Factors Influencing Oncology Nurses Discussing Cannabis Use with Patients Experiencing Chemotherapy-Induced Nausea

Background: Cannabis has been legalized in Canada since October 2018 and shown to be effective for chemotherapy-induced nausea (CIN). Purpose: Guided by the Ottawa Model of Research use, the aim was to determine factors influencing oncology nurses discussing cannabis use with patients experiencing CIN. Part I: A literature review to identify oncology nurses’ practices, knowledge, and attitude toward providing guidance on cannabis use for patients with CIN. Twelve articles were included. Results showed that health care professionals were hesitant to provide guidance for patients on using cannabis for medical purposes. But no studies specifically focus on nurses and CIN. Part II: A descriptive, cross-sectional study was conducted using survey methods. Twenty-five Canadian oncology nurses responded to the survey. Half (n=11) correctly answered the knowledge question about the effectiveness of cannabis. Most (n=18) did not feel confident providing guidance on use of cannabis for CIN. The top three barriers identified are social stigma, lack of knowledge, and lack of support in the workplace. Conclusion: Few Canadian oncology nurses discuss cannabis use for CIN. Identified barriers need to be addressed for oncology nurses to be prepared to discuss use of cannabis for CIN.

Icke-farmakologiska omvårdnadsåtgärder vid cytostatika-inducerat illamående hos vuxna patienter : En allmän litteraturstudie / Non-pharmacological nursing interventions in chemotherapy-induced nausea in adult patients : A literature review

Bakgrund: Cancerpatienter som genomgår cytostatikabehandling får ofta biverkningar i form av illamående och kräkningar. Ett långvarigt illamående kan ge upphov till både psykiska och fysiska konsekvenser, som ett försämrat allmäntillstånd, minskad livskvalitet och minskat välbefinnande. De antiemetika läkemedlen som finns mot Chemotherapy-induced nausea and vomiting (CINV) fungerar i många fall inte helt optimalt för patienterna, och många patienter är villiga att testa icke-farmakologiska metoder som ett komplement. Syftet: att belysa icke-farmakologiska omvårdnadsåtgärder mot cytostatika-inducerat illamående hos vuxna patienter. Metoden var en allmän litteraturstudie som genomfördes med tolv stycken vetenskapliga artiklar som granskades med hjälp av en fem-stegs modell. Resultatet visade på att icke-farmakologiska metoder kan ha effekt mot CINV och presenteras i tre huvudkategorier, Akupunktur, Akupressur och Avslappningsmetoder och muskelavslappning. Slutsatsen att icke-farmakologiska metoder har effekt mot CINV men att det behövs mer forskning i Europa och Sverige för att kunna tillämpa metoderna mer frekvent i den svenska hälso-och sjukvården än vad det görs idag. Det av vikt att sjuksköterskor som träffar dessa patienter som lider av CINV har kunskap om de olika icke-farmakologiska metoderna. / Background: Cancer patients undergoing chemotherapy often experience side effects in form of nausea and vomiting. Prolonged nausea can give rise to both psychological and physical consequences for the patient, the quality of life and well-being can decrease. The antiemetic drugs that are available against CINV does not work perfectly in many cases, and many patients are willing to try non-pharmacological methods as a supplement. The aim was to elucidate non-pharmacological nursing interventions against chemotherapy-induced nausea in adult patients. The study method was design as a structured literature review conducted with twelve scientific articled that was analyzed with a five-step model. The results of this literature study showed that treatment with non-pharmacological methods had a relieving effect. The result is presented in three main categories: Acupuncture, Acupressure and Relaxing Methods. The conclusion that non-pharmacological methods have an effect on CINV, but more research is needed in Europe and Sweden to be able to apply these methods more frequently in the Swedish health care system than it is done today. The conclusion is that non-pharmacological methods have an effect on CINV, but more research is needed in Europe and Sweden to be able to apply these methods more frequently in the Swedish health care system than is done today. Patientinddragelse -beskrivelse af kvalme og copingstrategier under kemoterapibehandling : et led i en sundhedsfremmende strategi / Patient participation in perception of nausea and coping strategies during chemotherapy : A health promotion strategy

Baggrund og Formål: Studier har vist at cancer patienter,der oplever kvalme under kemoterapibehandling,har en nedsat livskvalitet. Formålet med dette studie har væretat beskrive,hvordan cancerpatienter opleverkemoterapi-induceret kvalme og hvilke copingstrategier, de anvenderfor at håndtere og kontrollere kvalme. Metode:Der blev foretaget en kvalitativ dagborgsundersøgelse med 14 brystcancer patienter i adjuverende kemoterapibehandling på et universitetshospital i Danmark. Kvalitativ indholdsanalyse blev anvendt som analysemetode. Hovedresultat: Gennem analysen fremkom både et manifest og latent indhold,som beskrev patienterneserfaringer med og reflektioner over oplevelsen af kemoterapi-induceret kvalme. Der blev dannet fem kategorier,appetit, lokalisation, karakter, intensitet ogvarighed.Det latente indhold af disse kategorier blev beskrevet i temaet: kvalme er en fysisk oplevelse med komplekse og modstridende fornemmelser.Patienternes copingstrategier identificeredes gennemseks underkategorierog to kategorier. De to kategoriervar: at struktuere hverdagen med kvalme og at ændre fokus. Patienterne anvendte copingstrategier fra begge kategorier for at opnå kontrol over situationen og genoprette et velbefindende. Konklusion: Idette studie beskrivercancer patienterkemoterapi-induceret kvalme som en kompleks fysisk oplevelse, der håndteresved hjælp afen bred vifte af copingstrategier. Patientinddragelse er en mulig strategi i udviklingen afsundhedsfremmende aktiviteter,der kan højnelivskvaliteten hoscancerpatienter, somoplever kvalme i forbindelsen med kemoterapibehandlingen / Background and Aim: Several studies have reported low quality of life among cancer patients with chemotherapy-induced nausea.This study aimed to describe cancer patients’ perception of chemotherapy-induced nausea and the coping strategies used to ease nausea during chemotherapy. Method: Fourteenfemale cancer patients undergoing chemotherapy in a Danish university hospital participated in this study.Data were extracted from diaries, followed byqualitative content analysis. Result: Data analysis comprised both manifest and latent content and revealed the patients’ own experiences of and reflection about perceived chemotherapy-induced nausea.Five categories,appetite, localization, character, intensity, and durationemerged. The latent content of these categories is described by the theme: nausea is a physical experience of complex and contradictory sensations. The results also identified six subcategories and comprised two main categories, which described the women ́s coping strategies: structuring everyday life with nausea and shifting focus. To manage nausea, patients’ employed coping strategies from both categories. Conclusion: This study used patients’ own assessments to understand both the complexity of nausea and the patients’ description of coping with nausea.
