As gatas foram tratadas com 0,5 mg/kg de vedaprofeno (GV e GVT) ou gel placebo (GT e GP), via oral, uma hora antes da indução anestésica, 24 e 48h após o primeiro tratamento e tramadol 2 mg/kg (GT e GVT) ou solução salina (GV e GP), via subcutânea, uma hora antes da indução anestésica e a cada 8h por 72h. A anestesia foi induzida com propofol (8 mg/kg, IV) e mantida com isofluorano (1,8 ± 0,35 % expirado) em oxigênio. O escore de dor, avaliado pela escala de contagem variável (ECV) e escala analógica visual (EAV), e a hiperalgesia, determinada pelo limiar mecânico nociceptivo por meio dos filamentos de von Frey, foram registrados no período pré-operatório e nas 1, 2, 4, 6, 8, 12, 24, 28, 32, 48, 52, 56, 72 e 96 horas e no 7º dia após a cirurgia. Quando o escore da ECV atingiu pontuação acima de 33% do valor máximo da escala foi administrado 0,5 mg/kg de morfina IM, como analgésico de resgate. A concentração sérica de cortisol foi mensurada nos períodos pré, transoperatório e 1, 4, 8, 24 e 48 horas de pós-operatório. Estudo cardiorrespiratório comparativo da anestesia geral inalatória com o sevofluorano em cadela não-gestantes e no terço final da gestação, pré-tratados com acepromazina e propofol /

Orientador: Valéria Nobre Leal de Souza Oliva. / Resumo: Amplos conhecimentos existem a respeito da anestesia em mulheres gestantes, mas na medicina veterinária há escassez de informação científica a respeito deste tema. Este trabalho estudou as alterações cardiocirculatórias e respiratórias decorrentes da gestação em cadelas, comparando os efeitos da anestesia inalatória na cadela não-gestante e aos 45 dias de gestação. Foram avaliados os efeitos da anestesia inalatória com a utilização do sevofluorano por meio do monitoramento dos sistemas cardiorrespiratório da mãe e cardíaco dos fetos, além da realização de exames laboratoriais como: hemogasometria, hemograma e bioquímicos séricos. Para tanto foram utilizadas nove cadelas, sem raça definida, adultas, com 1 a 5 anos de idade. Os animais foram submetidos ao mesmo procedimento anestésico utilizando os fármacos acepromazina, propofol e sevofluorano, em dois períodos distintos, sendo o primeiro período na condição não-gestante e o segundo aos 45 dias de gestação. Não ocorreram alterações significativas na f, ETCO2, SatO2, nas variáveis hemogasométricas com exceção do pH, na concentração de sevofluorano no vaporizador e no tempo de recuperação anestésica entre os grupos ou entre os momentos. A freqüência cardíaca materna elevou-se na paciente gestante apenas no momento basal e 15 minutos após a MPA. Observou-se diminuição significativa da pressão arterial na paciente gestante, assim como da temperatura retal. Os valores de eritrócitos, hemoglobina, volume globular e albumina também diminuíram significativamente na paciente com 45 dias de gestação. Concluiu-se que o protocolo anestésico com o sevofluorano causa poucas alterações nas variáveis cardiocirculatórias e respiratórias na cadela com 45 dias de gestação, podendo ser utilizado no caso de necessidade de intervenção cirúrgica e/ou anestésica. / Abstract: Extensive human data have been published about anesthesia in pregnant women, but in veterinary medicine there are few articles about this subject. The aim of this study was to investigate the cardiocirculatory and respiratory alterations produced by pregnancy in bitches, comparing the effects of inhalatory anesthesia in nonpregnant bitch and that with 45 days of pregnancy. The effects of inhalatory anesthesia with sevoflurane were measured through the monitoring of cardiorespiratory systems of the mother and cardiac system of the fetus and by arterial blood samples for evaluate the acid-base and gases values, venous blood samples for haematology and serum biochemistry. For this were used 9 adults mongrel bitches from 1 to 5 years old. These animals were submitted to the same anesthetic procedure with acepromazine, propofol and sevoflurane in two different times. The first one was in nonpregnant bitch and the second in 45 days of pregnancy. There were no important differences in respiratory rate, ETCO2, pulse oximetry, some hemogasometric parameters (PaCO2, PaO2, HCO3-,CO2T, SatO2, BE), anesthetic concentration of sevoflurane and time of recovery of anesthesia either in time of each group, or among the groups. The arterial blood pressure, rectal temperature, red cell mass, hemoglobin, packed cell volume, albumin reduced in pregnant patient with 45 days of pregnancy. It was also noticed a marked increase in heart rate of mother during the first moments of the study. It was concluded that anesthetic protocol with sevoflurane cause few alterations in the cardiocirculatory and respiratory parameters in bitch with 45 days of pregnancy, which can used in case of need surgical and/or anaesthetic procedure to pregnant bitch and to the fetuses. / Mestre

O objetivo do presente estudo foi avaliar o efeito de três doses de sufentanil ,administradas em infusão contínua, na CAMiso em felinos. Oito gatos adultos e castrados (4.0±0.5 kg-1) foram anestesiados com isofluorano em três ocasiões distintas com um intervalo mínimo de sete dias mantidos em ventilação mecânica. A temperatura esofágica foi mantida na faixa de 38.5 a 39.0°C. Uma das três doses de sufentanil (0.01; 0.025 e 0.05 μg kg-1 minuto-1) foi selecionada aleatoriamente e infundida em cada experimento. A CAMiso basal individual, a CAMiso durante a infusões e a CAMiso após uma hora do término da infusão foram determinadas em todos os experimentos. As infusões eram feitas por 60 minutos antes da determinação da CAMISO em cada dose de sufentanil. As determinações da CAMISO foram realizadas de modo duplicado usando estímulo elétrico (50 V, frequência 50, 10 ms) aplicados nos antebraço. FC, PAS, PAM, PAD, EtCO2 e análise hemogasométrica eram registrados antes de cada determinação da CAMiso. Dados (média ± SD) foram analisados por ANOVA seguido do teste de Tukey (p<0.05%). Os valores de CAMbasal não diferiram estatisticamente (1.64±0.13; 1.61±0.24 e 1.62±0.31%). As infusões de sufentanil (0.01; 0.025 e 0.05 μg μg kg-1 minuto-1) reduziram significativamente a CAMISO 21.4±10.8; 18.5±10.5 e 18.0±13.7%, respectivamente. Não houve diferença significativa entre os valores de CAMISO durante as infusões.Os valores de CAMISO controle (1.56±0.26; 1.50±0.22 e 1.53±0.26%) foram inferiores da CAMISO basal indicando um possível efeito residual do opioide após a descontinuação da infusão. Os valores de FC e das pressões arteriais aumentaram durante as infusões de sufentanil 0.025 e 0.05 μg kg-1 minuto-1. As três doses de sufentanil infundidas resultaram em graus semelhantes... / Opioids reduce the MAC of Inhalants agents in many species. The aim of this study was to evaluate the effects of three sufentanil constant rate infusions (CRIs) on the MACISO in cats. Eight adult spayed cats (4.0±0.5 kg-1) were anesthetized with isoflurane under mechanical ventilation on three occasions with a minimum 7-day interval between. Esophageal temperature was maintained within a narrow range (38.5 to 39.0°C). One of three sufentanil CRIs (0.01; 0.025 and 0.05 μg kg-1 minute-1) was randomly selected to be administered on each day of MACISO determinations. On all study days, the individual basal MACISO (MACbasal), the MACISO during one of the infusion rates, and the control one hour post-infusion MAC (MACcontrol) were determined. CRI was continued for 60 minutes before MACISO was determined for each sufentanil dose. The MACISO determinations were performed in duplicate using an electrical stimulus (50V, 50 cycles second-1, 10 ms) applied to the antebrachium. HR, SAP, MAP, DAP, ETCO2, and arterial blood gases were recorded before and after each MACISO determination. Data (mean±SD) were compared by ANOVA followed by Tukey’s test (p<0.05%).MACbasal values were not significantly different (1.64±0.13; 1.61±0.24 and 1.62±0.31%). Sufentanil CRIs (0.01; 0.025 and 0.05 μg kg-1 minute-1) significantly reduced MACISO by 21.4±10.8; 18.5±10.5 and 18.0±13.7%, respectively. There were no significant differences between the MACISO values determined during the CRIs. Efeitos do sevofluorano e do desfluorano sobre variáveis intracranianas e hemodinâmicas em cães

Em seguida, administrou-se a dose inicial de rocurônio (0,6 mg/kg/IV) e ato contínuo, iniciou-se a infusão contínua do miorrelaxante (0,6 mg/kg/h), a qual foi mantida durante todo o período experimental. Procedeu-se a ventilação controlada, com amplitude e freqüência suficientes para manter a capnometria constante em 35mmHg. Aos animais do GSEVO foi fornecido sevofluorano e aos animais do GDES, desfluorano, diluídos em O2. A anestesia inalatória foi mantida em 1,5 CAM (concentração alveolar mínima) para ambos os agentes voláteis. A administração do anestésico inalatório teve início imediatamente após a introdução da sonda orotraqueal. Foram estudadas as seguintes variáveis: pressão intracraniana (PIC), pressão de perfusão cerebral (PPC), temperatura intracraniana (TIC), freqüência cardíaca (FC), pressões arteriais sistólica (PAS), diastólica (PAD) e média (PAM), débito cardíaco (DC), índice cardíaco (IC), volume sistólico (VS), índice sistólico (IS), pressão venosa central (PVC), resistência vascular sistêmica (RVS), índice de resistência vascular sistêmica (IRVS) e temperatura corpórea (TC). As mensurações tiveram início após 30 minutos da implantação do cateter de PIC (M1) e se repetiram a cada 20 minutos (M2, M3 e M4). A avaliação estatística das variáveis foi efetuada pela Análise de Perfil (p<0,05)... / The aim of this study was to compare the effects of sevoflurane and desflurane upon intracranial and hemodynamic variables in dogs under controlled ventilation. Eighteen adult mongrel dogs were used. The animals were randomly allocated into 2 groups of equal number (GSEVO and GDES). All dogs were induced with propofol (7.8l1.3 mg/kg/IV), intubated, and the tracheal tube was connected to the inhalant anesthesia circuit. The neuromuscular blockade was acomplished by a bolus dose of rocuronium (0.6 mg/kg/IV) followed by continuous infusion of the agent (0.6 mg/kg/h) throughout the experimental period. Controlled ventilation was immediately started, with adequate amplitude and frequency to maintain end-tidal CO2 (ETCO2) constant at 35 mmHg. The animals of GSEVO received 1.5 MAC of sevoflurane and the ones in the GDES,1.5 MAC of desflurane, both agents diluted in O2. The administration of the inhalant agents started immediately after the placement of the tracheal tube.The following variables were studied: intracranial pressure (ICP), cerebral perfusion pressure (CPP), intracranial temperature (ICT), heart rate (HR), systolic (SAP), diastolic (DAP) and mean arterial pressures (MAP), cardiac output (CO), cardiac index (CI), stroke volume (SV), stroke index (SI), central venous pressure (CVP), systemic vascular resistance (SVR), systemic vascular resistance index (SVRI) and body temperature (BT). Concluiu-se que o protocolo anestésico com o sevofluorano causa poucas alterações nas variáveis cardiocirculatórias e respiratórias na cadela com 45 dias de gestação, podendo ser utilizado no caso de necessidade de intervenção cirúrgica e/ou anestésica. / Extensive human data have been published about anesthesia in pregnant women, but in veterinary medicine there are few articles about this subject. The aim of this study was to investigate the cardiocirculatory and respiratory alterations produced by pregnancy in bitches, comparing the effects of inhalatory anesthesia in nonpregnant bitch and that with 45 days of pregnancy. The effects of inhalatory anesthesia with sevoflurane were measured through the monitoring of cardiorespiratory systems of the mother and cardiac system of the fetus and by arterial blood samples for evaluate the acid-base and gases values, venous blood samples for haematology and serum biochemistry. For this were used 9 adults mongrel bitches from 1 to 5 years old. These animals were submitted to the same anesthetic procedure with acepromazine, propofol and sevoflurane in two different times. The first one was in nonpregnant bitch and the second in 45 days of pregnancy. There were no important differences in respiratory rate, ETCO2, pulse oximetry, some hemogasometric parameters (PaCO2, PaO2, HCO3-,CO2T, SatO2, BE), anesthetic concentration of sevoflurane and time of recovery of anesthesia either in time of each group, or among the groups. The arterial blood pressure, rectal temperature, red cell mass, hemoglobin, packed cell volume, albumin reduced in pregnant patient with 45 days of pregnancy. It was also noticed a marked increase in heart rate of mother during the first moments of the study. It was concluded that anesthetic protocol with sevoflurane cause few alterations in the cardiocirculatory and respiratory parameters in bitch with 45 days of pregnancy, which can used in case of need surgical and/or anaesthetic procedure to pregnant bitch and to the fetuses.

Foram utilizados 18 animais adultos, distribuidos aleatoreamente em dois grupos de igual número (GSEVO e GDES). Em todos os animais, induziu-se a anestesia geral com propofol (7,8l1,3 mg/kg/IV). Os cães foram intubados e a sonda orotraqueal foi acoplada a aparelho de anestesia inalatória. Em seguida, administrou-se a dose inicial de rocurônio (0,6 mg/kg/IV) e ato contínuo, iniciou-se a infusão contínua do miorrelaxante (0,6 mg/kg/h), a qual foi mantida durante todo o período experimental. Procedeu-se a ventilação controlada, com amplitude e freqüência suficientes para manter a capnometria constante em 35mmHg. Aos animais do GSEVO foi fornecido sevofluorano e aos animais do GDES, desfluorano, diluídos em O2. A anestesia inalatória foi mantida em 1,5 CAM (concentração alveolar mínima) para ambos os agentes voláteis. A administração do anestésico inalatório teve início imediatamente após a introdução da sonda orotraqueal. Foram estudadas as seguintes variáveis: pressão intracraniana (PIC), pressão de perfusão cerebral (PPC), temperatura intracraniana (TIC), freqüência cardíaca (FC), pressões arteriais sistólica (PAS), diastólica (PAD) e média (PAM), débito cardíaco (DC), índice cardíaco (IC), volume sistólico (VS), índice sistólico (IS), pressão venosa central (PVC), resistência vascular sistêmica (RVS), índice de resistência vascular sistêmica (IRVS) e temperatura corpórea (TC). As mensurações tiveram início após 30 minutos da implantação do cateter de PIC (M1) e se repetiram a cada 20 minutos (M2, M3 e M4). (Resumo completo, clicar acesso eletrônico abaixo) / Abstract: The aim of this study was to compare the effects of sevoflurane and desflurane upon intracranial and hemodynamic variables in dogs under controlled ventilation. Eighteen adult mongrel dogs were used. The animals were randomly allocated into 2 groups of equal number (GSEVO and GDES). All dogs were induced with propofol (7.8l1.3 mg/kg/IV), intubated, and the tracheal tube was connected to the inhalant anesthesia circuit. The neuromuscular blockade was acomplished by a bolus dose of rocuronium (0.6 mg/kg/IV) followed by continuous infusion of the agent (0.6 mg/kg/h) throughout the experimental period. Controlled ventilation was immediately started, with adequate amplitude and frequency to maintain end-tidal CO2 (ETCO2) constant at 35 mmHg. The animals of GSEVO received 1.5 MAC of sevoflurane and the ones in the GDES,1.5 MAC of desflurane, both agents diluted in O2. Avaliação da suplementação intranasal de oxigênio em burros (Equus caballus x Equus asinus) anestesiados com a associação cetamina, butorfanol e éter gliceril guaiacólico /

Assim, o objetivo deste estudo foi avaliar o efeito da suplementação intranasal de oxigênio (SIO) em burros (Equus caballus X Equus asinus) anestesiados a campo com a associação cetamina/butorfanol/éter gliceril guaiacólico (EGG). Para isso, foram utilizados 6 burros adultos (278 a 365 kg) submetidos à medicação pré-anestésica (MPA) com 0,2 mg/kg de midazolam por via intramuscular (IM), após 15 minutos, 0,02 mg/kg de detomidina por via intravenosa (IV), seguido, após 5 minutos, de indução com administração IV da associação de cetamina (2mg/ml), butorfanol (22,5 μg/ml) e EGG (50 mg/ml) em solução de glicose a 5% (C/B/G) até o animal assumir o decúbito lateral. A manutenção foi realizada com a mesma associação anestésica da indução na taxa de infusão inicial de 2,1 ml/kg/h. Os animais foram submetidos duas vezes à MPA, indução e manutenção anestésica descrita anteriormente, com intervalo mínimo de 20 dias, formando, assim, dois grupos experimentais. Grupo SEM SIO - MPA, indução (0,92 ± 0,24 ml/kg (média ± DP)) e manutenção anestésica (2,2 ± 0,2 ml/kg/h) com a associação C/B/G sem SIO; Grupo COM SIO - MPA, indução (0,98 ± 0,17 ml/kg) e manutenção anestésica (2,3 ± 0,4 ml/kg/h) com a associação C/B/G com SIO, fluxo de 40 ml/kg/h de oxigênio. Durante a anestesia foi colhido sangue arterial a cada 20 minutos (T0, T20, T40 e T60) para hemogasometria e analisados os parâmetros cardiovasculares a cada 5 minutos. Os dados foram analisados pela ANOVA seguidos pelo teste de Bonferroni. Valores de p < 0,05 foram considerados significativos. Foi observado hipoxemia (PaO2 < 60 mmHg) dos burros no grupo sem SIO em todos os tempos avaliados ... / Abstract: The main complications of the field anesthesia in the mules are incident of hypoxemia and paucity of studies on anesthetic protocols in the species. The aim of this study was to evaluate the effect of intranasal oxygen supplementation (SIO) in mules (Equus caballus X Equus asinus) anesthetized under field conditions with combination of ketamine, butorphanol and guaifenesin. Six adult mules (278 to 365 kg) were used and underwent to premedication with 0.2 mg/kg of midazolam (i.m.), after 15 min, 0.02 mg/kg of detomidine (i.v.), after 5 min, anesthesia was induced with administration (free flow) a mixture of 5% guaifenesin with ketamine (2.0 mg/ml) and butorphanol (22.5 μg/ml) (C/B/G). When lateral recumbency occurred, the same mixture was used to maintenance (2.1 ml/kg/h). The animals were submitted twice to the MPA, induction and maintenance of anesthesia described above, with a minimum interval of 20 days, thus forming two experimental groups. WITHOUT SIO - MPA, induction (0.92 ± 0.24 ml/kg (mean ± SD)) and maintenance (2.2 ± 0.2 ml/kg/h) with C/B/G mixture without SIO; WITH SIO - MPA, induction (0.98 ± 0.17 ml/kg) and maintenance (2.3 ± 0.4 ml/kg/h) with C/B/G mixture with SIO in rate of 40 ml/kg/min. Arterial blood was collected each 20 minutes (T0, T20, T40 and T60) during anesthesia, for blood gas analysis, and the cardiovascular parameters measured each 5 minutes. The data were analyzed by repeated measures ANOVA followed by Bonferroni test and P values < 0.05 were considered significant. Hypoxemia (PaO2 < 60 mmHg) was observed in mules of the group without SIO at all times evaluated (T0 = 59 ± 5; T20 = 55 ± 5; 53 ± 7 = T40; T60 = 49 ± 8) and the means were considered significantly lower than the mean the supplemented group (T0 = 160 ± 34; T20 = 115 ± 34; T40 = 92 ± 25; T60 = 81 ± 19). Os animais foram induzidos (5 V%) e mantidos (2,05 ± 0,18 V%) à anestesia com isofluorano e, após 60 minutos, retiraram-se 12 mL/kg de sangue arterial para induzir a hipovolemia. Dez minutos após, os coelhos receberam bolus de 1 mL de solução de NaCl a 0,9% (GC100, GC60 e GC40) ou 5 mg/kg de tramadol (GT100, GT60 e GT40), seguido de infusão contínua dos mesmos (1 mL/kg/h e 0,025 mg/kg/min, respectivamente). As mensurações dos parâmetros hemogasométricos, hematológicos, respiratórios e cardiovasculares tiveram início 60 minutos após a indução anestésica (M0), dez minutos após a retirada total de sangue e imediatamente antes do bolus de tramadol ou NaCl 0,9% (M10) e em intervalos de 15 minutos (M25, M40, M55 e M70). Para as variáveis hematológicas, também foi colhida uma amostra antes da indução anestésica (MB). Os dados foram submetidos à análise de variância seguida pelo teste de Tukey (p<0,05). As pressões parciais de oxigênio (PaO2) e dióxido de carbono (PaCO2) no sangue arterial, pressão alveolar de oxigênio (PAO2) e diferença alvéolo-arterial de oxigênio [P(A-a)O2] foram maiores para a FiO2 maior. O índice respiratório (IR) e as relações entre PaO2 e PAO2 e entre PaO2 e FiO2 foram maiores no GT40. A pressão arterial média diminuiu após a hipovolemia em todos os grupos, exceto no GT40. A contagem de leucócitos, linfócitos, neutrófilos bastonetes e monócitos decresceu significativamente após a indução anestésica e foram agravadas após a hipovolemia. Para o plaquetograma, quanto maior a FiO2, maior a contagem desses fragmentos coroplasmáticos anucleados / To evaluate different inspired oxygen fractions (FiO2) and continuous infusion of tramadol on the response to acute hypovolemia, 48 adult rabbits were used and divided into six groups: tramadol 100% group (GT100), 60% (GT60) or 40 % (GT40) and control 100% group (GC100), 60% (GC60) or 40% (GC40). Isoflurane was used for induction (5% V) and maintenance of general anesthesia (2.05 ± 0.18 V%) and, after 60 minutes, hypovolemia was induced by removing arterial blood (12 ml/kg). After 10 minutes, the rabbits received a bolus of NaCl 0.9% (GC100, GC60 and GC40) or tramadol (GT100, GT60 and GT40), followed by continuous infusion of the same solution. The measurements of blood gas, hematological, cardiovascular and respiratory parameters were taken 60 minutes after anesthetic induction (M0), ten minutes after hypovolemia induction and immediately before a bolus of tramadol or NaCl 0.9% (M10), and then at 15-minute intervals (M25, M40, M55 and M70). For haematological variables also sample was collected before induction of anesthesia (MB). Numeric data were subjected to analysis of variance followed by Tukey test (p <0.05). The arterial partial pressures of oxygen (PaO2) and of carbon dioxide (PaCO2), alveolar oxygen partial pressure (PAO2) and alveolar-arterial oxygen gradient [P(Aa)O2] showed higher means with the highest FiO2 used. The GT40 showed the lowest respiratory index (RI), the highest oxygen tension ratio (PaO2/PAO2) and arterial oxygen partial pressure/inspired oxygen fraction ratio (PaO2/FiO2). The arterial pressures decreased after hypovolemia in all groups, except in the GT40 for systolic (SAP) and diastolic (DAP) arterial pressure. The leukocyte, lymphocytes, neutrophils and monocytes count decreased significantly after induction of anesthesia and again after hypovolemia.
