\"Detecção de esteróides androgênicos anabólicos por GC/MS em urina de esportistas e alterações séricas bioquímicas e hormonais\" / Detection of anabolic steroids by GC/MS in urine and correlation with biochemistry and hormonal alterations

A cromatografia gasosa acoplada a espectrometria de massa (GC/MS) é na atualidade a técnica analítica de escolha para a detecção de esteróides androgênicos anabólicos (EAA) em amostras biológicas. Os EAA, apesar de determinarem alterações endócrinas e bioquímicas graves em seus usuários, são amplamente utilizados no âmbito esportivo. O presente trabalho teve como objetivo a validação de um método para a detecção de EAA e/ou seus produtos de iotransformação em urina e avaliação de seus efeitos tóxicos sobre os parâmetros séricos laboratoriais. O estudo foi realizado em 20 voluntários freqüentadores de academias da cidade de São Paulo (SP), os quais responderam a um questionário e cederam amostras de urina e sangue. Os dados obtidos dos 10 voluntários usuários de EAA (teste) foram estatisticamente comparados com os de 10 voluntários não usuários (controle). O método analítico validado apresentou-se adequado para a aplicação em programas de controle de dopagem no esporte e os dados séricos demonstraram aumento nos níveis de LDL (178,2 vs 117,6 mg/dL; p< 0.001) e queda nos de HDL (13,8 vs 44,6 mg/dL; p< 0.001), LH (0,45 vs 2,69 mUI/dL; p< 0.001) e FSH (0,67 vs 3,88 mUI/dL; p< 0.001) nos voluntários usuários de EAA. Os questionários demonstraram diferentes padrões de uso para os EAA, os quais eram obtidos sem receita médica em farmácias ou em lojas de suplementos alimentares e utilizados em associação com outros fármacos ou drogas de abuso. Os resultados desse trabalho demonstram alterações no perfil lipoprotéico e supressão do eixo hipofisário em usuários crônicos de EAA, além da necessidade da adoção de ações educativas junto à população expostas a tais substâncias. / Gas chromatography-mass spectrometry (GC/MS) has been applied as standard analytical method for detection of anabolic steroids (AAS) in biological samples. The AAS have been used extensively in sports, despite of their reported toxicological effects in biochemistry and endocrine profile. The aim of this study was the validation of an analytical method for the detection of AAS and their metabolites in urine samples and evaluation of their effects on blood laboratories parameters. The study was conducted with 20 volunteers of professionally equipped private gym in São Paulo. Urine and blood samples were drawn and a questionnaire was answered. The data from 10 active users of AAS (test group) was statistically compared to 10 nonusers (control group). The optimized method was shown to be suitable for sports antidoping control programs and significant differences were found in laboratory parameters of test group: increase of LDL (178,2 vs 117,6 mg/dL; p< 0.001) and reduction in HDL (13,8 vs 44,6 mg/dL; p< 0.001), LH (0,45 vs 2,69 mUI/dL; p< 0.001) and FSH (0,67 vs 3,88 mUI/dL; p< 0.001) serum levels. The questionnaire demonstrated distinct user\'s profile of AAS, which were bought in drugstores or food supplemental stores and were frequently used in association with other medicines and drugs of abuse. These results indicate that chronic anabolic steroids intakes cause an alteration in serum lipoprotein profile and suppression of the hypothalamus-pituitary glandgonad axis as well as the necessity of educational programs to people exposed to AAS.

Pesquisa do Estado da Bahia - FAPEB / The use of anabolic androgenic steroids (AAS) has grown among practitioners of recreational bodybuilding, with significant contributions of "Designer Steroids" (DS), EAA designed aiming muscle hypertrophy in healthy subjects. The abusive use of AAS in general is associated with adverse effects, one of the most worrisome is cancer development. Given that cancer is a genetic disease resulting from changes in genes critical in maintaining genomic stability and in the control of proliferation and differentiation, biomarkers able to identify these changes can take predictive value and contribute to reducing the high rates of morbidity and mortality by this disease. The aim of this study was to evaluate the effectiveness of the Cytokinesis Block Micronucleus Test (CBMN) in human lymphocytes in identifying risk groups for cancer development in users of AAS. Was collected 5ml of blood from 15 AAS users bodybuilders (G1), 20 nonusers bodybuilders (G2) and 20 nonusers sedentary (G3). Lymphocytes were cultured with blocking of cytokinesis and subsequent processing for making slides. MN analysis was performed on a minimum of 1000 binucleated lymphocytes. The occurrence of MN was significantly higher (p <0.05) in individuals of G1 compared to G2 and G3. The results indicate the sensitivity of CBMN in human lymphocytes in the identification of chromosomal damage in consequence of AAS using, however, further studies are needed to determine their potential to predict the cancer risk in users of AAS. / O uso de esteroides anabolizantes androg?nicos (EAA) tem crescido entre praticantes recreativos de muscula??o, com contribui??es significativas dos Designer Steroids (DS), EAA projetados para fins de hipertrofia muscular em indiv?duos saud?veis. O uso abusivo de EAA em geral est? associado a efeitos adversos, sendo um dos mais preocupantes o desenvolvimento de c?ncer. Among the many chemicals that have been identified as mutagenic action, include the Androgenic Steroids (AAS), hormones widely used in the search for improved physical performance and increased muscle mass. Objective: In this context, the aim of the development of this study was to evaluate the potential of androgenic anabolic steroid nandrolone decanoate, testosterone propionate and testosterone cypionate to induce chromosome damage, apoptosis and necrosis, using the micronucleus test in exfoliated cells from the oral mucosa of users of AAS with a view to its application as a tool in cancer prevention. Method: The study sample consisted of 55 volunteers, male, divided into two (02) groups, matched for age: 25 subjects (G1) users of nandrolone decanoate, testosterone propionate and testosterone cypionate (alone or simultaneously ) and 30 subjects in the control group (G2). The collection methodology and cytological analysis followed the protocol of Tolbert et al. (1992) and Thomas et al. (2009), which includes, in addition to micronuclei, the computation of degenerative nuclear changes indicative of apoptosis (cariorr?xis, condensed chromatin and pyknosis) and necrosis (karyolysis, cariorr?xis, condensed chromatin and pyknosis). Statistical analysis of the endpoints analyzed (micronucleus, cariorr?xis, condensed chromatin, karyolysis, pyknosis and broken eggs) was performed using the conditional test to compare proportions in situations of rare events. Results: Statistical analysis revealed no significant difference in the occurrence of micronuclei, karyolysis and broken eggs between groups. The occurrence of apoptosis was significantly greater in cells from control individuals. Strenght training and anabolic steroids : a comparative study of the trapezius, a shoulder muscle and the vastus lateralis, a thigh muscle, of strength trained athletes

Strength training is widely used to increase performance in sports with high physical demands. The use of drugs such as anabolic steroids among athletes is a wellknown phenomenon, and the effects of these drugs on physical performance documented. The studies presented in this thesis focused on the mechanisms of muscle fiber hypertrophy in the vastus lateralis and the trapezius muscles of strength trained elite athletes. The main hypothesis was that the muscle adaptations to strength training and anabolic steroids are muscle specific. Biopsies were obtained from the trapezius and the vastus lateralis from three groups of elite power lifters. Nine used drugs, ten did not and seven had previously used drugs. Six sedentary males served as controls. The biopsies were frozen and cut in serial cross sections. Histological and immunohistochemical staining techniques were used to analyze muscle fiber morphology and pathology. Fiber type distribution, fiber area, myonuclei number and distribution, satellite cell number and proportion of split fibers were counted and compared for the two muscles within and between the groups. The main findings were that: a) Muscle fiber hypertrophy by strength training is further increased by anabolic steroids. b) The number of nuclei per muscle fiber is higher in power lifters using anabolic steroids compared to non-steroids using lifters. c) Among power lifters who have withdrawn from anabolic steroid usage and training for several years, the number of myonuclei, both subsarcolemmal and internal, remains high. d) In active power lifters, anabolic steroids have no further effect on the number of satellite cells per fiber. e) Power lifters have a high proportion of split fibers. High intensity resistance training increases muscle strength and banned substances such as testosterone and anabolic steroids can enhance the training effects. The studies on muscle cell morphology presented in this thesis reveals that anabolic steroids and testosterone increases muscle fiber size and adds more nuclei to the muscle cell. Based on the morphological appearance of muscle sections from doped and nondoped power lifters, we conclude that testosterone and anabolic steroids enhances the hypertrophic effects of training without adding new features. The addition of myonuclei by training and doping appears to be longer lasting in some muscles than in others. The high proportion of split fibers in power lifter is probably due to high mechanical stress. The findings and conclusions in this thesis raise questions regarding relevant suspension times for athletes caught with banned substances in the body.

The study is based on the perception of AAS as a constructed social problem and analyzes the discourses surrounding AAS depicted in three Swedish newspapers. The sample was prepared on the basis that it represents different aspects of daily Swedish press. The method used in the paper is a discourse analysis with social constructivism and gender as theoretical tools used to analyze the sample material. An important conclusion of the analysis is that the use of AAS is a socially constructed problem, which incorporates both a stereotyped, aggressive male individual who desires an ideal body, and the use of AAS defined as an illegitimate abuse by society. Another important aspect of the analysis is how gender roles are highlighted in the sample and how these from a gender perspective are used to perpetuate male superiority and female subordination in society.

BEHAVIORAL INTENTIONS AND NONMEDICAL ANABOLIC STEROID USE AMONG NON INTERCOLLEGIATE ATHLETE MALES AGES 18-30

The purpose of this study was to increase understanding of factors associated with nonmedical anabolic steroid use among males ages 18-30 who do not participate in intercollegiate athletics. The Behavioral Intentions and Ergogenic Aid/Performance Enhancer use among non-intercollegiate athlete males survey instrument was developed, reviewed for content validity by a jury of experts, and pilot tested. The pilot testing results (n=25) demonstrated acceptable reliability (Cronbach’s alpha= 0.74). The final version of the Behavioral Intentions and Ergogenic Aid/Performance Enhancer use among non-intercollegiate athlete males survey instrument was administered at two distribution sites which included Ford’s Fitness Center in Lexington, Kentucky and the Johnson Center on the University of Kentucky’s campus to non-intercollegiate athlete men between the ages of 18-30 (n=121). The final version of the survey instrument was also found to be reliable (Cronbach’s alpha= 0.86).Of the 121 respondents, 7 (5.9%) reported using nonmedical anabolic steroids at least 1-2 days a week or more. A total of 9 (7.4%) men reported intending to use nonmedical anabolic steroids within the next year. Age was found to have a statistically significant association with intention to use nonmedical anabolic steroids (p=.037).Perceived behavioral control (p=.029) was found to be the strongest predictor variable of study participants’ intention to use nonmedical anabolic steroids. Muscle mass builder use (p=.011) and muscle mass builder use in combination with multivitamin use (p=.000) were found to be significant predictors of actual nonmedical anabolic steroid use. Study participants were more likely to use nonmedical anabolic steroids if they were currently using a muscle mass builder or using a muscle mass builder in combination with a multivitamin. No decision about the effectiveness of the components of perceived behavioral control (self-efficacy and control) as a one or two part construct was possible because of the small number of study participants. Two additional demographic predictor variables were found to be statistically significant with predicting the intention to use nonmedical anabolic steroids. Being a competitive bodybuilder (p=.001) was positively correlated and being satisfied with body image (p=.025) was negatively correlated with the intention to use nonmedical anabolic steroids.

Los agentes anabolizantes prohibidos por la Comisión Médica del Comité Olímpico Internacional (COI) deben estar ausentes en las muestras de orina objeto de análisis. El límite de detección está condicionado por la sensibilidad del instrumento y la relación señal/ruido debida al material biológico coextraído junto con los analitos de interés. Las nuevas exigencias de sensibilidad del COI como la detección de nuevas sustancias utilizadas en dosis sensiblemente inferiores, obliga al replanteamiento de la estrategia analítica utilizada. El objetivo general de esta tesis es: desarrollar nuevas técnicas analíticas de alta sensibilidad para la detección de esteroides y otros anabolizantes en orina humana mediante: 1.- Estudio de la cromatografía liquida capilar para la cuantificación de las concentraciones de testosterona y epitestosterona en orina humana. 2.- Estudio de la purificación de la muestra previa al análisis instrumental. - Estudio del uso de la cormatografía de inmunoafinidad (IAC) en la purificación selectiva dela muestra. - Desarrollo de procedimientos mediante cromatografía líquida de alta resolución para la purificación de fracciones en los análisis de confirmación. 3.- Incremento de la sensibilidad en el análisis instrumental. - Estudio de la formación de tert-butildimetilsilil derivados como alternativa a los trimestilsilil derivados habitualmente utilizados. - Estudio de la utilización de nueva instrumentación basada en la espectrometría de masas de trampa de iones y en tándem (MS/MS) para alcanzar la sensibilidad y selectividad requerida por los nuevos estánderes impuestos por las organizaciones internacionales del área de la salud, en particular en el deporte.

A utilização não-terapêutica de drogas anabolizantes está documentada na literatura médica como prejudicial à saúde e, na legislação brasileira, como infração penal ante à Lei das Drogas. Apesar dos alertas em diferentes recantos da mídia, cresce o uso destes medicamentos em meio à população brasileira. Entre os usuários, a maioria deles jovens praticantes de musculação, é possível perceber que muitos não apenas usam tais drogas com o propósito de obter ganho muscular, mas também desenvolvem conhecimentos cada vez mais sofisticados, e de várias ordens, sobre os diferentes efeitos dos anabolizantes no corpo humano visando seu melhor uso. Dada à relevância social deste tema e da relação direta com o campo da Educação Física, o presente relatório de pesquisa versa acerca do empreendimento de uma investigação strito sensu que buscou compreender o processo de aquisição, elaboração e compartilhamento dos conhecimentos acerca do uso de anabolizantes entre estudantes de graduação em Educação Física. O marco teórico que subsidiou este estudo contempla, principalmente, as discussões sobre as formulações de Nikolas Rose acerca das biopolíticas contemporâneas (políticas da própria vida), em articulação com os conceitos de enhancement corporal e expertise, além de também versar acerca da teoria da biossociabilidade de Paul Rabinow e da bioascese de Francisco Ortega. A parte mais densa do material empírico foi obtida a partir de entrevistas episódicas, entretanto, sites da Internet também serviram como fontes de informação complementar no processo de identificação sobre como ocorre a organização e sistematização do aprendizado em ambiente virtual. Os entrevistados descreveram o aprendizado sobre a utilização de medicamentos anabolizantes como algo considerado “proibido” frente aos elementos éticos e jurídicos e envolto em um clima de clandestinidade na formação inicial. A partir das entrevistas foi possível identificar que o aprendizado sobre a utilização dos medicamentos anabolizantes é uma temática que apenas tangencia o percurso curricular formal dos acadêmicos de um curso de Educação Física. Também foi possível identificar, a partir do material empírico, que além da dimensão formal de aprendizagem na Educação Física, estes estudantes encontram, fora da graduação, as seguintes dimensões de aprendizagem sobre o tema: as academias de musculação, os fóruns virtuais da Internet e os coaches, considerados os guias da construção corporal por meio do uso do anabolizante. Há também uma última dimensão de aprendizagem que consiste no momento em que os sujeitos têm sua expertise reconhecida pelos pares do grupo quando, então, tornam-se coaches, passando a aplicar os conhecimentos sobre as modificações corporais não apenas em si mesmos, mas também em outras pessoas. / The non-therapeutic use of anabolic drugs is documented in the medical literature as harmful to health and, in Brazilian legislation, as a criminal offense. In spite of warnings in different media spaces, there is an increased use of these drugs among the Brazilian population. Among the users, most of them young bodybuilders, we observed that many not only use these drugs in order to develop muscle hypertrophy, but also acquire increasingly sophisticated knowledge, from several orders, about the different effects of anabolic drugs on the human body aiming at a safer use. Taking into account the social importance of this theme and its direct relation with the Physical Education area, this study aimed to understand the process of acquisition, development and sharing of knowledge about the use of anabolic steroids among undergraduate students of Physical Education. The theoretical framework that supports this study includes mainly discussions on the formulation by Nikolas Rose about contemporary biopolitics of life, in conjunction with de concepts of body enhancement and expertise, and also discusses about the biosociality theory by Paul Rabinow and the bioasceticism by Francisco Ortega. The densest element of the empirical material was obtained from episodic interviews; however, websites also served as data sources in order to identify how learning is organized on the internet. Interview participants described learning about the use of anabolic drugs as something considered "forbidden" in face of ethical and legal elements, and surrounded by a clandestine environment in initial training. From the interviews, we could identify that learning about the use of anabolic steroids is an issue that only touches the formative trajectory of the undergraduate students of Physical Education. We also identified, from the empirical data, that, besides the formal dimension of learning in Physical Education, these students find, outside the undergraduate course, the following learning dimensions about the theme: gyms, virtual forums, and the coaches. The coaches are the last level of learning about the use of anabolic steroids. In this stage, the subjects have their expertise recognized by their peers.

This research aims to understand, within a sociological perspective, the relationship of confidence and fear in the uses of anabolic steroid hormones, as well as their own motivations for the practice of bodybuilding and the use of these drugs masculinizing. Given the above, it is clear that in contemporary society, the body became a central element in the lives of many individuals, this reality, the result of the phenomenon of the cult of the body that in recent decades took radicalized forms. In this context, while the corpolatria intensifies, paradoxically grows in the same proportions, the people's dissatisfaction with their body image, especially when compared to ideal body widely socially valued. This situation is aggravated in a society in which we are charged, tried, identified and classified by body image. Thus, the overestimation of body image is causing many people to commit excesses in the use of many different ways in the pursuit of body aesthetically "perfect." These excesses are increasingly "blind", a symptom of the culture of presenteeism, which maintains the "now" as most important time for those seeking the transformation of body shape. These phenomena affect many individuals to embark in the underworld of the use of anabolic steroids with intent to build a body "healed" in a short time. In these circumstances the trust and fear is particularly important, because from these two elements, individuals direct their actions and speeches for use or not use of these substances. / O presente trabalho versa sobre o problema do uso intensivo dos hormônios esteroides anabólicos androgênicos (EAAs), também conhecidos popularmente como esteroides anabolizantes, por homens frequentadores de academia de ginástica de um bairro popular na cidade de João Pessoa. Esta pesquisa tem como objetivo, compreender, dentro de uma perspectiva sociológica, a relação de confiança e medo nos usos dos hormônios esteroides anabolizantes, bem como, as próprias motivações para a prática da musculação e a utilização destas drogas masculinizantes. Diante do exposto, percebe-se que na sociedade contemporânea, o corpo tornou-se elemento central na vida de muitos indivíduos, realidade esta, fruto do fenômeno do culto ao corpo que nas últimas décadas tomou formas radicalizadas. Neste contexto, ao mesmo tempo em que a corpolatria se intensifica, paradoxalmente cresce, nas mesmas proporções, a insatisfação das pessoas com sua imagem corporal, principalmente quando comparados aos ideais corpóreos amplamente valorizados socialmente. Esta realidade se agrava em uma sociedade em que somos cobrados, julgados, classificados e identificados pela imagem corporal. Assim, a supervalorização da imagem corporal está fazendo com que muitas pessoas cometam excessos na utilização dos mais diferentes meios na busca pelo corpo esteticamente perfeito . Estes excessos estão cada vez mais cegos , sintoma da cultura do presenteísmo, onde sustenta o agora , como tempo mais importante para os que procuram a transformação da forma corporal. Tais fenômenos condicionam muitos indivíduos a embarcarem no submundo da utilização dos esteroides anabolizantes com a intenção de construir um corpo sarado em um curto espaço de tempo. Nestas circunstâncias a relação de confiança e medo tem importância particular, pois, a partir destes dois elementos (e também de outros), os indivíduos direcionam suas ações e discursos para o uso ou não uso destas substâncias.
