INTRODUÇÃO: O Comprometimento Cognitivo Leve amnéstico (CCL-A) frequentemente representa a zona de transição entre o envelhecimento saudável e a demência, sendo considerado um problema de saúde pública, sobretudo devido ao crescimento da população idosa. Assim, é crucial o desenvolvimento de tratamentos eficazes que possam minimizar os déficits cognitivos presentes, tal como o treino de estratégia de memória (TEM). Destaca-se o TEM com foco na associação entre faces e nomes, alvo relevante, uma vez que esquecer nomes é uma queixa frequente em idosos com CCL-A. Entretanto, até o momento há pouca evidência quanto à eficácia dessa modalidade de intervenção a essa população, assim como à generalização dos possíveis efeitos, e o correlato neural subjacente. OBJETIVO: Investigar a eficácia do TEM com foco na associação face-nome e o correlato neural através da ressonância magnética funcional (RMf), em comparação a um grupo controle ativo (Psicoeducação - PED), em idosos com CCL-A. MÉTODOS: Trinta idosos com CCL-A foram randomizados em dois grupos: Grupo Experimental (GE, N=15), submetido ao TEM, e o Grupo Controle (GC, N=15), submetido a PED. Em cada programa, os participantes completaram quatro sessões de uma hora cada, duas vezes por semana. Os participantes foram avaliados nos momentos pré e pós intervenção, através de testes neuropsicológicos, tarefas de associação face-nome, escalas de humor e metamemória, além de uma tarefa na RMf. Ademais, no momento préintervenção, os participantes realizaram a coleta do líquido cefalorraquidiano; e, após a intervenção, responderam a um questionário de avaliação dos programas. A fim de investigar a manutenção dos efeitos ao longo do tempo, os voluntários foram reavaliados após um mês e após três meses. RESULTADOS: Não houve diferenças significativas entre ambos os grupos quanto ao perfil clínico, neuropsicológico e biomarcadores liquóricos. Quanto às mudanças após as intervenções, o GE apresentou melhor desempenho que o GC na tarefa de associação face-nome para o material treinado, o que generalizou em parte para o material não-treinado, com benefícios persistindo após três meses. No que se refere à generalização para as medidas neuropsicológicas, o GE exibiu melhora em teste de memória visual, porém não houve mudanças em testes de memória verbal e de atenção após ambos os programas. Além disto, em relação ao GC, o GE reportou uso mais frequente de estratégia associativa, e menor frequência de falhas de memória no cotidiano. Após os programas, ambos os grupos reportaram sentimentos mais positivos quanto à própria memória e avaliaram positivamente as intervenções. Ademais, não foram observadas mudanças em sintomas de depressão e ansiedade após os programas. Os achados de neuroimagem indicam que após as intervenções o GE mostrou maior ativação cerebral que o GC em regiões parietais (pré-cuneo, cíngulo posterior e giro angular bilaterais) e frontais (giro frontal medial à esquerda, giros frontal médio e pré-central à direita) ao codificarem o material treinado. Quanto ao material não-treinado, o GE mostrou maior ativação cebrebral que o GC em região anterior temporal (sulco temporal superior, giro temporal superior e médio). CONCLUSÃO: Os benefícios do TEM são observáveis tanto nos estímulos treinados, como transferem, em parte, para estímulos não-treinados e testes neuropsicológicos padronizados. Além disto, após o TEM, houve aumento no uso de estratégias associativas e ganhos de metamemória. Embora o GE tenha reportado menos falhas de memória que o GC, este aspecto não parece associado ao humor, uma vez que não ocorreram mudanças em sintomas de depressão e ansiedade após os programas. Os dados de neuroimagem corroboram os achados comportamentais sugerindo que o cérebro de idosos com CCL-A permanece altamente plástico. Thus, it is critical to develop effective treatments that can minimize the cognitive deficits, such as mnemonic strategy training (MST). We highlight the MST focused on face-name associations, a relevant target since forgetting names is a frequent complaint in elders with MCI. However, at the moment there is little evidence of the efficacy of this modality of MST to this population, as well as, its transfer effects and underlying neural correlate. OBJECTIVE: Investigate the efficacy of MST focused on face-name associations, and the underlying neural correlate, in comparison with an active control group (Psychoeducation - PED) in elders with A-MCI. METOHDS: Thirty elders with A-MCI were randomized in two groups: Experimental Group (EG, N=15), underwent to MST, and the Control Group (CG, N=15), underwent to PED. In each program, the participants completed four individual one-hour sessions, twice a week. The participants were assessed at pre and post intervention through neuropsychological tests, face-name association tasks, mood and metamemory scales, beside a fMRI task. Moreover, at pre-intervention, the participants performed the cerebral spinal fluid (CSF) puncture; and at post-intervention, answered a questionnaire to evaluate the programs. In order to investigate the maintenance of the effects over the time, the volunteers were reassessed after 1 and 3-months. RESULTS: There was no significant differente between groups regarding the clinical and neuropsychological profiles, as well as, the CSF biomarkers. With respect to the changes after intervention, the EG showed a better performance than CG in the face-name association task to the trained material, and transfered, in part, to the untrained material, with the benefits persisting after three months. Regarding the transfer effects to the neuropsychological measures, the EG exhibited an improvement in the visual memory test, however no changes were observed in the verbal memory and attention tests after both programs. In comparison to CG, the EG reported a more frequent use of associative strategy, and less frequent memory mistakes. After the programs, both groups reported more positive feelings regarding their own memory and evaluated positively the interventions. In addition, there was no change in depression and anxiety symptoms after the programs. The neuroimaging findings indicate that after the interventions the EG showed greater cerebral activation than CG in parietal (bilateral precuneus, posterior cingulare and angular gyru), and frontal regions (left medial frontal gyru, right precentral and middle frontal gyrus), when enconding the trained material. For the untrained material, the EG showed greater brain activation than CG in an anterior temporal region (superior temporal sulcus, superior and middle temporal gyru). CONCLUSION: The MST benefits were observed in the trained and transfer, in part, to the untrained stimuli and neuropsychological tests Moreover, after the MST, there was an increase of associative strategy use and gains in metamemory. L’objet de ce travail est de mieux comprendre les modifications cognitives et cérébrales liées à la sclérose en plaques (SEP) et les facteurs pouvant les affecter afin de contribuer à améliorer la prise en charge cognitive des personnes vivant avec une SEP. Nous avons orienté nos travaux selon 2 axes. Le premier visait à optimiser les évaluations cognitives. Nous avons montré une diminution de la qualité de vie (QdV) liée à l’atteinte cognitive et à son retentissement sur le statut professionnel, ainsi que l’intérêt d’utiliser l’Echelle d’Humeur Dépressive version auto-questionnaire que nous avons validée et dont la dimension « perte de contrôle émotionnel » était particulièrement associée à la QdV. Nous avons aussi élaboré des tâches « écologiques » en environnement virtuel et montré leur potentiel à évaluer plusieurs composantes cognitives, voire leurs interactions, suggérant leur intérêt pour évaluer l’impact des troubles cognitifs dans les activités quotidiennes. Le second, pour lequel nous avons présenté les bases théoriques, la méthodologie et les résultats préliminaires, consistait à améliorer la prise en charge cognitive grâce à son évaluation et à la réhabilitation (étude REACTIV, en cours). Nous avons créé des paradigmes en IRMf et un programme de réhabilitation spécifique visant à guider les phénomènes de plasticité cérébrale afin d’optimiser le transfert des acquis dans la vie quotidienne et améliorer la QdV. Lors des résultats préliminaires, nous avons identifié en imagerie multimodale des mesures IRM de deux structures prédictives de la progression de certaines performances cognitives au cours de nos thérapies : le thalamus et le corps calleux. / The purpose of this work is to better understand the cognitive and brain changes associated with multiple sclerosis and the influencing factors to help improve cognitive support for people living with MS. We organized our work according to two axes. The first one was to optimize cognitive assessments. We have shown i) a decrease in quality of life (QoL) linked to cognitive impairment and its impact on the professional status and ii) interest in the use of Depressed Mood Scale self-questionnaire that we validated and whose lack of emotional control dimension was particularly associated with QoL. We also developed ecological tasks in virtual environment and showed their potential to evaluate several cognitive components or their interactions, suggesting an interest in evaluating the functional impact of cognitive impairment in daily activities of PwMS. The second, for which we presented the theoretical basis, methodology and preliminary results, was to improve cognitive care through its assessment and rehabilitation (REACTIV study in progress). We created fMRI paradigms and a specific rehabilitation program to guide brain plasticity phenomena in order to optimize the transfer of training in daily life and improve QoL. Preliminary results in multimodal imaging revealed MRI measures of two structures, the thalamus and the corpus callosum, which would be predictive of progression of certain cognitive performance during our therapies.

Conception et Validation Expérimentale d’un Assistant Numérique pour l’Inclusion Scolaire d’Enfants avec Troubles du Spectre Autistique en Classe Ordinaire / Design and Experimental Validation of a Technological Assistant for School Inclusion of Children with Autism Spectrum Disorders in Mainstream Classrooms

Bien que reconnue comme critique pour le devenir socio-professionnel desenfants avec Troubles du Spectre Autistique (TSA), l’inclusion scolaire en classeordinaire demeure en France peu acces- sible pour ce public. En effet, lefonctionnement cognitif atypique associé aux limitations des comportements socioadaptatifs(communication, socialisation, autonomie etc.), se heurte bien souvent auxconditions normées des milieux ordinaires tels que l’école. Les nouvelles technologiessont aujourd’hui pressenties comme leviers prometteurs pour surmonter ces barrières àl’inclusion scolaire. Cependant, malgré un marché pléthorique de technologies ciblantles TSA, les études scientifiques manquent pour statuer sur leur efficacité mais aussisur les fondements mêmes de leur conception.Ce travail présente la conception et la validation d’applications mobiles pourl’inclusion scolaire d’enfants avec TSA en classe ordinaire au travers de trois études.Dans une approche centrée- utilisateur, l’Étude 1 présente des principes de conceptiond’applications d’assistance aux activités de classe et activités communicationnelles desenfants avec TSA pour une utilisation in situ. Dans une approche centrée utilisateur etde recherche clinique pilote, l’Étude 2 présente les principes de conception et lavalidation expérimentale d’une application d’assistance à la régulation émotionnelledes enfants TSA en classe ordinaire. Les résultats indiquent des bénéfices sur lescomportements d’auto-régulation ainsi que sur les processus sociocognitifs sousjacents.Enfin, dans une approche cross-syndromes, l’Étude 3 présente les résultatsd’une intervention globale reposant sur des ap- plications d’assistance et deremédiation cognitives (dispositif Collège+) déployées auprès de 48 enfants et visant àsoutenir la primo-inclusion en classe ordinaire d’enfants avec TSA et d’enfants nonTSA (avec Déficiences intellectuelles ou troubles globaux de l’apprentissage). Desbénéfices sont rapportés pour tous les enfants équipés en termes de comportementssocio-adaptatifs, de réponse sociale et de fonctionnement sociocognitif. Aussi, de pluslarges bénéfices sont observés pour les enfants TSA révélant ainsi la pertinence del’intervention Collège+ pour le public avec TSA.En conclusion, un approche systémique dans la conception et l’expérimentationd’applications mobiles a permis des améliorations dans l’adaptation descomportements et du fonctionnement socio-cognitif, cruciaux dans la réussite d’uneinclusion scolaire en classe ordinaire. Cette approche semble donc prometteuse poursoutenir l’inclusion scolaire en milieu ordinaire des enfants avec TSA, et offre delarges perspectives de travail, tant sur l’enrichissement des contenus, la conception denouvelles applications que des méthodes de validation expérimentale. / School inclusion of children with Autism Spectrum Disorders (ASD) inmainstream classrooms remains dramatically limited in France, even though it hasbeen recognized as critical for socio- professional perspectives. In fact, the atypicalcognitive functioning, associated with socio-adaptive behavior difficulties(communication, social skills, autonomy, etc.), are usually confronted to nor- malizedexpectations in these mainstream environments, such as schools. New technologiescan be seen as promising levers to overcome the barriers of school inclusion.However, despite a plethoric offer of technologies for children with ASD, scientificstudies are lacking to establish their efficacy, as well as the relevance of their design.This work presents the design and validation of mobile applications to support schoolinclusion of children with ASD in mainstream classrooms through three studies. Thefirst study presents design principles for assistive applications addressing schoolroutines and verbal communication activities of children with ASD; these applicationsare to be used in situ. Combining a user-centered approach and pilot clinical research,the second study presents design principles and experimental validation of an emotionregulation application targeting children with ASD in mainstream classroom. Theresults reveal benefits on self-regulation behaviors, as well as underpinning sociocognitiveprocesses. Finally, in across-syndrome approach, the third study presents theresults of a global intervention, based on cognitive assistive and rehabilitationapplications, involving 48 children and supporting the first inclusion in mainstreamclassrooms of children with ASD and children without ASD (with IntellectualDisabilities or learning disabilities). Benefits are reported for both equipped groups interms of socio-adaptive behaviors, social response and socio-cognitive functioning.Larger benefits have been observed for equipped children with ASD, revealing therelevance of Collège+ intervention for this population.A systemic approach to designing and experimenting mobile applications allowed forimprove- ments in socio-adaptive behaviors and socio-cognitive functioning, crucialfor the success of main- stream school inclusion. Such approach seems promising tosupport school inclusion of children with ASD in mainstream classrooms, and offersbroad perspectives by enriching contents, designing new applications as well asexperimenting validation methodologies for mainstream environments.

Volet clinique de la conception et de l’évaluation d’une technologie d’assistance à la préparation de repas conçue avec et pour des personnes ayant subi un Traumatisme CranioCérébral (TCC) grave

Introduction : Au Canada, 100 000 personnes sont victimes d'un traumatisme craniocérébral (TCC) annuellement. Les 16-24 ans présentent l’un des taux d’incidence les plus élevés, ce qui signifie qu’une grande partie des victimes et ces personnes vivront en moyenne 50 ans avec les séquelles physiques et cognitives du TCC. De plus, 10% des personnes qui ont subi un traumatisme crânien souffriront des séquelles d’un TCC grave leur causant d’importantes difficultés de fonctionnement, particulièrement au niveau des activités plus complexes de la vie quotidienne, comme la préparation de repas. Les technologies d’assistance à la cognition (TAC) ont démontré leur pertinence pour faciliter le fonctionnement dans leurs habitudes de vie des personnes vivant avec les séquelles d’un TCC. Toutefois, aucune TAC spécifique à la préparation de repas et à cette clientèle n’est actuellement disponible. Méthodologie : La présente thèse présente le volet clinique de la conception d’une technologie d’assistance à la préparation des repas, conception réalisée dans le cadre d’un projet interdisciplinaire joignant les sciences de la réadaptation et de l’informatique. Cette technologie d’assistance nommée COOK (Cognitive Orthosis for coOKing) a été développée avec et pour des personnes qui ont subi un TCC grave vivant en résidence spécialisée. Une approche de conception centrée sur l’utilisateur a d’ailleurs été retenue et organise le projet en trois grandes étapes : 1- l’analyse des besoins 2- le design de la TAC et 3- l’évaluation de ses effets. Une analyse des besoins a d’abord été réalisée auprès des futurs utilisateurs ainsi qu’auprès des acteurs clés afin de dresser le profil des futurs utilisateurs, d’identifier les interventions pertinentes pour optimiser leur fonctionnement et pour ensuite traduire ces interventions en exigences cliniques pour faciliter le design. La phase de design a permis de développer une technologie basée sur les données probantes en réadaptation cognitive et répondant aux besoins spécifiques des futurs utilisateurs. Ces derniers ont d’ailleurs collaboré avec l’équipe tout au long de cette phase. COOK a ensuite été implanté chez les trois participants afin d’évaluer les effets de son utilisation à court et long terme (1, 3 et 6 mois post-implantation) et d’améliorer son utilisabilité. Résultats : Grâce à l’analyse des besoins, trois profils de futurs utilisateurs ont été dressés et les meilleures pratiques en réadaptation cognitive pour répondre à leurs besoins spécifiques ont été identifiées, facilitant ainsi la conception interdisciplinaire de COOK. Avec l’aide de COOK et d’interventions cliniques complémentaires, les trois participants ont tous repris la préparation des repas de façon sécuritaire. De plus, COOK a démontré une efficacité intéressante et les participants étaient satisfaits de la technologie, sauf auprès d’un participant qui ne voyait pas l’utilité pour lui. Conclusion : Cette technologie semble donc prometteuse pour la réadaptation et le maintien à domicile de clientèles présentant des incapacités cognitives. D’autres travaux seront nécessaires afin d’adapter cette technologie à d’autres clientèles et différents milieux de vie. / Introduction: In Canada, 100,000 people suffer from traumatic brain injury (TBI) each year. The incidence is highest in individuals between 16-24 years of age, which means that people living with TBI will live an average of 50 years with the physical and cognitive sequelae. Of these individuals, 5% will have sustained a severe TBI, which will cause significant difficulties in their functioning, particularly in complex daily activities such as meal preparation. Assistive Technology for Cognition (ATC) has been shown to have high potential to facilitate the functioning of people with TBI. However, no ATC for meal preparation is currently available or suitable for this clientele. Methodology: As part of an interdisciplinary project, combining the disciplines of rehabilitation and computer sciences, COOK (Cognitive Orthosis for coOKing), an assistive technology to support meal preparation, was designed with and for people with severe TBI. This thesis presents the clinical aspect of the conception. For the design phase, a user-centered design methodology was chosen and organized into 3 main steps: 1- ATC needs analysis 2-design, and 3- evaluation of usability. As a result, a needs analysis was first conducted with future users and key stakeholders (3 future users, their relatives, the staff of the living environment and their healthcare professionals, and key external stakeholders). The design addressed the needs of residents with evidence-based practice guidelines in the cognitive rehabilitation field and these were translated into technological features. Future users were constantly consulted throughout the design process. Next, COOK was implemented with 3 future users in order to evaluate and improve its usability (1-, 3- and 6-months post-implementation). In addition, COOK demonstrated interesting efficiency and participants were satisfied with the technology, except for one participant who did not see how COOK could be of use for him. Results: The 3 participants with severe TBI have all resumed safe meal preparation with COOK. This technology has high potential for rehabilitation and home care for clients with cognitive impairments. Further work will be necessary to adapt this technology to other clienteles and different living environments.

[pt] Prejuízos cognitivos persistentes foram descritos em pacientes pós-COVID-19 independentemente da severidade da infecção, indicando que mesmo pacientes com infecção de grau leve ou moderado podem apresentar complicações cognitivas contínuas. Estratégias não-farmacológicas para reabilitação cognitiva ainda não foram revisadas de maneira sistemática neste grupo de pacientes. Dessa forma, o objetivo do presente estudo foi sistematicamente identificar, resumir e avaliar estudos de intervenção não-farmacológica avaliando desfechos cognitivos em pacientes com prejuízos neuropsicológicos persistentes após casos de COVID-19. Uma busca sistemática por artigos foi conduzida no dia primeiro de Julho de 2022. No total, 833 artigos foram identificados, dos quais 13 estudos foram incluídos na revisão final. Dentre esses, 7 eram estudos de grupo e 6 estudos de caso de sujeito único. Os estudos de grupo incluíram entre 12 e 50 participantes, e 2 destes também incluíram grupos controle. A maior parte dos estudos (11/13) testou a efetividade de intervenções multimodais de reabilitação, em geral combinando exercícios físicos com estimulação cognitiva. Dentre outros protocolos, reabilitação respiratória, ocupacional, psicossocial e fonoaudiológica também foram investigados. Com exceção de dois artigos, todos estudos descreveram resultados positivos após os processos de intervenção, incluindo aumento em escores de testes neuropsicológicos, redução em percepção de fadiga cognitiva e aumento de funcionalidade em atividades da vida diária. No entanto, as diversas limitações da presente literatura indicam que tais resultados devem ser interpretados com cautela. / [en] Persistent cognitive impairment has been described in people after COVID-19 irrespective of infection severity, indicating that patients with mild to moderate infection may also experience persevering cognitive problems. Non-pharmacological strategies for cognitive rehabilitation and their outcomes in this group have so far not been systematically summarized. Therefore, the aim of this study was to systematically identify, summarize, and appraise non-pharmacological rehabilitation intervention studies assessing neuropsychological outcomes in people with cognitive impairment after COVID-19. On July 1st, 2022, a systematic search was conducted using PubMed, Web of Science, PsycNET, and CENTRAL. In total, 833 papers were retrieved, of which 13 were included. Among those, 7 were group studies and 6 were single-subject case reports. The group studies included between 12 and 50 participants, of which two studies also included a control group. Most of the studies (11/13) tested multimodal interventions, mainly combining physical exercise with cognitive stimulation. Respiratory rehabilitation, occupational and speech-language therapy, EEG neurofeedback, tDCS, and psychosocial interventions were also investigated. With the exception of two, all studies described positive post-intervention results, including increased cognitive performance, reduced cognitive fatigue, and improved physical functionality. However, limitations of the current literature suggest that results should be considered carefully.

Évaluation de l’efficacité du Programme d’enrichissement cognitif auprès de personnes ayant subi un traumatisme craniocérébral à un âge avancé

Le vieillissement normal comporte une diminution légère des fonctions cognitives supérieures qui n’entrave pas le fonctionnement quotidien de la personne. Cependant, ce déclin peut rendre la personne âgée plus vulnérable si celle-ci subit une atteinte cérébrale acquise, tel un traumatisme craniocérébral (TCC). Devant le vieillissement démographique de la population, l’intérêt pour l’étude du vieillissement cérébral normal et pathologique a significativement augmenté au cours des dernières années. Malgré ce fait, on constate une absence de programmes validés de réadaptation cognitive pour les personnes âgées qui subissent un TCC, alors que la cause la plus fréquente de TCC chez la personne âgée sont les chutes de sa propre hauteur, dont la fréquence augmente avec l’âge. Ces faits permettent de présager une augmentation significative de cas de personnes âgées qui subissent un TCC, ce qui représentera un fardeau additionnel pour le déjà surchargé système de santé. Le présent projet de recherche clinique visait à compenser une telle carence en évaluant l’efficacité d’un programme d’entrainement cognitif, le Programme d’enrichissement cognitif (PEC), spécifiquement développé par l’auteur de cette thèse pour les personnes qui ont subi un TCC à l’âge de 55 ans et plus. Le PEC est composé de trois modules intégrant des méthodes de réadaptation qui visent des processus cognitifs fréquemment atteints tant chez la personne âgée que chez les personnes ayant subi un TCC : conscience de soi, mémoire épisodique et fonctions exécutives. Deux études de l’efficacité du PEC, effectuées par le biais d’un devis contrôlé semi-randomisé à simple aveugle, sont présentées dans la présente thèse. La première étude visait à démontrer l’efficacité du PEC pour améliorer la mémoire épisodique. Le deuxième module du PEC, composé d’exercices d’entrainement mnémonique tirés du Programme MEMO, vise tout particulièrement cette dimension mnésique. Le Programme MEMO a été originalement conçu et démontré comme étant efficace pour les personnes âgées présentant un déclin cognitif léger (ou mild cognitive impairment). Il s’agit de la première étude impliquant le Programme MEMO auprès de personnes ayant subi un TCC. Une ANCOVA a mis en évidence des améliorations significatives sur une mesure psychométrique de mémoire épisodique et d’une mesure auto-rapportée de généralisation dans la vie quotidienne des participants du groupe expérimental. De plus, une amélioration cliniquement significative a été constatée au niveau de leur perception de bien-être psychologique. La deuxième étude évaluait l’efficacité du PEC pour le fonctionnement exécutif des participants, tel que mesuré par des instruments psychométriques évaluant des composantes exécutives, ainsi que des mesures de généralisation à la vie quotidienne. Les résultats ont montré une amélioration significative dans le groupe expérimental pour des sous-tâches d’une épreuve de fonctionnement exécutif, ainsi qu’une réduction significative des différences entre les participants au PEC et leurs proches sur une échelle de fonctionnement exécutif dans la vie quotidienne. De plus, le nombre d’activités quotidiennes abandonnées était significativement réduit 6 mois plus tard dans le groupe expérimental. L’intégration de programmes de réadaptation cognitive comme le PEC dans les services de réadaptation nous paraît tout à fait nécessaire et hautement recommandée, tout comme la formation de neuropsychologues en méthodes et stratégies de réadaptation cognitive et neuropsychologiques afin d’améliorer significativement l’offre de services à la population vieillissante et générale. / Aging is a natural process that includes decline in higher cognitive functions. This decline doesn’t impede daily functioning but may render a person more vulnerable if they acquire a traumatic bran injury (TBI). The world’s demographic aging has motivated a rise of the interest for the study of normal and neuropathological cognitive decline. Despite this fact, there is a gap in terms of validated neurocognitive training programs specifically conceived for ageing persons who sustain a TBI, even though the main causes of TBI are falls from one’s own height, which become more frequent with age. These facts point to an expected rise of TBI among older people, creating a burden on the already overloaded public health system. This clinical research project aimed to compensate this gap by evaluating the efficacy of a cognitive training program specifically developed by the author of this thesis for people who suffered a TBI at the age of 55 years old or more. This program is named Cognitive Enrichment Program (CEP). The CEP is composed of three modules integrating rehabilitation methods of cognitive functions frequently disturbed during aging and in TBI: self-awareness, episodic memory, and executive functions. Two studies evaluating the efficacy of the CEP, using a controlled semi-randomized single-blind design, are reported in this thesis. The first study aimed to demonstrate the CEP’s efficacy in improving episodic memory. The CEP’s second module is composed of the training mnemonic exercises of the MEMO Program, which focuses mainly on this memory dimension. The MEMO program was originally conceived and demonstrated to be effective for people suffering from mild cognitive impairment. This is the first study to use the MEMO Program with individuals having sustained a TBI. ANCOVA showed significant improvements in a psychometric episodic memory measure, and on a daily-life generalisation self-reported measure in experimental group participants. In addition, they showed clinically significant improvement in self-perceived psychological well-being. The second study assessed CEP efficacy on participants’ executive functioning using psychometric measures of executive functions, and on daily-life generalization measures. Results showed significant improvement in the experimental group on subtasks of a psychometric executive function measure, as well as a significant reduction in CEP participant-significant other differences on a self-reported scale assessing executive functioning in daily life. Also, the number of forsaken daily activities was significantly reduced 6 months later in the experimental group. Integration of cognitive rehabilitation programs such as the CEP into holistic rehabilitation programs is necessary and highly recommended, as is training neuropsychologists on cognitive and neuropsychological rehabilitation methods and strategies, to improve services offered to the aging and general population.
