Treatment of Comorbid Conduct Problems and Depression in Youth: A Pilot Study

The purpose of this study was to pilot a cognitive behavioral treatment protocol for adolescents with co-occurring conduct problems and depression and to examine changes in diagnostic status, symptomatic improvement, and related areas of functioning associated with this treatment. Manualized treatment consisted of 11 to 16 sessions that targeted problem solving skills, cognitive restructuring, affect regulation skills, and parenting strategies. A non-concurrent multiple baseline design was used to assess the effectiveness of the intervention. A sample of five adolescents, aged 11 to 14 years, participated; all five families completed the protocol. Measures of diagnostic status, symptoms, and global impairment were administered at baseline, post-treatment, and 1-month post-treatment. Decreases in depression and conduct problem symptoms as well as changes in diagnostic status were evident for three of the five participants. Clinician severity ratings improved for all of the children at post-treatment. Preliminary results suggest that integrated cognitive behavioral treatment for adolescents with conduct problems and depression is associated with symptomatic improvement as well. Results of this study have implications for improving treatment outcomes for youth with these comorbid conditions and may also lead to increased conceptual understanding of the nature of these comorbid problems. Randomized clinical trials are required to assess the effectiveness of this combined treatment program. / Ph. D.

Conduct disorders in children are very common. The consequences of such problems are far-reaching not least because they impact upon the quality of the parent-child relationship. Parent Management Training (PMT) is a widely recognised treatment for childhood conduct disorders although the outcomes for many children are disappointing. Theoretically, the current study considers why outcomes are not more favourable and illustrates that attention should be paid to the role of maternal cognition in the quality of mother-child relationships. The aims of this study were to: • orientate the reader to the nature of the problem • describe the development of a tool that measures the quality of the mother-child relationship, The Parent-Child Relationship Questionnaire (PCRQ) • assess the psychometric properties of the PCRQ • discuss the findings, strengths and weaknesses of the study and consider future research ideas Control mothers completed the PCRQ once. Referred mothers completed the PCRQ and a measure of depression pre and post PMT. These mothers also completed a measure of parenting stress pre group only. A further sample of mothers completed the PCRQ twice. Principal components analysis was used to determine a) the factor structure of the PCRQ and b) whether this differed between a control and referred sample. One factor was extracted from the control sample and two from the referred. The two factors extracted were named Negative Cognition and Emotion (NC-E) and Behavioural Management (BM). Reliability analysis revealed adequate levels of alpha (Cronbach's) for the PCRQ and NC-E subscale although only barely adequate for BM subscale in both samples. Testretest reliability analysis on a small sample revealed a high Pearson's correlation coefficient. Intra-class correlation coefficients for total PCRQ and the BM subscale were reasonable whereas for NC-E the coefficient was weak. Criterion validity was explored by comparing the control and referred sample means. The differences between the means were significant. Concurrent and convergent validity were investigated by computing correlations between the PCRQ and the Parenting Stress Index Short-Form (PSI-SF; Abidin, 1995). This showed moderate correlations between the measures. The correlations between two PSI-SF dimensions and the BM subscale were low. Discriminant validity was investigated by computing correlations between the PCRQ and the Beck Depression Inventory (BDI; Beck & Steer, 1993). This showed a moderate relationship. Sensitivity to change following PMT was investigated by computing a paired samples t test in the referred sample. This test was also used to compare the pre and post group scores for the individual subscales. The changes were significant. There was no relationship between PCRQ scores and either age or gender. Normative values for the PCRQ and its subscales were calculated using a theoretical normal distribution. The results suggest that the PCRQ can be used to investigate the extent of negative maternal cognition underlying the quality of the mother-child relationship. Its usefulness is enhanced by employing a measure of maternal depression.

Enlightenment politeness and the female reader : the role of didactic literature in teaching politeness to women in Virginia and Scotland, 1750-1850

This thesis explores the notion of gentility among wealthy women in Virginia from 1750 to 1850 by comparing it to Scottish Enlightenment-inspired codes of politeness practiced among the Scottish gentry residing in Edinburgh, the Highlands, and London in the same era. It analyzes how books that taught the codes of polite morality, here referred to as didactic literature, were read by genteel, young women in Scotland and Virginia and the ways in which this literature was applied to their education, courtship practices, and social behaviors. Scots and Virginians in this era were linked through migration patterns, correspondence between families, and a transatlantic book trade, but they were also linked through the interpretation of politeness. The polite manners of genteel individuals in Britain, instilled as a part of Scottish moral philosophy, were adopted by many who aspired to gentility in America, but original, archival research has indicated that this was especially true among the elites of Virginia society from the middle of the eighteenth to the middle of the nineteenth centuries. This comparison serves to emphasize the connection between Virginian and Scottish standards of politeness, indicating similarities in the interpretation of politeness, but also a divergence over time as a result of the influences of the American Revolution and evangelical religion. It has concluded that, by the middle of the nineteenth century, while the standards of didactic literature did not entirely disappear with regards to shaping Scottish manners, the codes taught in conduct books and instructive novels of an earlier era were more widely regarded in Virginia and came to form a uniquely Virginian interpretation of politeness.

Treatment of fourth class midshipmen hazing and its impact on academic and military performance and psychological and physical health

In 1992, the General Accounting Office conducted a study at our Nation's service academies to determine the prevalence of hazing and its impact on psychological and physical health; and academic and military performance. Based on the literature it is theorized that the prevalence of hazing has changed. Furthermore, it is hypothesized that hazing will negatively impact both academic and military performance; and psychological and physical health. These hypotheses were tested using a series of cross tabulation analyses, Pearson-correlation coefficients, and multiple hierarchical regressions on population data, classes 2005 through 2008. Cross tabulation analyses measured the prevalence of 23-hazing behaviors for the sample population. Correlation coefficients measured the relationship between hazing and several performance and health outcomes. Finally, multiple hierarchical regression analyses assessed the impact of hazing experiences on academic and military performance; and psychological and physical health. The study concludes that hazing has declined in the past 12-years. Furthermore, hazing is correlated as hypothesized to all of the outcomes examined except for academic and military performance. Finally, regression analyses determined that hazing impacts psychological and physical health, but only partially impacts academic and military performance.
