244424.pdf: 2891840 bytes, checksum: 2382847c5088cd044143a2890f83faf5 (MD5) / Este estudo teve por objetivo compreender as implicações do processo de trabalho no sofrimento psíquico, nos danos à saúde e na capacidade de resiliência das equipes de saúde que atuam junto a crianças gravemente enfermas e / ou com risco de vida em hospitais públicos. Assim, buscou reconhecer os fatores que podem levar ao adoecimento do profissional de saúde a partir da organização de seu trabalho, na relação com a instituição, com a equipe, com a criança e seus familiares; e os recursos de saúde por ele utilizados com vistas a construção individual e coletiva de estratégias de enfrentamento. Para constituir o objeto de análise, em sua complexidade e em suas fronteiras com diferentes conhecimentos, foram articuladas idéias e pressupostos do campo da psicologia e da saúde, que compuseram a base teórica, subdividida em quatro tópicos: sobre o ato de conhecer e a produção de um saber sobre a saúde e a doença; sobre o trabalhar com o sofrimento e a doença - processo de trabalho, a instituição e o profissional de saúde; sobre a criança e seu adoecer, a hospitalização e a iminência da morte. A investigação desenvolveu-se em um hospital público de referência no sul do país, envolvendo vinte profissionais que compõem equipes de saúde de um ambulatório e duas enfermarias com atendimento na especialidade em estudo. A pesquisa foi de natureza qualitativa com delineamento do tipo estudo de caso, com foco na equipe de saúde em sua diversidade e especificidades. A coleta de dados se deu sob o vértice da observação participante # com acompanhamento dos profissionais em suas atividades diárias nas unidades de atendimento, consultas, atendimentos ao leito, procedimentos e reuniões de equipe além de situações do cotidiano em vários espaços da instituição hospitalar -; entrevistas semi-estruturadas de profissionais, considerados pelo critério de representatividade das categorias que compõem as equipes e de profissionais em cargos de direção e chefias; e estudo documental. Os dados coletados foram centralizados em três categorias principais: a instituição hospitalar e os aspectos subjetivos deste lugar; o sofrimento institucional, os limites da saúde no processo de adoecimento do trabalhador de saúde, o significado que lhes conferem e como, nesse cenário, a resiliência se constitui; e, por fim, as articulações finais acerca de todas as escutas, refletindo-se para onde nos levam e quais são seus limites e possibilidades. Os resultados desse estudo apontaram na direção de um insidioso processo de sofrimento e adoecimento dos trabalhadores de saúde frente ao ato de cuidar de crianças e adolescentes gravemente enfermos ou com risco de morte, em uma conjuntura que evidencia aspectos do trabalho como a alienação e a fragmentação, somando-se às características do cenário político institucional no qual ele se desenvolve. As implicações de enfrentamentos constantes nesse contexto de trabalho contribui para o descrédito na sua capacidade de trabalhar criativamente com a equipe e instituição, enfraquecendo a capacidade de resiliência e o desenvolvimento de estratégias defensivas de saúde na busca de alternativas de superação aos impasses colocados no seu exercício profissional, no âmbito das políticas de saúde, bem como da organização do trabalho.
This application has as a goal to know the envolment of the process of psychic work of suffering, in the damage that it brings to health and in the capacity of the health teams that act together with serious sick children , and or be in danger of death in public hospitals. In this way seek the factors that can take the health professional to be sick, from organizing his work, his relationship with the institution, with the team, with the child and its relatives and the resources of health that were used by him to build individual and general strattegies of facing it. To build the object of analysis, inside its complexity and on the boarders with different knowlegment, were articulate pressuposed from psychology and health areas that composed the theorical basis, subdivided in four topics: about the act of knowing and the production of health knowlege and the disease, about working with suffer and the disease, working process, the institution and the health professional, about the child and how he or she get sick, to be in a hospital and the eminence of death. The investigation was developed in a public hospital that is a reference in the south of the country, envolving twenty professionals of the health area from an ambulatory and two emercency places with attending inthe especiality application. The research was of qualitative nature with outlined by studying the case, focusing on the health team, its diversity and specifities. The data gathering was given by participant observation, with attendance of the professionals in their daily activities at their workplaces attends, consultation, attendences in berths, procedures and meetings of the team, beyond the daily situations in various spaces of the hospital, interviews non-structured by professionals, considered by the criteria of representative of the categories that compound the team and from professional function management and leadership, and ducumental application. The cataloged data were focused in three main categories: the hospital institution and the subjective aspects of this place, the institutional suffering, the limits of health in the process of getting sick of the health worker, the meaning that refers to them,in this scenery,the resilience is constituend, and finaly all the articulations of all hearings, reflecting where do they take us and which are their limitis and possibilities. The results of this application bet on the direction of an insidious process of suffering and getting sick of health workers facing the act of taking care of sick and damage children and teenagers or that are in danger of death, in a conjecture that makes evidence work aspects, with the alienation and grinding adding to the characteristics of the institutional political scenery in which it has been developed. The facing constant implications in this work context helped in descredit in his capacity in working creatively with the team and the institution weaken the resilience capacity and the development of defensive health strategies looking for alternatives to overcome the deadlocks in his professional job in the range of the health policies, as well as the work organization.
