The participation of children in HIV/AIDS clinicals trials : ethical and legal considerations

This dissertation examines the legal position relating to the participation of children in research, especially in HIV preventive clinical research in South Africa. HIV/AIDS presents a real threat to humanity and particularly to the welfare of children. The participation of children in this type of trials is therefore vital. Children, as vulnerable participants, must also be protected from harm resulting from research. The study also considers the nature of HIV preventive clinical research, pointing to the inconsistencies in the legislation governing children’s participation in HIV preventive vaccine trials. The dissertation concludes that the question of the participation of children in HIV preventive clinical research poses many challenges, as the position in the South African law and relevant ethical guidelines are inconsistent and contradictory. The study recommends in the final instance that the relevant statutory provisions and ethical guidelines be harmonised in order to clear up the inconsistencies. / Jurisprudence / LL.M. How does emotionality affect memory in children with autism?

Indiana University-Purdue University Indianapolis (IUPUI) / The purpose of the current study was to investigate the impact of emotionality on the memory of children with an autism spectrum disorder. Although emotional events enhance memory in adults and children without an autism spectrum disorder, there are different memory patterns among individuals on the autism spectrum. Specifically, individuals with autism may show a decreased advantage in memory for emotional content and may have deficits in memory for information that is not presented visually. Currently, however, there are no studies that look at how emotional content affects memory specifically in children with autism. In the current study, children with and without autism were presented with stimuli contrasting emotional and neutral content using one of two modalities, auditory and visual, and then completed memory recognition tasks for the stimuli. Results indicate that children with an autism spectrum disorder did not demonstrate enhanced memory for emotional information. Rather, they were equally able to remember emotional and neutral stimuli. Additionally, individuals on the spectrum demonstrated better memory for visual stimuli compared to their neurotypical peers. These results support the notion that individuals with an autism spectrum disorder may learn and remember material differently than those without the disorder and that educators need to acknowledge these differences as children with autism spectrum disorders continue to be integrated into classroom settings.

Evaluation of multiple cytokine levels to improve our understanding of protective immune responses against Tuberculosis and to develop novel diagnostic methods

Thesis (MScMedSc)--Stellenbosch University, 2013. / ENGLISH ABSTRACT: Important steps towards the global control of Tuberculosis include the improvement of diagnosis, the development of effective vaccines and the identification of correlates of protection/protective immunity to Mycobacterium tuberculosis. This study has of three objectives: 1. To validate the findings of a previous study that showed increased levels of IL-1β and decreased levels of IL-17 in children who are exposed to tuberculosis but remain uninfected compared to those who are exposed/infected and unexposed/uninfected. 2. To define the protective immunological phenotype in children with negative IGRA’s and TST following exposure to Mycobacterium tuberculosis. 3. To evaluate a number of cytokines in both serum and saliva samples of identified tuberculosis cases and controls for their diagnostic potential and to evaluate saliva as a possible new diagnostic sample type. The study designs were as follows: Objectives1, and 2: Children with documented tuberculosis exposure and with Mycobacterium tuberculosis infection as assessed through interferon gamma release assays, children with exposure but no infection and a control group with no exposure nor infection were investigated. These participants were selected according to their exposure and infection phenotypes from a larger TB household contact study that was conducted in communities in Cape Town. Whole blood was stimulated in QuantiFeron tubes overnight and ten cytokines were measured in antigen stimulated and unstimulated supernatants by Luminex multiplex Immunoassay. Differential production of cytokines in the three groups was evaluated. Objective 3. Saliva and serum samples were collected from thirty eight adults with suspected tuberculosis who were recruited from a community health centre in Cape Town, after which the levels of thirty three host markers were evaluated in the samples using the Luminex platform. The main findings of the studies included: 1. Increased levels of IL-1β and decreased levels of IL-17 in children who are tuberculosis exposed but remain uninfected compared to those who are exposed/infected and unexposed/uninfected could not be confirmed. 2. Immune responses other than IFN-γ are different in children with different exposure and infection phenotypes. Higher IL-23 and IL-33 levels in children with tuberculosis exposure without subsequent Mycobacterium tuberculosis infection compared to children with no exposure were shown. 3. In both the tuberculosis cases and controls, the levels of most markers were above the minimum detectable limit in both serum and saliva, but marker levels were not consistently higher in one sample type. The levels of fractalkine , IL-17, IL-6, IL-9, MIP-1β, CRP, VEGF and IL-5 in saliva, and those of IL-6, IL-2, SAP and SAA in serum, were significantly higher in tuberculosis patients, in comparison to the levels obtained in those without active tuberculosis (p<0.05). The area under the ROC curve was ≥ 0.70 for most of these markers, thereby confirming their diagnostic potential for TB disease. The work presented in this thesis has identified markers that may grant an improved understanding on the mechanisms that are associated with protection against Mycobacterium tuberculosis in children. The preliminary results presented show that the identification of host markers in saliva is possible and the utility of saliva for the development of rapid immune-based tests for active tuberculosis is promising. / AFRIKAANSE OPSOMMING: Noemenswaardige vooruitgang in die globale beheer van Tuberkulose is onderworpe aan verbeterde diagnose, die ontwikkeling van doeltreffende vaksienes en die identifikasie van aanwysers van immuniteit teen Mycobacterium tuberculosis. Die doel van hierdie studie is: 1. Om die bevindinge van ‘n vorige studie te bevestig, waar verhoogde vlakke van IL-1β en verlaagde vlakke van IL-17 waargeneem is in kinders wat aan tuberkulose blootgestel is, maar nie geïnfekteer is nie. Om sekere sitokines, in beide serum en speeksel monsters van tuberkulose gevalle en kontroles, te evalueer as potensiële diagnosemiddels, asook die moontlikheid dat speeksel kan dien as ‘n nuwe diagnostiese monstertipe. Die studieraamwerk was as volg: Doel 1 &2:Die volgende groepe was onder meer ondersoek – Kinders blootgestel aan tuberkulose en wat gevolglik geïnfekteer is, soos vasgestel deur interferon gamma vrystellingstoetse; kinders wat wel blootgestel is maar nie geïnfekteer is nie en ‘n kontrolegroep wat geen blootstelling aan Mycobacterium tuberculosis gehad het nie. Hierdie individue is geselekteer volgens hul blootstellingsprofiel en infeksiefenotipes, uit ‘n groter blootstellingstudie op Kaapse huishoudings. Heelbloed is oornag gestimuleer en tien sitokiene is gemeet in antigeen-gestimuleerde en ongestimuleerde supernatante, deur middel van Luminex multipleks Immunotoetse. Differensiële produksie van sitokienes in hierdie groepe is gevolglik geëvalueer Doel 3: Speeksel en serummonsters van 38 volwassenes met vermeende tuberkulose, is versamel en die vlakke van drie en dertig gasheermerkers is gemeet deur middel van die Luminex platvorm. Die hoof bevindinge van hierdie studie sluit in: 1.Vehoogde vlakke van IL-1β en verlaagde vlakke van IL-17 kon nie bevestig word in die verskeie kindergroepe (Sien doel 1) nie. 2. Die immuunrespons, uitsluitend die IFN- γ respons, is veskillend in kinders met uiteenlopende blootstelling en infeksiefenotipes. Hoër vlakke van IL-23 en IL-33 is gevind in kinders wat blootgestel is aan tuberkulose, maar nie geïnfekteer is nie, in teenstelling met nie-blootgestelde kinders.. 3. In beide die pasiënte en kontroles was die meeste sitokienvlakke hoër as die minimum meetbare limiet in beide speeksel en serummonsters, hoewel merkervlakke nie konstant hoër was in enige van die twee monstertipes nie. Die vlakke van fractalkine, IL-17, IL-6, IL-9, MIP-1β, CRP, VEGF en IL-5 in speeksel en IL-6, IL-2, SAP en SAA in serum, was merkbaar hoër in tuberkulosepasiënte, in vergelyking met vasgestelde vlakke in individue sonder aktiewe tuberkulose. (p<0.05). Die oppervlak onder die ROC kurwe was ≥ 0.70 vir die meerderheid van die merkers. Dit is ‘n sterk aanduiding dat hierdie merkers potensiaal het as diagnostiese merkers vir tuberkulose. Hierdie navorsing het merkers geïdentifiseer wat die begrip van die megansime waarmee beskerming teen Mycobacterium tuberculosis gebied word in kinders, verbreed. An assessment of needs and programmes for children living on the street

M.A. / The street child phenomenon is a world-wide phenomenon. However ; it is perceived to be most prominent in third world countries as compared to first world countries. The main issue is that children are forced to leave the comfort or discomfort of their homes to live on the street. This results from a number of factors. According to Schurink and Tiba in Schurink (1993:10), an extensive literature study revealed the following key factors: rapid urbanization, high rates of unemployment, poverty, inadequate housing, maladjustment to family disintegration. According to Maphatane (1993:1), today's children and youngsters face many problems and pressures arising from the changing structure of the family and the community and the breaking down of traditional systems of support and preparations for adult life. The emergence of the street child phenomenon results from poor soci-economic and political conditions. For instance according to Agnelli , as cited by Schurink (1993:13), the roots of the present clay street child phenomenon seem to lie in the historical context of economic conditions and in national and international policies accepted by various countries.

Enrolment of children in clinical trials : Botswana perspective.

Paediatric clinical trials are crucial to ensure safety and efficacy of medicines in children. This study explored parents’ perceptions in Gaborone, Botswana, regarding children’s participation in clinical trials. One hundred participants completed a self-administered questionnaire. The results showed that 93% of the participants had a good knowledge of clinical trials; 74% thought that children would benefit from clinical trial participation; 63% would not enrol their children in clinical trials; 55% suggested that children should only be enrolled once they have reached the age of 18 years; and 71% reported that only children with an active disease should be enrolled. A large proportion (82%) reported that children’s participation should be voluntary, while joint parental consent was supported by 93% of responders. Regarding children’s assent 91% deemed respect for children’s assent essential, although 52% thought that children’s assent should not override the parental decision. There was a statistically significant correlation between finding clinical trials in general important and children’s participation in clinical trials (p=0.008, Fisher’s Exact Test), as well as the need for individual consent p<0.0001, Chi-Square). There was also a statistically significantly association between respondents, who would allow their children’s participation in clinical trials and who would encourage their family members and friends’ participation in clinical trials (p=0.0001, Chi-Square). An overwhelming 94% advocated for special regulations in Botswana to govern paediatric clinical trials. M.Ed. / A great deal of concern is being expressed about the high teenage pregnancy rate as well as the leaping AIDS and sexual disease figures. There are programmes in place which aim to bring these numbers down, but it appears as if neither the abortion rate nor premarital sexual experimentation have been affected. This situation is cause for concern. The question thus arises - why have these programmes not made any headway on the statistics? Perhaps the answer lies in the fact that most researchers have not asked the children themselves about their needs with regard to sex education. It is for this reason that this study has been undertaken. In this study a qualitative research method has been followed, where focus group interviews with Standard 7 pupils have been conducted. The original question posed by this researcher was as follows: "What are the needs of Standard 7 pupils with regard to sexual education and what guidelines could be suggested in order to provide a workable pupil-orientated education programme for high schools?" In turn, the posing of the above question led to the following aim: To gauge what the needs of Standard 7 pupils are with regard to sexual education in order to design guidelines for the development of a sex-education programme for high schools. The qualitative research which flowed from this is exploratory, descriptive and contextual. The analysis of the data indicate that there is a need for sexual education in schools, more than is at present available. Pupils would like this subject to be compulsory and that it be taught during stipulated times/periods. There is also a need for a cumulative curriculum in which enough time for discussions is allowed. The children believe that sexual education at school should be presented by a person whom they can trust. Boys and girls differ as to whether this person should be attached to the school or not. The girls are especially critical of the teacher as sex educator. The parent as sex educator has also been criticised. The children would prefer someone out of their peer group to guide them. They also feel that adequate training, openness and neutrality are important characteristics in a sex education specialist. The boys feel that this type of education should not be over-emphasised. Another need expressed is that the sex educator should work from a holistic perspective, thereby putting sex education in a natural context. The requirements with regard to the content of the sexual education programme include the following: knowledge of sexual responsibilities, as well as an understanding of anatomy and physiology, knowledge of the sexual act and premarital sex, as well as an understanding of pregnancy, rape and the way the opposite sex experiences sexuality The children also believe that the way sex is presented in the media should be put into perspective. Knowledge of deviant sexual behaviour, the availability of source material, for example where one would obtain a supply of contraceptives and where to go if one suspects that one is pregnant, are also needs which teenagers expressed with regard to programme content and sex education. From the study it became apparent that, even though the girls maintained that they all receive sex education at home, there are nevertheless many questions which still require addressing. A number of misconceptions were also apparent. Information which is highlighted by these themes and which is furthermore supported by the other research includes aspects such as parent and teacher involvement. A number of guidelines based on the themes identified by the analysis of the focus group interviews, have been developed for exposure and for possible use by compilers of sex education curriculi. A shortfall in this study has been identified and the following recommendations have been suggested: Further research should be undertaken as to the need for sex education o fpupils of other cultures and standards as well as the teachers' views about sex education.

M.Ed. (Multiculturalism and Education) / It appears that much research has been done in the past on parent education, but with the emphasis on the white community. As a result of the present movement in the RSA towards multicultural education, the need for parent education in all population and ethnic groups has been increased. This area of study has been grossly neglected up to now. Due to the problematic nature of the above-mentioned situation, this study is aimed at compiling guidelines for an education program for parents of behaviorally disturbed children in the multicultural school. An investigation into the characteristics of the behaviorally disturbed child was attempted by means of a study of literature. Existing parent education programs were also studied and evaluated. Insights derived from this study were then adapted to create a set of general and a set of specific guidelines for and education program for parents of behaviorally disturbed children in the multicultural school.
