Experimental-Computational Analysis of Woodpeckers' Beaks/Hyoid Apparatus for Damping of Stress Waves

This dissertation proposes engineering principles for stress wave dissipation found in woodpeckers. From the experimental study of a woodpecker’s beaks via electron microscopy and mechanical testing, the three main design factors were pointed out. First, a woodpecker’s beak has wavy lines inside of the beak for local shearing. The waviness of wavy lines found in the woodpecker’s beaks was 1 while chicken’s was 0.3, and toucan’s was 0.05. Second, the woodpecker showed elongated the keratin scales to the pecking direction with a dimension ratio of 3.67 (width/height) while chicken’s and toucan’s were 3 and 1, respectively. Third, a woodpecker’s beak bone was less porous for structural strength. The porosity of a woodpecker’s beak bone was about 9.9 % while chicken’s and toucan’s were 42.3 % and 61.5 %, respectively. Also, by using computational simulations, unique geometries including hyoid apparatus and suture interfaces found in woodpeckers were investigated to assess their damping capabilities. Surrounding a woodpecker’s head, the hyoid apparatus composed of core cartilage and muscle encasing a core cartilage. The spiral and thinning geometry of the hyoid apparatus converted the normal waves into shear waves. Then shear waves generated lateral displacement of the hyoid bone, and lateral displacement brought strain energy into surrounding muscle, in which energy loss occurred by viscoelastic behavior of the muscle. Quantitatively, as the stress wave traveled from the anterior to the posterior end of the hyoid apparatus, its pressure decreased 75 % and the impulse decreased 84 %. Suture interfaces, which is another unique feature observed from woodpecker’s beak, was investigated for their geometrical effects on the dynamic impact mitigation. A sinusoidal pattern of suture interfaces induced wave scattering at its boundary causing conversion of longitudinal waves into shear waves. The suture gap also brought pressure decay by storing strain energy in its viscoelastic material. As a result, a bar with a suture interface attenuated stress waves about 37 % more than a bar with a flat interface. Based on the results and ideas presented herein, one can develop bio-inspired material for energy absorbing.

Protocolo de atendimento e fluxo em acidente com material biológico: uma construção coletiva entre profissionais e serviços de saúde / Assistance protocol and flow in accident with biological material: a collective and participatory construction by professionals and health services

INTRODUÇÃO: As exposições aos materiais biológicos constituem-se um sério risco para contaminações de doenças. Desta forma medidas preventivas e profiláticas devem ser tomadas a fim de evitar estes eventos ou a contaminação por eles. OBJETIVO: Elaborar um protocolo de assistência e fluxo de atendimento para acidente com material biológico na rede de atenção à saúde de Passos-MG, coerente com as orientações de documentos governamentais das políticas públicas de saúde já existentes. MÉTODOS: Trata-se de um estudo qualitativo, realizado por meio do método da Pesquisa Convergente Assistencial. A coleta de dados ocorreu nos meses de fevereiro a maio de 2015, através de grupo focal. A pesquisa foi realizada com os profissionais de saúde de nível universitário, independente da categoria profissional, atuantes em unidades de saúde, públicas e privadas de Passos-MG, nas quais eram feitos atendimentos as vítimas de acidente com material biológico. RESULTADOS: Participaram deste trabalho, 30 profissionais de saúde, os quais através de um processo participativo colaboraram na elaboração do \"Protocolo de Assistência e Fluxo em caso de acidente com Material Biológico de Passos/MG\". Os participantes da pesquisa relataram que este protocolo ajudará muito na padronização das condutas, pois a cada plantão e conforme o profissional, as condutas eram tomadas de modo diferente. CONCLUSÃO: O processo de construção deste protocolo reiterou a necessidade do mesmo, pois os profissionais demonstraram inicialmente desconhecimento, insegurança, desconforto a cada dia que acontecia um acidente com material biológico. Houve aprendizado em cada passo da construção deste protocolo, tanto para a pesquisadora como para os envolvidos, tendo sido enriquecedor trabalhar multidisciplinarmente e perceber as mudanças de perspectiva acerca do tema pelos profissionais da rede de atenção à Saúde de Passos/MG. Foi solicitado e proposto a implantação do protocolo com discussões e treinamentos para todos os profissionais de saúde do município, pertencentes a rede de atenção, a fim de conhecerem o \"Protocolo de Assistência e Fluxo em caso de acidente com Material Biológico de Passos/MG\" / INTRODUCTION: Accidents and the exposition to potentially contaminated biological materials are a serious risk for disease contamination. Preventive and prophylactic measures should be taken to avoid events or contamination by them. OBJECTIVE: To develop a protocol of care and service to be followed in accidents with biological material by health care service from Passos - MG, according to government guidance documents of public health policies. METHODS: This is a qualitative study, by the method of Convergent Care Research. Data were collected in the months from February to March 2015, through focus group. The survey was conducted in the Municipal Health System from Passos - MG, with university-level health professionals who work in of to public and private health attention in Passos - MG. Were attended 30 professionals from the health care in Passos - MG. RESULTS: It was possible to build a collective and participatory \"Assistance Protocol and flow in an accident with Biological Material in Passos - MG.\" Survey participants reported that this protocol will greatly assist in the standardization of pipelines, for each shift and as professional, the pipes were made differently. CONCLUSION: The construction process of this protocol, reiterated the need of it, since the professionals initially demonstrated ignorance, insecurity, uncomfortable when happen an accident with biological material. There was learning every step of the construction of this protocol, both the researcher as to those involved, having been enriching multidisciplinary work and realize the changes of perspective on the subject by professionals of attention to health in Passos-MG. Este estudo teve por objetivo compreender, a partir da análise das atividades de trabalho, os aspectos que estão presentes nas origens dos acidentes de trabalho com material biológico que ocorrem com os profissionais de saúde em hospital universitário. Foi realizado no Instituto Central de um grande complexo hospitalar, que conta com um efetivo de enfermagem de pouco mais 2.000 profissionais. Foram utilizados dois métodos. Um, a Análise Coletiva do Trabalho ACT, método pelo qual os trabalhadores são levados a falar como realizam seu trabalho em reuniões de grupos. O outro, o Modelo de Análise e Prevenção de Acidentes de Trabalho M.A.P.A., que consiste na análise em profundidade de acidentes de trabalho, no qual se faz entrevistas e observação buscando compreender os antecedentes causais e distais situados na origem das ocorrências de acidentes. As constatações da ACT revelaram a característica dinâmica do trabalho e suas variabilidades. Evidenciando que para dar conta de todas as tarefas e atividades há uma alta exigência de tempo, que o trabalho exige uma constante vigilância/atenção dos AE/TE para com os detalhes, que mobiliários e equipamentos são antigos e carecedores de manutenção, a pouca valorização do pessoal de enfermagem, que o trabalho sofre interrupções constantes, efetivo trabalhando no limite mínimo. Com o MAPA foram revelados aspectos de sobrecarga de trabalho, realização de procedimentos em ritmo acelerado, falta de desenho ou planejamento da gestão de segurança que considere as peculiaridades e variabilidades das situações de trabalho, bem como falha no cumprimento de Normas Regulamentadoras do MTe / This study aimed to understand, from the analysis of work activities, the aspects that are present in the origins of accidents with biological material that occur with health professionals in a university hospital. It was held at the Central Institute of a large hospital complex, which has a staff of little more than 2,000 nursing professionals. Two methods were used. One of them , Analysis Collective Labor ACT method by which workers are brought to say the way they perform their work in a group meeting. The other one, the Model Analysis and Prevention of Accidents M.A.P.A, which consists of in depth analysis of accidents in which one does interviews and observation in order to understand the causal antecedents and distal located at the origin of the occurrences of accidents. The findings of the ACT revealed the dynamic nature of work and its variability. Showing that to meet all the tasks and activities there is a high requirement of time, the job requires constant vigilance and attention of the nursing technicians for the details, the furniture and equipment are old and they need maintenance, the devaluation of nursing staff, the duty suffers constant interruptions, professionals working in the limit. The M.A.P.A revealed aspects of overload labor, perform procedures at a fast pace, lack of design or planning of security management that considers the peculiarities and variability of work situations, as well as failure to comply with the Regulatory Standards of MTE

A prática de colecionar material biológico humano na forma de Biorrepositórios (coleção de material biológico humano, coletado e armazenado ao longo da execução de um projeto de pesquisa específico) se tornou essencial para o avanço da pesquisa clínica. Contudo, normativas nacionais e internacionais recomendam a utilização de uma padronização mínima nas diversas práticas que abrangem a manipulação, gerenciamento e armazenamento de tais amostras e suas informações associadas de modo a garantir a qualidade dos resultados das pesquisas, ao mesmo tempo assegurar os padrões éticos e legais, respeitando o princípio da dignidade humana. Nesse contexto, o presente trabalho teve como objetivo caracterizar os setores de pesquisa do HU-UFMA (Hospital Universitário da Universidade Federal do Maranhão/HU-UFMA) que armazenam Biorrepositórios, quanto aos recursos humanos, estrutura física e padrões técnicos existentes, bem como desenvolver os Procedimentos Operacionais Padrão (POP’s) para manipulação e guarda das amostras biológicas pertencentes á estes Biorrepositóriose e outros que poderão vir a ser cirados. Para cumprir tais objetivos foram feitas visitas aos setores do hospital aonde são desenvolvidas as pesquisas; e aplicação de questionário com os Pesquisadores Responsáveis pelos grupos de pesquisa e com os Gestores dos setores de pesquisa na instituição. Os resultados dos questionários apontaram que alguns setores de pesquisas não contêm, e outros, estão em fase de desenvolvimento de seus POP’s para coleta, manipulação, gerenciamentos e guarda de Biorrepositórios, e que alguns grupos de pesquisas possuem, de forma isolada, POP’s para alguma das etapas de manipulação das amostras biológicas de seus Biorrepositórios. Foram identificados, ainda, os materiais biológicos que compõem os Biorrepositórios dos estudos realizados na instituição. A partir daí, realizou-se consultas a alguns dos POP’s existentes nos grupos de pesquisas e POP’s assistenciais de um dos setores estudado, para um melhor embasamento sobre os métodos utilizados nas pesquisas da instituição, além das diretrizes nacionais e internacionais sobre o tema abordado. O resultado foi a elaboração de POP’s para coleta, processamento e armazenamento das coleções de Biorrepositórios armazenados nos setores de pesquisas; elaboração de um manual de coleta, processamento, armazenamento e descarte das amostras visando à criação de Biorrepositórios; criação de formulários para registro das variáveis pré-analíticas existentes nas etapas de manipulação e guarda das amostras; e criação de modelos de formulários para cadastro do participante da pesquisa e de amostras armazenadas, no hospital. / The practice of collecting human biological material in the form of bio-repositories (collection of human biological material, collected and stored throughout the execution of a specific research project) has become essential for the advancement of clinical research. However, national and international regulations recommend the use of a minimum standardization in the various practices that cover the handling, management and storage of such samples and their associated information in order to guarantee the quality of the results of the research, at the same time assuring the ethical and respecting the principle of human dignity. In this context, the objective of the present study was to characterize the research sectors of the HU-UFMA (University Hospital of the Federal University of Maranhão / HU-UFMA) that store Biorrepositories in terms of human resources, physical structure and existing technical standards, as well as to develop the Standard Operating Procedures (SOPs) for manipulation and storage of the biological samples belonging to these bio-repositories and others which may be drawn. In order to fulfill these objectives visits were made to the sectors of the hospital where the research is carried out; and questionnaire application with the Researchers Responsible for the research groups and with the managers of the research sectors in the institution. The results of the questionnaires pointed out that some research sectors do not contain, and others, are in the development phase of their POPs to collect manipulation, management and custody of bio-repositories, and that some research groups have, in isolation, POP's for some steps of manipulating the biological samples of their bio-repositories. It was also identified the biological materials that make up the bio-repositories of the studies carried out at the institution. From then on, consultations were carried out with some of the existing POPs in the research groups and welfare POPs of one of the studied sectors, for a better basis on the methods used in the research of the institution, besides the national and international guidelines on the subject addressed. The result was the elaboration of POP's for the collection, processing and storage of the collections of bio-repositories stored in the research sectors; preparation of a manual for the collection, processing, storage and disposal of samples for the creation of bio-repositories; creation of forms for recording the pre-analytical variables in the sample handling and storage stages; and creation of template forms for registration of the research participant and of stored samples in the hospital.

From then on, consultations were carried out with some of the existing POPs in the research groups and welfare POPs of one of the studied sectors, for a better basis on the methods used in the research of the institution, besides the national and international guidelines on the subject addressed. The result was the elaboration of POP's for the collection, processing and storage of the collections of bio-repositories stored in the research sectors; preparation of a manual for the collection, processing, storage and disposal of samples for the creation of bio-repositories; creation of forms for recording the pre-analytical variables in the sample handling and storage stages; and creation of template forms for registration of the research participant and of stored samples in the hospital.

Análise de elementos potencialmente tóxicos em material biológico da Ilha da Trindade, Brasil / Analysis of potentially toxic elements in biological material of Trindade Island, Brazil

A Ilha da Trindade está localizada a aproximadamente 1150 km da costa de Vitória-ES, é uma das ilhas oceânicas brasileiras e está sob o domínio da Marinha do Brasil. Desde o início de sua ocupação sofreu algumas influências antrópicas, como a derrubada de árvores e a introdução de animais (porcos e cabras). Os metais oriundos da costa ou de navios que passam pela região, estão em destaque entre os contaminantes ambientais que podem alterar o ecossistema da ilha. Dessa forma, o objetivo desse trabalho foi quantificar Al, As, Cd, Cu, Fe, Mn, Ni, Pb, Se, Zn e Hg em 300 amostras, compostas por músculo de duas espécies de caranguejo e de fígado e músculo de peixes (duas espécies) e aves (três espécies). Foram verificadas se existiam relações entre parâmetros biológicos (sexo, comprimento, peso), diferenças entre tecidos e entre os períodos de coleta. Nos caranguejos a concentração de Fe em G. lagostoma, e Zn em G. grapsus, foram maiores nos meses mais quentes, nos quais a atividade biológica é maior, sendo uma possível causa para maiores concentrações. Diferentemente dos caranguejos, a espécie M. niger (peixe) apresentou maiores concentrações (Al no músculo e Fe e Zn no Fígado) nos meses mais frios, as concentrações em peixes envolvem muitos fatores além da sazonalidade. Para ambas as espécies de peixes as concentrações de Fe e Zn foram maiores no fígado, assim como para P. arminjoniana (ave) as concentrações de Cu, Fe e Zn também foram maiores no fígado, como era esperado, pois, o fígado é um tecido com grande capacidade de acumular poluentes. Alguns elementos apresentaram correlações com o peso e/ou comprimento, porém todas muito baixas, com exceção do Zn em G. grapsus. Todas as análises de Hg realizada ficaram abaixo do limite de quantificação do método (17,7 µg kg-1). Os metais e semimetais apresentaram resultados semelhantes a estudos desenvolvidos em outras regiões. / The Trindade Island is located approximately 1150 km from the coast of Vitória-ES, is one of Brazilian oceanic islands and is under the control of the Brazilian Navy. At the time the occupation began the island suffered some anthropogenic influences, such as the removal of trees and the animals introduction (pigs and goats). Metals originating from the shore or from ships passing through the region, are highlighting among the environmental contaminants that can change the ecosystem of the island. So the aim of this study was to quantify Al, As, Cd, Cu, Fe, Mn, Ni, Pb, Se, Zn and Hg in three hundred samples of muscle of two species of crab and liver and muscle of fish (two species) and seabirds (three species). The relationships between biological parameters (sex, length, weight), differences between tissues and between sampling periods were checked. The concentration of Fe, G. lagostoma (crab), and the Zn, in G. grapsus (crab), showed higher concentrations in the warmer months, in which biological activity is greater, being a possible cause for higher concentrations. Different from crabs, the species M. niger (fish) presented higher concentrations (Al in the muscle and Fe and Zn in the liver) in the colder months, concentrations in fish involve many factors besides the seasonality. For both species of fish concentrations of Fe and Zn were higher in the liver as well as for P. arminjoniana (seabird) concentrations of Cu, Fe and Zn were higher in the liver, like was expected, because the liver is a tissue with a great capacity to accumulate pollutants. Some elements showed correlations with the weight and/or length, but all very low, with the exception of Zn in G. grapsus. All Hg analyzes performed were below the method quantification limit (17.7 µg kg-1). The metals and semimetals showed similar results to studies conducted in other regions.

A survey was carried out among 406 doctors, nurses, technicians or nurses assistants. After signing a document stating their consent, they answered a questionnaire. The prevalence of accidents last year was 18%. Forty percent of these cases were not reported. Doctors presented 76% more risks of having accidents when compared to technicians or nurses assistants (p=0.045). The risk in hospital B was twice higher than hospital A (PR=2,06 CI95% 1,30-3,27; p=0,002). The majority of accidents occurred with percutaneous exposure (60%), fingers being the body part most injured (47%), and the Surgical Center was the place where more accidents were reported (24%). This finding is in accordance with the fact that among the surgeous the prevalence of accidents was 48%. The prevalence of accidents during the professional lifetime was 64%. Thirty six percent of these cases were not reported. The factors related to accidents in the last year were young age, to work in hospital B and to be a medical doctor or nurse. The higher time in the function, not mattering age and hospital, was associated to accidents during the professional experience / Investigou-se a ocorrência de acidentes com material biológico entre profissionais dos Hospitais Universitários de Pelotas. A amostra incluiu 406 médicos, enfermeiros e técnicos e auxiliares de enfermagem que, após consentimento assinado, responderam um questionário aplicado por entrevistador. A prevalência de acidentes no último ano foi de 18%, dos quais 40% não foram notificados. Os médicos apresentaram risco 76% maior de acidentes quando comparados a técnicos e auxiliares de enfermagem (p=0,045). O hospital B apresentou risco duas vezes maior do que o hospital A (RP=2,06 IC95% 1,30-3,27; p=0,002). A maioria dos acidentes ocorreu com materiais pérfuro-cortantes (60%), os dedos foram a parte do corpo mais atingida (47%), o local mais freqüente foi o Centro Cirúrgico (24%), achado consistente com a proporção de acidentes entre cirurgiões (48%). A prevalência de acidentes durante a vida profissional foi de 64%, dos quais 36% não foram notificados. Os fatores associados a acidentes no último ano foram a menor idade, trabalhar no hospital B e ser médico ou enfermeiro, e, para acidentes na vida, o maior tempo de função, independente da idade e do hospital

Prevalência de subnotificação de acidentes com material biológico pela equipe de enfermagem de um hospital de urgência / Prevalence of underreporting of accidents with biological material by the nursing team of an emergency hospital.

Introdução: a subnotificação de acidentes ocupacionais com material biológico é um desafio para prevenção e controle deste tipo de acidente. Objetivo: identificar a prevalência de subnotificação de acidentes ocupacionais com material biológico pela equipe de enfermagem de um hospital de urgência terciário brasileiro. Materiais e Métodos: trata-se de um estudo de corte transversal aprovado pelo Comitê de Ética em Pesquisa de um hospital publico especializado em urgência e emergência e de nível terciário. Dos 512 profissionais de enfermagem da instituição, 451 aceitaram participar do estudo e foram entrevistados individualmente, no período de abril a julho de 2009. Resultados: dos 451 sujeitos entrevistados, 237 referiram ter sofrido 425 acidentes com material biológico. A prevalência de subnotificação foi de 23,63% e houve indivíduos que não notificaram até sete acidentes. Em relação aos acidentes subnotificados 53,47% foram percutâneos, 10,89% cutâneo-mucosa e 35,64% pele íntegra. Destaca-se que em 67,33% das subnotificações o fluido envolvido foi o sangue. Os principais motivos referidos para não notificação foram: falta de conhecimento sobre mecanismo de notificação, medo de comunicar a chefia, excesso de burocracia para preenchimento dos formulários e atribuição de baixo risco ao acidente. Conclusões: a prevalência de subnotificação foi baixa quando comparada com outros estudos, porém exposições classificadas como de alto risco, ou seja, por via percutânea e envolvendo sangue deixaram de ser notificadas, evidenciando a subestimação do risco. Ao se considerar que a notificação dos acidentes confere respaldo legal aos profissionais e contribui para o real conhecimento da epidemiologia dos acidentes, mecanismos para aumentar a notificação devem ser implementados pelas instituições de saúde. / Introduction: underreporting of occupational accidents with biological material is a challenge for the prevention and control of this type of accident. Objective: to identify the prevalence of underreporting of occupational accidents with biological material by the nursing team of a Brazilian tertiary care emergency hospital. Material and method: this is a cross-sectional study approved by the Ethics Research Committee of a public tertiary hospital specialized in urgency and emergency. Of the 512 nursing professionals in the facility, 451 agreed to participate in the study and were individually interviewed between April and July 2009. Results: Of the 451 interviewed individuals, 237 reported 425 accidents with biological material. The prevalence of underreporting was 23.63% and there were individuals who did not report up to seven accidents. In relation to under reported accidents, 53.47% were percutaneous, 10.89% mucocutaneous and 35.64% intact skin. We highlight that blood was the fluid involved in 67.33% of underreporting. The main reported reasons for under notification were: lack of knowledge about the notification mechanism, fear of communicating the direction, excess of bureaucracy in filling out forms, and considered the accident of low risk. Conclusions: the prevalence of underreporting was low when compared to other studies, however exposure classified as high risk, whether through percutaneous via or involving blood were not reported evidencing that risks were underestimated. When we consider that the report of accidents ensures legal support to professionals and contribute to the knowledge about the real scenario of the epidemiology of accidents, mechanisms to increase notifications should be implemented by health institutions.
