The impact of HIV/AIDS on the South African labour market

Thesis (MBA)--Stellenbosch University, 2000. / ENGLISH ABSTRACT: No disease in modern times has created as much fear and panic as HIV/AIDS - undoubtedly one of the most formidable public health problems facing South Africa today. Traditionally, HIV/AIDS has not been regarded as a workplace issue, except for "high-risk" professions such as surgeons, dentists and emergency medical technicians. The stark reality is that HIV/AIDS will have an impact on all sectors of society and all aspects of human activity throughout South Africa. Because of the nature of the spread of the disease, it hits predominantly sexually active people. These are the people that make up the workforce. It can therefore be expected that HIV/AIDS will impact directly on the patterns of employment in South Africa. Furthermore, the economic consequences of the epidemic will result in pressures on the South African economy, which again will influence employment. Apart from affecting the general population, HIV/AIDS will have a direct impact on businesses in various areas. HIV/AIDS will also have a significant impact at community level that will, in turn, impact on businesses. If business in South Africa is to survive, these impacts have to be managed so that productivity can be maintained and costs can be contained. The objective of this study is to explore the impact of HIV/AIDS on employment by examining the South African labour market. From the study, it is clear that South Africa is certain to experience severe consequences arising from the HIV/AIDS epidemic. Even without taking the immense impact of HIV/AIDS into account, there are various problems facing the' South African labour market. Specific problems discussed are unemployment, low productivity and a shortage of skills. These are not the only problems associated with the South African labour market, but the extent of these problems is further exacerbated by the impact of HIV/AIDS. All sectors of the society need to be mobilised into action. This action must encompass both prevention of the spread of the disease, as well as caring for the sick and dying. / AFRIKAANSE OPSOMMING: Geen ander siekte in die moderne tyd het al soveel vrees en paniek geskep as MIV/VIGS nie ongetwyfeld een van die mees formidabele gemeenskapsgesondheidsprobleme wat Suid-Afrika huidiglik in die gesig staar. Oorspronklik was MIV/VIGS slegs met die werksplek geassosieer in terme van "hoë-risiko" beroepe soos dokters, tandartse en nood mediese personeel. Die naakte waarheid is dat MIV/VIGS 'n impak sal hê op alle sektore en aspekte van die samelewing in Suid-Afrika. As gevolg van die aard van die verspreiding van die siekte, tref dit hoofsaaklik seksueel aktiewe persone. Hierdie persone is die lewensaar van die arbeidsmag. Dit kan dus verwag word dat MIV/VIGS 'n direkte impak op werkverskaffingspatrone in Suid-Afrika sal hê. The impact of South African social welfare policies on pensioners raising orphaned grandchildren

Thesis (MPhil)--Stellenbosch University, 2004. / ENGLISH ABSTRACT: Very little attention has been paid in South Africa to the situations where grandmothers have become surrogate parents to their grandchildren and other vulnerable children who have been left destitute due to the HIV/Aids epidemic. These women, who are in great need of support are not directly factored into Aids-related policy and care strategies, even though they play a vital role in assuming the roles of surrogate parents to their grandchildren. Clearly elderly women are valuable resources and are both crucial and valuable for the role they play in the care of both Aids sufferers and Aids orphans. Grandmothers who assume such care-giving roles are not receiving specific and targeted support from government structures. Instead they only benefit peripherally from the legislation within the jurisdiction of Department of Social Development and the programs that are established by them. This study determined where government social and welfare polices can improve the quality of life of black female pensioners who are taking care of their orphaned grandchildren and other vulnerable children. Policy makers need to recognise that grandmothers are increasingly obliged to assume care-giving responsibilities for dying adult children and later for orphans, when their own physical and cognitive abilities may be declining. Instead of using their state pensions to enjoy their old age and spend their money on looking after their health, proper housing etc., black female pensioners are forced to assume additional responsibilities in looking after orphaned grandchildren and other vulnerable children who require food, clothing, proper housing, education etc. Such expenses imply that their pensions are not put to their intended uses. Due to their advanced age they find it increasingly difficult to earn income to support young children. Until now the South African government has done little to support older women who find themselves in such precarious circumstances and to recognise their invaluable contributions. Various recommendations are made in the report to improve this situation. / AFRIKAANSE OPSOMMING: Baie min aandag is tot dusver nog in Suid-Afrika geskenk aan die omstandighede waar grootmoeders hulself bevind in die posisie van surrogaat ouer vir hul kleinkinders en ander kwesbare kinders wat haweloos gelaat is as gevolg van die HIVNigs epidemie. Hierdie vroue wat ondersteuning dringend nodig het, is nie regstreeks in berekening gebring by die verskeie Vigs beleide en ander versorginstrategiee en maatreels nie, al speel hulle 'n beslissende rol as surrogaat ouers vir hulle kleinkinders. Destigmatisation within the HIV/AIDS pandemic : wowards a pastoral anthropology of embodiment

Thesis (MTh (Practical Theology and Missiology))--University of Stellenbosch, 2010. / ENGLISH ABSTRACT: The focus of the thesis is on the HIV and AIDS-related stigma and stigmatisation of people who try to live positively with HIV/AIDS within the pandemic. The basic assumption is that there is interplay between the HIVAIDS-related stigma as a cultural phenomenon and the negative perception of the human body. Since a human being is created corporeal and re-created due to the fact that human embodiment is a fundamental ingredient for the understanding of soul, It is argued that in a pastoral approach, a person should be understood holistically. Anthropology within the traditional kerygmatic approach focused mainly on the notion of sin (corruption totalis) within the theological understanding of God’s judgement (judgemental attitude). I have proposed that pastoral anthropology should adopt constructive paradigms and point towards the integration of embodiment (wholeness) in a realistic approach rather than emphasising the notion of sin and forms of dualism. The thesis departs from an eschatological and pneumatological view of the human being, in which the concepts of resurrection and hope are equally crucial. I further argue that a Christian spiritual perspective on embodiment is potentially destigmatising itself. In terms of a pastoral hermeneutic I have shown that in destigmatisation the transformation of the HIV and AIDS-related stigma corresponds to the transformation of the mindset and paradigm of a person (habitus). Through the process of destigmatisation people discover meaning and are enabled to live fully embodied and responsible lives. The thesis is designed as a literature study based on text analysis and hermeneutical reflection. Moreover, in order to develop a pastoral anthropological view, the Scripture is used as a reference point. / AFRIKAANSE OPSOMMING: Die navorsing fokus op die fenomeen van stigmatisiering binne die HIV/AIDS pandemie. Die kernargument is dat stigmatisering as 'n sosiaal-kulturele konstrukt binne die netwerk van verhoudinge direk in verband staan met 'n bepaalde destruktiewe persepsie wat die vraagstuk van liggaamlikheid onmiddellik raak. Vandaar die verdere fokus op die verband tussen liggaamlikheid en die verstaan van die menslike siel binne die raamwerk van 'n pastorale antropologie. Die teologiese invalshoek is die eskatologiese paradigma, die mens as 'n pneumatiese wese en nuwe skepping. Liggaamlikheid deel gelykoorspronklik aan hierdie nuwe wees-funksie van die mens sodat verstaan van die mens as „beliggaamde siel“ en „besielde liggaam“ alle vorme van dualisme in teologiese antropologie teëwerk. Die totale mens is as ‘n beliggaamde mens geskep sodat in pastorale antropologie die menslike persoon holisties verstaan moet word. Om menswees bloot vanuit die perspektief van sonde te benader hou nie rekening met die realisme van die Bybel wat die mens binne die raamwerk van die wysheidsliteratuur sien vanuit die perspektief van genade en vernuwing. In die lig van die opstandingsperspektief is die „dood van stigma“ totaal uitgewis. Hierdie opstandingperspektief moet verreken word in teologiese model wat gerig is op prosesse van destigmatisering binne pastorale hermeneutiek. Die implikasie hiervan is die transformasie van stigmatisernde paradigmas en die skep van pastorale houding (habitus) van begrip en medelye. Deur ‘n dergelike proses van destigmatisasie word mense in die kern van hul weesfunksie kwalitatief bemagtig ten einde vervulde lewens te kan ly. Die tesis volg kwalitatiewe benadering. Dit is voorts literêre studie gebaseer op teks-analises, kritiese reflektering en hermeneutiese metodologie.

Searching for meaning in the face of HIV/ AIDS among men who have sex with men (MSM) in Cape Town, South Africa

Thesis (MTh)--Stellenbosch University, 2012. / ENGLISH ABSTRACT: This research paper set out to discover whether a Christian Spiritual perspective (within a framework of a theology of affirmation and a psychological strengths perspective of fortigenesis) on basic threatening existential issues, such as HIV/AIDS Stigma, Homophobia, etc. can help MSM to reformulate the quest for meaning and be integrated in a holistic approach to spiritual healing in order to overcome the schism in HIV/AIDS: Friend or Foe? Setting the stage for exploring the world‟s worst global challenge within the health sector, and how it relates to the South African context for MSM. The research uncovered that MSM face the greatest challenge and burden of HIV/AIDS in many countries, however little research has focused on MSM and HIV/AIDS in South Africa. Literature links this to the fact that throughout Africa, the predominant view of same sex relationships as being un-African, sinful and an abomination. Pointing out that for MSM, the development of God-images usually occurs during the process of growth and maturation as an individual, where a multitude of experiences are based primarily through the filter of belonging to a minority, which are seen predominantly from society as being sinful. The possible multitude of traumatic and often fearful life experiences, including the potentially highly stressful and difficult process of informing their parents and families of their sexuality, may influence their emotional experience and ultimately their God-image. The God-image of parental love, acceptance and caring may be impacted on by fear of rejection and the lack of acceptance. The development of ambivalence may strongly impact on MSM God-images, resulting in conflicted experiences and understanding of God, where the God-image could become distant and judgmental (Hoffman, 2009:15-18). Something that is exacerbated by the stance of some belief systems promote the concept of “love the sinner, hate the sin”, which only results in MSM being ignored or having their partners/relationships rejected, thus isolating them within their own faith community. This impacts their spiritual/religious development and their participation in a non-affirming faith community can result in levels of increased internalized homonegativity (this is defined as negative attitudes to one‟s own homosexuality), causing poorer psychological health and lower self-regard. The dignity, identity and meaning of MSM are further impacted on by their minority existence within the context of South Africa, which places various traumatic stresses on MSM. These traumatic stresses are amplified by the constant negotiation of fear regarding the contracting of HIV/AIDS or even being diagnosed with the life threatening disease. Despite all these external and the resultant internal stressors, Graham and Kiguwa (2005) found that religion and spirituality is important to MSM-youth as it provides possible supportive structures and environments. Many of the MSM-youth believe in the Bible or ancestors (or even both), yet have chosen to adapt their beliefs by taking what was important for them from the Bible and ancestral belief systems. This they have done as the traditional view of religion and the Bible held by most people, where the scriptures are taken literally, is that homosexuality is a sin (Graham and Kiguwa, 2005:7-8). Within this research, the interviewees experienced all this; however the results also showed that all the participants found some beneficial aspects out of contracting HIV/AIDS, with various levels of meanings relating to a sense of growth for each interviewee, while 4 out of 5 indicated that spirituality played an important role in assisting them in improving their lives. It aided in helping them to develop a sense of connection to themselves and others, something that many felt was missing within their lives. They felt that if it hadn‟t been for HIV, they would not have increased their levels of spirituality. This is an important empirical finding with respect to the research question and hypothesis, which hypothesized that HIV/AIDS promoted changes within MSM that results in spiritual growth and the development of internal strengths that allow them to transcend above the stressor of HIV/AIDS. The idea that MSM experience spirituality as something helpful and essential, makes it important to note that comments in the empirical research indicated that they tended to believe that if a Higher Power is involved, that it is unconditionally loving, kind and friendly. This is an important description of a God image within Christian Spirituality, a possible God-image of a Partner for Life or Friend. Thus, for Christian Spirituality to play a role within the healing of MSM and to better enhance spirituality and lay a better spiritual foundation, and develop better internal strengths and existential states, it thus becomes important to explore how Christian Spirituality will be able to address these needs of MSM, within a theological framework of affirmation.

Grief care to children ophaned by HIV and AIDS within Nairobi province Kenya : towards a pastoral strategy for identity formation

Thesis (PhD)--Stellenbosch University, 2014. / ENGLISH ABSTRACT: This thesis deals with pastoral grief care for children orphaned through HIV and AIDS in Nairobi City County in Kenya (formerly Nairobi Province in Kenya). This concern developed as a result of realising that children’s grief is rarely attended to before, during and after burial in most Kenyan communities. The research question that guided the research was: What theoretical, theological and contextual pastoral intervention strategy can the church use to provide grief care to children orphaned through HIV and AIDS? To respond to this question, the research had four objectives, which helped the researcher to remain focused on the key problem. An empirical research design was adopted to establish the impact the death of parents has on children and to examine how their grief has been responded to, particularly by the church. It was clear that children do experience grief that manifests in various forms including emotional, behavioural, cognitive, spiritual and physical responses. Grief is also heightened by the events that transpire after the death of parents plus the status of the children as double orphans, paternal orphans or maternal orphans. Despite the fact that children are overwhelmed by grief and grief-related issues when their parents die, the family and community as well as the church, rarely provide children with grief care. Through a literature study the research engaged with other disciplines in order to broaden the understanding of children’s grief. The study revealed that children’s development in general is affected by their grief and their stage of development, in turn, has an effect on their grief. However, it should be noted that African cultures have great impact on the development of African children and their grief. It was also noted that African people have their own way of grieving and mourning when a family loses a person through death. Various rituals that are calculated to help the mourning family work to through the grief process are performed. Interestingly, children rarely participate in the performance of such rituals. It was very clear that, in order to provide pastoral grief care to children orphaned through HIV and AIDS, it is of utmost importance to understand the nature of human beings in relation to God. Such an understanding could determine how the church responds to the grieving children’s situation. It is suggested that the church should work closely with the family, community and institutions in order to provide meaningful pastoral grief care within an African setting. It was argued that various rituals performed during funeral ceremonies are of help in grieving and the mourning process; hence it is suggested that children should also be given the opportunity to participate in performing the rituals. It is noted that the Christian and cultural rituals that are relevant to grief care could be employed. Given the above understanding, a pastoral intervention strategy for grief care to children orphaned through HIV and AIDS is proposed. Different interventions that respond to children’s grief and grief-related issues before, during, and after burial are discussed. / AFRIKAANSE OPSOMMING: Hierdie tesis ondersoek die pastorale vertroosting van kinders wat in die Nairobi City County in Kenya (die voormalige Nairobi Provinsie) as gevolg van MIV en vigs wees gelaat is. Dit bied 'n pastorale ingrypingstrategie in reaksie tot die verdriet van kinders wat as gevolg van MIV en vigs as wese atergelaat is. Kommer hieroor het ontstaan by die besef dat die bedroefdheid van kinders in die meeste Keniaanse gemeenskappe selde vóór, tydens of ná ‘n begrafnis aandag kry. Die navorsingsvraag wat die navorsing gerig het, was: Watter teoretiese, teologiese en kontekstuele pastorale intervensiestrategie kan deur die kerk gebruik word om vertroosting vir treurende kinders wat vanweë MIV en vigs wese geword het, te bied? Dat mense van Afrika droefheid en rou op ‘n eie manier bedryf wanneer ‘n familielid te sterwe kom, moet ook in ag geneem word. Daar is ‘n verskeidenheid rituele wat daarop gemik is om die roubeklaers in staat te stel om die rouproses te deurwerk. Kinders neem selde hieraan deel. Dit was duidelik dat dit van uiterste belang is om die natuur van die mens in verhouding tot God te verstaan om vertroostende pastorale sorg aan kinders wat vanweë MIV en vigs wees geword het, te bied. Die verstaan hiervan kan bepaal hoe die kerk op die situasie van kinders wat treur, reageer. Die voorstel is dat die kerk noue samewerking met die familie, die gemeenskap en instellings moet handhaaf om betekenisvolle vertroostende pastorale sorg binne die Afrika-opset te bied. Daar word aangevoer dat die verskillende rituele wat gedurende begrafnis seremonies uitgevoer word, van hulp is in rou en die rouproses, dus word voorgetel dat kinders ‘n geleentheid moet kry om aan die rituele deel te neem. Christelike en kulturele rituele wat van toepassing is kan gebruik word. 'n Pastorale intervensiestrategie vir vertroostende sorg aan kinders wat as gevolg van MIV en vigs wees gelaat is, word aan die hand van die bogenoemde insig voorgestel. HIV and AIDS in the business sector with reference to Eskom

The untimely death of millions of people due to a disease is totally unnatural and basically unimaginable and it is therefore not surprising that people, organisations and government tend to shy away from the problems. Businesses drive growth and development, mobilise investment, nurture innovation, provide employment, develop skills, pay taxes and provide returns to investors. Outside the political arena businesses is the only group that do have the capacity and resources to make a major and effective contribution to the fight against the HIV and AIDS epidemic. This study provides a theoretical introduction to HIV and AIDS but more importantly it indicates that the business world has woken up to the threat of the HIV and AIDS epidemic and are reacting to it. The study concludes that the battle is not won as yet. There is room for improvement and major enhancements are required to current strategies and programmes to make it more effective. Eskom was selected as the case study to substantiate the abovementioned opinions and conclusions. / AFRIKAANSE OPSOMMING: Nasionale en internasionale studies oor die impak van die MIV en VIGS-epidemie dui op die ernstige negatiewe gevolge vir die Suid-Afrikaanse ekonomie. Dit veroorsaak ook geweldige lyding en wanhoop onder mense. Die ontydige dood van miljoene mense weens 'n siekte is heeltemal onnatuurlik en ondenkbaar. Dit is daarom geen wonder nie dat mense, organisasies en die regering daarvan wegskram. Sakeondernemings dryf groei en ontwikkeling, mobiliseer beleggingsmoontlikhede, moedig vernuwing aan, skep werkgeleenthede, ontwikkel vaardighede, betaal belasting en verskaf 'n opbrengs aan beleggers. Buite die politieke arena is sakeondernemings die enigste groep wat die kapasiteit en hulpbronne het om op grootskaal 'n doeltreffende bydrae te lewer tot die bekamping van die MIV en VIGS epidemie. Die studie is 'n teoretiese inleiding tot MIV en VIGS. Die belangrikste aspek wat na vore kom, is dat sakeondernemings wakker geskrik het oor die bedreiging van MIV en VIGS en dat hulle daarop reageer. Die studie kom tot die gevolgtrekking dat die geveg nog nie gewen is nie. Daar is baie ruimte vir verbetering en grootskaalse veranderinge is nodig om die huidige strategieë en programme meer doeltreffend te maak. Eskom is as gevallestudie gebruik om bogenoemde menings en gevolgtrekkings te ondersteun.

Thesis (DTh (Practical Theology and Missiology))--Stellenbosch University, 2008. / ENGLISH ABSTRACT: This work is about a missiological praxis for the creation of 'Circles of Hope' as an entry point for faith based organisations and, particularly, local churches in Zambia for an effective battle in the fight against HIV (Human Immunodeficiency Virus). The HIV pandemic is one of the worst tragedies to have befallen humankind in the 21st century. Lost to many people is the fact that it does not affect all regions of the globe equally. Figures show that over 70% of PLWHA (people living with HIV) are in sub Saharan-Africa while most affected are young and therefore, in theory, energetic. With an overall HIV prevalence rate of 16% and a life expectancy of 34 years, HIV has severely impacted the lives of Zambians across the country. Stigma remains one of the most significant challenges in Zambia across the prevention-to-care continuum. The wider environment of these effects and statistics has provided for us the wider contextual situation. The Church in Zambia and, indeed, in the entire sub-Saharan African region cannot afford to hide its head in the sand anymore. The impact of HIV is being felt at all levels of society. This has posed a threat to economic progress and human development by attacking the most economically productive age group and reversing gains in life expectancy and child survival. The increasing burden on health budgets has stretched national and community resources to the limit, leaving no room for complacency or pretence about the magnitude of the problem. Since some members of the Church are positively infected, we can safely say of the Church: the body of Christ has HIV. HIV is a national disaster. It cannot be managed without mobilising all the sectors within a nation. The Church in Zambia needs to make HIV prevention a matter of compelling priority. The Church is an instrument for the missional praxis of the triune God. Theology in this work is not so much a scientific endeavour that begins and ends with analysing contexts and texts, rather it is an imaginative way of finding new answers to the new situation brought about by the presence of HIV (Hendriks, 2004: 30). In this work the researcher contends that measures are required to develop strong theological reflections and education which would result in the establishment of Circles of Hope in all local congregations. These Circles of Hope would act as a fountain for promoting behavioural change, support action for safer sexual behaviour, and combat stigmatisation and discrimination against people with known HIV infection. There must be appropriate theological reflections that touch on the work of the reign of God. A relevant HIV theology will necessarily involve the laity, will watch out for fundamentalist views, will be biblical in nature and will draw from Trinitarian understanding. The basis of moving from a theology of punishment to that of care, truth, freedom, justice and peace is the theology of community and companionship. This reflection is an attempt to have constructive interpretation of the present realities brought about by a time of HIV. One can only discern God‘s will for the present situation through critical and sensitive leadership in order to bring about genuine healing. The role of the local church and networking activities are essential commodities to realise a vision of a Zambia that is HIV competent. This then is the new ministerial strategy being spearheaded by the 'Circles of Hope' programme of the Council of Churches in Zambia. It is a challenge for Zambian churches. / AFRIKAANSE OPSOMMING: Die navorsing handel oor ʼn missiologiese begronde praksis vir die skepping van 'Kringe van Hoop' as 'n beginpunt vir geloofsgebaseerde organisasies en spesifiek die plaaslike kerk in Zambië in hulle stryd teen MIV (Menslike Immuniteits Virus). Die MIV pandemie is een van die ergste tragedies wat die mensdom in die 21ste eeu getref het. Wat baie mense nie altyd besef nie, is dat dit nie oral in die wêreld dieselfde impak het nie. Statistiek wys daarop dat meer as 70% van alle MIV positiewe mense Suid van die Sahara woon. Die meerderheid van die geïnfekteerdes is jonk met baie potensiaal. 16% van die bevolking in Zambië is MIV positief en het 'n lewensverwagting van 34 jaar. Die uitwerking hiervan raak die land en al sy mense. Naas voorkoming en versorging bly een van die grootste uitdagings wat die gemeenskap in Zambië moet aanspreek stigmatisering. Die groter konteks Suid van die Sahara vorm die agtergrond van elke land se spesifieke probleme. Ook Zambië en met name die kerk in Zambië sal die feite in die oë moet kyk. Die pandemie kan nie bestry word sonder dat al die sektore van die samelewing daarteen gemobiliseer is nie. Die kerk in Zambië moet die voorkoming van MIV as 'n uiters noodsaaklike prioriteit beskou. Die Kerk is 'n instrument vir die missionêre praksis van die drie-enige God. Die navorsing beskou teologie nie as 'n wetenskaplike onderneming wat bloot handel oor 'n analise van kontekste en tekste nie. Teologie is iets waarin jy handelend en verbeeldingryk toetree tot die aanspreek van 'n veelbewoë situasie en antwoorde probeer vind op die probleme (Hendriks, 2004: 30). Die navorsing werk met die hipotese dat die probleem vanuit 'n teologiese hoek benader moet word sodat teologiese beginsels die praktykteorie van Kringe van Hoop in plaaslike gemeentes sal onderskraag. Die studie voorsien dat die Kringe van Hoop die hoof dryfveer sal wees wat gedragsverandering sal bevorder, veiliger seksuele gedrag sal aanmoedig, en die stryd teen die stigmatisering en diskriminasie sal voer. Gepaste teologiese refleksie oor die heerskappy van God is noodsaaklik. 'n Revelante HIV teologie sal gewone lidmate insluit, sal bedag wees op fundamentalistiese sieninge, sal bybels wees en sal gebaseer wees op ʼn trinitariese godsbegrip. Die teologiese basis vir die wegbeweeg van 'n teologie van straf/oordeel na een van versorging, waarheid, vryheid, geregtigheid en vrede, is geleë in gemeenskap en kameraadskap. Hierdie refleksie is 'n poging om in 'n tyd van HIV 'n konstruktiewe interpretasie te gee van die huidige realiteite. Kritiese en sensitiewe leierskap behoort in die huidige situasie te poog om God se wil te soek om die gebrokenheid van 'n MIV siek gemeenskap aan te spreek. Die rol van die plaaslike kerk en netwerkingsaktiwiteite is onontbeerlik vir die realisering van die visie van ʼn Zambië sonder MIV. Die 'Kringe van Hoop'-program van die Zambiese Raad van Kerke is 'n bedieningstrategie wat die MIV pandemie wil aanspreek en wat die kerk in Zambië uitdaag om mee te doen.

Experiences and coping strategies of women living with HIV/AIDS: case study of Khomas region, Namibia

This study focuses on the impact of HIV/AIDS on women in Namibia. Namibia, with a population of only 1.7 million people, is ranked as the seventh highest country in the world in terms of HIV/AIDS infections. The percentage of women living with HIV/AIDS in Namibia accounts for 54% of the total of 68 196 people in the country living with the virus. Women are also diagnosed with the disease at a younger age (30) in comparison to their male counterparts (35 years). Desoite their needs, women living with HIV/AIDS bear a triple burden of caring for those living with HIV/AIDS, caring for themselves and coping with the responses to their infection. There are few focused intervention strategies to support and care for women living with HIV/AIDS in Namibia.

Compassion fatigue : pastoral care to HIV and AIDS caregivers within the realm of the healing professions

Thesis (MTh)--Stellenbosch University, 2014. / ENGLISH ABSTRACT: This dissertation aims to address the phenomenon of compassion fatigue within the scenario - of the HIV and AIDS pandemic in South Africa where an estimated 5-6 million people are living with HIV and AIDS. One in every three patients is hospitalised at some stage of their sickness and sufferers of the pandemic come into contact with someone that gives care to HIV and AIDS infected persons, whether in a hospital, a clinic, in counselling or a hospice. Professional stress and emotional fatigue is an inherent feature of the life of caregivers, and there is growing evidence is that it is on the increase. Caregivers are generally compassionate people and the compassion that they have and share with suffering people can be depleted . Caregivers become fatigued by the plight and the situation that never lets up hence the concept compassion fatigue. In this regard, the meaning of this concept as applied to HIV and AIDS caregivers in the healing and helping professions in hospital care (chaplaincy) and different clinical settings was the focus of the research. Compassion fatigue relates to professional stress and can impair professional competency and performance within pastoral caregiving. The phenomenon was studied to distinguish between compassion fatigue burnout/burn up and vicarious traumatisation. A more comprehensive understanding of these concepts will help to clarify current misconceptions. Aspects that necessitate a closer look are whether compassion fatigue is fundamentally stress related or a kind of depletion within the self of the pastoral helper. Could it be some kind of affective exhaustion, or is it related fundamentally to the overexposure to human suffering? Is compassion fatigue then suffering related? The study focuses on the challenges that confront caregivers in the HIV and AIDS field. How can these challenges pose an existential threat to their being-function and human identity? The study also explores how the theology of the cross (from a pastoral approach) can assist caregivers to deal with the question of meaning in the suffering that their patients (and they) may experience. Research questions included how the phenomenon of compassion fatigue should be understood as an inherent feature of the life of caregivers to HIV and AIDS sufferers, and what caregiving should imply to the caregivers (“wounded healers”). What is clear is that those caregivers very definitely also need care. A viable possibility to render meaningful help to depleted caregivers is to take them out of the working place and give them time, space and means to become whole again. A retreat environment is indicated as a form of recuperation for caregivers. At this stage the study amounts to only first steps in that direction and emphasises the need to investigate the subject further. / AFRIKAANSE OPSOMMING: Hierdie verhandeling beoog om die fenomeen van medelye-moegheid te verstaan vanuit die agtergrond van die HIV en VIGS pandemie in Suid-Afrika waar daar 'n beraamde 5-6 miljoen mense met HIV en VIGS lewe. Dit word geskat dat 1 uit elke 3 pasiënte in 'n stadium of stadia van die siekte gehospitaliseer word en derhalwe kom die meeste lyers in aanraking met iemand wat sorg verleen aan MIV/VIGS geïnfekteerde persone, hetsy in 'n hospitaal, 'n kliniek, in berading of 'n hospice. Professionele stres en emosionele moegheid is 'n inherente kenmerk van die lewe van versorgers en daar is toenemende bewys dat dit vanweë die erns van die situasie toeneem. Versorgers is oor die algemeen deernisvolle mense en die deernis wat hulle het en deel met mense wat ly, kan uitgeput raak. Te wyte aan ʼn situasie wat toenemend versleg en nie in die afsienbare toekoms sal weggaan nie ervaar versorgers dat die blootstelling aan menslike lyding en ellende hulle vermoë om lydendes pastoraal te troos en by te staan, drasties opgebruik vandaar die begrip medelye-uitputting. In hierdie verband fokus die navorsing op die betekenis van die konsep medelye-uitputting, soos toegepas op MIV- en VIGS- sorggewers binne die kader van die genesing- en hulpverleningprofessies in die hospitaal (kapelaansdienste) en kliniese instellings. Die verskynsel van medelye-uitputting met betrekking tot professionele stres raak die professionele bevoegdheid en prestasie van versorgers in pastorale hulpverlening. In hierdie verband is die doel om te onderskei tussen medelye-uitputting, uitbranding en plaasvervangende traumatisering. Dit is aanvaarbaar dat 'n meer omvattende begrip van hierdie konsepte huidige wanopvattings daaroor beter kan toelig. 'n Aspek wat 'n nader beskouing noodsaak, is of medelye-moegheid fundamenteel stresverwant is of 'n soort uitputting van die pastorale helper self, of 'n soort affektiewe uitputting, en of dit verband hou met te veel blootstelling binne die sfeer van menslike lyding. Die studie fokus op die uitdagings wat deur versorgers in die HIV en VIGSversorgingsveld die hoof gebied moet word, en hoe hierdie uitdaging ’n eksistensiële bedreiging vir hulle kan inhou wat hulle menswees en identiteit negatief kan beïnvloed. Die studie verken hoe daar vanuit die verstaan van die kruis-teologie pastorale ondersteuning gebied kan word met die hantering/verstaan van lyding en die doel en betekenis daarvan. Navorsingsvrae sluit o.a. in hoe ons die fenomeen van medelye-moegheid as ʼn ontwikkelende verskynsel in die lewe van versorgers wat met MIV- en VIGS-lyers werk, behoort te verstaan en wat pastorale sorg aan uitgeputte versorgers sou impliseer. Wat duidelik na vore gekom het, is dat die helpende hande baie beslis self ook helpende hande benodig. ʼn Haalbare moontlikheid om betekenisvolle hulp aan uitgeputte versorgers te verleen, is aan die hand gedoen, naamlik om hulle uit die werkomgewing weg te neem. By ʼn rusplek (“retreat”) kan aan hulle die kans gegun word om te herstel, self ook weer heel te word. Hierdie vorm van herstel vir uitgeputte versorgers is kortliks hanteer en word voorgestel, maar dit is maar die eerste treë in so ’n navorsingsrigting, wat die noodsaaklikheid van verdere navorsing beklemtoon.
