Pesquisa do estado de Minas Gerais (FAPEMIG) / As doen?as cr?nicas n?o transmiss?veis constituem a principal causa de mortalidade em todo o mundo e t?m aumentado em ritmo acelerado. Elevadas taxas de interna??o e re-interna??o hospitalares causadas pela descompensa??o cl?nica da insufici?ncia card?aca t?m sido descritas e representam um grande problema. O objetivo deste estudo foi conhecer as caracter?sticas cl?nicas, epidemiol?gicas e terap?uticas das interna??es devido ? Insufici?ncia Card?aca em um hospital do interior de Minas Gerais. Realizou-se o estudo quantitatitativo, descritivo e observacional de 391 prontu?rios de interna??es pelo SUS devido a Insufici?ncia Card?aca, no per?odo de 2 anos. Foram realizadas an?lises de estat?stica descritiva utilizando-se do software Statistical Package for Social Science (SPSS) para obten??o de m?dia e mediana dos dados coletados. O n?vel de signific?ncia adotado foi de 95% (p<0,050). Os resultados demonstraram que a m?dia de idade foi de 67,75 anos ?15,5, sendo 51,2% do sexo feminino. Dentre as comorbidades: 77,0% apresentaram Hipertens?o Arterial Sist?mica, 19,7% Diabetes e 30,3% Chagas. A frequ?ncia de infarto do mioc?rdio pr?vio foi de 10,3%. A principal queixa identificada foi dispn?ia (81,6%). A m?dia do tempo de interna??o foi de 6,23 dias, e o custo m?dio de R$ 1.202,78. A taxa de mortalidade foi de 12,5%. N?o houve associa??o estatisticamente representativa entre ?bito e as vari?veis sexo, comorbidades e quantidade de doen?as. Os principais medicamentos prescritos foram: anticoagulantes 104,60%, diur?ticos 81,84% e digit?licos 53,19%. O uso de inibidor da ECA e ?-bloqueador foram inferiores ao recomendado nas diretrizes. Apenas 26,6% (n=104) chegaram ao hospital com um documento de refer?ncia e 39,9% (n=156) receberam algum tipo de encaminhamento ao sair de alta. Dentre as interna??es hospitalares a maior propor??o foi de mulheres com idade avan?ada e muitas comorbidades associadas. The non- transmissible chronic diseases are the leading cause of mortality around the world and have been increasing at a rapid pace. High rates of hospitalization and re-hospitalization caused by hospital clinical decompensation of heart failure have been described and represent a major issue. The aim of this study was getting to know the characteristics of the clinical, epidemiological and therapeutic hospitalizations due to heart failure in a hospital in Minas Gerais. We carried out a study of the quantitative, observational and descriptive records about 391 SUS admissions due to heart failure, in the period of 2 years. It was hold analysis including descriptive statistics using the Statistical Package for Social Science (SPSS) to obtain mean and median rates of the data collected. The significance level applied was 95% (p <0.050). The results showed that the average age was 67.75 ? 15.5 years, 51.2% of them, female. Among the comorbidities: 77.0% had systemic hypertension, 19.7% had Diabetes wounds and 30.3% had Chagas disease. The frequency of before myocardial infarction was 10.3%. The main complaint identified was dyspnea (81.6%). The average length of stay was 6.23 days and the average cost of R$ 1202,78. The mortality rate was 12.5%. There wasn?t a representative association between death and the variables gender, comorbidities, and number of diseases. The main drugs prescribed were: 104.60% anticoagulants, diuretics 81.84% and digitalis 53.19%. The use of ACE inhibitors and ?-blocker were lower than the recommended guidelines. Only 26.6% (n = 104) of the patients arrived at the hospital with a reference document and 39.9% (n = 156) of them received some type of referral to leave the hospital. The highest proportion among hospital admissions was women with advanced age and lots of comorbidities. The results showed that it is possible to improve the quality of care offered contributing to improved quality of life, decrease the number of hospitalizations, costs and have a positive impact in reducing the mortality rate in the hospital environment.

Prediction of Clinical Symptoms and Psychological Flexibility Using a Novel Values Card Sort ActivityKimball, Ryan S. 01 August 2018 (has links)
Values are directly relevant in a number of theoretical orientations in psychology, including Acceptance and Commitment Therapy (ACT). In ACT, clarification of one’s personal values is paramount. The present study examined the ability to predict clinical symptoms and psychological flexibility using variables derived from one’s performance on the ACT Values Card Sort (ACT-VCS), a novel values clarification exercise. The independent variables, obtained from the ACT-VCS, included 1) the number of values endorsed as very important in the initial sort (i.e., valuing propensity), 2) the number of values domains represented in the final sort (i.e., values diversity), and 3) the extent to which one’s values were oriented toward uncontrollable experiences (i.e., control agenda endorsement). Three hierarchical regressions were conducted to examine the extent to which these three predictors accounted for the variance in scores of a) depression, anxiety, and stress, b) psychological flexibility, and c) psychological inflexibility. The psychological flexibility model was significant (∆R squared = .25 (F (3, 69) = 12.20, p < .001) with valuing propensity (squared semi-partial correlation r = .13, p < .001) and control agenda endorsement (squared semi-partial correlation r = .05, p < .01), but not values diversity, independently accounting for a significant portion of the variance. Prior therapy experience moderated some of these relationships. These findings provide preliminary evidence for using performance variables from the ACT-VCS to predict clinical variables.

Apesar de ser uma criação recente, o cartão de crédito desponta como um dos mais utilizados meios de pagamento, além de instrumento de viabilização de crédito na sociedade atual. É diferenciação relevante a ser notada entre o cartão de crédito, peça plástica, e o sistema de cartão de crédito. Enquanto o primeiro se refere apenas ao documento de legitimação, ou seja, instrumento físico capaz de demonstrar uma relação existente entre o titular do cartão e o emissor, um dos contratos que integram o sistema, o segundo é o produto dos contratos coligados que possibilitam a concretização da operação. Além do contrato já indicado, o contrato entre fornecedor e emissor e o contrato entre fornecedor e titular formam, em um modelo básico, os contratos envolvidos na operação, coligados funcionalmente, ou seja, apesar de manter sua estrutura formal segmentada, formam um conjunto que viabiliza a operação. Os contratos envolvidos, apesar da tipicidade social, com a reiteração de contratações semelhantes, abrangendo os mesmos direitos e obrigações que identificam o modelo, são legalmente atípicos, já que as prestações envolvidas não permitem identificação com os tipos legais atualmente positivados nem assemelhação que permita aplicação das regras próprias daqueles, restando, no direito brasileiro, a vigência de normas e princípios gerais aos contratos e obrigações envolvidos. Os contratos coligados demandam uma recompreensão dos princípios tradicionais dirigidos e aplicáveis aos contratos atomizados, com diversos efeitos para o estudo dos contratos de cartão de crédito e a busca de soluções, até mesmo para as tradicionais questões controvertidas. / Despite being a recent creation, credit cards rise as one of the most used means of payment, besides being instruments of credit viabilization in the current society. There is a relevant differentiation to be observed between credit cards, plastic pieces, and the credit card system. While the former only refers to the legitimation document, that is, to the physical instrument capable of evidencing a relationship existing between the card holder and the issuer, one of the contracts that integrate the system, the latter is the product of the linked contracts that allow the operation execution. Besides that already indicated contract, the contract between suppliers and issuers and the contract between suppliers and holders form, in a basic model, the contracted involved in the operation, functionally linked, that is, although keeping their formal structure segmented, they form a set that feasibilizes the operation. The involved contracts, despite the social typicity, with the reiteration of similar contractings, involving the same rights and obligations that identify the model, are legally atypical, as the considerations involved do not allow identification with the legal types currently ascertained, nor similarity allowing application of their own rules, there being, in the Brazilian laws, the validity of general norms and principles governing the contracts and obligations involved. The linked contracts require a re-understanding of the traditional principles directed towards and applicable to atomized contracts, with various effects for the study of credit card contracts and for the quest for solutions, even for traditionally controvert issues.

Empreende à análise das decisões judiciais a respeito do contrato de cartão de crédito como meio de prevenir ou tratar o superendividamento do consumidor, por ser o contrato que representa a maior dívida dos consumidores brasileiros. A pesquisa consistiu em uma revisão bibliográfica dos temas superendividamento e cartão de crédito e uma pesquisa empírica documental, das decisões do Superior Tribunal de Justiça (STJ). O método utilizado foi análise de conteúdo, compreendendo a elaboração de fichas de leitura previamente estabelecidas. As fichas foram catalogadas de acordo com o assunto tratado pelo acórdão. Obteve-se um total de 7 (sete) grupos de análise: ações revisionais, ações indenizatórias, ações relativas a práticas abusivas, ações de prestação de contas, ações de exibição de documentos, ações de cobrança e declaratórias de inexistência de débitos. A partir da análise dos dados, depreendeu-se as seguintes conclusões: 1. O tema superendividamento ainda não faz parte do repertório da Corte, não tendo sido nem mesmo alegado pelas partes em algum processo. 2. A maior dos processos são ações indenizatórias que discutem meramente o valor exorbitante ou irrisório da indenização. 3. As decisões da Corte são, repetidamente, obstadas pelas Súmulas 5 e 7 do STJ. 4. As revisionais tratam apenas da taxa de juros nos contratos de cartão de crédito. / Undertakes the analysis of judgments about the credit card agreement as a means of preventing or treating consumer over-indebtedness, as the contract represents the largest debt of Brazilian consumers. The research consisted of a literature review of over-indebtedness and the contract of credit card and documentary empirical research of the decisions of the Superior Tribunal de Justiça (STJ). The method used was content analysis, including the development of previously established reading summaries. The sumaries were listed according to the subject matter of the judgment. We obtained a total of seven (7) analysis groups: revisional actions, compensation claims, actions related to abusive practices, accountability actions, document display actions, charging actions and declaratory absence of debts. From the data analysis, the following conclusions can be surmised: 1. The over-indebtedness issue is not yet part of the Court\'s repertoire and was not even alleged by the parties in any case. 2. Most of the cases are compensation claims than merely discuss the exorbitant or negligible amount of compensation. 3. The Court\'s decisions are repeatedly hindered by Precedents 5 and 7 of the STJ. 4. Revisional actions treat only about the interest rate on credit card contracts.

Objetivo: Descrever o perfil cardiovascular de pacientes portadores de acromegalia matriculados no Centro de Diabetes e Endocrinologia do Estado da Bahia (CEDEBA). Casu??stica e M??todos: Trata-se de uma s??rie de casos, descritiva correlacional, em que foram avaliados 37 pacientes portadores de acromegalia, cadastrados no CEDEBA, tendo sido realizados ecocardiograma transtor??cico e duplex scan de car??tidas, al??m de coletadas informa????es sociodemogr??ficas e cl??nicas nos prontu??rios desses pacientes. Esses dados foram analisados e cruzados por meio do software Statistical Package for Social Sciences (SPSS Inc., Chicago, IL,EUA), vers??o 14.0 for Windows. Resultados e Conclus??o: Constatou-se que h?? um prov??vel subdiagn??stico importante da patologia na Bahia e uma preval??ncia muito elevada de altera????es cl??nicas e cardiovasculares na amostra estudada que pode ser atribu??da ao controle hormonal inadequado de maior parte dos pacientes com a mensura????o de IGF-1 no prontu??rio, refletindo a car??ncia no fornecimento regular dos medicamentos para a patologia. Use of the Individualized Behavior Rating Scale Tool (IBRST) as a Self-Monitoring Tool to Improve Classroom BehaviorMartinez, Dominique Frances 30 June 2016 (has links)
Research on self-monitoring in the classroom setting has shown decreases in problem behavior and increases in academic engagement in students. Behavior rating scales can be used by teachers to monitor student progress or by students as a self-monitoring tool. The current study examined the impact of using the Individualized Behavior Rating Scale Tool (IBRST) as a self-monitoring tool on problem and on-task behavior in students in a high-need public elementary school. The results indicated that the self-monitoring with the IBRST successfully decreased disruptive behavior and increased on-task behavior in all 3 participating students during targeted academic time periods, evidenced by both direct observations and teacher- collected IBRST data. Improved levels of behaviors were maintained during fading with all 3 students and at 1-week follow-up with 2 students. The results also showed that both teachers and students had high levels of satisfaction with the self-monitoring procedures using the IBRST.

Le travail réalisé dans le cadre de cette thèse a pour but de fournir de nouvelles approches méthodologiques pour caractériser les communautés microbiennes en milieu océanique profond et de mesurer leurs activités. Dans le cadre de cette thèse, nous avons tout d'abord amélioré une technique d'hybridation in situ (CARD-FISH) pour dénombrer les groupes phylogénétiques majeurs en milieu marin profond. En Mer Tyrrhénienne, le pourcentage des Bacteria est toujours plus élevé que celui des Crenarchaea et des Euryarchaea. Alors que le pourcentage des Crenarchaea augmente avec la profondeur, celui des Euryarchaea est relativement homogène le long de la colonne d’eau. Dans le cadre d'une collaboration avec le Centre de Physique des Particules de Marseille sur le programme ANTARES (télescope sous-marin à neutrino), nous avons étudié quel pouvait être le rôle des bactéries luminescentes dans le bruit lumineux détecté quelquefois par le télescope. Une première étape a été de développer une approche quantitative par la méthode CARD-FISH sur les ARNm des gènes lux qui n'a été qu'en partie concluante mais donne des résultats préliminaires encourageants. Notre attention s'est portée sur l'identification puis la caractérisation d'une souche isolée à 2200 m de profondeur à proximité du site ANTARES (nommée Photobacterium phosphoreum ANT-2200). Pour mieux comprendre le rôle des bactéries bioluminescentes en milieu profond, nous avons développé un système hyperbare spécifique pour évaluer l’effet de la pression hydrostatique sur cette souche. Alors que cette souche a été caractérisée comme piezotolérante (croissance non sensible à des variations de pression), la lumière produite est plus élevée à la pression de 22 MPa (2200 m simulée) qu'à 0.1 MPa (pression atmosphérique). Enfin, une étude complémentaire m'a conduit à étudier l'effet de la pression hydrostatique sur une souche hydrocarbonoclaste Marinobacter aquaeolei #5. Là encore, nous montrons que la modification de la pression hydrostatique n’influence pas le taux de croissance (souche piezotolérante), mais peut fortement modifier les voies métaboliques, notamment la quantité et la nature des cires produites. Les cires intracellulaires s’accumulent sous formes de corps lipidiques avec une proportion moins importante à 35 MPa alors que le rapport d'insaturation des acides gras membranaires et la quantité de cires di-insaturées augmentent à la pression 35 MPa. Par ces études, nous suggérons que la pression hydrostatique joue un rôle important au niveau de la physiologie des bactéries marines et la distribution des procaryotes dans les écosystèmes marins profonds. / The purpose of this work is to provide new methodological approaches to characterize the microbial communities in deep-sea waters and to measure their activities. Within the framework of this thesis, first of all we improved the technique of in situ hybridization (CARD-FISH) to count the major phylogenetic groups in deep-sea zones. At the Tyrrhenian Sea, the percentage of Bacteria was always higher than Crenarchaea and Euryarchaea. Whereas the percentage of Crenarchaea increases with depth, while Euryarchaea is relatively homogeneous along the water column. A second step, within the framework of the ANTARES program in collaboration with the Centre de Physique des Particules de Marseille, we have studied the role of the luminescent bacteria in the luminous background detected sometimes by the telescope. We first developed a quantitative approach using CARD-FISH on mRNA of the Lux genes which was only partly conclusive but gave encouraging preliminary results. Then, we characterized a luminous strain isolated at 2200 m-depth close to the ANTARES site (named Photobacterium phosphoreum ANT-2200). For better understanding the role of the bioluminescent bacteria in deep-sea, we developed a specific hyperbaric system to evaluate the effect of the hydrostatic pressure on this strain. Whereas this strain was characterized as piezotolerante (growth non sensible to variations of pressure), bioluminescence was always higher at 22 MPa (2200 m simulated) than at 0.1 MPa (atmospheric pressure). Finally, a complementary study was done to study the effect of the hydrostatic pressure on a hydrocarbonoclastic bacteria Marinobacter aquaeolei #5. We have showed that the modification of the hydrostatic pressure does not influence growth rate (piezotolerante strain), but can strongly modify the metabolic ways, in particular the quantity and the nature of produced waxes. The intracellular waxes accumulated in the cells were proportionally less important at 35 MPa than at O.1 MPa. At the contrary, the unsaturation ratio of the membrane fatty acids and the quantity of diunsaturated waxes esters increased at 35 MPa. By these studies, we demonstrated that the hydrostatic pressure plays an important role on the physiological level of the marine bacteria and the distribution of the procaryotes in the deep sea ecosystems.
