MANAGING PREOPERATIVE ANXIETY IN CHILDREN UNDERGOING SURGERYChow, Cheryl H.T. January 2017 (has links)
Objectives: 1) To examine the effectiveness of Audiovisual (AV) interventions at reducing preoperative anxiety (PA) and its associated outcomes in children undergoing surgery; 2) To assess the psychometric properties of a new scale, the Children’s Perioperative Multidimensional Anxiety Scale (CPMAS); 3) To examine the feasibility and acceptability of a novel tablet-based intervention, Story-Telling Medicine (STM), for reducing children’s PA; 4) To examine the relation between temperament and PA in the surgical context.
Methods: A systematic review of studies where the primary outcome was children’s PA was conducted (Study 1). A study of the reliability and validity of the CPMAS were assessed at preoperative assessment (T1), on the day of surgery (T2), and 1 month postoperatively (T3) was also undertaken (Study 2). The feasibility and acceptability of STM were then examined and compared its effect to Usual Care (UC) (Study 3). Finally, children’s temperament was examined using the Colorado Childhood Temperament Inventory at T1 along with the CPMAS assessed at T1 and T2 (Study 4).
Results: Fourteen of the 18 studies led to reductions in children’s PA (Study 1). The CPMAS demonstrated good internal consistency, stability and convergent validity across all visits (Study 2). The participant recruitment and study procedures were shown to be feasible and children in the STM group demonstrated greater reductions in CPMAS compared to the UC group (Study 3). Shyness predicted lower PA at T1, while sociability predicted higher PA at T1 and T2 (Study 4).
Conclusions: AV interventions appear useful but full-scale RCTs of these treatments are required to pinpoint those that are most effective. The CPMAS is a promising tool for evaluating children’s PA and preliminary evidence suggests that STM is a feasible intervention for reducing children's PA. Finally, our findings highlight the importance of considering individual differences in predicting anxiety in the surgical setting. / Thesis / Doctor of Philosophy (PhD) / Preoperative anxiety affects up to 5 million children in North America annually and is associated with many adverse psychological, behavioural, and health effects. These problems not only produce significant distress for children and families, but can have immediate and long-term effects on a child’s mental health and development.
Current methods to reduce preoperative anxiety in children (e.g., medications and psychological preparation programs) are limited by their expense and/or time-intensive nature. In an attempt to reduce children’s preoperative anxiety and its associated negative outcomes, this work examined the usefulness of audiovisual interventions in reducing children’s preoperative anxiety, sought to develop a new instrument to measure children’s preoperative anxiety, tested a new tablet-based application to reduce children’s preoperative anxiety, and examined the relation between children’s temperament and preoperative anxiety. The goal is to improve the surgical experience for children and families, and to reduce psychological and physical problems in children undergoing surgery.

O objetivo do estudo é contribuir para a análise e avaliação dos recursos educativos utilizados nas ações de promoção e educação em saúde. Foram selecionados seis vídeos do acervo da videoteca do Centro de Formação dos Trabalhadores da Saúde (CEFOR) sobre os assuntos serviços de saúde/ serviços públicos, saúde da mulher e doenças sexualmente transmissíveis/ aids, sendo três de ficção e três do gênero reportagem/ documentário. Os materiais foram analisados buscando interpretar quais os significados da configuração do corpo nas mensagens veiculadas. Para orientar a análise utilizaram-se as categorias de corpo individual/ existencial, social e político. Observou-se também como a mensagem foi estruturada segundo a forma de utilização de alguns recursos de linguagem audiovisual como roteiro, edição, movimento de câmera, enquadramento, áudio e cenário. Foram identificadas algumas oposições básicas através das quais pudemos interpretar os significados atribuídos às várias dimensões do corpo. Dentre elas podemos citar as oposições natureza/ cultura, indivíduo/ sociedade e parte/todo. A partir da análise foram elaboradas algumas considerações e sugestões com relação à utilização dos recursos de linguagem visando uma maior eficácia na comunicação, assim como reflexões a respeito das concepções de corpo apresentadas. / This study objective is to contribute with the analysis and evaluation of educational resources used in promotion and education actions in health area. It was also observed how the message was structured according to the utilization form of some audiovisual language resources such as script, edition, camera movement, frame, audio and scenery. It were identified some basic oppositions through which we could interpret the meanings given to the several body dimensions. Among them we can mention nature/ culture, individual/ society and part/ whole oppositions. From the analysis some considerations and suggestions related to the language resource utilization were elaborated aiming a better communication effectiveness, as well as thoughts about the presented body conceptions.

Motivating Students to Engage in Preparation for Flipped Classrooms by Using Embedded Quizzes in Pre-class VideosJones, Emily P., Wahlquist, Amy E., Hortman, Melissa, Wisniewski, Christopher S. 01 January 2021 (has links)
BACKGROUND: The success of flipped classrooms is dependent upon students' preparation prior to class, the lack of which is the most common challenge associated with this teaching methodology. To mitigate this limitation, it is important to develop and assess methods of engaging learners during pre-class activities. OBJECTIVE: To determine if quiz delivery method (embedded throughout versus after pre-class videos) affects students' knowledge retention, grades, and video viewing behavior. METHODS: Participating students were randomized to take quizzes, either during pre-class videos via Panopto (EQV) or after pre-class videos in the traditional manner via Moodle (TMM). Outcomes assessed included students' knowledge retention, scores on pre- and post-class quizzes, and pre-class video viewing behavior (total views and minutes viewed per student) during a three-week period. Having experienced both quizzing modalities during the semester, the perceptions from students in the EQV group were surveyed. RESULTS: Baseline assessment results of both groups (n = 27 per group) were comparable with a median score of 33% (IQR: 17, 50) in both groups. Performance was also similar on knowledge retention [TMM: 67% (50, 83) vs. EQV: 83% (50, 83)], pre-class quiz scores [TMM: 90% (87, 97) vs. EQV: 93% (90, 95)], and post-class quiz scores [TMM: 93% (80, 100) vs. EQV: 87% (80, 100)], while students in the EQV group had more total views [10 (8, 12)] vs. [5 (2, 11)] and minutes viewed [71 (36, 108) vs. 35 (15, 81)]. Results from the perception survey administered to students in the EQV group (74.1% response rate) indicated a preference for embedded quizzes overall (58%) and for class preparation (75%) when compared with post-video quizzes. CONCLUSION: Students' knowledge retention and performance were similar in both EQV and TMM groups, though students in the EQV group were more engaged with videos and most of them preferred this quiz delivery. Using embedded quizzes for formative rather than summative assessment might be an appropriate mechanism to encourage students' viewing of pre-class videos and their preparation for flipped classes.

INTRODUÇÃO: O vídeo instrucional \"Seja doce com os bebês\" foi desenvolvido no Canadá como ferramenta de tradução do conhecimento direcionada a pais de recém-nascidos (RN) sobre o uso de medidas analgésicas durante procedimentos dolorosos. OBJETIVO: Avaliar o efeito do vídeo instrucional \"Seja doce com os bebês\" no envolvimento das mães no manejo da dor de RNs durante procedimentos dolorosos. MÉTODO: Ensaio clínico pragmático, não randomizado, desenvolvido no Alojamento Conjunto (AC) do Hospital Universitário da Universidade de São Paulo em julho de 2017. Foram incluídas 73 mães, alocadas em grupos controle (GC) (n=38) e experimental (GE) (n=35). Todas as participantes receberam um panfleto sobre o manejo da dor neonatal no momento da admissão no AC. O GE foi composto pelas mães que, voluntariamente, assistiram ao vídeo instrucional nas sessões educativas diárias realizadas pelos enfermeiros. Os dados foram coletados em entrevista. O desfecho primário foi a prevalência do uso de medidas analgésicas durante o teste do pezinho. Os desfechos secundários foram: viabilidade, aceitabilidade e utilidade do vídeo instrucional; tipo de envolvimento das mães durante o procedimento; medidas analgésicas utilizadas (amamentação, contato pele a pele e oferta de solução adocicada); dor e comportamento do RN; sentimentos maternos; satisfação com o controle da dor e informações recebidas. A pesquisa foi conduzida conforme Resolução 466/2012, mediante aprovação dos Comitês de Ética em Pesquisa das instituições envolvidas e registrada na plataforma do Registro Brasileiro de Ensaios Clínicos. RESULTADOS: Os grupos foram homogêneos quanto às características maternas e neonatais, com exceção da média do número de filhos, que foi maior no GC. O vídeo Seja doce com os bebês foi considerado útil, fácil de entender e de aplicar em cenários reais, com duração ideal e recomendável para outros pais. As mães dos dois grupos se envolveram no manejo da dor do RN. Contudo as estratégias analgésicas foram utilizadas em 40,0% dos casos no GE e em 23,7% no GC. Houve diferença clinicamente significativa de 16,3 pontos percentuais entre os grupos, embora não tenha sido constatada diferença estatisticamente significativa. Prevaleceu o uso da amamentação, seguido do contato pele a pele e da sucção não nutritiva. Em análise ajustada por outras variáveis, o vídeo ampliou a chance de uso das estratégias analgésicas em 2,1 vezes, enquanto a sugestão de uso por parte do enfermeiro aumentou a chance em 5,5 vezes. Não houve diferença entre os grupos quanto à intensidade da dor e comportamento do RN, percebidos pela mãe, e sentimentos maternos durante o procedimento. A maioria das mães ficou satisfeita com a assistência de enfermagem no controle da dor do RN, mas no GE houve maior satisfação com a quantidade de informações recebidas sobre o manejo da dor neonatal. Por fim, quase a totalidade das participantes afirmou que utilizaria as estratégias analgésicas em procedimentos dolorosos futuros. CONCLUSÃO: Confirmou-se que mães expostas ao vídeo Seja doce com os bebês apresentam maior participação no uso de medidas para alívio da dor de RNs submetidos a procedimentos dolorosos e maior satisfação com as informações recebidas. Essa participação aumenta quando há incentivo do enfermeiro. / INTRODUCTION: The instructional video \"Be sweet to babies\" was developed in Canada as a knowledge translation tool directed to parents on the use of analgesic strategies during painful procedures performed in newborns. AIM: To assess the effect of the Portuguese version of the \"Be sweet to babies\" video on maternal involvement in neonatal pain management during painful procedures. METHODS: A non-randomized, pragmatic clinical trial conducted in the rooming-in unit of the University of São Paulo affiliated hospital in July 2017. The study included 73 mothers, allocated in the control group (CG) (n=38) and in the experimental group (EG) (n=35). All participants received a pamphlet regarding neonatal pain management at the time of admission in the rooming-in. The EG was composed by mothers who voluntarily watched the video during daily educational sessions taught by nurses. Data were collected by means of interviews. The primary outcome was the prevalence of analgesic strategies during newborn screening test. The secondary outcomes were: feasibility, acceptability and usefulness of the instructional video; type of mothers involvement during this procedure; analgesic strategies used (breastfeeding, skin-to-skin care and sweet tasting solution); pain intensity and newborn behavior; maternal feelings; satisfaction with pain control and information received. The study was conducted according to the 466/2012 Resolution, under approval of the Research Ethics Committees of the involved institutions, and after registration in the Brazilian Registry of Clinical Trials platform. RESULTS: Groups were homogeneous regarding maternal and neonatal characteristics, except for the mean number of children, which was higher in the CG. The \"Be sweet to babies\" video was considered as useful, easy to understand and easy to apply in real scenarios, with ideal length, and recommendable to other parents. Mothers of both groups were involved in newborns pain management. However, analgesic strategies were used in 40.0% of cases in the EG and 23.7% in the CG. There was a clinically significant difference of 16.3 percentage points between groups, although no statistical significance was found. The use of breastfeeding prevailed, followed by skin-to-skin care and non-nutritive sucking. In an adjusted analysis by other variables, the video increased the chance of analgesic strategies use by 2.1 times, while the nurse suggestion increased the chance of analgesic strategies use by 5.5 times. There was no difference between the experimental and control groups regarding pain intensity and newborns behavior perceived by mothers. There was also no difference between groups with respect to maternal feelings during the procedure. Most of the mothers were satisfied with the nursing care in controlling the newborns pain, however there was greater satisfaction with the amount of information received on neonatal pain management in the EG. Finally, almost all participants stated that they intend to use analgesic strategies in future painful procedures. CONCLUSION: It was confirmed that mothers exposed to the Portuguese version of the \"Be sweet to babies\" video have greater participation in the use of analgesic strategies during painful procedures performed in newborns and greater satisfaction with the information received. Mothers participation increases when there is incentive provided by nurses.
