Desvelando a percepção do homem obeso mórbido sobre sua sexualidade

No. of bitstreams: 0 Previous issue date: 2016-12-20 / Morbid obesity is now recognized as a public health problem, with multifactorial etiology and broad spectrum pathophysiology. The general aim of this study was to reveal the perceptions regarding the sexuality aspects of subjects with morbid obesity. It is a qualitative study, with a phenomenological approach, developed in the capital of the State of Ceará, Fortaleza, in the Medical Specialties Ambulatory of the Hospital Geral César Cals, with 17 morbid obese men from the Bariatric Surgery Program, from August to December. 2016. Inclusion criteria were defined as being over 18 years of age, having a BMI ¿ 35kg/m² with comorbidities, or BMI ¿40kg/m², with or without comorbidities, and excluding those who presented at the time of collection any signs of mental confusion That would put his interview at a loss. Data collection took place systematically with immersion in the field of research and the approach of the participants following the ethical precepts. Initially the sociodemographic data was surveyed followed by the application followed by the application of the phenomenological interview using a trigger question. Collected and treated the data, the emerging themes were identified, which were grouped in thematic units according to their similarities, giving rise to the themes finally grouped into five categories: sociability, sexuality, obese representation and medicalization. For the analysis of the data we chose to transcribe the narratives reliably and discuss the findings under the lens of Merleau-Ponty's Phenomenology of Perception. The research findings indicate a man who experiences and suffers in his daily life with prejudice and discrimination and thus ends up being violated institutional, moral, and social rights by a patriarchy that determines the right and wrong and the destinies of bodies From the appropriate and inadequate. The study confirms the difficulty of living a morbid obese man in his multiple dimensions. He also points out the need to perceive man not only as a strong and virile figure, but as a body and as any body that suffers from illnesses having an interference in their social, emotional, sexual, professional life and other situations. / A obesidade mórbida é reconhecida hoje um problema de saúde pública, com etiologia multifatorial e fisiopatologia de amplo espectro. O objetivo geral deste estudo foi desvelar as percepções frente aos aspectos da sexualidade de sujeitos com obesidade mórbida. É um estudo de natureza qualitativa, com abordagem fenomenológica, desenvolvido na capital do Estado do Ceará, Fortaleza, no Ambulatório de Especialidades Médicas do Hospital Geral César Cals, com 17 homens obesos mórbidos do Programa de Cirurgia Bariática, no período de agosto a dezembro de 2016. Como critérios de inclusão foram instituídos ser maior de 18 anos, estar com IMC ¿ 35kg/m² com comorbidades, ou IMC ¿40kg/m², com ou sem comorbidades, sendo excluídos aqueles que apresentassem no ato da coleta quaisquer sinais de confusão mental que colocassem em prejuízo sua entrevista. A coleta de dados ocorreu, sistematicamente, com a imersão no campo de pesquisa e a abordagem dos participantes seguindo os preceitos éticos. Inicialmente foi feito o levantamento dos dados sociodemográficos seguido da aplicação da seguido da aplicação da entrevista fenomenológica usando uma pergunta disparadora. Coletados e tratados os dados, foram identificados os temas emergentes, que foram agrupados em unidades temáticas de acordo com suas semelhanças, originando os temas por fim agrupados em cinco categorias, sociabilidade, sexualidade, representação de obeso e medicalização. Para a análise dos dados optou-se por transcrever as narrativas fidedignamente e discutir os achados sob a lente da Fenomenologia da Percepção de Merleau-Ponty. Os achados da pesquisa indicam um homem que experimenta e sofre, em seu cotidiano com o preconceito e a discriminação e assim acaba sendo violentado institucional, moral, social e de seus direitos por um patriarcado que determina o certo e o errado e os destinos dos corpos a partir do adequado e inadequado. O estudo ratifica a dificuldade de viver do homem obeso mórbido em suas múltiplas dimensões. Aponta, ainda, a necessidade de perceber o homem não apenas como uma figura forte e viril, mas como um corpo e como qualquer corpo que sofre adoecimentos havendo uma interferência na sua vida social, emocional, sexual, profissional e outras situações.

Avaliação de fatores clínicos e biológicos associados ao prognóstico da cirurgia bariátrica

Introdução: A prevalência de obesidade vem aumentando na maioria dos países, praticamente dobrando nos últimos 30 anos. A cirurgia bariátrica é o tratamento mais efetivo para a obesidade grave, levando a uma perda de peso expressiva e que usualmente se mantém em longo prazo. Apesar de sua eficácia comprovada, nem todos os pacientes apresentam perda de peso igualmente satisfatória após a cirurgia. A possibilidade de que estas diferenças nos desfechos de perda de peso possam estar relacionadas a características individuais pré-operatórias tem sido investigada. Até o momento, porém, a literatura a respeito de possíveis preditores de sucesso da cirurgia bariátrica não se mostrou conclusiva. Obesidade e dependência química parecem compartilhar alguns mecanismos neurobiológicos, sobretudo a disfunção de sistemas neurais envolvidos na modulação da resposta a estímulos recompensadores, e há evidências de que a história familiar positiva para transtorno por uso de álcool possa estar associada à obesidade. Além disso, evidências recentes sugerem que a proteína ligante de FK506 51 (FKBP51), implicada na regulação da resposta ao estresse, possa estar envolvida também na regulação da homeostase energética, influenciando o peso corporal. Objetivos: Avaliar se a história familiar positiva de transtorno por uso de álcool e a presença do alelo T do polimorfismo de nucleotídeo simples (SNP) rs1360780 no gene FKBP5 poderiam influenciar a perda de peso após a cirurgia bariátrica. Método: Neste estudo prospectivo longitudinal, a presença de história familiar de transtorno por uso de álcool e do SNP rs1360780 do gene FKBP5 foram avaliadas em pacientes com idades entre 18 e 69 anos, com índice de massa corporal (IMC) igual ou superior a 35 Kg/m2, que aguardavam a realização da cirurgia bariátrica no Hospital de Clínicas de Porto Alegre. O procedimento cirúrgico foi realizado de acordo com a indicação da equipe assistente, e em todos os casos a técnica cirúrgica empregada foi a gastroplastia em Y de Roux. No período pós-operatório, foi realizado seguimento da perda de peso pelo período de 26 meses, através de dados contidos no prontuário. Resultados: A história familiar positiva para transtorno por uso de álcool e a presença do alelo T da SNP rs1360780 do gene FKBP5 mostraram associação com menor perda de peso após a cirurgia bariátrica. No primeiro artigo, pacientes com história familiar positiva apresentaram perda do excesso de peso (EWL, do inglês excess weight loss) 18,4% menor em 2 anos de seguimento quando comparados a pacientes sem história familiar, e esta associação se deu de modo independente da presença de história pessoal de transtorno por uso de álcool ou de compulsão alimentar. No segundo artigo, pacientes portadores do alelo T da SNP rs1360780 apresentaram EWL 19,9% menor, e perda do peso total 9,0% menor em 2 anos de seguimento quando comparados a pacientes com o genótipo C/C. Conclusões: A cirurgia bariátrica interage com características individuais, como genes envolvidos na regulação metabólica e características indicativas de uma maior fragilidade em sistemas neurais relacionados à modulação da resposta a estímulos recompensadores, levando a diferentes desfechos de perda de peso. A identificação de possíveis preditores de sucesso da cirurgia bariátrica possibilitaria o desenvolvimento de intervenções específicas, visando melhorar os desfechos da cirurgia. / Introduction: The prevalence of obesity is increasing worldwide, nearly doubling in the last three decades. Bariatric surgery is the most effective treatment choice for severe obesity, leading to expressive weight loss that is usually sustained in the long term. Despite its proven efficacy, not all patients have equally successful weight loss after the surgery. The possibility that this difference in weight outcomes could be attributed to preoperative individual factors has been considered. However, up until now, the literature about potential predictors of success after bariatric surgery has not been conclusive. Obesity and drug addiction seem to share some neurobiological mechanisms, more specifically a disruption in neural systems which modulate the response to reward, and recent literature indicates that a positive family history of alcohol use disorder could be associated with obesity. Moreover, recent evidence indicates that the FK506-binding protein 51 (FKBP51), which is known to play an important role in the regulation of stress response, could also play a role in energy homeostasis, influencing body weight. Objectives: To evaluate if a positive family history of alcohol use disorder and the presence of the T allele of the FKBP5 SNP rs1360780 could influence weight loss after bariatric surgery. Methods: In this prospective observational study, the family history of alcohol use disorder and the presence of the FKBP5 rs1360780 polymorphism were evaluated in patients between 18 and 69 years of age, who presented a body mass index (BMI) of at least 35 kg/m2 and who were awaiting bariatric surgery at Hospital de Clínicas de Porto Alegre. The surgical procedure was carried out according to the indication of the assistant team, and in all cases the surgical technique employed was the Roux-en-Y gastric bypass. In the postoperative period, a 26-month follow-up of weight loss was performed using data from medical records. Results: A positive family history of alcohol use disorder and the presence of the T allele of the FKBP5 SNP rs1360780 were associated with significantly lower weight loss after bariatric surgery. In the first paper, patients with a positive family history of alcohol use disorder presented an excess weight loss (EWL) that was 18.4% lower at 2 years of follow-up when compared to patients with no family history, independent of a personal history of alcohol use disorder or binge eating. In the second paper, carriers of the T allele of the FKBP5 SNP rs1360780 presented EWL 19.9% lower, and total weight loss 9.0% lower at 2 years of follow-up when compared to patients with the C/C genotype. Conclusions: Bariatric surgery can interact with individual characteristics, more specifically with genes involved in metabolic regulation and with characteristics suggestive of a higher vulnerability in neural systems which modulate the response to reward, leading to different weight loss outcomes. The identification of potential predictors of success after bariatric surgery would enable the development of interventions to improve outcomes.

Subnotificação da ingestão energética entre obesas candidatas à cirurgia bariátrica

Quando um indivíduo saudável e em balanço energético equilibrado relata um consumo energético muito baixo, implausível com a viabilidade biológica, este é identificado como um subnotificador ou sub-relatador. O objetivo deste estudo foi comparar seis diferentes abordagens para avaliação da subnotificação da ingestão energética entre obesas que aguardam na fila de espera para a cirurgia bariátrica, bem como verificar a associação da subnotificação com algumas características das voluntárias. Os resultados obtidos foram analisados em dois artigos científicos, sendo que o primeiro foi focado na comparação de diferentes abordagens para avaliar a subnotificação e o segundo se destina aos fatores associados à subnotificação na amostra. Participaram do estudo 100 candidatas à cirurgia da obesidade da Clínica Bariátrica de Piracicaba, localizada no interior do estado de São Paulo, Brasil. Dados como idade, cor, estado civil, grau de escolaridade, hábito de fumar, consumo de bebida alcoólica e alteração de peso recente foram obtidos por meio de uma entrevista. Para divisão das participantes em classes econômicas foi utilizado o questionário da Associação Brasileira de Empresas de Pesquisa. Medidas antropométricas e de composição corporal também foram coletadas. A subnotificação do consumo alimentar foi avaliada baseada na diferença entre ingestão energética relatada (IErel) e requerimentos energéticos calculados: gasto energético de repouso (GER) e o nível de atividade física (NAF). When a healthy individual in a balanced energy state reports a very low energy intake, implausible with biological viability, this individual is called an underreporter. The objective of this study was to compare six different approaches for assessing energy intake underreporting in obese women in the waiting line for bariatric surgery, as well as to verify the association between underreporting and some characteristics of these women. The results were analyzed in two scientific articles. The first article focused on the comparison of different approaches for assessing underreporting and the second article focused on the factors associated with underreporting by the studied sample. The sample consisted of 100 women in the waiting line for bariatric surgery at the Bariatric Clinic of Piracicaba, a city located in upstate São Paulo, Brazil. An interview was used for collecting the following data: age, skin color, marital status, education level, smoking status, intake of alcoholic beverages and recent weight changes. The questionnaire created by the Brazilian Market Research Association (Associação Brasileira de Empresas de Pesquisa) was used to determine the socioeconomic classes of the participants. Anthropometric measurements and body composition data were also collected. Underreporting was determined by the difference between reported energy intake (rEI) and calculated energy requirements: resting metabolic rate (RMR) and physical activity level (PAL). In the first paper, patients with a positive family history of alcohol use disorder presented an excess weight loss (EWL) that was 18.4% lower at 2 years of follow-up when compared to patients with no family history, independent of a personal history of alcohol use disorder or binge eating. In the second paper, carriers of the T allele of the FKBP5 SNP rs1360780 presented EWL 19.9% lower, and total weight loss 9.0% lower at 2 years of follow-up when compared to patients with the C/C genotype. Conclusions: Bariatric surgery can interact with individual characteristics, more specifically with genes involved in metabolic regulation and with characteristics suggestive of a higher vulnerability in neural systems which modulate the response to reward, leading to different weight loss outcomes. The identification of potential predictors of success after bariatric surgery would enable the development of interventions to improve outcomes.

Sua prevalência aumentada é o resultado da combinação da disponibilidade de uma dieta com altos teores energéticos com o estilo de vida sedentário. Atualmente, a obesidade não é mais um problema exclusivo dos chamados países desenvolvidos; ao contrário, afeta porções crescentes dos estratos de população menos privilegiados. A OMS preconiza o Índice de Massa Corporal (IMC) para classificação da obesidade. A obesidade grau III ou mórbida, definida pelo IMC maior ou igual a 40 kg/m2 está relacionada com mortalidade aumentada e ocorrência de diversas co-morbidades como: hipertensão arterial, diabetes, dislipidemia, apnéia do sono, doenças cardiovasculares, artropatias, colecistopatias e câncer. Devido à necessidade de uma intervenção mais eficaz no manejo de obesos graves, a indicação da cirurgia bariátrica vem crescendo nos dias atuais. O objetivo deste estudo foi analisar a prevalência da obesidade mórbida no Brasil, bem como as tendências, custos, complicações e mortalidade relacionadas à cirurgia de obesidade mórbida no sistema público de saúde. Quanto à metodologia, o estudo contemplou uma nova análise de bancos de dados de pesquisas nacionais do IBGE levando em conta o desenho amostral, dados coletados do DATASUS e uma amostra de conveniência de prontuários de um hospital credenciado pelo SUS para este tipo de cirurgia em São Paulo. Os resultados demonstram que a obesidade mórbida apresentou um crescimento de 255%, passando de 0,18% (1974-5) para 0,33% (1989) e 0,64% (2002-3). Este tipo de obesidade foi mais freqüente no Sul nas primeiras duas pesquisas, mas subiu de modo acelerado no Sudeste alcançando a prevalência de 0,77% em 2002-3, que supera a do Sul. Os custos associados à cirurgia bariátrica representam um grande impacto para os SUS. Na amostra estudada a mortalidade decorrente da cirurgia encontra-se dentro dos níveis esperados segundo a literatura internacional. As complicações mais freqüentes foram: dor abdominal, vômitos, litíase biliar e hérnia incisional. Complicações de alta gravidade como embolia pulmonar, fístulas gástrica e enteral, enterorragia, trombose venosa profunda e pancreatite também foram registradas mais raramente. O Ministério da Saúde deve adotar medidas de prevenção e promoção à saúde, previstas nas recentes Portarias publicadas, na tentativa de reduzir a tendência no aumento da prevalência da obesidade e, sobretudo, da obesidade mórbida no Brasil. Recomenda-se adoção de uma escala de identificação de risco no preparo pré-operatório; uso precoce de antibioticoterapia profilática e adoção da correta suplementação nutricional no acompanhamento em longo prazo dos pacientes operados. Os hospitais de referência do SUS devem manter dados sobre os pacientes que foram submetidos à cirurgia bariátrica. A equipe multidisciplinar deve estar alerta para a importância de coletar estes dados e de notificar as complicações pós-operatórias. É crucial para o SUS a implementação de um sistema informatizado para este acompanhamento. ________________________________________________________________________________________ ABSTRACT / Obesity is a hard to treat chronic illness and an important public health issue. Currently it affects over 300 million individuals. Its increased prevalence is the result of a combination of availability of highly caloric diets and a sedentary life style. Obesity is no longer an illness exclusive to developed countries; in fact it affects growing proportions of less privileged populations. The World Health Organization (WHO) recommends the use of Body Mass Index (BMI) to classify obesity. Obesity Class III, or morbid obesity, which is defined by a BMI equal or greater than 40 kg/m2, is related to increased mortality and several co-morbidities such as hypertension, diabetes, dyslipidemia, sleep apnea, cardiovascular diseases, arthropathy, cholecystopathy and cancer. The need for more effective interventions in treating severe obesity has caused an increase in the indication for bariatric surgery. The aim of this study was to analyze morbid obesity prevalence in Brazil as well as trends, costs, complications, and mortality related to the morbid obesity surgery in the Brazilian public health system. Study methodology involved new analysis of IBGE national surveys taking into account the complex sample design, an analysis of data collected from the DATASUS system, and a sample of patient charts of a SUS bariatric surgery reference hospital in S?o Paulo, Brazil. The results showed an increase of 255% in the prevalence of morbid obesity, rising from 0.18% (1974-5) to 0.33% (1989) and 0.64% (2002-3). Morbid obesity was more frequent in the South in the first two surveys, but the prevalence in the Southeast rose steadily and in 2002-3 reached 0.77%, surpassing the South. Costs associated with bariatric surgery greatly impact the public health budget. In the sample analyzed mortality associated with the surgery was within the expected levels according to international literature. The most frequent complications were abdominal pain, vomiting, biliary lithiasis and incisional hernia. Severe complications like pulmonary embolism, gastric and enteral fistulas, enteral bleeding, deep venous thrombosis and pancreatitis were also present, although more rarely. The Ministry of Health should urgently seek the adoption of preventive measures and the promotion of healthy lifestyles, as predicated in recently issued Administrative Decrees, aiming to reduce the growing trend in the prevalence of obesity in Brazil. We recommend the adoption of risk identification scale in pre-surgery preparations, early prophylactic antibiotic therapy, and appropriate nutritional supplementation in the long term follow up of bariatric surgery patients. SUS reference hospitals must adequately manage follow-up databases from patients who were submitted to bariatric surgery. The multidisciplinary team should be conscious about the importance of such data collection, as well as about notification of post-operative complications. Implementation of a computer-based follow-up system is indeed crucial for SUS.

The objective of this work was to evaluate the estimated costs of bariatric surgery, in order to compare the resources utilized by morbid obese patients before and after surgery in South Brazil. Methods: This was a retrospective cohort study with the review of the medical charts of 200 patients who met the inclusion criteria of the study. We analyzed the costs of the doctor/clinic visits and tests and medications prescribed, before and after surgery. Results: The study demonstrated that there was variation in the pre- and postsurgery expenses based on the existence of comorbidities and the form of payment. We found that the improvement or resolution of associated comorbidities produced in the long run a reduction in expenses for medical fees, tests and medications. There were statistical differences among the expenditures for fees, tests and medications, pre- and postsurgery. Conclusion: The costs of bariatric surgery are high, but in the long run, the positive clinical effects of the surgery and the reduction of costs for treatment prove that the surgery is cost-effective. / INTRODUÇÃO : Com a alta prevalência da obesidade e de comorbidezes associadas, o investimento financeiro nos serviços de saúde gera um grande impacto econômico. O objetivo deste trabalho foi avaliar o custo orçamentário da cirurgia bariátrica, no intuito de comparar os recursos utilizados no pré e pós-cirúrgico de pacientes obesos mórbidos no Sul do Brasil.MÉTODOS : Estudo de coorte histórica, com revisão de prontuário de 200 pacientes que preencheram os critérios de inclusão para o estudo. Foram analisados os medicamentos utilizados, as consultas e os exames realizados nos períodos pré e pós-cirúrgico. RESULTADOS : O estudo demonstrou que ocorreu variação das despesas no pré e póscirúrgico conforme a existência de comorbidezes e a forma de pagamento. Podemos observar que a melhora ou remissão das comorbidezes associadas, gera em longo prazo, uma redução das despesas com medicamentos, exames e profissionais. Houve associação estatística entre os gastos de medicamentos, exames e profissionais no pré e pós-cirúrgico. CONCLUSÃO : Os custos da cirurgia são elevados, mas em longo prazo, os efeitos clínicos positivos da cirurgia e a redução dos custos com o tratamento, comprova que a cirurgia é custo efetiva.

Steatosis affects most patients with morbid obesity and may progress to more severe forms of liver disease nonalcoholic fatty, such as nonalcoholic steatohepatitis and cirrhosis. The aim of this study is to evaluate the body fat of bariatric patients followed at the Center for Obesity and Metabolic Syndrome of PUCRS and analyze its relationship with hepatic hepatic steatosis. METHODS: This is a historical cohort study that compares the disease of nonalcoholic fatty liver for liver biopsies and the percentage of fat by bioimpedance preoperatively segmented and in these same patients 1 year after surgery. The degree of hepatic steatosis and body fat percentage, was analyzed by relating them to the body mass index, waist-hip ratio, weigth loss, blood glucose and other comparative data regarding age and gender. RESULTS: Of 25 patients undergoing gastrointestinal bypass surgery will be included in the study met all inclusion criteria and 5 were male and 20 female with mean age of 37,6±12,6 years. In this study, the difference in the percentage of fat in preoperatively and postoperative period in an average of 25,9±10,2. Of the 12 (48%) individuals who regressed 2 degrees of the disease, had a percentage difference of 14,3±4,2 in the amount of body fat, 5 (20%) patients regressed 3 degrees with a mean of 11,4±9,0 and 3 (12%) subjects regressed 4 degrees with an average of 11,0±2,4 percentage of difference of body fat in relation to preoperative and one year after surgery. CONCLUSION: Of 25 patients analyzed %, showed improvement in hepatic steatosis. However, the patients who showed a smaller loss in percentage fat, were the patients who regressed more in steatosis or nonalcoholic steatohepatitis after one year of bariatric surgery. / INTRODUÇÃO: A prevalência da esteatose hepática aumentou concomitante ao crescimento da epidemia global que a obesidade se tornou. A esteatose acomete grande parte dos pacientes obesos mórbidos e pode evoluir para formas mais graves de doença hepática gordurosa não-alcoólica, como a esteatohepatite não-alcoólica e a cirrose. O objetivo deste estudo é avaliar o percentual de gordura corporal em relação á esteatose hepática no pós-operatório de pacientes bariátricos acompanhados no Centro de Obesidade e Síndrome Metabólica da PUCRS.MÉTODOS: Trata-se de um estudo de coorte histórica que compara as doenças do fígado gorduroso não-alcoólico por biópsias hepáticas e o percentual de gordura por bioimpedância no pré-operatório e, nesses mesmos pacientes, 1 ano após a cirurgia. O grau de esteatose hepática e o percentual de gordura corporal, foram analisados relacionando-os com o índice de massa corporal, relação cintura-quadril, perda de peso, glicemia e outros dados comparados em relação à idade e gênero. RESULTADOS: Dos 25 pacientes submetidos á cirurgia por bypass gastrointestinal incluídos no estudo por preencherem todos os critérios de inclusão, 5 são homens e 20 são mulheres com idade média de 37,6±12,6 anos. Neste estudo, a diferença do percentual de gordura no pré e no pós-operatório teve resultado significativo quanto sua relação com a regressão da esteatose hepática e esteatohepatite (P= 0,023). Dos 25 pacientes, apenas 1(4%) manteve-se no mesmo grau de esteatose, 4 (8%) pacientes regrediram apenas 1 grau da doença hepática e tiveram uma diferença de percentual de gordura no pós-operatório em uma média de 25,9±10,2. Dos 12 (48%) indivíduos que regrediram 2 graus da doença, obtiveram uma diferença percentual de 14,3±4,2 na quantidade de gordura corporal, 5 (20%) pacientes regrediram 3 graus com uma média de 11,4±9,0 e 3 (12%) indivíduos regrediram 4 graus com uma média percentual de 11,0±2,4 de diferença de gordura corporal em relação ao pré-operatório e após 1 ano de procedimento cirúrgico. CONCLUSÃO: De25 pacientes analisados96%, obtiveram melhora na esteatose hepática. No entanto, os que apresentaram uma perda menor no percentual de gordura, foram os pacientes que mais regrediram a esteatose ou esteatohepatite não-alcoólica no período de 1 ano após cirurgia bariátrica.

É fator de risco para demências, doenças cardiovasculares, cânceres e diabetes, e está associada a transtornos psiquiátricos. A cirurgia bariátrica é uma alternativa para os casos de obesidade mórbida, pois, inibe a progressão de comorbidades. Contudo a literatura é escassa quanto ao funcionamento cognitivo e emocional dos pacientes que vivenciam a rápida redução de peso por meio do processo cirúrgico em contraste com a perda de peso lenta decorrente de reeducação alimentar. O objetivo deste estudo prospectivo foi avaliar o funcionamento emocional e mnemônico de indivíduos obesos, antes e quatro meses depois do emagrecimento. Participaram do estudo 76 voluntários divididos em dois grupos, conforme a fase do tratamento: redução de peso por meio de cirurgia bariátrica (RPCB, N=40) ou redução de peso preparatória para bariátrica (RPPB, N= 36), avaliados por meio de testes cognitivos para memória episódica e operacional, bem como escalas comportamentais. Os instrumentos utilizados para a avalição eram os mesmos em ambas as avaliações. O teste t de Student indicou diferença estatística entre a 1ª avaliação e a 2ª avaliação. Houve mudanças mais robustas para o grupo RPCB, que vivenciou o emagrecimento rápido no âmbito comportamental para os âmbitos de depressão, ansiedade, compulsão e obsessão, resiliência e em especial para cinco categorias da escala que medem a qualidade de vida. It is a risk factor for dementia, cardiovascular diaseses, cancer and diabetes, and is associated with psychiatric disorders. Bariatric surgery is an alternative for cases for morbid obesity, since it inhibits the progression of comorbidities. However, the literature is scarce regarding the cognitive and emotional functioning of patients who experience rapid weight reduction through the surgical process in constrast to the slow weight loss due to dietary reeducation. The purpose of this prospective study was to evaluate the emotional and mneumonic functioning of obese individuals before and after weight loss. Participants were 76 volunteers divided into two groups, according to the treatment phase: weight reduction through bariatric surgery (RPCB, N = 40) or preparatory weight reduction for bariatric (RPPB, N = 36) evaluated by means of Cognitives tests for episodic and operational memory, as well as behavioral scales. The instruments used for the assessment were the same in both evaluations. Student's test indicated statistical difference between the 1 st evaluation and the 2nd evaluation. There were more robust changes for the RPCB group, which experienced rapid weight loss in the behavioral setting for the areas of depression, anxiety, compulsion and obsession, resilence and in particular for five categories of the scale that measures the quality of life. The RPCB group still had better performance in the follow-up regarding operational memory capacity, verbal memory and attention assessed by the Span Span test. The improvement of the RPPB group in the 2nd evaluation may be associated to the multidisciplinary follow-up as preparation for CB. In particular, this condition has not been investigated in Brazil. The objective of this work was investigate the frequency of NES symptoms in candidates for bariatric surgery and assess its implications in sleep quality and depressive symptoms. 100 patients from Bariatric Surgery Outpatient Clinic of a tertiary hospital in Fortaleza were selected, and these night eating symptoms were measured by the Night eating Questionnaire (NEQ), sleep quality by the Pittsburgh Sleep Quality Index (PSQI), the daytime sleepiness by the Epworth Sleepiness Scale (ESS), the depressive symptoms, by Beck Depression Inventory short form and the risk for obstructive sleep apnea by Berlin Questionnaire. It was performed a comparative analysis of the results obtained from patients with night eating symptom scores suggestive of NES. The patients included in the study were predominantly women (76%), with age between 18 and 64 (mean ± SD = 34.8 ± 9.3) years and BMI between 31.3 and 72.4 (46.9 ± 8.5) kg/m². Poor quality sleep (PSQI > 5) was observed in 56 patients, excessive daytime sleepiness (ESS > 10) in 27; depressive symptoms (Beck > 4) in 80 and high risk for obstructive sleep apnea in 73. Seventeen patients showed symptoms suggestive of NES, without difference in age, BMI, degree of daytime sleepiness and risk of sleep apnea in obese when compared to the other patients. The group with symptoms suggestive of NES had a worse sleep quality (9.3 ± 4.9 and 6.6 ± 3.7, p = 0.01) and more depressive symptoms (16, 7 ± 7.6 and 9.5 ± 7.0, p = 0.001) than the other obese. A higher proportion of patients with symptoms suggestive of NES was in use of antidepressants than other patients (29.4 and 10.8%,p = 0.04). These data show that NES symptoms are common in bariatric surgery candidates and their presence is associated with more depressive symptoms and worse quality of sleep. Also, in particular, the high frequency of severe depressive symptoms among patients with a suggestive picture of NES highlights the importance of identifying this condition in the context of specialized services to treat obesity. Further studies on the impact of the NES and its treatment are needed to allow a better approach to this problem. / A síndrome alimentar noturna (SAN) tem freqüência elevada nos obesos, embora ainda não tenha sido estabelecida uma relação causal. De modo particular, essa condição não foi investigada no Brasil entre os candidatos a cirurgia bariátrica. O objetivo deste estudo foi investigar a frequência de sintomas alimentares noturnos sugestivos de SAN em candidatos a cirurgia bariátrica, bem como avaliar sua relação com a qualidade do sono e sintomas depressivos. Foram estudados 100 pacientes do ambulatório de cirurgia bariátrica de um hospital terciário da rede pública de saúde de Fortaleza. Os sintomas alimentares noturnos foram avaliados através da Escala de Sintomas Alimentares Noturnos (ESAN), a qualidade do sono pelo Índice de Qualidade do Sono de Pittsburgh (IQSP), a sonolência diurna, pela Escala de Sonolência de Epworth (ESE), os sintomas depressivos pelo Inventário de Depressão de Beck e o risco para apneia obstrutiva do sono pelo questionário de Berlim. Foi realizada uma análise comparativa entre resultados obtidos de pacientes com e sem sintomas alimentares noturnos sugestivos da SAN (ESAN>25). Os indivíduos foram predominantemente mulheres (76%), com idades entre 18 e 64 (média±DP = 34,8 ±9,3) anos e IMC entre 31,3 e 72,4 (46,9 ± 8,5) kg/m². Má-qualidade do sono (IQSP>5) foi observada em 56 pacientes; sonolência excessiva diurna (ESE> 10) em 27; sintomas depressivos (Beck > 4) em 80 e o risco elevado para apneia obstrutiva do sono em 73. Dezessete pacientes apresentaram sintomas sugestivos de SAN, sem diferença quanto a idade, IMC, grau de sonolência diurna e risco de apneia do sono, em relação aos demais obesos. O grupo com sintomas sugestivos de SAN, comparado aos demais, apresentou pior qualidade de sono (9,3±4,9 e 6,6±3,7; p= 0,01) e mais sintomas depressivos (16,7±7,6 e 9,5±7,0; p= 0,001). Uma proporção maior dos pacientes com sintomas sugestivos de SAN fazia uso de antidepressivos (29,4 e 10,8%; p=0,04). Nossos resultados indicam que sintomas alimentares noturnos são comuns nos candidatos a cirurgia bariátrica e sua presença está associada a mais sintomas depressivos e pior qualidade do sono. A freqüência elevada de sintomas depressivos graves entre os pacientes com quadro sugestivo de SAN destaca a importância da identificação dessa condição no tratamento da obesidade. Estudos adicionais sobre o impacto da SAN e de seu tratamento são necessários para permitir uma abordagem mais adequada deste problema.
