Investigation of socio-demographic, clinical and genetic factors associated with blood pressure and glycaemic control among indigenous South African adult patients

Doctor Scientiae / Achieving blood pressure and glycaemic treatment targets remain a major public health challenge in individuals with hypertension and diabetes mellitus (DM). This research project was, therefore, designed to investigate the socio-demographic, clinical and genetic factors associated with blood pressure and glycaemic control among indigenous South African adult patients. The main aims of the project were as follows: (1) To assess the prevalence and socio-demographic factors associated with uncontrolled hypertension, in individuals receiving chronic care in primary healthcare facilities, based in the rural areas of Mkhondo Municipality (Study 1). (2) To investigate the association of nineteen single nucleotide polymorphisms (SNPs) with blood pressure control among adult patients treated with hydrochlorothiazide (Study 2). (3) To assess the level of association between twelve SNPs with uncontrolled blood pressure for adult patients treated with amlodipine (Study 3). (4) To examine the association of five SNPs in selected genes (ABO, VEGFA, BDKRB2, NOS3 and ADRB2) with blood pressure response to enalapril treatment, and further assess interaction patterns that influence blood pressure response (Study 4). (5) To determine the prevalence of poor glycaemic control and its influencing factors among adult patients from Mkhondo Municipality attending chronic care for DM (Study 5). (6) To evaluate the level of association between polymorphisms found in the SLC22A1, SP1, PRPF31, NBEA, SCNN1B, CPA6 and CAPN10 genes, and glycaemic response to metformin and Sulphonylureas (SU) combination therapy among South African adults with DM. Also, to investigate interaction patterns that influence glycaemic control in response to metformin and SU combination therapy (Study 6).

O presente estudo foi realizado com o objetivo de se desenvolver e validar uma metodologia analítica indicativa de estabilidade para separação e quantificação de anlodipino e seus produtos de degradação no medicamento besilato de anlodipino 5 mg por comprimido. Para tanto, realizou-se um estudo de degradação forçada, como forma de identificar os principais produtos de degradação, que podem vir a formar num estudo de estabilidade, bem como estabelecer possíveis rotas de degradação. Foi utilizada a técnica instrumental de separação, cromatografia a líquido (CL), com dois tipos de detectores: arranjo de diodos (DAD) e corona CAD (Detector de aerossol carregado), coluna cromatográfica Zorbax SB-Phenyl 1,8µm 4,6 x 50 mm Agilent® e fase móvel constituída por tampão pH3,0, metanol e acetonitrila sob condições de gradiente (75% de tampão pH 3,0, 15% de metanol e 10% de acetonitrila; no tempo de 10 minutos tem-se 20% de tampão pH3,0, 60% de metanol e 20% de acetonitrila; no tempo de 13 minutos tem-se 75% de tampão pH3,0, 15% de metanol e 10% de acetonitrila; e no tempo de 15 minutos tem-se 75% de tampão pH3,0, 15% de metanol e 10% de acetonitrila); fluxo de 1,0mL/min até 10 minutos; de 10,1 a 13 minutos tem-se fluxo de 0,5 mL/min; e de 13,1 a 15 minutos volta ao fluxo de 1,0 mL/min; e detecção em 238 nm. For this development was performed a forced degradation study, in order to identify the main degradation products that may form in a stability study, as well as establish possible degradation routes. It was used the instrumental separation technique, liquid chromatography (LC) with two types of detectors: diode array (DAD) and corona CAD (Charged Aerosol Detector), chromatographic column Zorbax SB-Phenyl 4.6 x 50 mm 1,8µm Agilent® and mobile phase of buffer pH3,0, methanol and acetonitrile under gradient conditions (75% buffer pH3,0, 15% methanol and 10% acetonitrile; 10 minutes has 20% buffer pH3,0, 60% methanol and 20% acetonitrile; 13 minutes has become 75% of buffer pH3,0, 15% methanol and 10% acetonitrile; and 15 minutes has become 75% of buffer pH3,0, 15% methanol and 10% acetonitrile); flow 1.0 mL/ min up to 10 minutes; a flow 0,5 mL/min is obtained from 10,1 to 13 minutes; and from 13,1 minutes the flows returns to 1,0 mL/min; and detection in 238 nm. L'ischémie myocardique est caractérisée par la survenue de désordres ioniques et métaboliques responsables de la perte de l'intégrité structurale et fonctionnelle cellulaire, notamment au niveau des mitochondries et, en conséquence, de l'altération de l'activité électromécanique cardiaque. Dans le cadre de cette thèse, nous nous sommes intéressés : 1) à l'étude des lésions d'ischémie-reperfusion (I/R) et des moyens pharmacologiques de cardioprotection avec amlodipine (antagoniste calcique), perindorpilate (Inhibiteur de l'enzyme de conversion). Après avoir mis en évidence les différentes altérations électrophyiologiques, hémodynamiques, structurales et fonctionnelles mitochondriales induite par un épisode d'I/R, les molécules précédemment citées sont administrées seules ou en association, en bolus IV, avant l'induction de l'ischémie myocardique par ligature de l'artère interventriculaire antérieure dans sa partie distale et à une minute seulement après la reperfusion (Etude I, n=36 porcelets domestiques) ; 2) à la cardiotoxicité aigue d'un anesthésique local en l'occurrence la lévobubivacaine à la suite d'une injection intravasculaire (IV). Cette toxicité a été évaluée dans deux situations distinctes : circulation coronaire préservée ou ischémie myocardique. Ces deux situations permettent de « mimer » respectivement une intoxication à la levobupivacaïne chez un sujet sain ou coronarien ; 3) à l'effet bénéfique éventuel des émulsions lipidiques (Intralipid®) dans les mêmes conditions expérimentales. Les émulsions lipidiques ont été administrées quelques minutes après l'injection (IV) de la levobupivacaïne (Etude II, n=48 porcelets domestiques). Ces études ont été menées, in vivo, sur des porcs anesthésiés et ventilés. Les paramètres électrophysiologiques et hémodynamiques ont été déterminés tout au long de l'expérimentation à des intervalles réguliers. A la fin des expériences, les animaux ont été euthanasiés et des fragments de ventricule gauche ont été prélevés pour l'évaluation structurale cellulaire et fonctionnelle mitochondriale / Myocardial ischemia is characterized by the development of ionic and metabolic disorders that result in the loss of the structural and functional cellular integrity, especially within mitochondria and, consequently, in alterations of cardiac electromechanical activity. In the context of this thesis, the following aspects have been investigated: (1) ischemia-reperfusion (I/R) lesions and pharmacological measures of cardioprotection involving the calcium antagonist amlodipine, and the converting enzyme inhibitor perindorpilate. After describing the various electrophysiological, hemodynamic, and mitochondrial (both structural and functional) alterations induced by I/R, amlodipine and perindorpilate were administered either alone or combined, via a bolus IV injection, prior to a distal ligation of the anterior interventricular artery, and then one minute after reperfusion (Study II; n = 36 domestic piglets); (2) the acute cardiotoxicity of the local anesthetic levobupivacaine following an IV injection. Cardiotoxic effects were evaluated in two distinctive situations: preserved coronary circulation and experimental myocardial ischemia. Using both situations allows for mimicking levobupivacaine overdose in a healthy patient or a patient with coronary disease, respectively; (3) the possibly beneficial effect of lipid emulsions (Intralipid®) in both experimental conditions. Lipid emulsions were administered a few minutes following the IV injection of levobupivacaine (Study II, n=48 domestic piglets). These investigations were conducted in vivo on piglets anesthetized and ventilated. Electrophysiological and hemodynamic parameters were measured at given intervals throughout the study. At the end of the study, the animals were sacrificed and tissue samples of the left ventricles were withdrawn to measure the structure and function of mitochondria

Avaliação comparativa dos efeitos do anlodipino e da pimobendana na insuficiência valvar crônica de mitral em cães / Comparative evaluation of the effects of amlodipine and pimobendan in chronic mitral valve regurgitation in dogs

A insuficiência valvar crônica de mitral (IVCM) é a principal cardiopatia de cães, correspondendo a 75-85% dos casos de cardiopatias. É causada pela degeneração mixomatosa da valva mitral (endocardiose de mitral) sendo, então, uma doença degenerativa adquirida e que pode ocasionar a insuficiência cardíaca congestiva (ICC). Pode acometer qualquer raça de cão, mas é mais frequentemente observada nas raças de pequeno porte, dentre as quais, Poodle miniatura, Spitz Alemão, Dachshund, Yorkshire Terrier, Chihuahua e Cavalier King Charles Spaniel. Na endocardiose de mitral, o volume sanguíneo regurgitado causa sobrecarga do lado esquerdo do coração, devido ao aumento das pressões atrial e ventricular esquerdas, seguida de dilatação e hipertrofia dessas cavidades cardíacas. A elevação da pressão ventricular esquerda pode causar hipertensão pulmonar, congestão e, em estágios avançados, edema pulmonar. A doença pode evoluir assintomática, enquanto que naqueles casos que evoluem para insuficiência cardíaca congestiva (ICC) os sintomas mais usuais são: tosse, intolerância ao exercício, dispneia e síncope. Em 2009 o colégio americano de medicina interna veterinária (American College of Veterinary Internal Medicine -ACVIM) elaborou diretrizes para o tratamento da IVCM, tendo por base a classificação funcional adaptada do American College of Cardiology. Neste trabalho foram utilizados os fármacos anlodipino e pimobendana em associação a outros usualmente indicados no tratamento da ICC em cães, segundo consenso de 2009, indicados no tratamento da ICC em cães. Dois grupos (A e B) de cães, cada um constituído por 10 pacientes com IVCM em estágio C, foram tratados com furosemida e maleato de enalapril, sendo que os animais do grupo A receberão pimobendana e os do grupo B, anlodipino. Os animais foram avaliados em diferentes momentos (T0, T30, T60) observando-se as alterações nos exames ecodopplercardiográfico e eletrocardiográfico, bem como de pressão arterial sistólica / Chronic mitral valve regurgitation is the most frequent heart disease of dogs, accounting for 75-85 % of cases of heart diseases. It is caused by myxomatous mitral valve degeneration (mitral endocardiosis), therefore, an acquired degenerative disease that can lead to congestive heart failure (CHF). It affects any breed of dog, but is most often seen in small-sized breeds, like Miniature Poodle, German Spitz, Dachshund, Yorkshire Terrier, Chihuahua, and Cavalier King Charles Spaniel. In mitral endocardiosis, regurgitated blood volume causes overload on the left side of the heart due to increased left atrial and ventricular pressures, followed by dilation and hypertrophy of these heart chambers. The increase in left ventricular pressure can cause pulmonary hypertension, congestion and, in advanced stages, pulmonary edema. The disease can be asymptomatic, but in those cases that progress to congestive heart failure (CHF), the most common symptoms are coughing, exercise intolerance, dyspnea and syncope. In 2009 the American College of Veterinary Internal Medicine (ACVIM) has developed guidelines for the treatment of IVCM, based on the adapted functional classification of the American College of Cardiology. In this paper, amlodipine and pimobendan were used in combination with other commonly used drugs indicated in the treatment of CHF in dogs. Two groups (A and B) of dogs were used, each one consisted by 10 patients in stage C. Both received furosemide and enalapril maleate and animals of group A, additionally, received pimobendan and those of group B, amlodipine. All animals were evaluated at three different times (T0, T30, T60) observing changes in echodopplercardiographic and electrocardiographic examinations, as well as systolic blood pressure

Desenvolvimento e caracterização de minicomprimidos contendo besilato de anlodipino / Development and characterization of mini-tablets containing amlodipine besylate

Este trabalho tem por objetivo desenvolver e caracterizar minicomprimidos de besilato de anlodipino para liberação imediata. Para tanto, foi realizado um estudo de pré-formulação de amostras do fármaco provenientes de diferentes fornecedores. Os resultados indicaram que o ativo é de alta solubilidade, de acordo com o Sistema de Classificação Biofarmacêutica. Além disso, não foram encontradas diferenças nas estruturas cristalinas das matérias-primas estudadas. Em seguida, foi feito um estudo de velocidade de dissolução intrínseca (VDI) do besilato de anlodipino, utilizando planejamento fatorial fracionado (33-1) cujas variáveis foram o meio de dissolução, velocidade de rotação e pressão de compactação. Verificou-se, que a VDI do besilato de anlodipino é afetada principalmente pelo meio de dissolução. Por fim, nove lotes de minicomprimidos (F1 a F9), contendo o fármaco para liberação imediata, foram produzidos de acordo com um planejamento fatorial completo 32, no qual foi avaliado o impacto do diluente e do desintegrante nas características físicas da formulação e na dissolução do ativo. Os resultados indicam que o diluente influencia no peso médio e relação diâmetro / espessura das formulações, o que é evidenciado pelas diferentes propriedades plásticas da celulose microcristalina e do fosfato de cálcio diidratado. Por outro lado, a eficiência de dissolução é uma resposta influenciada pelo uso do desintegrante. Considerando as recomendações da OMS para dissolução de medicamentos contendo fármacos de classe I ou III do Sistema de Classificação Biofarmacêutica, tal resultado indica que, somente com o uso de crospovidona ou croscarmelose, é possível alcançar uma liberação de 85% da dose em não mais do que 15 minutos, mesmo se tratando de um fármaco de alta solubilidade. / The purpose of this study was to develop and characterize immediate release mini-tablets containing amlodipine besylate. Accordingly, a pre-formulation study of samples of the drug from different suppliers was carried out. The results indicate that the active ingredient is highly-soluble, as defined by the Biopharmaceutics Classification System. Furthermore, no differences in the crystalline structures of the raw material in question were encountered. Subsequently, a study of the intrinsic dissolution rate (IDR) of amlodipine besylate was carried out using a fractional factorial design (33-1), whose variables were the dissolution medium, rotation speed and compaction pressure. We ascertained that the IDR of amlodipine besylate is mainly affected by the dissolution medium. Finally, nine batches of mini-tablets (F1 to F9) containing the drug for immediate release were produced in accordance with a complete factorial design (32), in which the impact of the diluent and the excipient on the physical characteristics of the formulation and on the dissolution of the active ingredient was assessed. The results indicate that the diluent influences the average weight and the diameter / thickeness ratio of the formulations, which is evidenced by the different plastic properties of microcrystalline cellulose and calcium phosphate dihydrate. On the other hand, dissolution efficiency is influenced by the use of the disintegrant. Considering WHO recommendations for the dissolution of medications containing class I or III drugs of the Biopharmaceutics Classification System, this result indicates that it is possible to attain 85% of the dosage dissolved in less than 15 minutes only with the use of crospovidone or croscarmellose, even for a highly-soluble drug.

Combinação de drogas para o tratamento da Hipertensão Arterial: estratégia para um melhor controle pressórico / Drugs combination for treating arterial hypertension: strategy for a better control

A taxa de controle da hipertensão arterial permanece subótima apesar dos amplos e intensos programas institucionais e o número das novas medicações. A combinação de drogas de diferentes mecanismos de ação vem se tornando uma alternativa para aumentar a redução na pressão arterial (PA) e aumentar seu controle, aumentar aderência ao tratamento e reduzir os eventos adversos. Um estudo fatorial 4X4 foi desenhado para determinar a eficácia e a segurança de telmisartana (T) mais anlodipino (A) em pacientes hipertensos estágios I e II. Pacientes hipertensos adultos (N=1461) estágios I e II (pressão arterial basal 153,212,1 &#8260;101,74,3 mm Hg) foram randomizados para 1 de 16 grupos de tratamento com T 0, 20, 40, 80 mg e A 0, 2.5, 5, 10 mg por oito semanas. A maior redução na média das pressões sistólica e diastólica foram observadas com T 80 mg mais A10 mg (- 26,4 &#8260;20,1 mm Hg; p<0,05 comparados com as monoterapias). A taxa de controle da PA foi também maior no grupo T 80mg mais A 10mg (76,5% [controle total] e 85,3% [controle da PA diastólica ]), e taxa de controle da PA >90% com esta combinação. O edema periférico maleolar foi o evento adverso mais frequente e ocorreu no grupo A 10mg (17,8%), porém, esta taxa foi marcadamente menor quando A foi usada associada com T: 11,4% (T20+A10), 6,2% (T40+ A10), e 11,3% (T80+A10). Um subestudo utilizando a monitorização ambulatorial da pressão arterial (MAPA) foi realizado na fase basal e após oito semanas de tratamento. A maior redução média das pressões nas 24 horas a partir do período basal foi registrada para a combinação de telmisartana 80 mg e anlodipino 10 mg e encontrou-se queda de 22,4/14,6 mmHg, de 11,9/6,9 mmHg para anlodipino 10 mg monoterapia e de 11,0/6,9 mmHg para telmisartana 80 mg (p< 0,001). Além disso, resultados relevantes foram também constatados numa análise post hoc de subgrupos incluindo idosos, obesos, diabéticos tipo 2 e hipertensão sistólica. A resposta anti-hipertensiva da combinação foi semelhante, independente de qualquer característica de cada subgrupo. Estes dados demonstram que telmisartana e anlodipino em combinação oferecem substancial redução e controle nas 24 horas superior às respectivas monoterapias em hipertensos estágios I e II. / The rate of control of hypertension remains suboptimal despite widespread educational programs and the increasing number of novel medications. The combination of drugs with different mechanism of action has become an alternative to improve blood pressure reduction and control, enhance adherence to the treatment and reduce adverse events. This randomized 4X4 factorial study determined the efficacy and safety of telmisartan (T) plus amlodipine (A) in hypertensive patients. Adults (N=1461) with stage 1 or 2 hypertension (baseline blood pressure (BP) 153.212.1 &#8260;101.74.3 mm Hg) were randomized to 1 of 16 treatment groups with T 0, 20, 40, 80 mg and A 0, 2.5, 5, 10 mg for 8 weeks. The greatest leastsquare mean systolic &#8260; diastolic BP reductions were observed with T80 mg plus A10 mg (- 26.4 &#8260;20.1 mm Hg; P<.05 compared with both monotherapies). BP control was also greater in the T80-mg plus A10-mg group (76.5% [overall control] and 85.3% [diastolic BP control]), and BP response rates >90% with this combination. Peripheral edema was most common in the A10-mg group (17.8%); however, this rate was notably lower when A was used in combination with T: 11.4% (T20 &#8260; A10), 6.2% (T40 &#8260; A10), and 11.3% (T80 &#8260; A10). Ambulatory BP monitoring (ABPM) was performed, at baseline and after 8 weeks of treatment; the endpoints of interest were the changes from baseline in 24-hour systolic and diastolic BP. Mean reductions from baseline in 24-hour BP for the combination of the highest doses of telmisartan 80 mg and amlodipine 10 mg were -22.4/-14.6 mmHg versus -11.9/-6.9 mmHg for amlodipine 10 mg and -11.0/-6.9 mmHg for telmisartan 80 mg (p< 0.001 for each comparison. This study also presents most of the relevant results in hypertensive patients and a post hoc analysis of subgroups including elderly, diabetics type 2, systolic hypertension and obese patients. These findings demonstrate that telmisartan and amlodipine in combination provide substantial 24 hour BP efficacy that is superior to either monotherapy in patients with stages 1 and 2 hypertension.

Verificou-se, que a VDI do besilato de anlodipino é afetada principalmente pelo meio de dissolução. Por fim, nove lotes de minicomprimidos (F1 a F9), contendo o fármaco para liberação imediata, foram produzidos de acordo com um planejamento fatorial completo 32, no qual foi avaliado o impacto do diluente e do desintegrante nas características físicas da formulação e na dissolução do ativo. Os resultados indicam que o diluente influencia no peso médio e relação diâmetro / espessura das formulações, o que é evidenciado pelas diferentes propriedades plásticas da celulose microcristalina e do fosfato de cálcio diidratado. Por outro lado, a eficiência de dissolução é uma resposta influenciada pelo uso do desintegrante. Considerando as recomendações da OMS para dissolução de medicamentos contendo fármacos de classe I ou III do Sistema de Classificação Biofarmacêutica, tal resultado indica que, somente com o uso de crospovidona ou croscarmelose, é possível alcançar uma liberação de 85% da dose em não mais do que 15 minutos, mesmo se tratando de um fármaco de alta solubilidade. / The purpose of this study was to develop and characterize immediate release mini-tablets containing amlodipine besylate. Accordingly, a pre-formulation study of samples of the drug from different suppliers was carried out. The results indicate that the active ingredient is highly-soluble, as defined by the Biopharmaceutics Classification System. Furthermore, no differences in the crystalline structures of the raw material in question were encountered. Subsequently, a study of the intrinsic dissolution rate (IDR) of amlodipine besylate was carried out using a fractional factorial design (33-1), whose variables were the dissolution medium, rotation speed and compaction pressure. We ascertained that the IDR of amlodipine besylate is mainly affected by the dissolution medium. Finally, nine batches of mini-tablets (F1 to F9) containing the drug for immediate release were produced in accordance with a complete factorial design (32), in which the impact of the diluent and the excipient on the physical characteristics of the formulation and on the dissolution of the active ingredient was assessed. The results indicate that the diluent influences the average weight and the diameter / thickeness ratio of the formulations, which is evidenced by the different plastic properties of microcrystalline cellulose and calcium phosphate dihydrate. On the other hand, dissolution efficiency is influenced by the use of the disintegrant. Considering WHO recommendations for the dissolution of medications containing class I or III drugs of the Biopharmaceutics Classification System, this result indicates that it is possible to attain 85% of the dosage dissolved in less than 15 minutes only with the use of crospovidone or croscarmellose, even for a highly-soluble drug.

A insuficiência valvar crônica de mitral (IVCM) é a principal cardiopatia de cães, correspondendo a 75-85% dos casos de cardiopatias. É causada pela degeneração mixomatosa da valva mitral (endocardiose de mitral) sendo, então, uma doença degenerativa adquirida e que pode ocasionar a insuficiência cardíaca congestiva (ICC). Pode acometer qualquer raça de cão, mas é mais frequentemente observada nas raças de pequeno porte, dentre as quais, Poodle miniatura, Spitz Alemão, Dachshund, Yorkshire Terrier, Chihuahua e Cavalier King Charles Spaniel. Na endocardiose de mitral, o volume sanguíneo regurgitado causa sobrecarga do lado esquerdo do coração, devido ao aumento das pressões atrial e ventricular esquerdas, seguida de dilatação e hipertrofia dessas cavidades cardíacas. A elevação da pressão ventricular esquerda pode causar hipertensão pulmonar, congestão e, em estágios avançados, edema pulmonar. A doença pode evoluir assintomática, enquanto que naqueles casos que evoluem para insuficiência cardíaca congestiva (ICC) os sintomas mais usuais são: tosse, intolerância ao exercício, dispneia e síncope. Em 2009 o colégio americano de medicina interna veterinária (American College of Veterinary Internal Medicine -ACVIM) elaborou diretrizes para o tratamento da IVCM, tendo por base a classificação funcional adaptada do American College of Cardiology. Neste trabalho foram utilizados os fármacos anlodipino e pimobendana em associação a outros usualmente indicados no tratamento da ICC em cães, segundo consenso de 2009, indicados no tratamento da ICC em cães. Dois grupos (A e B) de cães, cada um constituído por 10 pacientes com IVCM em estágio C, foram tratados com furosemida e maleato de enalapril, sendo que os animais do grupo A receberão pimobendana e os do grupo B, anlodipino. Os animais foram avaliados em diferentes momentos (T0, T30, T60) observando-se as alterações nos exames ecodopplercardiográfico e eletrocardiográfico, bem como de pressão arterial sistólica / Chronic mitral valve regurgitation is the most frequent heart disease of dogs, accounting for 75-85 % of cases of heart diseases. It is caused by myxomatous mitral valve degeneration (mitral endocardiosis), therefore, an acquired degenerative disease that can lead to congestive heart failure (CHF). It affects any breed of dog, but is most often seen in small-sized breeds, like Miniature Poodle, German Spitz, Dachshund, Yorkshire Terrier, Chihuahua, and Cavalier King Charles Spaniel. In mitral endocardiosis, regurgitated blood volume causes overload on the left side of the heart due to increased left atrial and ventricular pressures, followed by dilation and hypertrophy of these heart chambers. The increase in left ventricular pressure can cause pulmonary hypertension, congestion and, in advanced stages, pulmonary edema. The disease can be asymptomatic, but in those cases that progress to congestive heart failure (CHF), the most common symptoms are coughing, exercise intolerance, dyspnea and syncope. In 2009 the American College of Veterinary Internal Medicine (ACVIM) has developed guidelines for the treatment of IVCM, based on the adapted functional classification of the American College of Cardiology. In this paper, amlodipine and pimobendan were used in combination with other commonly used drugs indicated in the treatment of CHF in dogs. Two groups (A and B) of dogs were used, each one consisted by 10 patients in stage C. Both received furosemide and enalapril maleate and animals of group A, additionally, received pimobendan and those of group B, amlodipine. Combinação de drogas para o tratamento da Hipertensão Arterial: estratégia para um melhor controle pressórico / Drugs combination for treating arterial hypertension: strategy for a better control

A taxa de controle da hipertensão arterial permanece subótima apesar dos amplos e intensos programas institucionais e o número das novas medicações. A combinação de drogas de diferentes mecanismos de ação vem se tornando uma alternativa para aumentar a redução na pressão arterial (PA) e aumentar seu controle, aumentar aderência ao tratamento e reduzir os eventos adversos. Um estudo fatorial 4X4 foi desenhado para determinar a eficácia e a segurança de telmisartana (T) mais anlodipino (A) em pacientes hipertensos estágios I e II. Pacientes hipertensos adultos (N=1461) estágios I e II (pressão arterial basal 153,212,1 &#8260;101,74,3 mm Hg) foram randomizados para 1 de 16 grupos de tratamento com T 0, 20, 40, 80 mg e A 0, 2.5, 5, 10 mg por oito semanas. A maior redução na média das pressões sistólica e diastólica foram observadas com T 80 mg mais A10 mg (- 26,4 &#8260;20,1 mm Hg; p<0,05 comparados com as monoterapias). A taxa de controle da PA foi também maior no grupo T 80mg mais A 10mg (76,5% [controle total] e 85,3% [controle da PA diastólica ]), e taxa de controle da PA >90% com esta combinação. O edema periférico maleolar foi o evento adverso mais frequente e ocorreu no grupo A 10mg (17,8%), porém, esta taxa foi marcadamente menor quando A foi usada associada com T: 11,4% (T20+A10), 6,2% (T40+ A10), e 11,3% (T80+A10). Um subestudo utilizando a monitorização ambulatorial da pressão arterial (MAPA) foi realizado na fase basal e após oito semanas de tratamento. A maior redução média das pressões nas 24 horas a partir do período basal foi registrada para a combinação de telmisartana 80 mg e anlodipino 10 mg e encontrou-se queda de 22,4/14,6 mmHg, de 11,9/6,9 mmHg para anlodipino 10 mg monoterapia e de 11,0/6,9 mmHg para telmisartana 80 mg (p< 0,001). Além disso, resultados relevantes foram também constatados numa análise post hoc de subgrupos incluindo idosos, obesos, diabéticos tipo 2 e hipertensão sistólica. A resposta anti-hipertensiva da combinação foi semelhante, independente de qualquer característica de cada subgrupo. Estes dados demonstram que telmisartana e anlodipino em combinação oferecem substancial redução e controle nas 24 horas superior às respectivas monoterapias em hipertensos estágios I e II. / The rate of control of hypertension remains suboptimal despite widespread educational programs and the increasing number of novel medications. The combination of drugs with different mechanism of action has become an alternative to improve blood pressure reduction and control, enhance adherence to the treatment and reduce adverse events. This randomized 4X4 factorial study determined the efficacy and safety of telmisartan (T) plus amlodipine (A) in hypertensive patients. Adults (N=1461) with stage 1 or 2 hypertension (baseline blood pressure (BP) 153.212.1 &#8260;101.74.3 mm Hg) were randomized to 1 of 16 treatment groups with T 0, 20, 40, 80 mg and A 0, 2.5, 5, 10 mg for 8 weeks. The greatest leastsquare mean systolic &#8260; diastolic BP reductions were observed with T80 mg plus A10 mg (- 26.4 &#8260;20.1 mm Hg; P<.05 compared with both monotherapies). BP control was also greater in the T80-mg plus A10-mg group (76.5% [overall control] and 85.3% [diastolic BP control]), and BP response rates >90% with this combination. Peripheral edema was most common in the A10-mg group (17.8%); however, this rate was notably lower when A was used in combination with T: 11.4% (T20 &#8260; A10), 6.2% (T40 &#8260; A10), and 11.3% (T80 &#8260; A10). Ambulatory BP monitoring (ABPM) was performed, at baseline and after 8 weeks of treatment; the endpoints of interest were the changes from baseline in 24-hour systolic and diastolic BP. Mean reductions from baseline in 24-hour BP for the combination of the highest doses of telmisartan 80 mg and amlodipine 10 mg were -22.4/-14.6 mmHg versus -11.9/-6.9 mmHg for amlodipine 10 mg and -11.0/-6.9 mmHg for telmisartan 80 mg (p< 0.001 for each comparison. This study also presents most of the relevant results in hypertensive patients and a post hoc analysis of subgroups including elderly, diabetics type 2, systolic hypertension and obese patients. These findings demonstrate that telmisartan and amlodipine in combination provide substantial 24 hour BP efficacy that is superior to either monotherapy in patients with stages 1 and 2 hypertension.
