O iodo em excesso promove o efeito Wolff-Chaikoff. Oligominerais já foram descritos como potenciais reguladores da expressão de proteínas. Tornou-se interessante avaliar se o iodo interferiria com a expressão do mRNA da NIS, em curtos períodos de tempo. Foram realizados, em ratos e células (de 30 min24h), estudos de expressão, comprimento de cauda poli-A e recrutamento para polissomos, do mRNA de NIS. Observou-se, in vivo e in vitro, que o excesso de iodo promoveu diminuição da expressão e do comprimento da cauda poli-A do mRNA de NIS, em todos os períodos estudados, além de promover menor recrutamento deste mRNA para os polissomos. A diminuição da cauda poli-A do mRNA de NIS pode ter aumentado sua instabilidade/degradação e também ter sido responsável por uma menor eficiência de tradução deste transcrito. Conclui-se que: (a) o iodo regula pós-transcricionalmente a expressão gênica da NIS, sendo fundamental nos processos que norteiam o efeito Wolff-Chaikoff e (b) oligoelementos têm relevância na regulação da expressão de proteínas relacionadas ao seu transporte. / Iodide in excess exerts the Wolff-Chaikoff effect. It is described that some minerals can regulate the expression of proteins. This study aimed to investigate if the iodide could modify the expression of NIS mRNA, in short periods of time. Rats and cells, divided in time-groups of 30 min up to 24h, were used in studies of expression, poly-A tale length and polysomal profile of NIS mRNA. Both in vivo and in vitro studies showed that the iodide treatment promoted a reduction in the expression and the poly-A length of NIS mRNA, in all time-groups, and decreased its recruitment to the polysomes. It is possible that the reduction of NIS mRNA poly-A tale length has increased the instability/degradation of this transcript, and impaired the translation efficiency of it. Concluding: a) the iodine exerts a post-transcriptional regulation of NIS mRNA expression, being essencial in the processes that guide the Wollf-Chaikoff effect; b) the oligoelements have an extremely important role in the expression regulation of proteins related to their transport.

Illness perceptions in patients and parents in paediatric oncology during acute treatment and follow‐up care

Objective: Recent evidence suggests that illness perceptions in paediatric patients and their parents may differ, with parents holding more negative views compared to their children. Little is known about illness perceptions of very young patients and their parents. This study investigates illness perceptions in paediatric cancer patients aged 4–18 years and their parents in acute treatment or follow‐up care, distinguishing patients by age (4–11, 12–18) and stage of medical treatment. - Methods: N = 45 patient–parent dyads in acute treatment and n = 95 dyads in follow‐up care were examined. Parents and older children aged 12–18 years completed the Illness Perception Questionnaire—Revised (IPQ‐R) and younger children aged 4–11 years were examined using an age‐adapted hand puppet interview containing the IPQ‐R questions. Difference scores of illness perceptions (symptoms, timeline‐acute/chronic, timeline‐cyclical, personal control, illness coherence, consequences, emotional representations) between children and parents were tested for significance using Wilcoxon signed‐rank tests. - Results: Overall, parents perceived more symptoms associated with their child's illness/treatment than the children themselves. In acute treatment, younger children indicated more negative and older children more positive views regarding chronicity than parents. Younger children held less negative views on consequences, and all children reported less negative emotional representations than parents. In follow‐up care, all children held less negative views on consequences and emotional representations. Older children reported less negative views on chronicity, cyclicity and illness coherence. - Conclusion: Differences in illness perceptions of paediatric patients and their parents should be considered during and after treatment/medication and psychosocial care to support illness coping in person‐ and family‐centred interventions.

The main theme of the dissertation work are allocations of sources in regular dialysis treatment in ethical context. The aim of the work is detection, description and understanding to ethical problems forming in context with questionable allocation of sources in regular dialysis treatment. The theoretical part of the work is concentrated on mapping above mentioned problems from various point of view. By the help of analysis of pertinent literature it is referred to historical determination and social-economic context of difficult allocation of rare sources in health service, which subsequently caused development of specific ethical problems in clinical practice of dialysis treatment. Above mentioned ethical problems are discussed chronologically, namely from the first using of a dialysis device up to now, when this therapy is considered to be a standard clinical method. The practical part of the work contains results of high-quality research, the aim of which was to explore the experience of respondents with allocation of resources in regular dialysis treatment and so in ethical context.
