O uso de LECO na população pediátrica ocorreu de forma lenta devido à baixa casuística e a incerteza dos efeitos das ondas de choque sobre os rins em desenvolvimento, eficiência, complicações e efeitos sobre órgãos adjacentes. No presente estudo, foram avaliadas 138 crianças, com idades entre 1 e 12 anos, portadoras de litíase do trato urinário tratadas com LECO, para avaliar a eficiência e complicações precoces do procedimento. A taxa de sucesso dos cálculos com menos de 11mm foi significantemente maior que nos cálculos maiores. Não foi observada diferença estatisticamente significante quando relacionamos a taxa de sucesso com idade, intensidade da onda e localização do cálculo / The use of Extracorporeal Shock Wave Lithotripsy (ESWL) in children has evolved rather slowly due to the small number of cases and the uncertainty about the effects of shock waves on the immature kidneys, its efficacy, complications and effects on the surrounding organs. In the present study, 138 children aged 1 to 12 years treated for urinary tract lithiasis by ESWL were evaluated in order to assess the efficacy and early complications of the procedure. The success rate for calculi smaller than 11mm was significantly higher than for larger calculi. When the success rate was correlated with age, intensity of the shock wave and location of the calculi no significant statistical difference was observed

Objetivo: O Holmium:YAG laser é o método de litotripsia intracorpórea para cálculos urinários mais recentemente introduzido em nosso meio. O objetivo deste estudo é analisar a eficácia e as complicações imediatas do tratamento de pacientes com cálculos ureterais com essa fonte de energia. Casuística e Métodos: Foram tratados 16 pacientes, nove homens e sete mulheres, portadores de cálculos ureterais sintomáticos, maiores ou iguais a 6 mm, ou com evolução superior a 30 dias. A média de idade foi 42 anos (6- 68 anos). Quatro cálculos estavam localizados no ureter superior, seis no ureter médio e seis no ureter inferior. A técnica empregada foi a de vaporização do cálculo com Holmium:YAG laser, através de ureteroscopia. Utilizou-se exclusivamente ureteroscópio semi-rígido de 7 Fr. Resultados: A taxa de sucesso obtida foi de 87,5%, sem diferença estatisticamente significativa em relação ao relatado na literatura. Todos os cálculos foram fragmentados. Os dois insucessos da série ocorreram com cálculos localizados em ureter superior, em que fragmentos foram deslocados para o rim. As complicações observadas foram três perfurações ureterais e dois casos de febre. Conclusão: O Holmium:YAG laser é eficaz no tratamento endoscópico do cálculo ureteral. Medidas destinadas a prevenir a migração retrógrada do cálculo ou de seus fragmentos devem ser tomadas, principalmente no tratamento dos cálculos localizados em ureter superior. A litotripsia com Holmium:YAG laser não é, entretanto, um método isento de complicações, particularmente no que se refere ao tratamento de cálculos impactados de ureter superior. / Objetive: Holmium:YAG laser is the more recently method of intracorporeal lithotripsy of urinary calculi introduced in our area. The purpose of this study is to analyze the technique and to evaluate the immediate results and complications in the treatment of patients with ureteral calculi. Casuistry and Methods: Sixteen patients were treated, nine men and seven women, carrying symptomatic ureteral stones, bigger than 6 mm or with evolution superior to 30 days. The average age was 42 years old (6-88 years old). Four stones were localized in upper ureter, six in middle ureter, and six in lower ureter. The technique employed was the vaporization of the stone with Holmium:YAG laser, through ureteroscopy. It was used exclusively 7-Fr semirigid ureteroscope. Results: The rate of success attained was of 87.5%, with no statistically significant differences regarding the reports in literature. All the stones were fragmented. The two failures of the series occurred with stones localized in upper ureter, in which fragments were displaced to kidney. The complications observed were three ureteral perforations and two fever cases. Conclusion: Holmium:YAG laser is effective in the endoscopic treatment of ureteral stones. Measures aimed at preventing retrograde migration of stones or fragments should be taken, mainly when the stone are located in the upper ureter. However, lithotripsy with Holmium:YAG laser may be associated with complications, particularly in what concerns the treatment of impacted stones.

Efeitos do Phyllanthus niruri em parâmetros metabólicos de portadores de litíase urinária / Effect of Phyllanthus niruri on metabolic parameters of patients with kidney stone disease

Introdução: O Phyllanthus niruri (P. niruri) ou quebra pedra é uma planta com ação antilitogênica. No entanto, estudos clínicos nesta área ainda são escassos na literatura. O objetivo principal deste estudo foi avaliar prospectivamente os efeitos do P. niruri nos parâmetros metabólicos de pacientes com litíase urinária e, secundariamente, avaliar o impacto da ingestão do chá da planta na eliminação de cálculos urinários. Material e Métodos: Foram estudados 56 pacientes portadores de cálculos renais <10 mm. Avaliação clínica, metabólica e ultrassonográfica do trato urinário foram realizadas antes do uso do P. niruri (infusão de 500 ml/dia com 9 g do extrato seco da planta), após a administração deste por 15 semanas e, finalmente após 12 semanas sem o uso (período de \"Wash out\"). Utilizamos o teste ANOVA e o teste de Tukey para comparação entre os períodos do estudo. O nível de significância considerado foi de 5%. Resultados: Trinta e seis pacientes (64%) eram mulheres. A média de idade dos 56 pacientes foi 44,1±9,16 anos. O IMC médio foi 27,2±4,4 Kg/m2. Não se observou alteração nos parâmetros antropométricos, séricos, no volume urinário ou efeitos adversos significativos durante todo o período de estudo. Houve redução da pressão arterial diastólica de 76±10,5 para 72,5±10,5 mmHg (p=0,02), quando comparado o período de uso do chá e o período de \"Wash out\". Aumento significativo dos valores do potássio urinário de 50,5±20,4 para 56,2±21,8 mEq/vol.24h (p=0,017); da relação magnésio/creatinina de 58±22,5 para 69,1±28,6 mEq/gCr.24h (p=0,013) e da relação potássio/creatinina, de 39,3±15,1 para 51,3±34,7 mEq/gCr.24h (p=0,008) foi observado ao final do período de uso do chá quando comparado com a avaliação inicial. O número de cálculos renais por paciente reduziu de 3,21±2,02 para 2,02±2,07 cálculos (p < 0,001) após o consumo do P. niruri quando comparado com o momento inicial. Na avaliação inicial, 24 pacientes apresentaram hipercalciúria e hipocitratúria (42,8%), seis apresentaram hiperuricosúria (10,7%) e cinco hiperoxalúria (8,9%). Após o uso do P. niruri, nos pacientes portadores de hiperuricosúria houve redução de 0,77±0,22 para 0,54±0,07 mg/vol.24h (p=0,0057) no valor do ácido úrico urinário. Nos portadores de hipocitratúria, o citrato urinário aumentou de 211,8±123,7 para 322,3±145,8 mg/vol.24h (p=0,0282) e nos pacientes com hiperoxalúria houve a redução no oxalato urinário de 59,0±11,7 para 28,8±16,0 mg/vol.24h (p=0,0002). Conclusão: O consumo do P. niruri se mostrou seguro e não provocou efeitos adversos ou alterações séricas relevantes e elevou a excreção urinária de magnésio e potássio. Algumas alterações metabólicas urinárias predisponentes a formação de cálculos normalizaram em subgrupos de pacientes estudados. O consumo do P. niruri contribuiu na eliminação de cálculos urinários / Introduction: Phyllanthus niruri (P. niruri) or breake-stone is a plant commonly used to reduce stone risk, however, clinical studies on this issue are lacking. The aim of this study was to prospectively evaluate the effects of P. niruri in metabolic parameters of patients with kidney stones and secondarily to evaluate the impact of the plant intake in the elimination of urinary calculi. Material and Methods: We studied 56 patients with kidney stones < 10 mm. Clinical, metabolic, and imaging studies were performed prior to P. niruri (tea infusion of 500 ml/day with 9 g of the dried plant extract), after 15 weeks of tea administration and finally after 12 weeks without the intake plant (wash out). ANOVA test for repeated measures and Tukey test and McNemars test for categorial variables. The significance level was set at 5%. Results: Thirty-six patients (64%) were female and mean age was 44.1±9.16 years-old. The mean BMI was 27.2±4.4 Kg/m2. There was no change in anthropometric and serum parameters or urinary volume throughout the study period. There was a reduction in diastolic blood pressure from 76±10.5 during the tea use to 72.5±10.5 mmHg after the wash out (p=0.02). When the tea use period was compared to the baseline assessment, there was a significant increase in urinary potassium from 50.5±20.4 to 56.2±21.8 mg/24-hour (p=0.017), magnesium/creatinine ratio from 58±22.5 to 69.1±28.6 mg/gCr24-hour (p=0.013) and potassium/creatinine ratio from 39.3±15.1 to 51.3±34.7 mg/gCr24-hour (p=0.008). The number of kidney stones per patient decreased from 3.21±2.02 to 2.02±2.07 calculi (p < 0.001) after consumption of tea compared with the initial stage. Initial evaluation showed hypercalciuria and hypocitraturia in 24 patients (42.8%), hyperuricosuria in six (10.7%) and hyperoxaluria in five cases (8.9%). In patients with hyperuricosuria there was a decrease in the amount of urinary uric acid from 0.77±0.22 to 0.54±0.07 mg/24-hour (p=0.0057). After the use of P. niruri, in patients with hypocitraturia, urinary citrate increased from 211.8±123.7 to 322.3±145.8 mg/24-hour (p=0.0282). In patients with hyperoxaluria there was a reduction in urinary oxalate from 59.0±11.7 to 28.8±16.0 mg/24-hour (p=0.0002). Conclusion: P. niruri intake is safe and does not cause significant adverse effects or significative serum metabolic changes. The use of the tea plant increases urinary excretion of magnesium and potassium. Some urinary metabolic changes predisposing to the formation of calculi normalized in subgroups of patients studied. The consumption of P. niruri contributed to the elimination of urinary calculi

A formal approach to the modeling, simulation and analysis of nano-devices.

Nano-devices are molecular machines synthesized from molecular subcomponents whose functions are combined in order to perform the func- tion of the machine. It frequently results of relative motions of subcomponents triggered by chemical events such as excitement induced by light, acidity or tem- perature changes. Thus the function consists in the transformation of a chemical event into a mechanical event. An important and characteristic feature of these devices is their intrinsic compositional nature. Therefore process-algebra for- malisms are natural candidates for their modeling. To this aim we introduce a dialect of the -calculus, the nano calculus. It is a rule-based language, the basic agents are molecules, with explicit representa- tion of molecular complexations and internal states. Its stochastic semantics is governed by rules which correspond to chemical reactions. The stochastic rate of the rule, possibly in nite, corresponds to the kinetic rate of the reaction. We illustrated its relevance for the modeling and simulation of nano-devices with an example stemming from the collaboration with the chemistry department of bologna: the [2]RaH rotaxane. We modeled it in nano and simulated its behaviour under various conditions of concentration: rst we validate our model by checking its correspondance with the experimental data and then we investi- gate extreme conditions not observable in practice. We were able to show that some classical assumption about kinetic rates were not correct any longer in this setting. The calculus has many advantages for the modelling of biochemical sys- tems. It is in particular compact, easily reusable and modi able and maybe more importantly much biological-like and thus easier to learn for biochemists. On the other hand the -calculus, also often used to model biochemical sys- tems, has a much more developed theory and more available tools. We present an encoding from the nano calculus to the stochastic -calculus. It satis es a very strong correctness property: S ! T , [[S]] ! [[T]], where S and T are nano terms, is the rate of the reaction and [[:]] is the encoding. Thus it permits to use nano as a front-end formalism and still get the bene ts of the theory and tools of the -calculus. We carry on with a study of the chemical master equation. It probabilisti- cally describes the possible behaviours of the system over time as a di erential equation on the probability to be in a given state at a given instant. It is a key notion in chemistry. There have been many e orts to solve it, and methods such as the Gillespie's algorithm has been developed to simulate its solution. We introduce and motivate a notion of equivalence based on the chemical master equation. It equates state with similar stochastic behavior. Then we prove that this equivalence corresponds exactly to the notion backward stochastic bisimu- lation. This bisimulation di ers from the usual ones because it considers ingoing transitions instead of outgoing transitions. This results is worth in itself since it establishes a bridge between a chemical semantics and a computer semantics, but it is also the rst step towards a metrics for biochemistry. Finally we present an unexpected consequence of our study of the nano calculus. We study the relative expressiveness of the synchronous and asyn- chronous -calculus. In the classical setting the latter is known to be strictly less expressive than the former. We prove that the separation also holds in the stochastic setting. We then extend the result to the -calculi with in nite rates. We also show that under a small restriction the asynchronous -calculus with in nite rates can encode the synchronous -calculus without in nite rates. In- terestingly the separation results are proved using the encodability of the nano calculus. We also propose and motivate a stochastic -calculus with rates of di erent orders of magnitude: the multi-scale -calculus to which we generalize our results. Finally we prove that in the probabilistic settings the asynchronous -calculus can be encoded into the asynchronous one.

CT with 3D-Image Reconstructions in Preoperative Planning

Computed tomography is one of the most evolving fields of modern radiology. The current CT applications permit among other things angiography, 3D image reconstructions, material decomposition and tissue characterization. CT is an important tool in the assessment of specific patient populations prior to an invasive or surgical procedure. The aim of this dissertation was to demonstrate the decisive role of CT with 3D-image reconstructions in haemodialysis patients scheduled to undergo fistulography, in patients undergoing surgical breast reconstructions with a perforator flap and in patients with complicated renal calculi scheduled to undergo percutaneous nephrolithotomy. CT Angiography with 3D image reconstructions was performed in 31 patients with failing arteriovenous fistulas and grafts, illustrating the vascular anatomy in a comprehensive manner in 93.5% of the evaluated segments and demonstrating a sensitivity of 95% compared to fistulography. In 59 mastectomy patients scheduled to undergo reconstructive breast surgery with a deep inferior epigastric perforator flap, the preoperative planning with CT Angiography with 3D image reconstructions of the anterior abdominal wall providing details of its vascular supply, reduced surgery time significantly (p&lt; 0.001) and resulted in fewer complications. Dual Energy CT Urography with advanced image reconstructions in 31 patients with complicated renal calculi scheduled to undergo PNL, resulted in a new method of material characterisation (depicting renal calculi within excreted contrast) and in the possibility of reducing radiation dose by 28% by omitting the nonenhanced scanning phase. Detailed analysis of the changes renal calculi undergo when virtually reconstructed was performed and a comparison of renal calculi number, volume, height and attenuation between virtual nonenhanced and true nonenhanced images was undertaken. All parameters were significantly underestimated in the virtual nonenhanced images. CT with 3D-reconstructions is more than just “flashy images”. It is crucial in preoperative planning, optimizes various procedures and can reduce radiation dose.

ANOVA test for repeated measures and Tukey test and McNemars test for categorial variables. The significance level was set at 5%. Results: Thirty-six patients (64%) were female and mean age was 44.1±9.16 years-old. The mean BMI was 27.2±4.4 Kg/m2. There was no change in anthropometric and serum parameters or urinary volume throughout the study period. There was a reduction in diastolic blood pressure from 76±10.5 during the tea use to 72.5±10.5 mmHg after the wash out (p=0.02). When the tea use period was compared to the baseline assessment, there was a significant increase in urinary potassium from 50.5±20.4 to 56.2±21.8 mg/24-hour (p=0.017), magnesium/creatinine ratio from 58±22.5 to 69.1±28.6 mg/gCr24-hour (p=0.013) and potassium/creatinine ratio from 39.3±15.1 to 51.3±34.7 mg/gCr24-hour (p=0.008). The number of kidney stones per patient decreased from 3.21±2.02 to 2.02±2.07 calculi (p < 0.001) after consumption of tea compared with the initial stage. Initial evaluation showed hypercalciuria and hypocitraturia in 24 patients (42.8%), hyperuricosuria in six (10.7%) and hyperoxaluria in five cases (8.9%). In patients with hyperuricosuria there was a decrease in the amount of urinary uric acid from 0.77±0.22 to 0.54±0.07 mg/24-hour (p=0.0057). After the use of P. niruri, in patients with hypocitraturia, urinary citrate increased from 211.8±123.7 to 322.3±145.8 mg/24-hour (p=0.0282). In patients with hyperoxaluria there was a reduction in urinary oxalate from 59.0±11.7 to 28.8±16.0 mg/24-hour (p=0.0002). Conclusion: P. niruri intake is safe and does not cause significant adverse effects or significative serum metabolic changes. The use of the tea plant increases urinary excretion of magnesium and potassium. Some urinary metabolic changes predisposing to the formation of calculi normalized in subgroups of patients studied. Cette thèse étudie la théorie de la correspondance et la théorie des preuves pour la logique modale monotone et les logiques qui en sont proches.La première partie de la thèse établit une connexion formelle entre la théorie de la correspondance algorithmique et des résultats de caractérisation duale pour les treillis finis, similaire à la caractérisation par Nation d'une hiérarchie de variétés de treillis qui généralise les treillis distributifs. Cette connexion formelle est établie en utilisant la logique modale monotone. Nous adaptons l'algorithme ALBA pour la correspondance à l'environnement de la logique modale monotone, et nous utilisons un encodage, induit par une dualité, des treillis finis sous forme de 'neighbourhood frames' pour traduire les termes de la théorie des treillis en formules de la logic modal monotone.La deuxième partie de la thèse étend la théorie des 'display calculi' à la logique Baltag-Moss-Solecki pour les actions épistémiques et la connaissance (Epistemic Actions and Knowledge), à la logique modale monotone et à la logique propositionnelle dynamique (PDL). Nos résultats incluent plusieurs méta-théorèmes d'élimination de la coupure qui généralisent le théorème original de Belnap dans des dimensions différentes et indépendantes. Les deux principales généralisations des 'display calculi' traitées dans la thèse sont : la généralisation d'une théorie pour les langages ne contenant qu'un seul type à une théorie pour les langages contenant plusieurs types, et la généralisation d'une théorie pour les calculs satisfaisant la propriété de 'display' aux calculs ne la satisfaisant pas. / The present thesis focuses on Monotone Modal Logic and closely related logics from the point of view of Correspondence Theory and Proof Theory.The first part of the thesis establishes a formal connection between algorithmic corre- spondence theory and certain dual characterization results for finite lattices, similar to Nation's characterization of a hierarchy of pseudovarieties of finite lattices progressively generalizing finite distributive lattices. This formal connection is established through monotone modal logic. Specifically, we adapt the correspondence algorithm ALBA to the setting of monotone modal logic, and we use a certain duality-induced encoding of finite lattices as monotone neighbourhood frames to translate lattice terms into formulas in monotone modal logic.The second part of the thesis extends the theory of display calculi to Baltag-Moss- Solecki's logic of Epistemic Actions and Knowledge (EAK), Monotone Modal Logic (MML), and Propositional Dynamic Logic (PDL). Our results include several cut-elimination metatheorems, which generalize the original metatheorem of Belnap in different and mutually independent dimensions. The two main generalizations of display calculi treated in the thesis are: the generalization from single type to multi-type languages, and from the full or relativized display property to no display property.
