn from scientific and medical journals (251), medical texts (5), clinical practice guidelines (2), newspaper articles (230) and transcribed conversations with 12 young women diagnosed with advanced breast cancer. The main discourses identified within and across the various data sets were; the discourse of numeracy, the discourse of tragedy and several discourses of the body; the thin body, the declining body, the object body and the gendered body. While the emphasis of each of these discourses varied across the three data sets, they were all present in each to some degree, reflecting broader cultural stories within which the individual stories are located. Young women diagnosed and living with advanced breast cancer are currently being portrayed as living with a tragic disease, controlled and constrained by the statistics and probabilities and played out within and on a body in ‘perpetual disintegration’. The discourses of tragedy, numeracy and the thin, object, gendered and declining body all relate to larger stories of what it is to be dying before one’s time in Western society today.

Moral development in early childhood

This study is of moral development in young children, with a focus on justice as fairness, a term appropriate to the life experience of three- and four-year-old children. The subjects were twenty-eight children attending two groups in a preschool centre (sessional kindergarten), eighteen female and ten male. The ages ranged from three years four months to four years eight months at the commencement of the study. The children were studied for one year at their kindergarten. Five problem areas were explored: (1) moral reasoning; (2) behaviour in peer conflict interactions; (3) relationship between moral reasoning in response to hypothetical dilemmas and behaviour in real-life conflicts;(4) construction of an instrument to describe and measure early moral reasoning and behaviour; (5) moral education within a preschool setting. Semi-structured interviews using Kohlberg-type dilemmas, and naturalistic observations of spontaneous social interactions in free play situations, were the methods used. Pretest and post-test interviews and observations were six months apart. Teachers of one group of children participated in a moral education project which included a process for using naturally-occurring peer conflict in the kindergarten to facilitate moral development.

Holding your breath: predictive genetic testing in young people

A clash in perception is taking place. Some perceive predictive genetic testing in young people to be too potentially harmful to allow. Others perceive it to be an opportunity for benefit, even an opportunity for the prevention of harm. In this thesis I consider the issue of potential harm to mature young people who seek predictive genetic tests. / There are two parts to this thesis. In part one (chapters 1-4) I provide a background to the current debate. I describe the prohibitive stance purported within current guidelines, the arguments used to justify this stance and the opposition that has arisen in response. I discuss the psychological and social ways in which young people differ from adults, arguing that it is likely young people will react differently from adults in response to predictive genetic tests. However, I conclude that the lack of empirical evidence means we are unable to determine if these differences will confer a greater potential for harm or benefit when young people are tested. Finally, I present a discussion of two fundamental gaps in our knowledge about testing in young people: a lack of knowledge about current practice and a lack of first-hand evidence about the effects of testing. I argue that empirical research is required. / In part two of this thesis (chapters 5-7) I present the findings of my own empirical research. Firstly, I describe the findings of an international survey of clinical geneticists. Secondly, I describe the outcomes of 18 in-depth interviews performed with young people who have experienced predictive genetic testing for either Familial Adenomatous Polyposis or Huntington Disease. These young people ranged in age from 14 to 25 years. / The international survey uncovered 49 cases where predictive genetic tests had been provided to young people for non-medical reasons. When such tests are provided, the impacts are rarely followed-up as part of a formal research protocol. Clinicians’ reasons for providing and refusing tests are highly varied and are driven more by the nuances of individual cases than by any one ethical principle or set of guidelines. / When young people talk about the predictive genetic tests they have experienced, they refer to the entire experience of being at risk of a genetic condition, not simply the time after receipt of their test result. Young people speak about a far more extensive range of harms and benefits associated with the testing process than have been previously researched. / I argue that some young people growing up at risk of a genetic condition suffer several harms prior to their request for predictive genetic testing, because of their risk status. I argue that when we understand this, it becomes clear that for these mature young people who seek such testing, the provision of a test may not only serve to alleviate some of these harms, but may in fact create benefits for them, irrespective of their test result. In these cases, the provision of a predictive genetic test is appropriate, logical and ethical.
