Estudo prospectivo do valor da acupuntura no controle da náusea e vômitos em pacientes de câncer de mama submetidas a quimioterapia adjuvante / Prospective study on the use of acupuncture for nausea and vomiting control in patients with breast cancer submitted to adjuvant chemotherapy.

O objetivo deste estudo é avaliar a eficácia de duas técnicas de acupuntura no controle da náusea e vômito em pacientes portadoras de Carcinoma Invasivo de mama, submetidas à quimioterapia anti-neoplásica. Foram incluídas no estudo 64 pacientes no período de março de 2003 a fevereiro de 2007, do Hospital A.C. Camargo com diagnóstico anatomopatológico de carcinoma invasivo da mama, candidatas à quimioterapia adjuvante com esquemas que incluem doxorrubicina e ciclofosfamida. O estudo foi realizado no primeiro ciclo de quimioterapia. As pacientes foram randomizados em 3 grupos: As pacientes do grupo A, receberam tratamento antiemético convencional com medicamentos. As pacientes do grupo B, receberam o tratamento antiemético convencional e aplicação de acupuntura clássica. As pacientes do grupo C, receberam tratamento convencional e aplicação de acupuntura auricular. As pacientes foram submetidas à auto-avaliação nos primeiros 21 dias após o início da quimioterapia, e conforme os critérios da Common toxicity criteria of the National Cancer Institute (NCI CTC), preencheram a ficha de coleta de dados em relação à náusea e vômito neste período. Quanto à náusea do primeiro ao sétimo dia, encontramos pelo teste de Kruskal-Wallis diferença significativa (p=0.040), sugerindo que a acupuntura diminui a intensidade de náusea deste período, e quanto à duração de dias de pior intensidade também foi significativo (p=0,037) ou seja a acupuntura diminui os dias de pior intensidade.Quando utilizamos o teste de Mann-Whitney encontramos diferença significativa da intensidade de náusea somente entre os grupos A e B (p=0,013), significando que a acupuntura clássica diminui a intensidade de náusea em relação ao grupo controle. Em relação aos dias de pior intensidade de náusea encontramos diferença entre os grupos A e B (p=0,043) e entre grupos B e C (p=0,010) ou seja, a acupuntura clássica diminui os dias de pior intensidade de náusea em relação aos grupos controle e acupuntura auricular. Quanto à gravidade do vômito, do primeiro ao sétimo dia, encontramos pelo teste de Kruskal-Wallis diferença significativa (p=0,036) entre os grupos, sugerindo que a acupuntura diminui a gravidade de vômitos. Pelo teste de Mann-Whitney entre os grupos A e B encontrou-se diferença significativa (p=0,017), ou seja, a acupuntura clássica diminui a gravidade de vômitos comparado ao grupo controle. Quanto ao número de dias de vômito de pior grau entre o primeiro e o sétimo dia encontramos ao aplicar o teste ANOVA diferença significativa entre os grupos (p=0,029), sendo que a acupuntura diminui o número de dias com vômito. O teste t aplicado para os grupos em pares revelou entre grupos A e B (p=0,035), portanto, a acupuntura clássica diminui o número de dias de vômito comparado ao grupo controle. Ao estudarmos a intensidade de náusea entre oitavo e vigésimo primeiro dia encontramos pelo teste de Kruskal-Wallis entre os grupos diferença significativa (p=0,022), sendo que a acupuntura diminui a náusea entre oitavo e vigésimo primeiro dia. Pelo teste de Mann-Whitney entre grupos aos pares encontrou-se diferença significativa entre os grupos A e B (p=0,016), houve diferença entre grupos A e C (p=0,049). Portanto, há diferença significativa entre grupos B e C em relação ao grupo A, ou seja, a acupuntura clássica e auricular diminuem a náusea entre oitavo e vigésimo primeiro dias em relação ao grupo controle. Conclusão: A acupuntura clássica diminui a intensidade e duração de náusea e vômito do primeiro ao sétimo dia pós-quimioterapia. A acupuntura clássica e auricular diminuem a intensidade de náusea do oitavo ao vigésimo primeiro dia pós-quimioterapia. / The purpose of this study is to evaluate the efficacy of two acupuncture techniques in the control of the nausea and vomiting in patients diagnosed of Invasive Breast Carcinoma submitted to chemotherapy. Sixty four patients with pathological diagnosis of Invasive Breast Carcinoma were included in the study during the period of March 2003 to February 2007 at Hospital A.C. Camargo. The patients received adjuvant chemotherapy including doxorubicin and ciclophosphamide. The study was developed during the first chemotherapy cycle. The patients were randomized in 3 groups: Patients of the group A received conventional antiemetic treatment with drug administration. Patients of the group B received conventional antiemetic treatment with drug administration and application of classic acupuncture. Patients of the group C received conventional antiemetic treatment with drug administration and application of auricular acupuncture. During the first 21 days, the patients performed self-reports after receiving the chemotherapy shot, according to the Common toxicity criteria of the National Cancer Institute (NCI CTC). They filled out the data collection instrument related to the nausea and vomiting in this period. As for the nausea from the first to the seventh day, we found statistical significancy (p=0.040) using the Kruskal - Wallis test, suggesting that acupuncture reduces the intensity of nausea in this period. Regarding length of days having worse intensity of nausea in this period, results were statistically significant by ANOVA test (p=0.037) with acupuncture decreasing the number of days of worse intensity. Through the Mann Whitney test, we also found significant difference regarding the intensity of nausea among the groups A and B (p=0.013), showing that classic acupuncture reduces the intensity of nausea when compared to the control group. In relation to the days of worse intensity of nausea we found difference among the groups A and B (p=0.043) and among groups B and C (p=0.010) meaning that classic acupuncture decreases the days of worse intensity of nausea when compared to control group and auricular acupuncture group. As for the severity of the vomit, from the first to the seventh day, we found through Kruskal - Wallis test a significant difference (p=0.036) among the groups, suggesting that acupuncture reduces the severity of the vomits. Using the Mann - Whitney test among the groups A and B a significant difference was found (p=0.017) meaning that classic acupuncture reduces the severity of vomits compared to the control group. As for the number of days of vomit in worse degree between the first and the seventh day, we used the ANOVA test and found significant difference among the groups (p=0.029) with acupuncture reducing the number of days with vomit. The t test applied for the groups in pairs revealed statistical significance among groups A and B (p=0.035) and therefore, classic acupuncture reduces the number of days of vomit compared to the control group. We have analysed the intensity of nausea between eighth and twentieth first day. Through the Kruskal - Wallis test, we found significant difference among the groups (p=0.022) showing that acupuncture also reduces the nausea between eighth and twentieth first day. Analyzing the groups in pairs with the Mann - Whitney test, we found significant difference among the groups A and B (p=0.016) and among groups A and C (p=0.049). Therefore, there is significant difference among groups B and C in relation to the group A. Classic acupuncture and auricular acupuncture reduce the nausea between eighth and twentieth first days when compared to control group. Conclusion: Classic acupuncture reduces the intensity and duration of nausea and vomit from the first to the seventh day after chemotherapy. Classic acupuncture and auricular acupuncture reduce the intensity of nausea from the eighth to the twentieth first day after chemotherapy.

Terapia celular sob aquapuntura em modelos murinos para distrofia muscular de Duchenne / Cellular therapy under aquapuncture in murine models for Duchenne

O camundongo mdx é um modelo animal muito utilizado para estudar a distrofia muscular degenerativa de Duchenne (DMD) e apesar de o modelo apresentar uma degeneração mais branda, inúmeras pesquisas com este animal são realizadas devido à alta reprodutibilidade de fenótipos, obtenção comercial de várias linhagens e baixo custo de manutenção. A degeneração muscular progressiva ocorre devido a uma mutação genética que culmina na ausência ou diminuição da produção da proteína distrofina. Esta alteração gera uma cascata de eventos que pioram a degeneração muscular. Atualmente, a terapia preconizada é o uso de corticóide, que visa modular a inflamação muscular e possui uma série de efeitos colaterais, por isso uma série de estudos propondo outras formas de tratamento vem sendo desenvolvidos. Um tratamento promissor parece ser a terapia celular com células-tronco mesenquimais, a qual pode interferir no processo degenerativo por imunomodulação celular. Como a acupuntura pode controlar o processo inflamatório de doenças degenerativas, tal qual na doença de Parkinson, acredita-se que o uso consorciado das duas terapias possa diminuir o efeito degenerativo da DMD. Nesta pesquisa foram avaliados 22 camundongos mdx, machos, 4 a 6 semanas de idade. As células-tronco utilizadas para a aplicação foram de polpa dentária humana carregando a proteína verde fluorescente (EGFP) e a quantidade aplicada em cada acuponto foi de 1x104 células. Os acupontos selecionados foram B47 (Hunmen), B49 (Yishe) e B52 (Zhishi). Os animais foram distribuídos aleatoriamente em quatro tratamentos (A) células-tronco em acupontos falsos, (B) solução fisiológica em acupontos verdadeiros, (C) células-tronco em acupontos verdadeiros e (D) controle, sem nenhum tratamento. No total três injeções foram realizados em um período de 56 dias de estudo. As avaliações do tratamento foram feitas através das análises da força de tração dos músculos, da creatinofosfoquinase sérica (CPK), histológica através da estruturação das miofibrilas e morfometria do colágeno e imuno-histoquímica pela expressão da proteína distrofina. Ao décimo dia após a terceira aplicação de células-tronco observa-se uma diferença somente no grupo controle, a qual manifestou um aumento da CPK em relação aos outros tratamentos e sem diferença estatística para força de tração pelos membros torácicos. Na histologia dos músculos tibial cranial e diafragma foram observou-se infiltrados inflamatórios, regeneração tecidual e diminuição do colágeno nos grupos tratados p<0,05. Na imuno-histoquímica verificou-se uma melhora da quantidade de distrofina nos animais tratados principalmente no tratamento células-tronco em acupontos verdadeiros p< 0,05 e, pela análise do rastreamento das células-tronco com EGFP não se pode determinar o seu caminho uma vez que não observamos imunorreatividade nas análises utilizando o anti-GFP. Conclui-se que os tratamentos com terapia celular nos acupontos verdadeiros, acupontos falsos e solução fisiológica nos acupontos podem melhorar a doença degenerativa muscular no mdx pelo aumento da expressão da proteína distrofina. / Muscular dystrophy in mdx mouse is an animal model used to study Duchenne muscular dystrophy (DMD) in humans. Although this model presents a milder degeneration, there are several studies with this animal due to the high reproducibility of phenotypes, obtaining various lines available and low maintenance cost. It is known that progressive muscle degeneration occurs due to a genetic mutation that culminates in the absence or decreased production of dystrophin protein. This change generates a cascade of events which aggravate the muscle degeneration. Currently, the recommended therapy is the use of corticosteroids to modulate muscle inflammation; however, there are a number of collateral effects, so a series of studies suggesting other forms of treatment have been developed. A promising treatment seems to be a therapy with mesenchymal stem cells, which can interfere with the degenerative process by cellular immunomodulation. As acupuncture can control the inflammatory process of degenerative diseases such as Parkinson\'s disease, the use of both syndication therapies may reduce the DMD degenerative effect. In this study 22 mdx mice, males, 4-6 weeks of age were evaluated. Stem cells were used for the application of human dental pulp carrying the green fluorescent protein (EGFP) and the quantity applied to each acupoint was 1x104 cells. The acupoints selected were B47 (Hunmen), B49 (Yishe) and B52 (Zhishi). The animals were randomly assigned to four treatments groups: (A) stem cells in false acupoints, (B) saline in true acupoints, (C) stem cells in true acupoints and (D) control without any treatment. In total, three injections were performed over a period of 56 days of study. Evaluations of treatment were made by analyzing the tensile strength of muscles, measuring the serum creatine phosphokinase (CPK), structuring of myofibrils by histology and morphometry of collagen immunohistochemistry and by the amount of dystrophin protein. At day ten after the third application of stem cells there is a difference only in the control group, which manifests an increase in CPK compared to other treatments and no statistical difference in tensile strength. In the histology of cranial tibial muscles and diaphragm, inflammatory infiltrates were observed, as well as tissue regeneration and decreased collagen in the treated groups (p <0.05). On immunohistochemistry there was an improvement in the amount of dystrophin in animals treated mainly in acupoints true (p <0.05) and through analysis and tracking of stem cells with EGFP it was not possible to determine its path once immunoreactivity was not observed in analyzes using anti-GFP. We conclude that treatment with stem cell therapy on true and false acupoints and with saline solution on acupoints can improve muscular degenerative disease in mdx by increased expression of dystrophin protein.

Neurotransmitter systems and EEG related to acupuncture

Acupuncture has been used for thousands of years to treat a wide range of diseases, but the mechanisms involved in the process have remained a mystery. The present study measures EEG responses to stimulation of a specific acupuncture point, GB37 (Guang Ming), with two different types of manual needle stimulation. Previous studies stimulated for a maximum of 2 minutes. The present study reflects the normal acupuncture treatment time of 20 minutes, with EEG recordings during and for 10 minutes prior to and after stimulation. Our results show no changes in the global spatial and temporal properties of EEG during and shortly after acupuncture treatment of acupoint GB37. The second part of this study examines the global protein expression of glutamic acid decarboxylase (GAD) knockout mice. GAD is the rate-limiting enzyme in the synthesis of GABA, the major inhibitory neurotransmitter in the brain. The protein content of wild type, hetero-, and homozygous GAD knockout mice brains were determined using a LC-MS-based gel-free shotgun profiling of complex protein mixtures. The data was analyzed using the Raculovic algorithm to determine the proteins differences. A short list of 32 proteins was determined with four that have been shown to be significant proteins that influence cell survival and excitotoxicity in the brain and have potential relationships with GABA. These proteins include VATPase, Glutamine synthetase, Beta-synuclein, and Micortuble associated protein (MAP). The proteomics results provide a preliminary best guess list of proteins influencing GAD and GABA production. / by Michael L. O objetivo deste estudo foi comparar a resposta a diferentes formas de tratamento da discopatia toracolombar e lombar, com base na avaliação do quadro clínico de animais antes e após tratamentos específicos: cirurgia descompressiva, tratamento clínico com eletroacupuntura ou associação de cirurgia e eletroacupuntura. Os 40 pacientes selecionados foram atendidos no período de janeiro de 2003 a julho de 2007, com quadro clínico de discopatia toracolombar. Estes foram então classificados conforme o grau de envolvimento medular (de I a V, com severidade clínica crescente) segundo a classificação de adaptada de Takahashi et al. (1997), Schulz et al. (1998) e Chierichetti e Alvarenga (1999) e divididos em três grupos: G1 (Cirurgia), G2 (Acupuntura) e G3 (Acupuntura e Cirurgia), de acordo com o tipo de tratamento a qual foram submetidos. Para análise estatística utilizou-se o Teste de Goodman para contrastes entre e dentro de populações multinomiais com o nível de significância de 5% (p<0,05). Todos pacientes dos três grupos tinham quadro clínico medular grave (IV ou V), a exceção de um animal grau II do G2. No G1, houve melhora de 40% dos animais tratados, no G2 de 79% e no G3 de 72,7%. Em nenhum animal houve piora clínica. Concluiu-se que o grupo do tratamento com eletroacupuntura foi mais eficiente que o grupo da cirurgia descompressiva tardia para recuperação do quadro neurológico em cães portadores de discopatia. / Intervertebral disc disease is the most commom cause of neurologic disease in small animals. Spinal cord alterations in disc herniation are not completly understood. Treatment options are depending on neurological examination. The aim of this study was to compare the effectiveness of surgery, acupuncture or surgery followed by eletroacupuncture in dogs with disc disease. Dogs with thoracolumbar and lumbar intervertebral disc disease were selected from january 2003 to july 2007. Patients were classified according to a score adapted from Takahashi et al. (1997), Schulz et al. (1998) and Chierichetti and Alvarenga (1999) scale of myelopathy, ranging from I to V with a crescent severerity. Forty animals were treated with surgery (G1), acupuncture (G2) and surgery followed by acupuncture (G3). All of them had a high degree of myelopathy (grade IV-V) except one dog (grade II) in G2 group. Statistical analysis was performed using the Goodman test for comparison of differences between the polinomial populations (p<0,05). We obtained that 40% of the animals improved in G1, 79% in G2 and 72,7% in G3. Electroacupuncture was more effective than late surgery for neurological improvement in dogs with disc disease.

Defining the problem: Elektropuncture is a therapeutic method that uses electric current to the stimulation of acupuncture points or projections on the surface of the body. Because it is an alternative method of treatment, some specialists as well as nonprofessionals don't acknowledge the electropuncture as an effective method of treatment. My thesis is focused on issues elektropuncture approach and on evaluation of its therapeutic effectiveness and usefulness in the field of physical therapy by using low- frequency currents. Objectives of work: The aim is to bring the issue of the use of low-frequency currents in elektropuncture and to describe in detail the currents, which can be used, to evaluate and to process the resources available in the literature to find a scientific evidence of its therapeutic effectiveness and use elektropuncture in physiotherapy. It is considered a public health issue.The effectiveness of acupuncture in chronic headache is still a reason of controversies. Objectives: Evaluate the effectiveness of acupuncture as an adjuvant analgesic treatment for chronic headache and the influence of this treatment on quality of life. Methods: There were selected 34 patients by chronic pain service of a teaching hospital, divided in two groups: True acupuncture (group I), where were used the recommended points for the Traditional Chinese Medicine paragraph for each type of headache; and sham acupuncture (group II), where the needles were inserted into a device (moxa on button), used the same points in Group I. Both groups used the prescribed preventive medication to pain. The assessment methods were: Scale Verbal numerical before (ENV1) and after (ENV2) to treatment; the number of attacks and number of analgesics used during the first and second months of treatment. Quality of life was also assessed before and after the treatment through the Brazilian version of the Quality of Life Questionnaire (QOL) SF-6D 2002. Results: The true acupuncture group showed greater effectiveness in controlling pain in chronic headache, statistically significant in all items compared with the sham acupuncture group, including improvement in quality of life. Conclusions Acupuncture can be considered an adjuvant treatment of chronic headache, reducing the intensity of pain, the number of attacks, quantity of analgesics, as well as improve the patients’s quality of life. / INTRODUÇÃO: Mais de 90% da população mundial teve ou terá pelo menos uma crise de cefaleia na vida e 3% as tem regularmente. É considerada uma questão de saúde pública. A eficácia da acupuntura nas cefaleias crônicas ainda é motivo de controvérsias. OBJETIVOS: Avaliar a eficácia da acupuntura como tratamento analgésico adjuvante da cefaleia crônica e seu efeito na qualidade de vida dos portadores dessa condição dolorosa. MÉTODOS: Foram selecionadas 34 pacientes do Ambulatório de Dor Crônica de um hospital de ensino, divididas em dois grupos: acupuntura verdadeira, onde foram usados os pontos convencionais para tratamento de cefaleia pela Medicina Tradicional Chinesa; e acupuntura falsa, onde as agulhas foram inseridas em um dispositivo, a moxa em botão, nos mesmos pontos usados na acupuntura verdadeira. Ambos os grupos usaram medicamentos preventivos para a dor. Os métodos de avaliação foram: Escala Numérica Verbal antes e depois do tratamento; número de crises e de analgésicos utilizados durante o primeiro e segundo mês de tratamento. A qualidade de vida também foi avaliada antes e depois do tratamento através da Versão Brasileira do Questionário de Qualidade de Vida SF36 2002. RESULTADOS: O grupo da acupuntura verdadeira apresentou maior eficácia no controle da dor na cefaleia crônica, estatisticamente significante, em todos os itens avaliados quando comparada com o grupo da acupuntura falsa, inclusive na melhora da qualidade de vida. CONCLUSÕES: A acupuntura pode ser considerada como tratamento adjuvante da cefaleia crônica, reduzindo a intensidade da dor, o número de crises, quantidade de analgésicos, além de melhorar a qualidade de vida das pacientes.
